Acquired Eustachian tube atresia following turbinoplasty: multimodal surgical reconstruction, clinical outcomes, case series, and literature review.

BACKGROUND: Acquired atresia of the Eustachian tube (ET) is an uncommon but debilitating condition that can significantly impair middle ear ventilation and drainage, leading to chronic otitis media, conductive hearing loss, and aural fullness. Despite its impact, diagnosis and management remain challenging, with limited literature on standardized approaches. OBJECTIVE: This case series aims to describe the clinical presentation, imaging findings, surgical management, and outcomes of three adult patients with acquired ET atresia treated using multimodal surgical interventions, including balloo

Antibiotic resistance: a case of a chronic maxillofacial osteomyelitis complicating an odontogenic cellulitis.

INTRODUCTION AND IMPORTANCE: The post-antibiotic era is here. Oral diseases affect about half of the world's population, and hence, antibiotic resistance can have serious consequences given that antibiotics are the cornerstone of modern medicine. Nowadays, with the massive, abusive use of antibiotics, we are forcing bacteria to mutate genetically in order to survive. This antibiotic Darwinism creates resistant bacterial strains that encroach on practitioners' therapeutics. The European Centre for Disease Control estimates that 25 000 people die every year as a result of antibiotic resistance.

Retropharyngeal abscess revealing Granulomatosis with polyangiitis GPA (Wegener's): a case report.

INTRODUCTION: A retropharyngeal abscess (RPA) is an infection that develops in the deep spaces of the neck, more common in children under five due to their prominent lymph nodes. However, it can also occur in adults. To our knowledge, Case reports of RPA revealing Granulomatosis with polyangiitis (GPA) (Wegener's) have yet not been reported. CASE PRESENTATION: We present a case of a 62-year-old woman, originally from West Africa, with symptoms of sore throat, shortness of breath and otalgia. Imaging was compatible with RPA. Surgical drainage involved endoscopic and oral procedures with biopsi

Otogenic Sigmoid Sinus Thrombosis in a 34-Year-Old Woman: A Rare Intracranial Complication.

Otogenic sigmoid sinus thrombosis (SST) is a rare but potentially life-threatening intracranial complication of otitis media, particularly uncommon in adults in the antibiotic era. Diagnosis may be delayed due to nonspecific early clinical features, with rapid deterioration secondary to raised intracranial pressure and impaired venous drainage. We report the case of a 34-year-old woman who presented to the emergency department with a four-day history of progressive right-sided otalgia, headache, and vomiting. She initially presented alert with a Glasgow Coma Scale (GCS) score of 15/15 but rapi

Eustachian Tube Dysfunction Mimics.

Eustachian tube dysfunction (ETD) is a commonly suspected cause of aural fullness, otalgia, and hearing loss, but presentation varies among patients. In addition, these symptoms are not specific and can be indicative of other conditions. Some of these conditions are otologic in nature and can be localized to the external, middle, or inner ear. Others arise from outside the ear and can be attributed to functional, neurologic, or structural causes. While presentation may be similar to ETD, differentiation is possible with clinical history, physical examination, and objective testing such as otos

Eagle's Syndrome Presenting With Peripheral Facial Palsy-A Case Report.

BACKGROUND: Eagle's syndrome is a condition caused by an elongated styloid process and is characterized by various symptoms such as cervicofacial pain, foreign body sensation, headache, odynophagia, and otalgia. However, the occurrence of a unilateral peripheral facial palsy as a symptom of Eagle's syndrome has been described only rarely. METHODS: We report the case of a 60-year-old male patient who presented with a sudden and unprovoked onset of retroauricular pain on the left side, accompanied by peripheral left facial palsy. Imaging revealed an extensively elongated styloid process on the

Head and neck NUT carcinoma-radiotherapy dose, chemotherapy choice, and outcomes: lessons from a 7-year complete remission.

BACKGROUND: NUT carcinoma (NUTc) is a rare, aggressive squamous carcinoma defined by NUTM1 rearrangements. Outcomes in head and neck (H&N) disease are poor, and no uniform treatment standard exists; available evidence largely derives from case reports and small series. METHODS: We performed a structured narrative review of H&N NUTc with an emphasis on radiotherapy (RT) dose concepts, chemotherapy regimens, and outcome signals. RESULTS: Published evidence indicates poor overall survival, particularly in metastatic disease. Long-term remissions have been described in selected patients with non

Primary external auditory canal cholesteatoma in a young adult: Diagnostic challenges and surgical management of a rare otologic entity.

External auditory canal cholesteatoma is a rare and potentially destructive otologic condition characterized by the accumulation of keratinizing squamous epithelium within the external auditory canal, leading to progressive bony erosion. Despite being described in the 19th century, EACC remains an uncommon and frequently underrecognized entity, often misdiagnosed as keratosis obturans or chronic otitis externa. Its pathogenesis is multifactorial and incompletely understood, involving impaired epithelial migration, chronic microtrauma, local inflammation, and ischemic changes of the canal skin.

Inferior ST-Elevation Myocardial Infarction (STEMI) Presenting as Left Otalgia: A Case Report.

Diabetic populations are predisposed to atypical, or sometimes silent, responses to myocardial ischemia. This is often the result of chronic neuropathy involving the nociceptive sympathetic pathways of the heart that converge at the T1-T4/5 spinal columns. When these signals are blunted, ischemic pain signals may be delivered via the "Arnold's nerve," the auricular branch of the vagus that innervates the external auditory canal.  A 70-year-old woman with a history of type 2 diabetes (diagnosed 14 years before presentation), hypertension, hyperlipidemia, and chronic obstructive pulmonary diseas

Cryoneurolysis of auriculotemporal nerve for treatment of otalgia.

Otalgia in adults is often secondary to non-otologic pathology, particularly with normal otoscopic examination. Auriculotemporal neuralgia (ATN) is a rare and overlooked cause of secondary otalgia. We present a case of an Army veteran with chronic right otalgia and dizziness following blast-related mild traumatic brain injury who was initially diagnosed with vestibular migraine. Despite significant improvement in headache symptoms with migraine-directed preventive and abortive treatments, otalgia persisted. Auriculotemporal nerve block produced complete but short-lived pain relief, confirming

[From vestibular migraine to cochlear migraine].

While diagnosis and treatment consensus has been established for vestibular migraine, migraine-related cochlear manifestations (such as sudden/progressive sensorineural hearing loss, tinnitus, and aural fullness, otalgia) still face high rates of misdiagnosis and missed diagnosis. With the proposal of the "cochlear migraine" concept, the establishment of interdisciplinary precise classification is urgently needed. To this end, multidisciplinary experts held a discussion on the hotspots in this field. This article systematically reviews the cognitive evolution of migraine in neuro-otology, comm

Clinico-microbiological assessment of necrotizing otitis externa in subjects with diabetes mellitus in India.

Necrotizing otitis externa (NOE) represents a severe progression of otitis externa, particularly threatening in diabetic patients, with limited clinico-microbiological data for Indians. Therefore, it is of interest to evaluate the clinico-microbiological profile in 60 diabetic adults diagnosed with NOE at a tertiary center. Among participants (mean age 60.51±9.6 years; 60% male), universal findings included external auditory canal edema, otorrhea and severe nocturnal otalgia; pus cultures grew Pseudomonas spp. in 43.3% (n=26), with no growth in 13.3% (n=8), mild conductive hearing loss in 16.7

Spontaneous Rupture of a Peritonsillar Abscess Mimicking Successful Incision and Drainage: A Case Report.

Peritonsillar abscess (PTA) is a common deep infection of the head and neck that can present with significant clinical symptoms. Patients typically present with fever, malaise, sore throat, otalgia, dysphagia, and the characteristic muffled "hot-potato" voice. Although PTA is generally managed effectively with antibiotics, analgesics, and abscess drainage, spontaneous rupture is uncommon and may falsely suggest clinical resolution, posing a diagnostic and management challenge. We present the case of a 25-year-old woman who presented with a five-day history of sore throat, fever, odynophagia, d

The Role of Antibody Testing and Vaccination in Paediatric Recurrent Acute Otitis Media.

INTRODUCTION: Recurrent acute otitis media (RAOM) is a common condition most frequently affecting children under two years of age. The main causative agent is Streptococcus pneumoniae. Pneumococcal vaccination has been shown to be an effective method of RAOM prophylaxis. Currently, pneumococcal antibody testing can be performed to assess immunity levels, followed by booster vaccination if protection is suboptimal. We aimed to explore the role of antibody testing and vaccination in the context of managing paediatric RAOM. METHOD: A retrospective study was performed between January and December

Tongue Squamous Cell Carcinoma in a Young Patient Without Classical Risk Factors: A Case Report and Literature Review.

Squamous cell carcinoma of the tongue usually affects older patients with established risk factors such as tobacco and alcohol use. Its occurrence in very young patients without classical risk factors remains clinically challenging and may lead to diagnostic delay. We report the case of a 20-year-old male patient with no history of tobacco or alcohol use who presented with a three-month history of lingual swelling, dysphagia, right-sided otalgia, marked weight loss, and deterioration of general condition. Clinical examination revealed a firm right submandibular lymphadenopathy and an ulcerativ
