Operative Details and Clinical Outcomes of Chordal Preservation in Mitral Valve Replacement Surgery.

BACKGROUND: Chordal preservation in mitral valve replacement (MVR) has been historically associated with favorable postoperative left ventricular remodeling. We evaluated the outcomes of chordal preservation in a contemporary MVR cohort. METHODS: Consecutive MVR patients at a single institution (2015-2024) were divided into chordal preservation and non-preservation groups. Baseline differences were balanced by inverse probability of treatment weighting. Primary outcome was survival. Secondary outcomes included change in left ventricular ejection fraction and end-diastolic diameter between pre

Improved Visualization of Heart Valves With 4D-CT Using a Novel Motion Correction Algorithm.

OBJECTIVE: Motion artifacts remain a limitation of cardiac CT when assessing both native and prosthetic heart valves, particularly during phases of rapid motion. This may lead to reduced image quality, potentially impairing the evaluation of valvular movement and pathology. The purpose of this study was to evaluate whether a novel reconstruction-based motion correction algorithm improves image quality and reduces motion artifacts of native and prosthetic heart valves compared with standard reconstructions in photon-counting detector CT (PCD-CT). MATERIALS AND METHODS: In this retrospective si

Prognostic value of pulmonary artery pulsatility index for perioperative adverse cardiovascular events in patients undergoing surgery for mitral regurgitation.

BACKGROUND: The pulmonary artery pulsatility index (PAPi) demonstrates prognostic value across cardiopulmonary conditions, yet its utility in mitral regurgitation (MR) perioperative risk stratification remains undefined. This study aimed to evaluate whether the PAPi could independently predict perioperative adverse cardiovascular events in patients with MR, and to explore whether its addition to the European System for Cardiac Operative Risk Evaluation (EuroSCORE) II provided incremental value. METHODS: This ambidirectional cohort study enrolled 302 consecutive patients with moderate-to-sever

Multi-valvular burden score and one-year mortality in ICU patients undergoing echocardiography: an exploratory cohort study.

BACKGROUND: Multiple regurgitant lesions coexist in critically ill patients, yet cohort-level analyses typically represent each valve separately. We evaluated whether a Multi-Valvular Burden (MVB) score could summarize regurgitant involvement and whether its prognostic information differed from models retaining individual valve grades. METHODS: This retrospective cohort used MIMIC-IV v3.1 linked with MIMIC-IV-ECHO. Adults with an ICU admission and complete ordinal grading of tricuspid, mitral, and aortic regurgitation on an index echocardiogram within 72 h of ICU admission were included. MVB

Acute Rheumatic Fever: Atrial Flutter Associated With Carditis.

BACKGROUND: Acute rheumatic fever (ARF) is an immune-mediated sequela of group A streptococcal infection that can progress to rheumatic heart disease (RHD), defined by permanent rheumatic valvular damage. Although uncommon in the United States, delayed recognition may lead to advanced valve disease and arrhythmias. CASE SUMMARY: A 10-year-old African-American male presented after syncope following 4 months of unexplained bilateral ankle arthritis. He was found to be in atrial flutter. Echocardiography showed a flail mitral valve with severe regurgitation, moderate tricuspid regurgitation, and

Transcatheter Edge-to-Edge Repair in Adult Congenital Heart Disease: Anatomic Challenges, Device Adaptations, and Emerging Evidence.

Atrioventricular valvular regurgitation (AVVR) is a principal determinant of morbidity and mortality in adults with congenital heart disease (ACHD), and surgical management carries substantial operative risk owing to prior sternotomies, distorted intracardiac anatomy, impaired ventricular function, and lesion-specific hemodynamic fragility. Transcatheter edge-to-edge repair (TEER)-using MitraClip or TriClip (Abbott) or PASCAL/PASCAL ACE (Edwards Lifesciences)-has emerged as a compelling alternative in high-surgical-risk acquired valvular disease and is rapidly expanding into ACHD. We review th

Sex-specific differences in right heart remodelling and patient outcomes in secondary tricuspid regurgitation.

AIMS: Current guidelines lack sex-specific thresholds for assessing secondary tricuspid regurgitation (STR) severity and right ventricular (RV) and tricuspid annulus (TA) remodelling. We aimed to determine whether risk-based cut-offs for these parameters differ between men and women with STR. METHODS AND RESULTS: We included 554 patients (74 ± 13 years, 51% women) with moderate or severe STR. The primary endpoint was all-cause mortality or heart failure hospitalization. Women were older (P < 0.001) and had a higher prevalence of atrial fibrillation (P = 0.008) and atrial STR (P < 0.001), wher

Real-world Objects Scaffold Visual Working Memory for Features: Increased Neural Engagement When Colors Are Remembered as Part of Meaningful Objects.

Visual working memory is a core cognitive function that allows active storage of task-relevant visual information. Contrary to the common assumption that the capacity of this system is fixed with respect to a single feature dimension, recent research has shown that working memory performance for a simple visual feature-color-is improved when this feature is encoded as part of a real-world object relative to an unrecognizable scrambled object. Using EEG (n = 24), we here demonstrate that this performance benefit is supported by increased neural engagement during the retention period, as indexed

The importance of sex differences in valvular heart disease from an imaging point of view.

Sex differences in valvular heart disease (VHD) represent an emerging focus in cardiovascular imaging, with implications spanning aetiology, pathophysiology, chamber remodelling, and prognosis. This review aims to illustrate how multimodality imaging can be applied to address sex-specific differences in VHD, with the goal of improving disease grading, staging of extra-valvular cardiac damage, and risk stratification across the whole VHD spectrum.

Pitfalls and solutions in the imaging follow-up of valvular heart disease devices.

Doppler-echocardiography is the primary imaging modality for the evaluation and follow-up of valvular heart devices. This article highlights the unique pitfalls associated with these devices, including imaging challenges and flow dynamics, which often push echocardiography to its limits. The use of a multi-modality imaging approach including cardiac computed tomography (CT) and magnetic resonance, and positron emission tomography-CT is recommended to confirm the diagnosis of prosthetic valve dysfunction (PVD), quantify its severity and determine its mechanism and aetiology. Thus, the distincti

Randomized controlled trials in valvular heart disease: the evolving role of multimodality imaging.

Valvular heart disease represents a significant global health burden, with an estimated prevalence of 2.5% in high-income countries and projected increases due to population ageing. Randomized controlled trials in valvular heart disease have undergone substantial evolution, shifting from mortality-focused endpoints toward comprehensive assessments integrating imaging parameters and patient-centered outcomes. Cardiovascular imaging modalities, including echocardiography, cardiac computed tomography, and cardiac magnetic resonance, have become pivotal in trial design, patient selection, and endp

The Silent Cost of Gender in Mitral Valve Surgery: A Propensity-Score Matched Analysis.

OBJECTIVES: Mitral valve (MV) disease shows sex-specific differences in morphology and outcomes. Women often present later, undergo replacement more frequently, and experience worse survival. This study investigated sex-related disparities in surgical approach, repair rates, and outcomes of MV surgery. METHODS: A total of 1531 consecutive patients undergoing MV surgery with or without concomitant tricuspid valve procedure were analysed retrospectively. Baseline characteristics, operative strategies, and outcomes were compared between sexes. Propensity score matching was used to adjust for bas

ACC/AHA vs ESC/EACT Guidelines on valvular heart disease: EJPC Guideline comparison review.

Valvular heart disease poses persistent challenges for clinical decision-making, and international guidelines provide essential frameworks for its management. The American College of Cardiology/American Heart Association last issued a full update in 2020 and an expert consensus on tricuspid regurgitation in 2025. The European Society of Cardiology/European Association for Cardio-Thoracic Surgery released their updated guidelines in 2025. Although there is broad alignment in principles of intervention, prosthesis choice, and the role of Heart Teams, the European document incorporates substantia

Multivalve Involvement in Aortic Stenosis: Insights From a Narrative Review.

Many patients with severe aortic stenosis present with concomitant multivalvular heart disease. The management of this condition remains challenging and requires a multidisciplinary approach that integrates clinical, hemodynamic, and multimodality imaging data to define the most effective and durable treatment strategy. Although randomized evidence to guide treatment decisions in this setting is limited, percutaneous interventions to address additional valvular lesions are being increasingly adopted. This review provides an overview of the pathophysiology of valvular diseases commonly associat

Right Ventricular-Pulmonary Artery Coupling and Clinical Outcomes Following Mitral Transcatheter Edge-to-Edge Repair: A Systematic Review and Meta-Analysis.

BACKGROUND: Right ventricular-pulmonary artery (RVPA) coupling is an emerging prognostic marker in cardiovascular disease, but its predictive value in patients undergoing mitral transcatheter edge-to-edge repair (MTEER) remains uncertain. METHODS: We conducted a systematic review and meta-analysis following PRISMA 2020 guidelines. PubMed, Scopus, and Web of Science were searched for studies evaluating the prognostic impact of RVPA coupling in patients undergoing MTEER, using tricuspid annular plane systolic excursion (TAPSE)/pulmonary artery systolic pressure (PASP) or related markers. The Influence of Left Ventricular Ejection Fraction on Long-Term Survival of Patients after Transcatheter Edge-to-Edge Mitral Valve Repair.

INTRODUCTION: Transcatheter edge-to-edge repair (M-TEER) has emerged as an effective treatment for high-risk patients suffering from mitral regurgitation (MR), alleviating symptoms and improving outcomes. However, the prognostic relevance of left ventricular ejection fraction (LVEF) in this population remains unclear. METHODS: We analyzed data from 821 patients undergoing M-TEER at four German tertiary care centers between 2011 and 2022. Patients were stratified into heart failure subgroups based on LVEF: heart failure with preserved ejection fraction (HFpEF, LVEF ≥50%), mildly reduced ejecti
