Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

Data augmentation method for computer-aided diagnosis using specular reflection.

UNLABELLED: Drug-resistant Plasmodium falciparum has been on the rise, mainly due to point mutations in the pfDHFR or dihydrofolate reductase gene. This has heightened urgency to discover and develop new antimalarial drugs. This research utilized Pharmacophore Modeling, Virtual Screening, Molecular Docking and Molecular Dynamics Simulation techniques to identify potential pfDHFR inhibitors from virtual databases. Ligand-based pharmacophore hypothesis (H1) (r = 0.949, r2 = 0.883) was formulated using a training set of 28 compounds that demonstrated pfDHFR inhibitory activity across four orders

IgG4-related disease in correlation with left main coronary artery aneurysm as a rare cause of heart transplantation.

BACKGROUND: Eagle syndrome is caused by an elongated styloid process affecting carotid arteries and cranial nerves. Pain, dysphagia, tinnitus, paresthesia (classic subtype), and neurovascular events (vascular subtype) may be triggered by head movements or arise spontaneously. However, Eagle syndrome remains underappreciated in the neurological community. We aimed to determine the most common neurological and non-neurological clinical presentations in patients with Eagle syndrome and to assess the clinical outcome post-surgical resection in comparison to non-surgical therapies. METHODOLOGY: We

Prevalence and Radiologic Manifestations of Internal Jugular Venous Stenosis in Patients with Pulsatile Tinnitus: A Case-Control Study.

BACKGROUND AND PURPOSE: The role of internal jugular venous stenosis (IJVS) in the development of pulsatile tinnitus (PT) is unclear. In this study, we aim to assess the prevalence of IJVS and to analyze the associated imaging findings in patients with and without PT. MATERIALS AND METHODS: We conducted a case-control study of consecutive adult patients with PT who underwent imaging evaluation with CTA/CTV with contrast between February 2018 and March 2022. Age- and sex-matched controls who received CTA and/or CTV for an indication other than PT, headache, or visual disturbance were included.

Longitudinal study of dietary intake and risk of persistent tinnitus in 2 large independent cohorts of women.

Persistent tinnitus is common, often disabling, and challenging to treat. Greater adherence to healthy diet patterns was associated with 30% lower risk of hearing loss. Dietary factors have also been implicated in tinnitus, but findings are not consistent and longitudinal studies are scarce. We examined the longitudinal associations of healthy diet patterns, specific food groups, individual foods, and risk of developing persistent tinnitus in 2 independent cohorts of 113 554 women, the Nurses' Health Study (1984-2022; n = 42 504) and Nurses' Health Study 2 (1991-2021; n = 71 050). Assessment and management of pulsatile tinnitus.

Pulsatile tinnitus refers to the perception of intermittent, often rhythmical sounds, which are typically throbbing, beating or whooshing in nature, and may or may not be synchronised to the patient's pulse. It may also be classified as either objective, where the clinician can also hear the sound on auscultation, or subjective, where only the patient hears it, often due to heightened awareness of blood flow in the ear. It is important for neurologists to have an understanding of pulsatile tinnitus as, especially when combined with other symptoms and signs, it may suggest a specific neurologic

Outcomes of Bariatric Surgery for Idiopathic Intracranial Hypertension: A Systematic Review and a Single-Arm Meta-analysis.

BACKGROUND AND AIMS: Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure (ICP) in the absence of structural pathology and is strongly linked to obesity. Weight reduction remains the only disease-modifying therapy, and bariatric surgery (BS) may offer durable ICP control and symptom improvement. This study evaluated postoperative clinical, visual, and physiological outcomes in patients with IIH undergoing BS. METHODS: A systematic review and single-arm meta-analysis were conducted in accordance with PRISMA and Cochrane guidelines. PubMed, Embase, and C

[Post-dural puncture headache : From the diagnosis to treatment].

Post-dural puncture headache (PDPH) is a frequent yet often underestimated complication of neuraxial anesthesia. The presumed mechanism involves cerebrospinal fluid loss due to accidental dural perforation, leading to intracranial hypotension and a secondary compensatory vasodilation. Meningeal irritation and neuroinflammatory processes also contribute to the complex pathophysiology. The main symptom is a severe orthostatic headache with pain exacerbation in an upright posture. This can be accompanied by neck stiffness, tinnitus, visual and other neurological disorders. Patient-specific risk f

Advances in the pathogenesis of pulsatile tinnitus induced by sigmoid sinus wall abnormalities: A narrative review.

Pulsatile tinnitus (PT) is characterized by the perception of rhythmic sounds synchronized with the heartbeat, with sigmoid sinus wall abnormalities (SSWA) being the most common etiology. The generation and transmission of sound in SSWA-PT are closely related to abnormal hemodynamics, elevated intracranial pressure, and anatomical abnormalities of the sigmoid plate and temporal bone pneumatization. Despite extensive research, the exact pathophysiological mechanisms of SSWA-PT remain unclear, often leading to misdiagnosis and suboptimal treatment outcomes. This narrative review synthesizes stud

Longitudinal analysis of the risk factors for onset and change in tinnitus in the Busselton Healthy Ageing Study.

INTRODUCTION: Effective prevention and intervention strategies for tinnitus rely on identifying risk factors and understanding its progression over time. However, longitudinal data on these aspects are limited. This study therefore aimed to (1) assess the incidence of tinnitus and identify risk factors associated with tinnitus development, and (2) assess the impact of tinnitus and change in impact over time and identify factors associated with this change. METHODS: Data from the Busselton Healthy Ageing Study, a population-based cohort of individuals born between 1946-1964, were used. Informa

Overlapping Clinical Presentation of Long COVID and Postacute COVID-19 Vaccination Syndrome: Phenotypes, Severity, and Biomarkers.

BACKGROUND: Postacute sequelae of COVID-19 (PASC), also known as long COVID, and postacute COVID-19 vaccination syndrome (PACVS) present overlapping but distinct clinical challenges. We hypothesize that PASC and PACVS share clinical features but differ in symptom patterns and biomarker profiles. This study aims to identify differences in presentation and distinguish immunologic biomarkers relevant to general clinical practice. METHODS: This cross-sectional study analyzed 181 patients from a PASC clinic at Columbia University Irving Medical Center. Patients were divided into PASC with myalgic

Assessment of 7-Item Eustachian Tube Dysfunction Questionnaire in a Clinical Population.

OBJECTIVE: Eustachian tube dysfunction (ETD) is a common condition among patients presenting to audiology clinics, often accompanied by nonspecific symptoms that complicate diagnosis. The 7-item Eustachian Tube Dysfunction Questionnaire (ETDQ-7) was developed to aid in identifying ETD; however, despite its widespread clinical use, its diagnostic validity has not been consistently demonstrated across patient populations. We evaluate ETDQ-7 performance in a diverse group of patients seen in an audiology clinic and compare it to an objective test of ETD based on modified impedance audiometry. ME

Length of Transverse Sinus Reinforcement Predicts Symptom Improvement After Venous Sinus Stenting for Idiopathic Intracranial Hypertension.

BACKGROUND AND OBJECTIVES: Chronically elevated venous sinus and intracranial cerebrospinal fluid (CSF) pressures in idiopathic intracranial hypertension (IIH) may weaken the dural walls covering these sinuses, increasing their compliance and susceptibility to compression by elevated CSF pressures. This may contribute to proximal or distal stent-adjacent stenosis after venous sinus stenting (VSS), explaining the persistent or recurrent IIH symptoms and elevated CSF pressures in some cohorts. We sought to assess whether increasing the length of transverse sinus (TS) reinforced by the stent (TS-

Initial compliance as a predictor of therapeutic outcomes in chronic tinnitus.

Tinnitus treatments incorporate various strategies, yet substantial challenges remain. Sound therapy (ST) is cost-effective and self-administered, but its efficacy and standardization continue to be debated. This study aimed to evaluate treatment outcomes and associated factors of ST combined with reinforced directive counseling (DC) in patients with chronic tinnitus. From January 2020 to August 2023, 53 patients underwent one year of ST-based self-training, with reinforced DC provided every 6 months. Tinnitus distress was assessed using the numeric rating scale (NRS, 0-100), tinnitus handicap
