AIMS: Significant secondary tricuspid regurgitation (STR) is associated with worse morbidity and mortality. To-date, evidence on isolated tricuspid valve surgery in STR remains controversial with uncertainty regarding management strategies. We aimed to investigate sex differences in the association of isolated tricuspid valve surgical repair/replacement (TVR) with outcomes of mortality and heart failure (HF) hospitalizations in STR.
METHODS AND RESULTS: Patients with severe STR without significant aortic valve or organic mitral valve disease, congenital heart disease, pacemakers, previous val

BackgroundRenewed interest in the Ross procedure in young adults has been tempered by questions over safety and early outcomes. This paper examines our institution's 1-year outcomes of the Ross procedure.MethodsFrom November 2019 to February 2025, 136 consecutive adult patients underwent a Ross procedure at an urban tertiary center. The first 50 patients had follow-up beyond 1 year and constitute the study group of this case series.ResultsMedian age was 41 years (IQR: 30, 47). The majority were male (n = 34, 68%). The most common indication for surgery was aortic stenosis with or without aorti

Valvular heart disease (VHD) is increasingly prevalent and associated with substantial morbidity and mortality. However, severe multivalvular heart disease (MVHD) remains poorly characterized; therefore, we sought to elucidate its prevalence, phenotypes, and clinical correlates. A single center study including all individuals with echocardiographic diagnosis of native VHD between 2010 and 2019 were included and grouped by the presence of severe MVHD. Multivariable logistic regression was used to determine the association between clinical and echocardiographic factors and presence of severe MVH

Background/Objectives: Infective endocarditis (IE) is a life-threatening cardiovascular infection with in-hospital mortality of 15-30% despite modern therapy. Contemporary IE demonstrates non-random valve involvement: aortic and mitral 35-45%, tricuspid 5-10% (30-50% in intravenous drug users [IVDU]), and pulmonary < 1%. These patterns implicate valve-specific anatomy and hemodynamics as central determinants of susceptibility. This narrative review examines the reported distribution of IE across the aortic, mitral, tricuspid and pulmonary valves and summarises the anatomical, haemodynamic, str

Background/Objectives: Native valvular heart disease (VHD) lacks established lesion-modifying pharmacotherapy. Sodium-glucose cotransporter 2 inhibitors (SGLT2is) improve heart-failure and cardiorenal outcomes, but their relevance to VHD differs by phenotype and treatment setting. Methods: MEDLINE/PubMed, Scopus, Web of Science Core Collection and Cochrane CENTRAL were searched from inception to 27 July 2026, together with trial registries, conference proceedings and reference lists. Evidence was synthesized by clinical setting, study design and mechanistic proximity to valve tissue. Results:

Prenatal diagnosis of coarctation of the aorta (CoA) remains challenging. Anticipated postnatal risk, level of care (LOC), was assigned using fetal echocardiography measures including mitral valve/tricuspid valve ratio, aortic valve/pulmonary valve ratio, and aortic isthmus size. Patients assigned LOC 1 (low risk, not expected to have instability within first weeks of life) underwent postnatal echocardiography in the community; patients assigned LOC 2 (moderate risk, expected stability at delivery, postnatal intervention before hospital discharge) were started on prostaglandin and transferred.

Cardiac amyloidosis (CA) is an infiltrative cardiomyopathy caused by extracellular deposition of amyloid fibrils, most commonly derived from immunoglobulin light chains (AL) or transthyretin (ATTR). While myocardial involvement has been extensively characterized, valvular disease in CA has received comparatively less attention, despite its frequent occurrence and significant clinical impact. Aortic stenosis (AS) represents the most prevalent valvular manifestation, particularly in wild-type ATTR amyloidosis, where shared demographic and pathophysiological factors contribute to a frequent overl

BACKGROUND: Neuroendocrine tumours (NETs) are uncommon cancers that secrete vasoactive hormones. Around half of those with metastatic small bowel NETs develop carcinoid syndrome, and historically, up to 50% develop carcinoid heart disease (CHD). We evaluated clinical characteristics, echocardiographic features, and outcomes of patients with NETs and CHD in a contemporary cohort to identify factors associated with CHD development and mortality.
METHODS: We identified patients with NETs who underwent an echocardiogram at a tertiary cancer centre-affiliated laboratory (2013-2023). Patients were

Pannus formation is a rare but clinically important complication of mechanical prosthetic valves. The incidence of pannus in aortic prostheses is much higher than that in mitral prostheses, the latter being more prone to thrombosis. However, the presence of pannus in both prostheses is an exceptional finding. The authors present the case of a 61-year-old woman with a long history of rheumatic multivalvular disease. She had undergone three cardiac surgeries for mechanical valve prosthesis replacement in the aortic, mitral, and tricuspid positions. She was hospitalized for atypical angina and bl

Valvular heart disease encompasses a diverse group of hemodynamic disorders that are frequently associated with heart failure, recurrent hospitalizations, and substantial morbidity despite advances in transcatheter and surgical therapies. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have emerged as a cornerstone of cardiovascular and renal disease management, demonstrating consistent benefits across a broad range of patient populations. Beyond their established effects in heart failure, diabetes mellitus, and chronic kidney disease, accumulating evidence suggests that SGLT2 inhibitors may

Background: Valvular heart disease frequently accompanies acute heart failure (AHF), yet most studies address single valve lesions, and the prognostic weight of concurrent multivalvular involvement is less well defined. We examined the prevalence of single- and multi-valve disease and its relationship with one-year mortality. Methods: We analyzed 530 consecutive patients enrolled in a prospective single-center AHF registry between February 2023 and June 2025 and followed for 12 months. Aortic, mitral and tricuspid disease was graded according to European Society of Cardiology/European Associat

Background: The population of patients surviving previous multivalve surgery is expanding, and structural Heart Teams increasingly encounter complex multivalvular disease requiring reintervention, for which redo surgery carries prohibitive risk. Patient selection, sequential procedural planning, prosthesis choice, and, in particular, pacing strategy and protection of pre-existing intracardiac hardware remain unstandardized. Case Presentation: We report a 73-year-old woman with a twenty-year history of rheumatic multivalvular heart disease, comprising mechanical mitral valve replacement, tricus

To fully understand valve function and evaluate valve dynamics in patients with valvular diseases, it is crucial to model the biomechanical behavior of the valve. The primary objective of this study is to develop an innovative, patient-specific computational framework to assess stress and strain deformation throughout the cardiac cycle using finite element analysis. Three aortic valve models-tricuspid (TAV), bicuspid type 0 (BAV Type 0), bicuspid type 1 (BAV Type 1), and quadricuspid (QAV)-were created from transesophageal echocardiography images using a combination of MATLAB and Blender funct

INTRODUCTION: Available work from North America indicates that same-sex attracted (SSA) individuals enjoy aspects of rural life but nonetheless report encountering homophobia and experiencing isolation from SSA networks. The experience of prejudice and social isolation are often associated with psycho-social distress among the general population of same-sex attracted individuals. Little is known of how SSA women experience life in rural areas of Australia and how this influences their psycho-social wellbeing.
METHODS: This was a small-scale qualitative study using guided interviews to explore

BACKGROUND: Sex-related disparities after transcatheter aortic valve replacement (TAVR) have been described in tricuspid aortic stenosis, but long-term data in bicuspid aortic valve (BAV) stenosis remain limited.
AIMS: To evaluate sex-based differences in long-term mortality and predictors of outcomes after TAVR in patients with BAV stenosis.
METHODS: We retrospectively analyzed 327 patients with BAV stenosis (183 men, 144 women) who underwent TAVR from 2014 to 2025. Baseline, anatomic, procedural, and clinical outcomes were compared by sex. Cox models evaluated predictors of all-cause morta

BACKGROUND: Concomitant mitral (MR) and/or tricuspid regurgitation (TR) in patients with aortic stenosis (AS) is often regarded as a late-stage disease manifestation, resulting from left ventricular damage, pulmonary hypertension, and right heart dysfunction. However, we assume that atrioventricular valve regurgitation in the context of severe AS may often develop as an independent entity, but knowledge regarding these patients and their risk factors is scarce.
OBJECTIVES: The aim of this study was to characterize patients who develop atrioventricular valve regurgitation after transcatheter a

Disseminated gonococcal infection (DGI) is an uncommon manifestation of Neisseria gonorrhoeae infection and may rarely involve the endocardium. We report the case of a 48-year-old man who presented with progressive right wrist pain and swelling, back pain, intermittent fever, weight loss, and diarrhea. MRI demonstrated inflammatory changes involving the L3-L4 facet joints and sacroiliac joints. Culture from wrist aspiration grew N. gonorrhoeae, whereas blood cultures remained negative. Additional infectious evaluation showed a negative HIV screen, nonreactive syphilis testing, a negative hepat

BACKGROUND: Rheumatic heart disease (RHD) poses a significant global health challenge due to its high rates of premature morbidity, mortality, and disability, particularly in low-income countries. Existing risk scoring systems for valve replacement in cardiac surgery have primarily focused on non-RHD cases, making them less applicable to RHD patients due to the distinct pathology of valvular damage. This study aims to evaluate the Model for End-stage Liver Disease, including sodium (MELD-Na) score, as a predictive tool for pre-operative mortality risk assessment following cardiac surgery in RH

BACKGROUND: Infective endocarditis (IE) complicated by aortic root abscess, fistulation and multivalve involvement is rare but carries high mortality. Fistulation from the left ventricular outflow tract (LVOT) to the right atrium (RA) represents an exceptionally uncommon sequela due to extensive perivalvular tissue destruction. Early surgical intervention remains crucial, but postoperative outcomes vary widely depending on host factors, organism control and the feasibility of durable reconstruction.
CASE PRESENTATION: We report two cases of severe aortic valve endocarditis with aortic root ab

BACKGROUND: Infective endocarditis (IE) remains a diagnostic and therapeutic challenge due to its heterogeneous presentation and potential for severe embolic complications. Right-sided infective endocarditis (RSIE) accounts for only 5–10% of all IE cases, most commonly associated with intravenous drug use. Psychiatric comorbidities and language barriers may further delay diagnosis, as symptoms are often nonspecific and communication may be impaired. We report a rare case of right- and left-sided pneumococcal endocarditis in a patient with chronic schizophrenia, initially presenting with isolat
