Severe mitral stenosis as a cause of paradoxical low-flow, low-gradient severe aortic stenosis: an explorative study on hemodynamics and outcomes.

BACKGROUND: Aortic stenosis (AS) and mitral stenosis (MS) are valvular heart diseases that may present concomitantly, particularly in regions where rheumatic heart disease remains prevalent. While each condition has been extensively studied in isolation, there is limited data on the clinical characteristics of patients with combined AS-MS. METHODS: We retrospectively identified patients with significant AS and concomitant significant MS from the echocardiography database between 2003 and 2018. Exclusion criteria included left ventricular ejection fraction <50%, other significant valvular lesi

Non-rheumatic calcific aortic valve disease as a global driver of heart failure: Burden and three-decade trends.

BACKGROUND: Global aging and lifestyle shifts raise cardiovascular disease burden, but the specific contribution of non-rheumatic calcific aortic valve disease (nrCAVD) to heart failure (HF) in older adults is incompletely defined. OBJECTIVES: To quantify global trends, drivers, inequalities, and future projections of HF attributable to nrCAVD among persons aged ≥60 and to assess causality. METHODS: Using GBD 2021 estimates (1990-2021) for prevalence, Years Lived with Disability(YLDs), and age-standardized rates across 204 countries, we applied decomposition analysis, Social Inequality Index

Effect of Nitisinone on Aortic Stenosis Disease Progression in Patients With Alkaptonuria: An Analysis of the Suitability of Nitisinone in Alkaptonuria (SONIA) 2 Study.

Background and aim Alkaptonuria (AKU) is a rare metabolic disorder characterised by the accumulation of homogentisic acid (HGA). Deposition of HGA in the aortic valve leading to progressive aortic stenosis is a serious complication. Nitisinone has been shown to improve morbidity and slow disease progression in AKU, but the effects of this treatment on aortic stenosis progression have not yet been described. The objective of this study was to evaluate whether treatment with nitisinone attenuated the progression of aortic stenosis, as assessed by peak trans-aortic valve pressure (Pmax), in patie

Endoscopic mitral valve surgery: picture from the real world-sub-analysis from the Mini-Mitral International Registry.

BACKGROUND: In the field of minimally invasive mitral valve surgery (MVS), recent technical and technological advances have made endoscopic approaches increasingly popular. However, enthusiasm for endoscopic cardiac surgery has not translated into routine clinical usage likely due to the perceived complexity of the technique and the lack of robust supporting evidence. This study aims to evaluate operative results, and assess the overall effectiveness of endoscopic approaches in current surgical practice using data from the Mini-Mitral International Registry (MMIR). METHODS: This is a retrospe

Transcatheter Aortic Valve Implantation in Young Patients: Challenges and Perspectives-A Narrative Review.

Although most commonly diagnosed in the elderly population, aortic stenosis can affect younger patients, being the most frequent valvular disease requiring replacement interventions, either through surgical procedures (SAVR = surgical aortic valve replacement) or through transcatheter aortic valve implantation (TAVI). In young patients, aortic stenosis generally occurs due to congenital malformations, such as bicuspid aortic valves (BAVs), or to rheumatic valve disease, both of which present specific anatomical characteristics. There is an upward trend among young patients regarding TAVI, due

Association of SGLT2 Inhibitors with Mortality and Bioprosthesis Valve Failure After TAVR: A Propensity-Matched Cohort Study.

Background: Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have shown cardioprotective effects beyond glucose control. In aortic stenosis, SGLT2 expression is upregulated in myocardium and valve tissue, contributing to inflammation, oxidative stress, thrombogenicity, and calcification. SGLT2 inhibition may counteract these mechanisms, potentially reducing bioprosthetic valve failure after transcatheter aortic valve replacement (TAVR), where the diseased native valve remains in place. Objectives: This study aimed to evaluate whether SGLT2i use is associated with improved clinical outcomes,

TAVR in Rheumatic Aortic Stenosis With Mechanical Mitral Prosthesis: Case Series on Short Annulus-Prosthesis Distance.

Patients with rheumatic aortic stenosis and prior mechanical mitral valve replacement present unique challenges for transcatheter aortic valve replacement, particularly when the aortomitral distance is short. This case series examines 4 such patients, focusing on procedural complexities, imaging strategies, and outcomes. Preprocedural multidetector computed tomography and intraprocedural transesophageal echocardiography were critical in assessing anatomical constraints and optimizing valve deployment. Key considerations included preventing mitral prosthesis interference, mitigating left ventri

Mitral Valve Replacement via Minithoracotomy Versus Conventional Median Sternotomy in Rheumatic Mitral Valve Disease: A Multicenter Retrospective Study.

Background and objectives This study aimed to compare surgical outcomes, early postoperative complications, and midterm recovery in patients with severe rheumatic mitral insufficiency undergoing either minimally invasive cardiac surgery (MICS) or mitral valve replacement via conventional median sternotomy (CMS). While CMS remains the standard approach, MICS has emerged as a less invasive option with potential benefits. However, comparative data in resource-limited settings remain scarce. Methods This multicenter retrospective study included 55 adults with severe rheumatic mitral Insufficiency

Total endoscopic valve surgery with the Ram device in combined mitral and tricuspid valve disease.

Total endoscopic valve surgery is considered safe and efficient, providing multiple advantages and significantly increasing patient satisfaction. However, prolonged surgical times due to high technical complexity may remain. Devices for automated suturing may address this problem. The following case report demonstrates our step-by-step approach for combined total endoscopic mitral valve replacement using the RAM technology and tricuspid valve repair in a patient with severe rheumatic mitral valve stenosis.

Perioperative challenges and management strategies for non-cardiac surgery in patients with rheumatic heart diseases: A narrative review.

Rheumatic heart diseases (RHDs) impose a substantial global burden, primarily affecting individuals under 25 years of age in low- and medium-income countries (LMICs) and poor and marginalized groups in high-income countries.[123] The underlying cause is a group A beta-hemolytic streptococcus, which triggers an immune-mediated attack on the heart and joints. Although acute rheumatic fever (ARF) is treatable, its occurrence and complications remain high in impoverished areas.[4] Variations in social structure contribute to differences in the incidence and progression of the disease, even in affl

Association Between Rheumatoid Arthritis and the Risk of Incident Degenerative Valvular Heart Disease: Evidence From a Prospective Cohort Study.

BACKGROUND: Accumulating evidence indicates that degenerative valvular heart disease (VHD) and rheumatoid arthritis (RA) share overlapping risk factors and intersecting inflammatory processes; however, their interrelationship remains insufficiently explored. METHODS: Among 492 745 UK Biobank participants without VHD at baseline, Cox proportional hazards models were conducted to assess the association between prevalent RA and new-onset degenerative VHD, with sequential adjustments for demographic factors, lifestyle variables, and comorbidities. The end points of degenerative VHD in this study

Evolution and long-term impact of concomitant valvulopathies in patients undergoing transcatheter aortic valve implantation.

BACKGROUND: Multiple valvular heart disease (M-VHD) is a common condition, often involving aortic stenosis (AS) plus a mitral or tricuspid valve disease. We aim to evaluate the evolution and prognostic impact of M-VHD in patients undergoing transcatheter aortic valve implantation (TAVI). METHODS: A retrospective cohort study was conducted on patients who underwent TAVI in a tertiary care center between January 2016 and December 2022. Echocardiography was performed before and after TAVI. The primary endpoint was the composite of all-cause mortality and cardiovascular hospitalizations during fo

Neovascularization of native cardiac valves, and correlation with histopathologic, clinical, and radiologic features.

Native cardiac valves in the setting of chronic injury undergo remodeling that includes fibrous thickening and dystrophic calcification, as well as neovascularization, that result in abnormal valve function. In order to characterize the presence of neovascularization in valves, a retrospective review of 1246 sequentially reviewed native cardiac valves of all types was performed, with correlation with other histopathologic features, and clinical and echocardiographic findings. Neovascularization was present in 55.5% of cases, with the greatest prevalence amongst aortic valves. While microvascul

Agreement between Handheld and Standard Echocardiography for Diagnosis of Latent Rheumatic Heart Disease in Brazilian Schoolchildren from High-Prevalence Settings (Agreement between Screening and Standard Echo for RHD).

Introduction: Handheld echocardiography (echo) is the tool of choice for rheumatic heart disease (RHD) screening. We aimed to assess the agreement between screening and standard echo for latent RHD diagnosis in schoolchildren from an endemic setting. Methods: Over 14 months, 3 nonphysicians used handheld machines and the 2012 WHF Criteria to determine RHD prevalence in consented schoolchildren from Brazilian low-income public schools. Studies were interpreted by telemedicine by 3 experts (Brazil, US). RHD-positive children (borderline/definite) and those with congenital heart disease (CHD) wer
