Limit of Blank Variability as a Determinant of Analytical Sensitivity in a DNA-Binding Dye-Based Digital Polymerase Chain Reaction for the Detection of Chlamydia pneumoniae.

Digital PCR is considered a highly sensitive technique for detecting low-abundance DNA in samples containing excess background DNA. This is particularly relevant for Chlamydia pneumoniae infections, as this obligatory intracellular bacterium can be disseminated by phagocytes from the respiratory tract throughout the whole body. Although persistent bacteria are associated with chronic inflammatory diseases, currently, no diagnostic tools for persistent C. pneumoniae infections exist. In this study, an EvaGreen-based digital PCR assay targeting the chlamydial 16S rRNA gene was initially develope

Neovaginal Cytologic and Molecular Screening Findings for Human Papillomavirus and Sexually Transmitted Infections Among Transgender Women.

Evidence regarding optimal screening strategies for neovaginal health in transgender women remains limited. Data describing cytologic and molecular detection of human papillomavirus (HPV) and sexually transmitted infections (STIs) in neovaginal tissue are scarce, particularly in populations with high burden of HIV and STI. We conducted a descriptive observational study of transgender women with neovagina receiving care at a specialized sexual health clinic in Mexico City. Neovaginal samples were collected for cytologic evaluation and polymersase chain reaction (PCR)-based molecular testing for

Can we predict reproductive complications after Chlamydia trachomatis infection? A cohort study integrating demographic, behavioural, host genetic and infection-related factors.

OBJECTIVES: Chlamydia trachomatis (chlamydia) infection has a variable course; most infections resolve but some lead to reproductive complications. Behavioural, immunological and host genetic factors likely influence risk. We aimed to identify predictors of complications in persons with and without chlamydia and develop prediction models for risk stratification. METHODS: We analysed data from a long-term cohort of Dutch individuals registered as female in municipal registries and previously screened for chlamydia, including demographic, behavioural, chlamydia-infection and serology variables,

Quantitative multiplex detection of 9 feline respiratory pathogens with enhanced sensitivity: a cost-effective integrated platform combining RT-PCR/PCR and capillary electrophoresis.

Pathogen infections and coinfections are critical determinants of feline upper respiratory tract disease; although the etiologic agents are diverse, their clinical manifestations often overlap. Established detection panels for feline upper respiratory tract infections (FURTIs) primarily focus on core pathogens-feline calicivirus (FCV), felid alphaherpesvirus 1 (FeAHV1), Mycoplasmopsis felis (MF), Chlamydia felis (CF), Bordetella bronchiseptica (BB), and influenza A virus (IAV)-and frequently do not incorporate emerging zoonotic agents (e.g., SARS-CoV-2) and overlooked streptococcal species (St

Respiratory pathogen dynamics in hospitalized children with lower respiratory tract infections: A 9-year hospital-based surveillance study in Beijing, China.

BACKGROUND: Lower respiratory tract infections (LRTIs) are a major cause of pediatric hospitalization. However, long-term, region-specific pediatric inpatient data remain limited on whether pathogen-specific seasonal patterns have returned to their baselines before nonpharmaceutical interventions (NPIs) or remain reconfigured. METHODS: Using routinely collected, deduplicated PCR results from a hospital-based multi-pathogen surveillance system, we conducted a retrospective analysis of children hospitalized with LRTIs from January 2017 to December 2025. Pathogen-specific detection rates were ca

Case Report: A cluster of cases with rash associated with Chlamydia pneumoniae infection.

We report a case of clustered rashes among teenagers associated by Chlamydia pneumoniae (CP) infection. Most patients only presented with rashes without significant fever or other respiratory tract symptoms. Metagenomic next-generation sequencing (mNGS) of blood samples identified the culprit - the nucleic acid sequence of CP was detected in the peripheral blood of the index case. The results of targeted next-generation sequencing (t-NGS) of throat swabs further supported the presence of CP. After antibiotic treatment, the patients' conditions improved. In this case, we should have a systemati

Community behavioural resilience and STIs: Predictors of pandemic preparedness in a low-resource setting in Cameroon.

BACKGROUND: Sexually transmitted infections (STIs) infect 374 million people annually, with sub-Saharan Africa bearing the highest burden. Community behavioural resilience remains poorly understood in low-income settings, creating a critical gap in STI pandemic preparedness. This study identifies predictors of willingness to adopt protective behaviours during a hypothetical STI outbreak in rural Cameroon. METHODS: We conducted a community-based survey in 2022 among residents of three riverine health areas in the Bafia Health District, Cameroon. Participants (N=372; 202 females [54.3%], 170 ma

A nationwide study of pathogens in hospitalized patients with lower respiratory tract infections by metagenomic next-generation sequencing.

Lower respiratory tract infections remain a major cause of morbidity and mortality, yet timely etiologic diagnosis is difficult because of the breadth of possible pathogens and the limitations of conventional microbiological tests. We retrospectively analyzed 30,819 clinical specimens from 27,507 hospitalized patients with lower respiratory tract infections across 31 provincial-level regions in mainland China from April 2019 to May 2025 to characterize the pathogen spectrum detected by metagenomic sequencing. In total, 323 pathogens were identified, 33.74% of which were fastidious or uncultura

From Chlamydia to Gonococcus: Emerging Trends of Infectious Conjunctivitis and Keratoconjunctivitis in a Tertiary Ophthalmology Center in Switzerland from 2005-2025.

Gonococcal and chlamydial conjunctivitis and keratoconjunctivitis are potentially sight-threatening ocular manifestations of sexually transmitted infections. Recent epidemiological data indicate rising rates of gonorrhoea and chlamydia infections worldwide, raising concerns about a possible increase in associated ocular infections. This retrospective single-center study analyzed the frequency and temporal trends of laboratory-confirmed gonococcal and chlamydial ocular infections in adults treated at the University Hospital Zurich between January 2005 and July 2025. Clinical characteristics, tr

Clinical significance of Chlamydia trachomatis and HPV coinfection across cervical lesion severity: a 10-year retrospective study in Southern China.

BACKGROUND: Persistent high-risk HPV infection is the major cause of cervical carcinogenesis, but the clinical relevance of Chlamydia trachomatis (CT) coinfection remains uncertain. We examined the epidemiological patterns of CT-HPV coinfection and their associations with cervical lesion severity among women in Southern China. METHODS: This retrospective study included 21,275 outpatients who underwent concurrent HPV genotyping, CT PCR testing, and cytological or histopathological evaluation between 2015 and 2024. Age-specific patterns and temporal trends were assessed. Age-adjusted logistic r

Trachoma prevalence surveys in 15 indigenous and non-indigenous evaluation units in Brazil, 2018-2023.

BACKGROUND: To provide the groundwork for a future declaration of elimination of trachoma as a public health problem in Brazil, we conducted house-to-house surveys following WHO methodological guidance. METHODS: An observational cross-sectional study was conducted in 10 non-indigenous and five indigenous evaluation units (EUs) from 2018 to 2023; data on six EUs are reported here for the first time. Two-stage cluster sampling was used: 30 clusters per EU, and 30 households per cluster. We estimated the prevalence of trachomatous inflammation-follicular (TF) in 1-9-y-olds and trachomatous trich

Does persistent active trachoma mandate antibiotic mass drug administration? A comparison of prevalence of trachomatous inflammation-follicular with that of conjunctival infection and anti-chlamydial antibodies, Western Province, Zambia.

BACKGROUND: The evaluation unit comprising Kaoma, Luampa, and Nkeyema districts, Western Province, Zambia, has persistent active trachoma. In 2023, we sought to compare the evaluation unit-level prevalence of the active trachoma sign, trachomatous inflammation-follicular (TF), to that of conjunctival Chlamydia trachomatis (Ct) infection and anti-Chlamydia trachomatis (Ct) seropositivity. METHODS: We conducted a cluster-sampled cross-sectional survey. In selected households, we examined all consenting residents ≥1 y of age for trachoma. We collected dried blood spots (DBSs) by finger-prick fro

Developing strategies to address persistent and recrudescent trachoma in Ethiopia using the Delphi techniques.

BACKGROUND: Despite significant progress, trachoma persistence and recrudescence in Ethiopia pose a challenge to global elimination of trachoma as a public health problem. Developing evidence-based, expert-informed strategies is crucial for achieving global elimination targets by 2030. Our aim was to develop strategies to address persistent and recrudescent trachoma using Delphi techniques. METHODS: The study reviewed literature and studies from districts in southern Ethiopia where trachoma prevalence remains above the elimination threshold. A modified Delphi technique was employed to achieve

A Randomised Trial of Three Face-Washing Methods for the Removal of Chlamydia trachomatis From the Faces of Children With Severe Active Trachoma.

OBJECTIVES: A three-arm, open, parallel-group randomised trial compared three face-washing methods for cleaning Chlamydia trachomatis from the faces of children with severe active trachoma. The impact of face-washing on Chlamydia trachomatis on the hands of children and their caregivers and Chlamydia trachomatis duration on faces and hands were investigated as secondary objectives. METHODS: Children aged 1-7 years in Oromia, Ethiopia, were screened for active trachoma. We aimed to recruit 470 children with severe conjunctival inflammation; 141 were expected to have concurrent conjunctival inf

Evaluation of fluorometholone as adjunctive medical therapy for trachomatous trichiasis surgery (FLAME): a parallel, double-blind, randomised controlled field trial in the Jimma Zone, Ethiopia.

BACKGROUND: In trachoma, trachomatous trichiasis mediates visual impairment. Trachomatous trichiasis surgery has an unacceptably high relapse incidence. We hypothesised that anti-inflammatory therapy with fluorometholone 0·1% suspension (hereafter fluorometholone) eyedrops perioperatively twice daily for 28 days would safely, efficaciously, and cost-effectively reduce postoperative trachomatous trichiasis relapse (PTT). METHODS: In this randomised, parallel, placebo-controlled trial, we compared placebo (artificial tears) with fluorometholone for eyes undergoing trachomatous trichiasis surger

The Effect of a Water, Sanitation, and Hygiene Intervention on Clinical Trachoma: A Cluster-Randomised Trial.

OBJECTIVES: Few randomised trials have been done to assess the impact of hygiene interventions on trachoma even though hygiene improvements feature prominently in the strategy for global elimination of trachoma. This study's aim was to determine if implementing an integrated water, sanitation, and hygiene (WASH) intervention in a hyperendemic region of Ethiopia would reduce clinical signs of trachoma. METHODS: In a parallel-group cluster-randomised trial, 40 Ethiopian communities were randomised in a 1:1 ratio to a WASH intervention or to delayed intervention (i.e., control) (ClinicalTrials.g

Age-Specific Prevalence of Trachoma in Remote Australian Communities.

PURPOSE: Trachoma is endemic in Australia, affecting remote Indigenous communities in northern, central and western Australia. The World Health Organization (WHO) defines a key requirement of elimination as a public health problem being the prevalence of active trachoma (characterised as trachomatis inflammation - follicular) below 5% in children aged 1-9 years. In Australia, screening is based in school settings and focuses on children aged 5-9 years. There is international evidence that active trachoma may be more common in children under five, meaning true Australian prevalence may be under

The Cost of Annual and More Frequent Than Annual Mass Drug Administration for Trachoma in Two Districts in Amhara, Ethiopia.

Ethiopia accounts for 59% of the global trachoma burden. To eliminate trachoma as a public health problem by 2030, modified mass drug administration (MDA) strategies have been proposed, including more frequent than annual MDA. In the present study, the cost of the "child MDA" (CMDA) strategy, defined as an initial community-wide MDA treatment followed by another treatment targeting children aged 6 months to 9 years, was estimated in the Lasta and Wadilla districts, Amhara, Ethiopia. A micro-costing analysis was conducted from a payer perspective, documenting the total financial and economic co
