Pregnancy-associated group B streptococcal endocarditis: a report of two fatal cases.

Group B streptococci commonly colonize parturient women, yet pregnancy-associated endocarditis due to this organism is rare. Most reports of group B streptococcal endocarditis are from the preantibiotic era and occurred in women with rheumatic mitral valve disease. Reported herein are two cases of fatal group B streptococcal endocarditis involving the aortic valve of women with no preexisting heart disease. One had undergone a second-trimester abortion and the other had a normal pregnancy and uncomplicated vaginal delivery.

Surgical pathology of combined aortic stenosis and insufficiency: a study of 213 cases.

The gross surgical pathologic features of the aortic valve were reviewed in 213 patients who had had clinically combined aortic stenosis and insufficiency and aortic valve replacement at our institution during the years 1965, 1970, 1975, and 1980. The three most common causes were postinflammatory disease (69%) and calcification of congenitally bicuspid (19%) and unicommissural (6%) aortic valves. Other causes included infective endocarditis (2%) and congenitally quadricuspid or malformed tricuspid aortic valves (1% each); the cause was indeterminate in 1%. In the postinflammatory and bicuspid

[Diagnosis and estimation of mitral regurgitation by two-dimensional pulsed Doppler echocardiography].

The sensitivity and specificity of detecting and estimating mitral regurgitation were assessed by two-dimensional pulsed Doppler echocardiography (PDE) for 85 patients, aged 17 to 79 years. Mitral regurgitation was clinically diagnosed in 10 patients, and confirmed by angiography in 60 patients. Using real time two-dimensional images and M-mode displays, the sample volumes were taken at 36 sites, including the mitral ostium, the left atrium, and the left ventricular inflow tract. Doppler signals were analyzed by the FFT method and the diagnosis of mitral regurgitation was based on the systolic

[Echocardiographic manifestations of nonrheumatic aortic regurgitation].

Recently, the frequency of nonrheumatic aortic regurgitation (AR) has apparently increased, accompanied by a decrease in frequency of rheumatic fever. The purpose of the present study was to ascertain the echocardiographic features of nonrheumatic AR. We had 24 surgically- or autopsy-proven cases of nonrheumatic AR admitted during a two year period. These were 10 cases of infective endocarditis (IE), five with ventricular septal defect of type I, three with syphilis, and two with prosthetic valve malfunctions, and the remainder five were difficult to diagnose clinically. These five were three

Histopathology of the aortic valve in patients with a previous history of acute rheumatic fever. An analysis of 63 surgical specimens.

In a series of 63 surgical patients with a positive history of rheumatic fever and whose aortic valve was removed either singly or in combination with other valves, the histological examination of the aortic valves showed functional lesions in 22 cases (35%). Organic lesions of inflammatory origin, destruction of the architecture with scarring and presence of hypertrophic vessels, were observed in 19 valves (30%). The other cases were doubtful. This ratio was identical in uni- and plurivalvular involvement. Anatomic aspects and significance of microscopic lesions of the aortic valves were disc

[Work test in acquired heart valve defects].

The physical working capacity to 34 patients with acquired cardiac valvular defects and right catheterization was determined via bicycle ergometer "Zimermann" (GDR). Reverse correlation was established between the physical capacity and pressure 0,93 +/- 0,13 kPa (7,0 +/- 1,0 mm Hg) in right auricle (p less than 0,01), 0,9 +/- 0,27 kPa (6,75 +/- 2,12 mm Hg)--in right auricle (p less than 0,01), 4,18 +/- 1,21 kPa (31,5 +/- 9,02 mm Hg)--in pulmonary artery (p less than 0,05) and 2,66 +/- 0,26 kPa (9,19 +/- 6,76 mm Hg)--in pulmonary capillaries (p less than 0,5). Emergency mitral and aortic valve replacement during acute rheumatic fever in a 5-year-old child.

Emergency combined mitral and aortic valve replacement was performed during an attack of acute rheumatic fever in a 5-year-old boy. He made an uneventful recovery and has remained asymptomatic 1 year after the operation. From this and other authors' clinical experience, we believe that the decision to operate on these patients should be based on the severity of the disability and the hemodynamic disturbances rather than on the activity or inactivity of the rheumatic process.

Felty's and Sjögren's syndromes and hypertrophic obstructive cardiomyopathy (case report).

Our findings refer to a female patient with Felty's and Sjögren's syndromes and hypertrophic obstructive cardiomyopathy. The rheumatoid granuloma found in the left ventricle at autopsy and the absence of other conditions which may be manifested by hypertrophic obstructive cardiomyopathy suggest that the case studied, probably represents an extraarticular manifestation of Felty's and Sjögren's syndromes.

Comparison of the echocardiographic and hemodynamic diagnosis of rheumatic tricuspid stenosis.

Two-dimensional echocardiography has proved to be reliable in the diagnosis of mitral, aortic and pulmonary stenosis. Its role in the diagnosis of rheumatic tricuspid stenosis is still being defined; therefore, the tricuspid valve echograms of 147 patients with rheumatic heart disease were examined. Thirty-eight of these patients also underwent hemodynamic evaluation. Tricuspid stenosis was defined echocardiographically as diastolic anterior leaflet doming, thickening and restricted excursion of the other two tricuspid leaflets and decreased separation of the leaflet tips. Using these criteria

Aortic regurgitation in scleroderma.

Two cases of scleroderma with isolated aortic regurgitation are described. Other known causes of aortic regurgitation were not apparent. None of the patients gave a history of rheumatic fever. It appears likely that aortic regurgitation is related to scleroderma in these two patients. Aortic regurgitation in scleroderma is, however, rare.

[Open-heart surgery in patients over 65 years of age. Lesions encountered and immediate postoperative mortality].

Open heart surgery after 65 is more and more frequent (16% of our surgical cases). Between January 1971 and December 1982, 8 425 open heart operations were performed in this Department, 1 377 of them in patients over 65. Most were cases of aortic valvulitis (620), and calcific aortic stenosis in particular, but also dystrophic aortic insufficiency; 217 patients underwent surgery for mitral valvulitis (rheumatic or dystrophic in origin) and 84 for involvement of more than one valve. Of the 2 440 patients with coronary lesions who underwent surgery, 255 were over 65; beyond this age, a higher ra

Two years experience with Medtronic-Hall prostheses in 75 patients with special reference to its thrombogenicity and hemolysis.

Seventy-five patients received 80 Medtronic-Hall valve prostheses during a 24-month period at the Beilinson Medical Center. The mean age was 50 years and the majority of patients had rheumatic heart disease. All operations were elective. There were 4 early and 2 late deaths (8%). Four patients needed re-operation for paravalvular leaks, 3 of them survived the second operation and one patient with prosthetic valve endocarditis did not. One patient was re-operated on successfully because of a mycotic aneurysm in the ascending aorta. Mild subclinical hemolysis (elevated SLDH and reduced haptoglob

Mitral regurgitation in hypertrophic cardiomyopathy. Non-invasive study by two dimensional Doppler echocardiography.

Mitral regurgitation and its haemodynamic features were investigated non-invasively in cases of hypertrophic cardiomyopathy by means of two dimensional Doppler echocardiography. There were 28 patients, 14 of whom showed systolic anterior motion (SAM) of the mitral echo; the other 14 did not. The following results were obtained. (1) Mitral regurgitation was detected by the Doppler technique in all cases with systolic anterior motion of the mitral echo and in half of those without it. (2) Doppler signals of mitral regurgitation started immediately after the first heart sound. (3) Mitral regurgit

Localized dystrophic amyloidosis of heart valves.

A survey of 152 surgically resected heart valves (90 aortic, 60 mitral, two pulmonic) from a consecutive series of 142 adult patients revealed congophilic, green birefringent, amyloid deposits in 81 valves (59 aortic, 22 mitral) from 77 patients from 41 to 79 years of age. Amyloid was found in 50 (82 per cent) of 61 valves showing degenerative calcific sclerosis and in 31 (53 per cent) of 58 valves showing post-rheumatic sclerosis but in none of 33 valves showing more acute disease processes (myxoid transformation, "stretch lesions," healed bacterial endocarditis). HLA B27-associated rheumatic diseases with severe cardiac bradyarrhythmias. Clinical features and prevalence in 223 men with permanent pacemakers.

A 15-fold increase in the prevalence of ankylosing spondylitis in a group of 223 men with permanent cardiac pacemakers has recently been demonstrated. In this study of the same patient group, the prevalence of other related rheumatic syndromes was investigated by clinical examination and HLA typing. The clinical picture and electrocardiographic features of all patients with HLA B27-associated rheumatic disease (seronegative spondarthritis) were analyzed. Altogether 28 patients, 12.6 percent (95 percent confidence limits: 8.2 to 17.0 percent), fulfilled inclusion criteria for seronegative spond

Chronic or fatal Q-fever infection: a review of 16 patients seen in North-East Scotland (1967-80).

Sixteen patients with serologically proven Q-fever infection are reviewed. Fifteen had significantly raised antibody titres to both phase I and phase II Coxiella burneti antigens, indicating persistent or chronic infection. One patient, a premature infant who died, had raised phase II titres only, but is included together with the mother who had chronic Q-fever and was the presumed source of infection. Chronic Q-fever infection has previously been regarded as virtually synonymous with Q-fever endocarditis, but only seven of the patients in this survey had evidence of valvular endocarditis. In

Coronary embolism in valvular heart disease.

Coronary embolism is considered to be rare but recent evidence suggests that it may be underdiagnosed, and implicated in acute myocardial infarction associated with angiographically normal coronary arteries. Twenty-six patients were studied. In six, coronary embolism was a primary cause of death confirmed at autopsy. In 20 patients, 23 episodes of coronary embolism were diagnosed clinically. The left coronary system was occluded in 65 per cent, transient electrocardiographic changes occurred in 30 per cent, and either no history or an atypical history of acute myocardial infarction occurred in
