OBJECTIVE: To examine whether prenatal opioid use disorder (OUD) diagnosis is associated with the risk of congenital anomalies (CAs) in offspring.
METHODS: We conducted a population-based study of mother-newborn dyads comprising. 4143 761 births delivered in Canada from 2006 to 2021. We used robust Poisson regression to examine the association between prenatal OUD diagnosis and risk of non-chromosomal CAs, adjusted for maternal age, parity, multiple gestation, co-morbidities (including mental health disorders, chronic illnesses and other substance use disorders), and infant sex.

INTRODUCTION: Heparin-induced thrombocytopenia (HIT) is a relatively rare condition burdened by a high rate of complications and mortality. Cardiac surgery is a high risk setting for this condition. But in this area, some particularities can make both diagnosis and treatment difficult. Warfarin is often needed after valvular surgery but may be dangerous if HIT is ongoing. Danaparoid is the only anticoagulant whose mechanism of action has been demonstrated to counteract the pathogenesis of HIT. However, the 24-hour half-life challenges its use after surgery.

Histoplasma is a rare cause of endocarditis. It usually occurs in immunosuppressed patients, but diagnosis can be challenging given its nonspecific clinical and laboratory findings. This report describes the case of a patient with end-stage renal disease and multiple hospitalizations who was diagnosed with blood culture-negative infective endocarditis and subsequently found to have Histoplasma endocarditis of his native mitral valve. Our patient's nonspecific symptoms posed a diagnostic challenge, which resulted in delayed diagnosis and treatment.
