Outcomes after tricuspid transcatheter edge-to-edge repair: a systematic review and meta-analysis.

BACKGROUND: Severe tricuspid regurgitation (TR) is associated with high morbidity and mortality. Tricuspid transcatheter edge-to-edge repair (T-TEER) improves TR severity and symptoms, yet survival benefit remains uncertain. This meta-analysis aims to evaluate 1-year outcomes after T-TEER and explore clinical and echocardiographic correlates of prognosis. METHODS: A systematic search of PubMed/MEDLINE, Web of Science, and Scopus was conducted through June 2025. Studies enrolling ≥100 patients undergoing T-TEER and reporting 1-year outcomes were included. Primary endpoints were 1-year all-caus

Mid-term outcomes after open-heart surgery for severe chronic rheumatic heart disease in a low-income country: an observational study with historical controls.

OBJECTIVES: The literature is sparse regarding outcomes after valvular surgery in patients with severe chronic rheumatic heart disease (RHD) in low- and middle-income countries (LMICs). We aimed to evaluate mid-term outcomes in patients with severe chronic RHD who underwent open-heart surgery in Ethiopia, and evaluate the durability of the operated valves during follow-up. METHODS: This study implemented an observational design, with 104 patients screened for cardiac surgery, of whom 52 were excluded. Outcome measures and clinical and echocardiographic data after cardiac surgery were availabl

The value of preoperative risk scores prior to tricuspid valve interventions: The TRI-SCORE and beyond.

Tricuspid regurgitation (TR) is increasingly recognized as a condition associated with poor clinical outcomes, including reduced quality of life, progressive right-sided heart failure, and increased mortality. Its heterogeneous etiologies and systemic consequences, affecting both cardiac and extracardiac organs such as the kidneys and liver, make patient management particularly challenging. Accurate risk stratification and timely intervention are therefore essential. The 2025 European Society of Cardiology and European Association for Cardio-Thoracic Surgery Guidelines acknowledge the multifac

When an Atrial Septal Defect Is No Longer Silent: A Case Report of a Large Symptomatic Defect Presenting in Adulthood.

Atrial septal defects (ASDs) are among the most common forms of congenital heart disease diagnosed in adulthood. Ostium secundum defects represent the majority of cases and may remain clinically silent for decades. Delayed diagnosis can lead to progressive right-sided cardiac chamber dilation, pulmonary hypertension, arrhythmias, and heart failure. The choice between percutaneous and surgical closure depends on anatomical characteristics such as defect size and the adequacy of surrounding septal rims. We report the case of a 49-year-old woman with hypertension, type 2 diabetes mellitus, and hy

Role of Cardiac Magnetic Resonance (CMR) as a Diagnostic and Risk Stratification Tool for a Patient With Arrhythmic Mitral Valve Complex Phenotype and Tricuspid Annular Disjunction (TAD).

Arrhythmogenic mitral valve disease (AMVD) is a newly established entity that carries a high risk of developing ventricular arrhythmia ranging from premature ventricular contraction to sudden cardiac death. Therefore, there is an urgent need for risk prediction and stratification by using different imaging modalities. A 77-year-old patient presented with a syncopal episode. Physical examination was unremarkable. Resting electrocardiogram (ECG) was normal. Echocardiography showed mild systolic dysfunction and mild aortic stenosis. Seven days of Holter monitor recording demonstrated frequent epi

Presentation, Treatment, and Outcomes of Non-bacterial Thrombotic Endocarditis in Pancreatic Cancer: A Systematic Review.

Non-bacterial thrombotic endocarditis (NBTE) is characterized by the formation of sterile vegetations on cardiac valves, typically in the setting of malignancy or systemic inflammatory states. These vegetations can cause valvular dysfunction and have a higher tendency to embolize relative to other vegetations, which may result in life-threatening complications. Some of the highest rates of malignancy-associated NBTE have been seen in pancreatic cancer. A review of the literature on pancreatic cancer-associated NBTE was conducted to better understand patient presentation, disease course/progres

The ELASTA-T Technique for TTVR After Failed T-TEER: An Operator-Oriented Guide to Planning, Execution, Pitfalls.

Failure of tricuspid transcatheter edge-to-edge repair (T-TEER) with persistent severe tricuspid regurgitation (TR) is an increasingly encountered clinical scenario in contemporary structural heart disease treatment. In this setting, transcatheter tricuspid valve replacement (TTVR) is emerging as an effective therapeutic option for recurrent TR after T-TEER. However, the presence of pre-existing tricuspid edge-to-edge devices significantly alters valve geometry, creating a fixed multiple-orifice configuration that may impair coaxial device crossing, generate asymmetrical expansion forces, and

Carcinoid Heart Disease: Surgical Timing, Right Ventricular Risk Stratification and Operative Strategy.

Carcinoid heart disease is a progressive right-sided valvulopathy caused by serotonin and other vasoactive mediators released by metastatic neuroendocrine tumours. As oncological therapies have extended survival, cardiac disease has become a leading determinant of mortality. Operative mortality has decreased to 5-6% in contemporary high-volume centres, and long-term survival appears increasingly determined by tumour biology rather than cardiac disease when surgery is appropriately timed. The principal determinant of operative outcome is preoperative right ventricular function; symptom-based re

Isolated aortic valve-sparing aortic root replacement in elective patients with bicuspid aortic valve: A single-center experience.

OBJECTIVE: The David-I procedure is the standard surgical treatment for patients with aortic root aneurysms and preserved aortic valve morphology. Its application in patients with bicuspid aortic valves presents additional challenges. This study compares long-term outcomes of the elective, isolated David-I procedure in patients with bicuspid aortic valves and tricuspid aortic valves. METHODS: We retrospectively reviewed all elective patients undergoing an isolated David-I procedure between 2004 and 2019. Patients with bicuspid aortic valves or tricuspid aortic valves and isolated aortic root

CE-MRA-FLOWnet: Fast and Accurate Generative AI-Based Aortic Hemodynamic Mapping from Contrast-Enhanced MRA.

BACKGROUND: 4D flow MRI hemodynamic biomarkers (e.g. peak velocity and wall shear stress [WSS]) have shown promise for improved risk-stratification in patients with thoracic aortic disease (TAD). However, lengthy 4D flow scan times, complex data analysis and need for dedicated expertise limit clinical translation. In this study, we developed a fluid-physics informed deep generative adversarial neural network, CE-MRA-FLOWnet, to predict aortic hemodynamics directly from standard-of-care contrast-enhanced magnetic resonance angiography (CE-MRA) images. METHODS: We retrospectively identified 139

Management of massive paravalvular leak (PVL): a case report of transcatheter mitral paravalvular leak closure.

BACKGROUND: Paravalvular leak (PVL), a complication following either surgical or transcatheter prosthetic heart valve replacement, can lead to severe clinical consequences including refractory heart failure and persistent hemolysis. Transcatheter paravalvular leak closure (TPVLC) represents an effective therapeutic option for PVL management; however, clinical experience remains relatively limited, particularly regarding its application in cases of massive paravalvular leaks, where reported cases managed with a single occluder are notably scarce. We present a 69-year-old female with rheumatic h

Valve-in-Valve Implantation for Early Failure of a Transcatheter Tricuspid Valve.

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) is increasingly used for severe tricuspid regurgitation in high-risk patients. Early prosthetic dysfunction may occur, but management strategies remain poorly described. CASE SUMMARY: A 79-year-old woman presented with cardiogenic shock 6 months after Topaz TTVR. Echocardiography and computed tomography showed leaflet thickening involving 2 of 3 leaflets with a mean gradient of 9 mm Hg. Endocarditis was excluded, and anticoagulation optimization failed. Because of persistent catecholamine dependence and prohibitive surgical risk, th

Effectiveness of Rhythm Control of Atrial Fibrillation in Moderate-to-Severe or Severe Secondary Tricuspid Regurgitation.

BACKGROUND: Secondary tricuspid regurgitation (STR) is heterogeneous and associated with increased morbidity and mortality, but there remains limited evidence on the optimal management strategy. Atrial fibrillation (AF) increases the risk of all types of STR. OBJECTIVES: The authors aimed to study the association of successful rhythm control of AF with decreased severity of moderate-severe/severe STR across clinically relevant subgroups and different etiologies of STR. METHODS: Patients with moderate-severe/severe STR, AF, and follow-up echocardiograms were identified retrospectively. STR ty

4D Flow MRI-Based Quantification of Tricuspid Regurgitation: A Possible Approach to Improve Measurement Repeatability.

PURPOSE: To address limitations in tricuspid regurgitation (TR) quantification by magnetic resonance imaging (MRI) or transthoracic echocardiography (TTE), a new method is proposed to directly quantify TR volume (TRV) using 4D Flow MRI with retrospective tricuspid valve (TV) annular tracking. METHODS: Thirty patients with TR were prospectively enrolled. TRV was quantified using four MRI-based methods: (i) indirect, (ii) direct (2D phase-contrast), (iii) 4D Flow with a static TV plane, and (iv) 4D Flow with dynamic annular tracking. Reproducibility and agreement were assessed in terms of TR gr

Rethinking Tricuspid Valve Regurgitation in Heart Failure - A Pathophysiology-based Holistic Approach for Individualized Patient Management.

Tricuspid regurgitation (TR) rarely exists as an isolated condition and is most frequently secondary due to heart failure (HF), or atrial fibrillation, with prevalent extracardiac comorbidities. Morphological assessment of the anatomy and severity of TR, together with evaluation of symptoms, currently drives interventional approaches, yet a more holistic integration of causal and contributing comorbid conditions might further improve outcomes. The present review explores such a pathophysiology-driven holistic approach to interpret functional changes and symptomatic burden in patients with TR.

The Tricuspid Valve Maladapts in a Pulmonary Hypertension Rat Model.

Tricuspid valve regurgitation is a frequent valve lesion and, if severe, an independent predictor of mortality. In most patients, the valve itself has historically been considered intact. Yet, we have previously shown that the valve may not be an innocent bystander. In multiple sheep models, we have shown that the tricuspid valve thickens and stiffens. This remodeling may contribute to valve disease. Our goal is to extend our investigation of tricuspid valve remodeling to a rodent model, potentially opening scientific opportunity and enabling scaling our studies. To this end, we used pulmonary

Radial-Sectional CT for Tricuspid Valve Intervention Planning: A 4-Patient Clinical Case Series.

BACKGROUND: Isolated tricuspid regurgitation (TR) is an independent mortality predictor. Despite improved surgical outcomes, complex tricuspid valve anatomy makes precise repair challenging. We evaluated the utility of computed tomography (CT) with radial-sectional view (RSV) for procedural planning and postoperative assessment. CASE SERIES: Four patients with severe TR underwent surgical repair. Preoperative CT-RSV, generated using standard imaging workstations, provided a comprehensive anatomical map. Radial planes centered on the septal leaflet visualized coaptation gaps with anterior/post

State of the art: Transcatheter tricuspid valve intervention.

Tricuspid regurgitation (TR) has a community prevalence of 3%, which increases with age and affects up to 7% of elderly patients. TR is secondary (functional) in 90% of cases (usually resulting from left heart disease and/or pulmonary hypertension), although atrial dilatation is an increasingly recognised cause. Awareness of the poor prognosis associated with increasing TR severity has refocused the quest for effective treatments. Surgical intervention has been hindered by high in-hospital mortality (up to 10%), even in carefully selected cases, and transcatheter tricuspid valve interventions
