Prevalence and Predictors of High Inflammatory Risk in Heart Failure Subtypes: Findings From the Global POSEIDON Study.

AIMS: Inflammation contributes to the pathophysiology of heart failure (HF), yet the global prevalence of elevated high sensitivity C-reactive protein (hsCRP) in patients with HF across the ejection fraction (EF) spectrum remains unclear. We sought to characterize the prevalence and clinical correlates of high inflammatory risk (defined as hsCRP ≥2 mg/L) in a global real-world HF cohort across the EF spectrum. METHODS AND RESULTS: POSEIDON prospectively enrolled 18,904 individuals across 317 sites in 18 countries (2023-2025) at routine visits, including 11,809 with HF and available hsCRP (371

Janus Kinase and Interleukin-6 Inhibitor-Induced Lipid Modulation and Its Association With Major Adverse Cardiovascular Events in Rheumatoid Arthritis: A Systematic Review of Clinical and Biomarker Evidence.

Rheumatoid arthritis (RA) is a chronic systemic inflammatory disease associated with increased cardiovascular (CV) risk and alterations in lipid metabolism. Janus kinase inhibitors (JAKi) and interleukin-6 inhibitors (IL-6i) have offered effective control of disease activity; however, their impact on lipid profiles and subsequent CV outcomes has yet to be fully elucidated. This systematic review aimed to evaluate the effects of JAKi and IL-6i on lipid profiles, inflammatory biomarkers, and major adverse cardiovascular events (MACE) in patients with RA. Nineteen peer-reviewed studies published

The Outcomes of Percutaneous Mitral Balloon Valvuloplasty in Patients With Severe Rheumatic Mitral Stenosis and Left Atrial Appendage Thrombus: Systematic Review and Meta-Analysis.

Persistent left atrial appendage thrombus despite anticoagulation is common. Current guidelines advise against percutaneous mitral balloon valvuloplasty (PMBV) because of perceived thromboembolic risk, leaving surgical mitral valve replacement-an option associated with substantial morbidity-as the primary alternative. However, the procedural risk of PMBV in the setting of left atrial appendage thrombus remains poorly established. We conducted a systematic review and meta-analysis to assess this. Of the 2,136 studies identified in the initial search, 17 were included in the analysis, comprising

Long-Term Outcomes of Open Mitral Commissurotomy vs Mechanical Mitral Valve Replacement in Rheumatic Mitral Stenosis.

BACKGROUND: Mechanical mitral valve replacement (MVR) is widely performed for severe rheumatic mitral stenosis (MS) but requires lifelong anticoagulation and carries prosthesis-related risks. Open mitral commissurotomy offers a valve-preserving alternative, yet contemporary long-term data are scarce. We compared long-term outcomes of open mitral commissurotomy and MVR in severe MS. METHODS: We retrospectively analyzed 249 patients undergoing mitral valve surgery for severe rheumatic MS (1995-2021): open mitral commissurotomy (n = 58) and MVR (n = 191). The primary outcome was a composite of a

Distance from tertiary care, pericardial effusion, and nutritional status predict all-cause mortality among Malawian children with rheumatic heart disease.

BACKGROUND: This study of Malawian children with rheumatic heart disease (RHD) sought to detect demographic, clinical, and echocardiographic risk factors for mortality. METHODS: Pediatric patients with RHD were recruited from March to October, 2018 from clinic rosters and inpatient consults in Lilongwe and Blantyre, Malawi. An echocardiogram was performed upon study enrollment. Cox regression analyses were performed to assess for factors associated with mortality over nearly 2 years of follow-up. RESULTS: Of 118 patients, nearly two-thirds were female (64.4%) and median age was 12 (IQR 10-14

Percutaneous mitral balloon commissurotomy in juvenile rheumatic mitral stenosis patients: a ten-year experience from Ethiopia.

BACKGROUND: Rheumatic heart disease (RHD) remains a major cause of acquired valvular heart disease in low- and middle-income countries, disproportionately affecting young people. The mitral valve is most frequently involved, and severe rheumatic mitral stenosis (MS) represents the dominant presentation. MS is a progressive disease that gradually impairs cardiac output and, if untreated, leads to pulmonary hypertension, right-sided heart failure, and premature death. Percutaneous mitral balloon commissurotomy (PMBC) is considered the first-line intervention for symptomatic severe MS with favora

A randomised open-label pilot trial comparing mycophenolate mofetil with no immunosuppression in limited cutaneous systemic sclerosis (MINIMISE-Pilot).

OBJECTIVES: Mycophenolate mofetil (MMF) is routinely used in early diffuse cutaneous systemic sclerosis (dcSSc) but not in limited cutaneous (lc)SSc. This may miss an opportunity to slow disease progression. MINIMISE-Pilot tested the feasibility of an open-label event-driven randomised trial of MMF vs no immunosuppression in lcSSc. METHODS: We tested the feasibility of a trial evaluating the impact of MMF on a novel event-driven composite endpoint. The MINIMISE endpoint measures time to worsening of lcSSc determined by progressive lung fibrosis, pulmonary hypertension, scleroderma renal crisi

Trends and Disparities in Chronic Rheumatic Heart Disease Mortality in the United States (1999-2024) and Projections to 2050.

Contemporary data on national mortality trends, projections, and evolving disparities remains limited. National rheumatic heart disease mortality data from 1999 to 2024 was assessed using Centers for Disease Control and Prevention's Wide-ranging Online Data for Epidemiologic Research database. Age adjusted mortality rates (AAMR) per 100,000 were calculated. Trends were assessed using joinpoint regression to compute the average annual percent change (AAPC), and predictive models were adopted to estimate rates through 2050. A total of 177,342 deaths were recorded from chronic rheumatic heart dis

Inflammation and cardiovascular risk in autoimmune diseases: Mechanisms, biomarkers, and clinical implications.

The intersection of autoimmune diseases and cardiovascular disease (CVD) represents a critical frontier in modern medicine. Patients with chronic inflammatory and autoimmune conditions exhibit a significantly elevated risk of premature and accelerated cardiovascular events, a phenomenon rooted not only in shared risk factors but also in a deeply intertwined pathophysiology. The persistent inflammatory state characteristic of these disorders creates a proatherogenic environment, leading to systemic effects that manifest as subclinical cardiac involvement long before overt clinical events occur.

Risk factors for atrial fibrillation in Ibadan, Nigeria: A case-controlled study.

BACKGROUND: Atrial fibrillation (AF) is a common arrhythmia that is a potent independent risk factor for stroke. The incidence of AF increases with age, and most affected people have underlying cardiac disease. This study aimed to describe the risk factors for AF in Nigeria. METHODS: This was a hospital-based case-controlled study. The cases were adult male and female clinic attendees aged 18 years and above who had 12-lead ECG-diagnosed atrial fibrillation. They were age- and sex-matched with clinic attendees who were free of AF. A uniform case report form was used for data collection for th

Cardiometabolic risk and vascular changes in rheumatic diseases.

Rheumatic diseases (RDs) are chronic immune-mediated disorders associated with disproportionately increased cardiovascular morbidity and mortality. Accelerated atherogenesis in these diseases is driven by persistent systemic inflammation, autoantibody-mediated endothelial injury, oxidative stress, and dysregulated lipid metabolism, resulting in premature vascular remodeling manifested by increased carotid intima-media thickness, arterial stiffness, impaired flow-mediated dilation, and coronary artery calcification. This review synthesizes evidence regarding subclinical atherosclerosis and card

Rare coexistence of unroofed coronary sinus, coronary sinus atresia and rheumatic mitral stenosis demonstrated by multimodality imaging.

Unroofed coronary sinus (CS) with CS orifice atresia and persistent left superior vena cava (PLSVC) is an exceptionally rare congenital anomaly that may coexist with acquired valvular disease, as illustrated in this case of severe rheumatic mitral stenosis. Multimodality imaging including echocardiography and cardiac CT was essential for precise anatomical delineation, guiding safe surgical intervention. Recognition of this complex anatomy is critical to avoid catastrophic complications such as inadvertent PLSVC ligation without ensuring adequate CS drainage. This case highlights the importanc
