High diagnostic yield of family echocardiographic screening of children with bicuspid aortic valve.

AIMS: Bicuspid aortic valve (BAV) is present in 0.8% of newborns, with associated aortopathy in 33%. While the prevalence of BAV and associated aortopathy in first-degree relatives of symptomatic BAV patients ranges from 5 to 10% and 5 to 32%, data from unselected probands are limited. Uniform family screening guidelines are lacking. We aimed to determine the prevalence of BAV and aortopathy in first-degree relatives of children diagnosed neonatally with BAV. METHODS AND RESULTS: In this case-control family study based on the Copenhagen Baby Heart Study (n > 25 000 newborns enrolled 2016-18),

One-year outcomes of a novel balloon-expandable valve for severe aortic stenosis.

OBJECTIVE: This study evaluated the safety and efficacy of the Renatus transcatheter aortic valve system in treating severe aortic stenosis. METHODS: This prospective, multicenter, single-arm clinical trial employed a target-value approach. The primary endpoint was the all-cause mortality rate at one year postoperatively, while secondary endpoints included assessments of safety and effectiveness. Safety was measured by the all-cause mortality rate at 30 days, the incidence of major adverse cardiovascular events within one year, and the device success rate. Effectiveness was assessed using tran

Current Challenges in Cardiac Device Management: Guidance for Practicing Clinicians: A Canadian Journal of Cardiology White Paper.

Cardiac implantable electronic devices (CIED) are medical devices that play a significant role in the management of heart disease by regulating cardiac rate and rhythm. CIEDs encompass pacemakers, implantable cardioverter defibrillators, cardiac resynchronisation devices (CRT), and implantable cardiac monitors. While robust evidence supports the use of these devices, there are several areas where technical developments have outpaced robust clinical trials, including: the role of primary prevention implantable cardioverter-defibrillators in the era of contemporary heart failure (HF) therapy, ri

Case Report: Persistent delusional memories after postoperative delirium in a woman with complex cardiac surgical history.

INTRODUCTION: Delirium is common after cardiac surgery and is associated with long-term cognitive and psychological morbidity. Many patients later recall vivid, often frightening delusional experiences, but sustained delusional conviction in such memories long after delirium resolution is rarely reported. CASE DESCRIPTION: We describe a woman in her late 60s with congenital aortic valve disease, three previous aortic valve operations and a recent tricuspid valve repair via right mini-thoracotomy. Her postoperative course was severely complicated by hemothorax, repeated thoracotomies, respirat

TAVR in Bicuspid Aortic Valve Disease: A Meta-Analysis of Surgical Comparison and Anatomic Predictors.

BACKGROUND: Despite the proven efficacy of transcatheter aortic valve replacement (TAVR) in treating tricuspid aortic valve stenosis, the bicuspid aortic valve (BAV) population has been excluded from most of the landmark trials. AIM: This study aimed to assess the outcomes of TAVR compared with those of surgical aortic valve replacement (SAVR) and examine the impact of BAV type, aortopathy, excess leaflet calcification, and raphe calcification on BAV TAVR outcomes. METHOD: We searched PubMed/MEDLINE, Embase, and Cochrane Library for studies that assessed the outcomes of TAVR in the BAV popul

Differential roles of calcification and fibrosis in aortic stenosis: a sex- and morphology-specific CT analysis.

AIMS: Aortic stenosis (AS) is characterized by fibrotic and calcific remodelling of the valve, but their sex- and morphology-specific contributions to disease severity remain incompletely characterized. METHODS AND RESULTS: This cross-sectional analysis enrolled 448 AS patients with preserved flow (left ventricular ejection fraction ≥50%, stroke volume index >35 mL/m²) who underwent comprehensive echocardiography and cardiac CT, severity was classified by both peak aortic jet velocity and indexed aortic valve area (AVA), and stratified by sex and valve morphology [bicuspid (BAV) or tricuspid

Right heart function matters: prognostic value of sPAP, TAPSE and RV-PA coupling in a real-world cohort of TAVI patients.

Right ventricular (RV) function is an established prognostic factor in structural heart disease, but is not routinely incorporated into pre-procedural risk assessment in patients undergoing transcatheter aortic valve implantation (TAVI). This study evaluated the prognostic impact of systolic pulmonary artery pressure (sPAP), tricuspid annular plane systolic excursion (TAPSE), and RV–pulmonary artery (PA) coupling (TAPSE/sPAP ratio) in a real-world TAVI population. This retrospective single-center study included 565 consecutive patients (mean age 82.1 ± 5.1 years; 48.7% male) undergoing transfe

Cardiac damage stages and prognosis in patients with bicuspid versus tricuspid aortic valve: insights from the CURRENT AS Registry-2.

Cardiac damage staging is a validated predictor of outcomes in tricuspid aortic valve aortic stenosis (TAV-AS), but its role in bicuspid aortic valve aortic stenosis (BAV-AS) remains unclear. We compared baseline characteristics, cardiac damage stages, and clinical outcomes between BAV-AS and TAV-AS patients. We analyzed 3365 patients with severe aortic stenosis (AS) from the CURRENT AS Registry-2, including 222 with BAV-AS and 3143 with TAV-AS. Cardiac damage stages were classified based on index echocardiographic findings. The primary outcome was a composite of death or heart failure hospita

Valve-Dependent Regional Heterogeneity of Wall Mechanics and Collagen Remodeling in Ascending Thoracic Aortic Aneurysms.

Ascending thoracic aortic aneurysm (ATAA) pathogenesis varies with aortic valve morphology, yet regional heterogeneity between inner curvature (IC) and outer curvature (OC) remains incompletely characterized. We hypothesized that regional differences between the outer and inner curvature of the ascending aorta are valve-morphology dependent and reflect distinct remodeling mechanisms in bicuspid versus tricuspid aortic valve-associated aortopathy. Ascending aortic tissue from 155 patients (69 tricuspid aortic valve [TAV], 68 bicuspid aortic valve [BAV], 18 non-aneurysmal heart transplantation [

Growth rate of the ascending aorta and risk of aortic dissection or reintervention in patients with bicuspid or tricuspid aortic valves undergoing valve interventions: A systematic review and meta-analysis.

BACKGROUND: Bicuspid aortic valve (BAV) is associated with premature valve dysfunction and progressive aortic dilation, potentially leading to dissection. The evolution of aortic dimensions after valve replacement remains uncertain, particularly compared with tricuspid aortic valve (TAV) disease. This systematic review and meta-analysis aimed to compare postoperative aortic growth and the risk of major aortic events between BAV and TAV patients. METHODS: A systematic search of MEDLINE, CENTRAL, and Web of Science (from inception to April 2025) identified prospective and retrospective cohort s

Transcatheter Aortic Valve Replacement for Severe Bicuspid Aortic Valve Stenosis.

Surgical aortic valve (AV) replacement has been the standard treatment for patients with severe symptomatic degenerative AV stenosis (AS). In recent years, transcatheter AV replacement has emerged as an established alternative in selected patient populations, supported by robust evidence in tricuspid AS. However, there has been limited evaluation of transcatheter AV replacement in bicuspid AV AS. Recently, several observational studies have demonstrated the feasibility and safety of transcatheter AV replacement in bicuspid AV AS, and the use of newer-generation devices has shown encouraging ou

Aortic Valve Phenotype Shapes Helical Flow in the Ascending Aorta: Insights from 4D Flow MRI.

PURPOSE: Helical flow in the ascending aorta (AAo) is recognized as beneficial to cardiovascular physiology. Previous in vivo studies of bicuspid aortic valve (BAV) disease have mainly relied on qualitative assessments or surrogate measures of helical flow, hampering its use as potential hemodynamic biomarker. Here, we leveraged a rigorous fluid mechanical framework for helical flow structures to (i) determine how aortic valve (AV) phenotype-tricuspid (TAV) versus bicuspid-influences AAo helical flow and (ii) identify its principal anatomical and hemodynamic determinants. METHODS: 4D flow MRI

Valvulo-Arterial Impedance in Patients with Severe Aortic Stenosis and Bicuspid Aortic Valve.

BACKGROUND: Risk stratification in patients with bicuspid aortic valve (BAV) and severe aortic stenosis (AS) remains challenging. Valvulo-arterial impedance (Zva), an integrated marker of global left ventricular (LV) afterload, has shown prognostic value in tricuspid AS; however, data in BAV are limited. This study aimed to evaluate the association of Zva with LV remodeling, symptoms, and all-cause death in patients with BAV and severe AS. METHODS: In this retrospective, two-center cohort study, 147 patients with severe AS and BAV were included. Zva was calculated at the time of the first ech

Age-Dependent Outcomes of Reductive Ascending Aortoplasty.

Background and Objectives: The optimal management of dilated ascending aorta during aortic valve replacement (AVR) in older or high-risk patients remains debated. While graft replacement is the standard procedure, reductive ascending aortoplasty (RAA) may offer a less invasive, tissue-preserving alternative. This study evaluated long-term RAA outcomes and identified the optimal patient profile. Materials and Methods: In a single-center cohort, 64 patients underwent AVR with unwrapped RAA (2005-2025). Patients were stratified by valve phenotype (tricuspid [TAV], n = 45; bicuspid [BAV], n = 19)

Pulmonary Atresia With Intact Ventricular Septum: Expert Review of the Surgical Management.

Pulmonary atresia with intact ventricular septum (PA/IVS) is a rare but highly heterogeneous congenital heart lesion defined by absence of continuity between the right ventricle (RV) and pulmonary arteries, a morphologically variable tricuspid valve (TV), RV hypoplasia and frequent right ventricle to coronary connections. Management has evolved from uniform surgical palliation toward nuanced, morphology-driven individualized strategies. Current approaches include transcatheter perforation, surgical valvotomy, staged ventricular recruitment, and single-ventricle or 1.5-ventricle palliation, wit
