Isolated aortic valve-sparing aortic root replacement in elective patients with bicuspid aortic valve: A single-center experience.

OBJECTIVE: The David-I procedure is the standard surgical treatment for patients with aortic root aneurysms and preserved aortic valve morphology. Its application in patients with bicuspid aortic valves presents additional challenges. This study compares long-term outcomes of the elective, isolated David-I procedure in patients with bicuspid aortic valves and tricuspid aortic valves. METHODS: We retrospectively reviewed all elective patients undergoing an isolated David-I procedure between 2004 and 2019. Patients with bicuspid aortic valves or tricuspid aortic valves and isolated aortic root

CE-MRA-FLOWnet: Fast and Accurate Generative AI-Based Aortic Hemodynamic Mapping from Contrast-Enhanced MRA.

BACKGROUND: 4D flow MRI hemodynamic biomarkers (e.g. peak velocity and wall shear stress [WSS]) have shown promise for improved risk-stratification in patients with thoracic aortic disease (TAD). However, lengthy 4D flow scan times, complex data analysis and need for dedicated expertise limit clinical translation. In this study, we developed a fluid-physics informed deep generative adversarial neural network, CE-MRA-FLOWnet, to predict aortic hemodynamics directly from standard-of-care contrast-enhanced magnetic resonance angiography (CE-MRA) images. METHODS: We retrospectively identified 139

Management of massive paravalvular leak (PVL): a case report of transcatheter mitral paravalvular leak closure.

BACKGROUND: Paravalvular leak (PVL), a complication following either surgical or transcatheter prosthetic heart valve replacement, can lead to severe clinical consequences including refractory heart failure and persistent hemolysis. Transcatheter paravalvular leak closure (TPVLC) represents an effective therapeutic option for PVL management; however, clinical experience remains relatively limited, particularly regarding its application in cases of massive paravalvular leaks, where reported cases managed with a single occluder are notably scarce. We present a 69-year-old female with rheumatic h

Valve-in-Valve Implantation for Early Failure of a Transcatheter Tricuspid Valve.

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) is increasingly used for severe tricuspid regurgitation in high-risk patients. Early prosthetic dysfunction may occur, but management strategies remain poorly described. CASE SUMMARY: A 79-year-old woman presented with cardiogenic shock 6 months after Topaz TTVR. Echocardiography and computed tomography showed leaflet thickening involving 2 of 3 leaflets with a mean gradient of 9 mm Hg. Endocarditis was excluded, and anticoagulation optimization failed. Because of persistent catecholamine dependence and prohibitive surgical risk, th

Effectiveness of Rhythm Control of Atrial Fibrillation in Moderate-to-Severe or Severe Secondary Tricuspid Regurgitation.

BACKGROUND: Secondary tricuspid regurgitation (STR) is heterogeneous and associated with increased morbidity and mortality, but there remains limited evidence on the optimal management strategy. Atrial fibrillation (AF) increases the risk of all types of STR. OBJECTIVES: The authors aimed to study the association of successful rhythm control of AF with decreased severity of moderate-severe/severe STR across clinically relevant subgroups and different etiologies of STR. METHODS: Patients with moderate-severe/severe STR, AF, and follow-up echocardiograms were identified retrospectively. STR ty

4D Flow MRI-Based Quantification of Tricuspid Regurgitation: A Possible Approach to Improve Measurement Repeatability.

PURPOSE: To address limitations in tricuspid regurgitation (TR) quantification by magnetic resonance imaging (MRI) or transthoracic echocardiography (TTE), a new method is proposed to directly quantify TR volume (TRV) using 4D Flow MRI with retrospective tricuspid valve (TV) annular tracking. METHODS: Thirty patients with TR were prospectively enrolled. TRV was quantified using four MRI-based methods: (i) indirect, (ii) direct (2D phase-contrast), (iii) 4D Flow with a static TV plane, and (iv) 4D Flow with dynamic annular tracking. Reproducibility and agreement were assessed in terms of TR gr

Rethinking Tricuspid Valve Regurgitation in Heart Failure - A Pathophysiology-based Holistic Approach for Individualized Patient Management.

Tricuspid regurgitation (TR) rarely exists as an isolated condition and is most frequently secondary due to heart failure (HF), or atrial fibrillation, with prevalent extracardiac comorbidities. Morphological assessment of the anatomy and severity of TR, together with evaluation of symptoms, currently drives interventional approaches, yet a more holistic integration of causal and contributing comorbid conditions might further improve outcomes. The present review explores such a pathophysiology-driven holistic approach to interpret functional changes and symptomatic burden in patients with TR.

The Tricuspid Valve Maladapts in a Pulmonary Hypertension Rat Model.

Tricuspid valve regurgitation is a frequent valve lesion and, if severe, an independent predictor of mortality. In most patients, the valve itself has historically been considered intact. Yet, we have previously shown that the valve may not be an innocent bystander. In multiple sheep models, we have shown that the tricuspid valve thickens and stiffens. This remodeling may contribute to valve disease. Our goal is to extend our investigation of tricuspid valve remodeling to a rodent model, potentially opening scientific opportunity and enabling scaling our studies. To this end, we used pulmonary

Radial-Sectional CT for Tricuspid Valve Intervention Planning: A 4-Patient Clinical Case Series.

BACKGROUND: Isolated tricuspid regurgitation (TR) is an independent mortality predictor. Despite improved surgical outcomes, complex tricuspid valve anatomy makes precise repair challenging. We evaluated the utility of computed tomography (CT) with radial-sectional view (RSV) for procedural planning and postoperative assessment. CASE SERIES: Four patients with severe TR underwent surgical repair. Preoperative CT-RSV, generated using standard imaging workstations, provided a comprehensive anatomical map. Radial planes centered on the septal leaflet visualized coaptation gaps with anterior/post

State of the art: Transcatheter tricuspid valve intervention.

Tricuspid regurgitation (TR) has a community prevalence of 3%, which increases with age and affects up to 7% of elderly patients. TR is secondary (functional) in 90% of cases (usually resulting from left heart disease and/or pulmonary hypertension), although atrial dilatation is an increasingly recognised cause. Awareness of the poor prognosis associated with increasing TR severity has refocused the quest for effective treatments. Surgical intervention has been hindered by high in-hospital mortality (up to 10%), even in carefully selected cases, and transcatheter tricuspid valve interventions

Forgotten but Striking: Complete Tricuspid Valve Degeneration Unmasked by Lancisi's Sign.

Tricuspid regurgitation (TR) is increasingly recognized as an important cause of right-sided heart failure and adverse clinical outcomes. Although severe TR typically presents with a holosystolic murmur, advanced disease may produce atypical findings that obscure the diagnosis. Lancisi's sign, characterized by prominent systolic jugular venous pulsations, remains an important but underrecognized bedside clue to severe TR. We present the case of a 60-year-old man with a remote history of presumed infective endocarditis who presented with progressive dyspnea, anasarca, and peripheral edema. Phys

Ebstein Anomaly: Imaging, Pathophysiology, and Therapeutics.

Ebstein anomaly is a rare congenital malformation defined by failed delamination and apical displacement of the tricuspid valve, which produces an atrialized segment of the right ventricle and variable right ventricular dysfunction. Its clinical expression is wide, ranging from severe neonatal cyanosis and heart failure, which carry high perinatal mortality, to incidental detection in adulthood. Because anatomy and physiology vary widely among patients, imaging is central to diagnosis, risk stratification, and treatment planning. Transthoracic echocardiography remains the first-line study and

Clinical Risk Assessment of Acute Intermediate-Risk Pulmonary Embolism Using Repeat Transthoracic Echocardiography.

BACKGROUND: Current risk stratification algorithms for intermediate-risk pulmonary embolism (PE) inadequately identify patients who may benefit from escalated care interventions (ECIs) beyond anticoagulation and do not incorporate temporal assessments of right ventricular (RV) function. OBJECTIVES: To evaluate whether changes in tricuspid annular plane systolic excursion (TAPSE) on repeat transthoracic echocardiography (TTE) before ECIs are associated with clinical outcomes in acute intermediate-risk PE. METHODS: Acute intermediate-risk PE patients at a single quaternary care center were ret

Four-year clinical and echocardiographic outcomes after tricuspid valve replacement using a novel bovine pericardial valve bioprosthesis.

OBJECTIVE: To describe our experience and evaluate the clinical outcomes and hemodynamic performance of a new bovine pericardial valve bioprosthesis when used off-label for tricuspid valve replacement (TVR). METHODS: We retrospectively reviewed 82 consecutive patients who underwent 83 TVR procedures (1 patient required early reoperation for paravalvular leak) with the new bioprosthesis between July 2021 and April 2025 at our institution. The mean clinical follow-up was 2.3 ± 1.1 years (94% completed), and the mean echocardiographic follow-up was 2.0 ± 1.1 years (75% completed). The primary en

Leadless Pacing for Delayed Heart Block After Transcatheter Tricupid Valve Replacement.

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) is a minimally invasive treatment for severe tricuspid regurgitation. However, atrioventricular block (AVB) requiring permanent pacemaker implantation occurs in 12.0% to 13.3% of patients without a pacemaker. CASE SUMMARY: An 81-year-old man presented 7 days after TTVR with dyspnea and dizziness. Electrocardiography showed alternating AVB, and monitoring indicated a transient episode of complete AVB, suggesting a premonitory sign of delayed conduction disease after EVOQUE implantation. A leadless pacemaker (Micra VR) was implanted t

Percutaneous Iatrogenic Atrial Septal Defect Closure Post Transcatheter Mitral Valve Repair: Revisiting MITHRAS Trial in a Larger Cohort.

Introduction Transcatheter mitral valve repair (TMVR) creates an iatrogenic atrial septal defect (iASD) that may persist and affect hemodynamics. Although the MITHRAS (Closure of Iatrogenic Atrial Septal Defects Following Transcatheter Mitral Valve Repair) trial showed no benefit of routine closure, its small size and selective population limit generalizability, and observational data remain conflicting. We hypothesized that percutaneous iASD closure (PC-iASD) would be associated with differences in in-hospital clinical and hemodynamic outcomes compared with no closure. Methods Using the Natio

Sex-Specific Remodeling Phenotypes of the Tricuspid Valve Leaflets in an Ovine Model of Functional Tricuspid Regurgitation.

BACKGROUND: Moderate to severe tricuspid regurgitation (TR) affects approximately 1.6 million Americans, yet more than 90% of patients with significant TR remain untreated. Women exhibit higher TR prevalence and more rapid disease progression than men, but the valve-intrinsic mechanisms underlying these sex disparities remain unclear. We hypothesized that sex and circulating testosterone influence tricuspid leaflet remodeling during right-sided pressure overload. METHODS: Female, castrated male (C-Male), and non-castrated male (NC-Male) adult Dorset sheep (n = 45) underwent pulmonary artery b

Cardiomegaly With an Absent Tricuspid Septal Leaflet: A Cadaveric Case Report.

The heart functions as a coordinated four-chambered pump, with four valves ensuring unidirectional blood flow. The tricuspid valve, located between the right atrium and right ventricle, consists of anterior, posterior, and septal leaflets that coapt during systole to prevent retrograde flow. Valvular disorders may result in stenosis or regurgitation, thereby increasing cardiac workload and predisposing to chamber dilation and heart failure. Congenital abnormalities, including partial or complete leaflet absence, are rare but can significantly disrupt normal hemodynamics. Reports of absent tric

Intracardiac Migration of an Iliac Venous Stent to the Right Ventricle Causing Tricuspid Regurgitation Managed by Surgical Extraction: A Case Report.

Venous stenting is increasingly used in the management of chronic obstructive venous disease, particularly in severe post-thrombotic syndrome, iliocaval obstruction, and extrinsic iliac vein compression. Although this technique is generally effective and safe, rare but potentially serious complications may occur, including stent migration to the right-sided cardiac chambers. We report the case of a 72-year-old man with a history of deep venous thrombosis treated by left iliac venous stenting approximately 20 years earlier. He presented with a cough and hemoptysis evolving over three months. Ch
