Risk of Sexually Transmitted Infections Among Pregnant Persons With Substance Use Diagnoses.

BACKGROUND: Sexually transmitted infections (STIs) in pregnancy have increased among reproductive-aged women and are associated with adverse maternal and infant outcomes. Identifying high-risk groups for perinatal STIs, including those with differential substance use diagnoses, is essential. This study examines the risk of STIs in pregnancy by specific isolated and co-occurring substance use diagnoses. METHODS: Data stem from a population-based study of pregnant individuals with liveborn, singleton deliveries in California from 2011 to 2021. Birth certificates were linked to maternal and infa

STI incidence and care discontinuation among HIV-PrEPusing black men who have sex with men in New Orleans, LA: a prospective cohort study.

Black men who have sex with men (MSM) have continued to face disproportionately high rates of HIV and STIs, yet few studies specifically address STI risk in Black MSM, particularly those on HIV pre-exposure prophylaxis (PrEP). The aim of this study was to examine STI incidence (gonorrhea, chlamydia, and syphilis) and PrEP care discontinuation in a prospectively enrolled cohort of young Black MSM engaged in PrEP care. We enrolled 63 Black MSM PrEP users; total follow -up time was 81.8 person -years (PY). Mean age was 29.9 (SD 4.3); 45 participants (71%) had insurance, and 30 (48.4%) had an STI

Mycoplasma genitalium prevalence, bacterial load, and antibiotic resistance in paired vaginal and anal specimens from Chlamydia trachomatis-infected women.

BACKGROUND: Mycoplasma genitalium (MG) frequently co-infects women with Chlamydia trachomatis (CT), but the epidemiology and clinical significance of this association remain poorly understood. This study assessed MG positivity, antimicrobial resistance, mgpB genotyping and bacterial load in paired self-collected vaginal and anal swabs from 426 women with urogenital CT infection, and assessed the risk factors for CT/MG coinfection compared with CT monoinfection. METHODS: MG detection and quantification were carried out using an in-house quantitative PCR targeting the MgPa adhesin gene. Macroli

Diagnostic performance of sexually transmitted infection screening in men with sterile leukocyturia detected by flow cytometry.

INTRODUCTION: Sexually transmitted infections (STIs) have increased in recent years, especially amongst young men. However, genitourinary symptoms continue to be initially managed as a urinary tract infection. The aim of this study was to evaluate the diagnostic yield of STI screening in urine samples submitted for culture from men with sterile leukocyturia and to assess the discriminative ability of leukocyturia for detecting STIs. METHODS: A prospective study (June 2019-May 2024) was conducted using midstream urine samples from men aged 15-59 years with leukocyturia ≥20 leukocytes/μL detect

Awareness, willingness to use and use of doxycycline post-exposure prophylaxis among gay, bisexual and other men who have sex with men in China: a national cross-sectional survey.

BACKGROUND: Bacterial sexually transmitted infections (STIs) disproportionately impact gay, bisexual and other men who have sex with men (GBMSM) in China. Doxycycline post-exposure prophylaxis (DoxyPEP) can prevent syphilis and chlamydia among GBMSM; however, it has not yet been officially endorsed in China. This study aims to investigate the perceptions and use of DoxyPEP among GBMSM in a nationwide sample. METHODS: We conducted a nationwide online cross-sectional survey in China between November 2024 and February 2025. Participants were recruited through community-based organizations and Ce

Loss of PmpC in Chlamydia trachomatis leads to altered bacterial development and impaired recovery from IFNγ- and penicillin-induced stress.

Chlamydia trachomatis is an obligate intracellular pathogen that can reversibly halt replication and developmental transitions in response to immunological or pharmacological stress, a phenomenon linked to chlamydial persistence. Although polymorphic membrane proteins (Pmps) are known adhesins, their broader roles in other pathogenesis-relevant processes remain poorly understood. Here, we characterized a pmpC-null mutant generated by insertional mutagenesis and found that while PmpC is not essential for primary growth, it is required for the efficient transition from reticulate bodies to infec

Global, Regional, and National Burden of Female Infertility Attributable to Chlamydia Infections, 1990-2023: An Analysis Based on the Global Burden of Disease Study 2023.

BACKGROUND: Chlamydia infection, the most common bacterial sexually transmitted infection worldwide, frequently leads to tubal infertility. With the rising global prevalence of female infertility, the World Health Organization has highlighted untreated sexually transmitted infections as a preventable cause. Chlamydia has been identified as a leading contributor to infertility attributable to sexually transmitted infections; however, comprehensive and up-to-date estimates specifically focused on chlamydia-attributable female infertility remain limited. This study aimed to quantify the global, r

Acceptability of a Prospective Chlamydia Vaccine in Sexually Active Young Adults: A Descriptive Survey.

BACKGROUND: Chlamydia infection rates are high among young adults (YA, ages 18-24). Fortunately, vaccines are being developed to target this pathogen, but chlamydia vaccine acceptability has remained largely unaddressed. The purpose of this descriptive study was to assess the acceptability of a prospective chlamydia vaccine and perceived barriers to receiving such a vaccine among YA. METHODS: Sexually active YAs in the United States were recruited for an online survey in May 2022. Participants were asked to indicate their likelihood of being vaccinated for chlamydia, receiving the vaccine if

Incident and Recurrent Sexually Transmitted Infections by Gender Identity Among Sex Workers on HIV Pre-Exposure Prophylaxis: A Prospective Cohort Study.

BACKGROUND: Sex workers on HIV pre-exposure prophylaxis (PrEP) experience high bacterial sexually transmitted infection (STI) rates. We assessed associations between gender identity and first/recurrent STI incidence to inform risk-stratified screening. METHODS: This prospective cohort study (Madrid, 2022-2026) evaluated sex workers initiating PrEP. Participants underwent quarterly screening for Neisseria gonorrhoeae, Chlamydia trachomatis, and Treponema pallidum. We used Cox and Prentice-Williams-Peterson models to evaluate first and recurrent STIs, adjusting for confounders. RESULTS: We inc

Current Data to Inform Point-of-Care Testing for Sexually Transmitted Infections in United States Healthcare Settings: A Narrative Review.

Point-of-care testing (POCT) for sexually transmitted infections (STIs) holds promise to improve care across clinical settings in the United States (US). However, multiple barriers to widespread adoption remain, and universal use will not be appropriate. At a minimum, decision-makers require robust, real-world evidence that STI POCTs improve patient outcomes within specific clinical contexts for these tests to be reimbursed and adopted, and data among priority patient groups may help guide targeted utilization. To inform decisions in the US and identify gaps in the evidence base, we reviewed l

Sphingolipids associate with the chlamydial nucleoid and mark developmental transitions in Chlamydia trachomatis.

Chlamydia trachomatis is an obligate intracellular bacterial pathogen and a leading cause of sexually transmitted infections worldwide. During its biphasic developmental cycle, infectious, non-replicative elementary bodies alternate with replicative reticulate bodies within a membrane-bound intracellular niche known as the inclusion. C. trachomatis relies heavily on host-derived metabolites, including sphingolipids, which are essential for inclusion integrity, bacterial growth and production of infectious progeny. Here, using expansion microscopy, we uncover an unexpected localization of sphin

How STI trends track changes in sexual behaviour, online dating and sexual orientation demographics: an analysis of reported bacterial STI incidence and three social behavioural surveys in the USA (2010-2024).

INTRODUCTION: Bacterial sexually transmitted infections (STIs) in the USA rose substantially in the 2010s and incidence remains high, despite reported declines after the COVID-19 pandemic. We aimed to better understand potential changes that may be contributing to these epidemic trends, particularly new behavioural and demographic patterns in the online dating era since ~2010. METHODS: We used joinpoint models to examine trends in reported incidence for three notifiable bacterial STIs (ie, chlamydia, gonorrhoea and syphilis) in the USA during 2010-2024, and analysed concurrent changes in sexu

A Rare Case of Community-Acquired Acinetobacter Urethritis in an Immunocompetent Adult Woman.

Urethritis is most commonly caused by sexually transmitted pathogens; however, refractory presentations may indicate an atypical etiology that does not respond to empiric therapy. Acinetobacter species are Gram-negative bacilli classically associated with nosocomial infections and are an uncommon cause of community-acquired lower urinary tract infections in immunocompetent women. We present a case of severe community-acquired urethritis caused by Acinetobacter baumannii complex in a previously healthy 44-year-old woman, with concurrent isolation of Acinetobacter lwoffii of uncertain pathogenic

FIRST-VOID URINE VERSUS URETHRAL SWAB FOR MOLECULAR DETECTION OF SEXUALLY TRANSMITTED INFECTIONS IN MEN WITH MULTIPLE SEXUAL PARTNERS.

BACKGROUND: Sexually transmitted infections remain a major clinical and global public health burden. Nucleic acid amplification tests are recommended for the molecular detection of urogenital sexually transmitted infections. Although physician-collected urethral swabs have traditionally been used for STI testing in men, swab collection is invasive and may cause substantial discomfort. First-void urine is a promising non-invasive alternative that can support molecular STI testing and screening in men. AIM OF STUDY: To compare first-void urine with physician-collected urethral swab specimens fo
