Development and Evaluation of a Dual-Target One-Step Nested PCR for the Detection of Spotted Fever Group Rickettsia spp. in Ticks.

Spotted fever group (SFG) rickettsioses are tick-borne infectious diseases caused by more than 30 Rickettsia species. As ticks may harbor and transmit multiple pathogens during a single blood meal, sensitive and specific molecular detection methods are essential for early diagnosis. Conventional nested PCR is commonly used but is time-consuming and prone to cross-contamination due to multiple amplification steps. This study evaluated a dual-target one-step nested PCR assay developed as a rapid alternative to conventional nested PCR for SFG Rickettsia detection. Gene-specific primers targeting

Japanese Spotted Fever With Marked Serological Cross-Reactivity to Rickettsia typhi Confirmed by Sequence-Based Molecular Analysis: A Case Report.

We report a case of Japanese spotted fever (JSF) showing marked serological cross-reactivity with Rickettsia typhi, which made serological differentiation difficult. An 80-year-old man presented with fever and disseminated erythematous macules. Laboratory finding suggested a rickettsial infection. Minocycline treatment led to clinical improvement. Real-time polymerase chain reaction screening was positive for spotted fever group rickettsiae. Serological testing using paired sera demonstrated comparable increases in antibody titers against R. japonica and R. typhi in the convalescent phase, pre

Molecular identification of Rickettsia conorii subsp. israelensis and other tick-borne pathogens in dog ticks from the Upper East region of Ghana.

Ticks continue to facilitate the spread of pathogens that affect both humans and domestic animals. Domestic dogs interact with humans and other domestic animals, playing a crucial role in the spread of ticks and tick-borne pathogens. This study examined the diversity of tick species infesting domestic dogs and the occurrence of tick-borne pathogens in the Upper East region. Domestic dogs were randomly selected and examined for tick infestation. The sampled ticks were morphologically identified, pooled and screened for tick-borne pathogens using polymerase chain reaction (PCR) and Sanger sequen

Rickettsiae of the spotted fever group in dogs of the Americas: A systematic review and meta-analysis.

Sentinel animals are defined as sensitive indicators of environmental hazards that provide an early warning system for public health interventions. Spotted fever group rickettsioses are zoonotic tick-borne diseases that are considered endemic in different American countries with significant rates of morbidity and lethality, mainly when involving species such as Rickettsia rickettsii and Rickettsia parkeri. Dogs have been recognized as good sentinels for these rickettsioses, since they are hosts to different species and stages of ticks, and they sustain a good immunological response after infec

Beyond the eschar: African tick-bite fever in a returning traveller - a diagnostic and public health consideration in Europe.

BACKGROUND: African tick-bite fever (ATBF), caused by Rickettsia africae, is the most common tick-borne rickettsiosis among travellers returning from sub-Saharan Africa. In non-endemic regions, its relevance lies mainly in diagnostic challenges in returning travellers and the need for continued vigilance in vector-borne disease surveillance. CASE PRESENTATION: report a 48-year-old woman presenting with fever, headache, myalgia, abdominal pain, and multiple eschars after a 9-day trip to South Africa. Clinical examination revealed fever (38.5 °C), tender inguinal lymphadenopathy, and a maculopa

A real-time PCR assay for detection of Rickettsia rickettsii to aid in vivo infection monitoring.

Rocky Mountain spotted fever (RMSF) is an important and potentially fatal tick-borne disease impacting people and dogs. RMSF, caused by Rickettsia rickettsii, is frequently reported in parts of the North, Central and South American countries. The treatment for RMSF is limited to tetracyclines, specifically to doxycycline. If untreated, the disease can progress rapidly to fatal outcomes, particularly in children. In recent years, fatality rates ranging from 30% to 80% have been reported throughout South America, Mexico, and in parts of Indian reservations in southwest regions of the USA. To aid

Rickettsia parkeri as a Probable Agent of Mild Spotted-Fever Group Rickettsiosis Identified by Seroreactivity in Villeta, Colombia.

Spotted fever group (SFG) rickettsioses are emerging zoonotic diseases of increasing relevance in Latin America, yet the specific species involved in human infections remain poorly defined in many endemic regions. This study aimed to determine the most probable antigen among SFG-seroreactive febrile patients from Villeta, Colombia. A panel of 25 convalescent-phase serum samples previously identified as positive for SFG Rickettsia spp. antibodies was analyzed by indirect immunofluorescence assay using antigens of Rickettsia rickettsii, R. amblyommatis and R. parkeri. Antibody titers were compar

Probable African tick bite fever identified during preoperative evaluation in Central Utah.

African tick bite fever (ATBF) is a spotted fever group rickettsiosis caused by Rickettsia africae and is often identified among travelers returning from sub-Saharan Africa. Clinical presentation may be mild or nonspecific, often delaying diagnosis despite characteristic findings such as inoculation eschars. A 52-year-old male presented to the preoperative holding area prior to a scheduled left-sided inguinal hernia repair after recently returning from a game-hunting trip to Africa. During preoperative intake, he was noted to have a fever of 106 °F despite being otherwise asymptomatic. Physica

Cutaneous manifestations of scrub typhus: a comprehensive review of diagnostic clues in a neglected tropical disease.

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic infection with significant diagnostic reliance on its cutaneous manifestations. The disease, transmitted by larval trombiculid mites, is endemic to the 'Tsutsugamushi Triangle' and extends to other regions globally. Dermatological features are pivotal in early recognition, with eschar formation being pathognomonic. Eschars, resembling 'cigarette burns', develop as necrotic lesions surrounded by erythema and often resolve with minimal scarring. Maculopapular rashes, although variably reported, predominantly affect the tru

Dengue and scrub typhus coinfection in pregnancy: A rare case with multiorgan dysfunction.

Tropical infections continue to cause significant morbidity and mortality in India. While mixed tropical infections are relatively common in the general population, they are rare during pregnancy. We report a 24-year-old pregnant woman with a history of previous lower segment caesarean section and preeclampsia, who developed dengue and scrub typhus infection near-term. The illness was complicated by multiple organ dysfunction syndrome, yet with timely multidisciplinary management, both maternal and neonatal outcomes were favourable. Literature on such mixed tropical infections in pregnancy rem

Temporary Spontaneous Remission of Adult Acute Lymphoblastic Leukemia Associated with Scrub Typhus Infection.

INTRODUCTION: Acute lymphoblastic leukemia (ALL) in adult patients usually poses a treatment challenge with intensive chemotherapeutic agents. Prognosis is worse in comparison to pediatric patients owing to disease biology and associated comorbidities. Spontaneous remission of ALL is rare and warrants close follow-up for progression. CASE PRESENTATION: Herein, we report the case of a male in his 40s initially admitted for febrile illness with pancytopenia; later, his serology was positive for scrub typhus infection, and he was managed for the same with antimicrobials.
