Association Between Age and PCI Effectiveness in Stable CAD: Secondary Analysis of ORBITA-2.

BACKGROUND: ORBITA-2 (Objective Randomized Blinded Investigation With Optimal Medical Therapy of Angioplasty in Stable Angina) was the first randomized placebo-controlled trial to show the efficacy of percutaneous coronary intervention (PCI) in patients with stable angina and single- or multivessel coronary artery disease without background antianginal medication. Whether the effect is consistent across age groups is unknown. OBJECTIVES: The authors sought to evaluate the interaction between age and symptom and stenosis severity, and the efficacy of PCI on the ORBITA-2 primary and secondary e

Incremental prognostic value of pericoronary fat attenuation index in diabetic patients with non-obstructive coronary artery disease.

BACKGROUND: Non-obstructive coronary artery disease (CAD) is highly prevalent in diabetic patients, yet there are currently limited evidence-based recommendations for risk stratification and management in this population. We aimed to investigate the additional prognostic value of pericoronary fat attenuation index (FAI) beyond coronary computed tomography angiography (CCTA) parameters and CCTA-derived fractional flow reserve (CT-FFR) in diabetic patients with non-obstructive CAD. METHODS: 310 diabetic patients with non-obstructive CAD (coronary stenosis 1–49% on CCTA) between January 2017 and

Prediction of heart failure as the first major cardiovascular disease event in type 2 diabetes.

AIMS: Heart failure (HF) may develop in type 2 diabetes without prior cardiovascular disease (CVD). This study aimed to identify risk factors that distinguish CVD that presents first as a HF event versus CVD that presents first as an atherosclerotic event (ASCVD), among people with type 2 diabetes. MATERIALS AND METHODS: ORIGIN and REWIND trial participants with type 2 diabetes but free of CVD (N = 6175; 64 ± 7 years, 50.1% female) were followed for ~5.8 years for incident CVD presenting first as either a HF event (HF hospitalization or HF death), ASCVD (myocardial infarction, unstable angina

Effects of Artificial Intelligence Recognition-Based Telerehabilitation on Exercise Capacity in Patients With Hypertension: Randomized Controlled Trial.

BACKGROUND: Hypertension remains a major global health challenge, significantly increasing cardiovascular and all-cause mortality risks. While exercise therapy is effective, conventional approaches face limitations in accessibility and personalization, compromising adherence. Artificial intelligence (AI)-assisted remote rehabilitation enables real-time monitoring and personalized guidance, offering a promising alternative. Nevertheless, its clinical benefits and applicability require further systematic validation. OBJECTIVE: This study aimed to evaluate the efficacy of an 8-week AI-assisted t

Acute Myocardial Infarction from Stent Thrombosis Following a Loss to Follow-up Triggered by Financial Barriers to PCSK9 Inhibitor Therapy.

A 44-year-old man with suspected familial hypercholesterolemia underwent percutaneous coronary intervention (PCI) with bioresorbable polymer sirolimus-eluting stent implantation for unstable angina. A proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor was initiated post-procedure but it discontinued five months later due to financial constraints, leading to a loss to follow-up and cessation of optimal medical therapy, including antiplatelet therapy. Four years later, he developed stent thrombosis, presenting as ST-elevation myocardial infarction with cardiogenic shock. Emergency P

Influence of Pullback Pressure Gradient on Residual Angina at One Year.

BACKGROUND: The pullback pressure gradient (PPG) is a novel physiological metric that quantifies coronary artery disease patterns as focal or diffuse on a scale from 0 to 1. This study assessed the relationship between PPG and residual angina at 1 year. METHODS: PPG Global is a prospective, investigator-initiated, single-arm, multicenter study that enrolled patients with at least 1 lesion with a fractional flow reserve ≤0.80 intended to be treated with percutaneous coronary intervention. After the PPG calculation, physicians could revise treatment assignment to medical therapy or coronary art

Cardiovascular risk among Giant cells arteritis patients.

OBJECTIVES: Giant cell arteritis (GCA) is a chronic large-vessel vasculitis affecting older adults, associated with significant cardiovascular (CV) complications. Understanding CV risk drivers among GCA, including classical CV risk factors and inflammation, is essential for improved patient management. We (i) assessed the association between GCA and risks of cardiovascular events, including aortic events, major adverse cardiovascular events (MACE), peripheral artery disease (PAD) events, and visceral artery events (ii) assessed the impact of traditional CV risk factors and GCA disease activity

[Angina in non-obstructive coronary artery disease: some examples from daily practice].

Angina in non-obstructive coronary artery disease (ANOCA) is a common clinical condition affecting 20-50% of patients undergoing coronary angiography for suspected angina. Despite recent 2024 European guidelines on chronic coronary syndrome emphasizing its significance and clinical implications, ANOCA remains often underdiagnosed and undertreated. The management of ANOCA differs from obstructive coronary artery disease and requires careful interpretation of diagnostic tests and tailored treatment based on accurate clinical assessment. ANOCA diagnosis may involve microvascular dysfunction or va

Percutaneous coronary intervention employing a double drug-eluting stent sandwich strategy for a right coronary artery aneurysm with severe calcified stenosis: a case report.

BACKGROUND: Coronary artery aneurysm(CAA) represents a rare subset of coronary artery disease and is associated with heightened risks of cardiovascular mortality. Because of its low prevalence, large-scale data are scarce, therefore specific therapeutic guidelines remain underdeveloped. We report the case of a 64-year-old female who presented to our hospital due to unstable angina. The coronary angiogram showed an aneurysm in the mid-segment of the right coronary artery, the arterial lumen preceded the aneurysm was severe stenosis with calcification. Considering the elevated risks of rupture a

Association of atherogenic index of plasma and cardiometabolic index with all-cause mortality and cardiovascular disease in NAFLD patients: NHANES 1999-2018.

BACKGROUND: The atherogenic index of plasma (AIP) serves as a crucial indicator for assessing atherosclerotic risk. It reflects the degree of dyslipidaemia and cardiovascular disease (CVD) risk. The cardiometabolic index (CMI) provides a comprehensive evaluation of obesity-related metabolic risk, acting as a key biomarker for predicting multiple cardiometabolic diseases. The relationship between AIP and CMI in patients with non-alcoholic fatty liver disease (NAFLD) and mortality or CVD risk remains unclear. METHODS: This study included 5792 adult (≥ 18 years) NAFLD patients from the US Nation

Does Sirolimus-Eluting Resorbable Magnesium Scaffold Plays a Role in Iatrogenic Flow-Limiting Coronary Artery Dissection?

BACKGROUND: The third-generation sirolimus-eluting resorbable magnesium scaffold could be a competitive or alternative to a drug-eluting stent in the future, in particular in iatrogenic coronary artery dissection, to avoid unnecessary metal stents to reduce restenosis and occlusion rate. CASE PRESENTATION: We report a 66-year-old female who presented with unstable angina. The significant medial Cx stenosis was treated with a drug-coated balloon. A small new dissection medial Cx occurred. 8 hours later, the patient developed lateral wall ST segment elevation myocardial infarction. Coronary ang

Causal relationships between psoriasis and coronary artery disease: A two-sample Mendelian randomization study.

Previous studies have revealed a potential association between psoriasis and coronary artery disease (CAD). However, the causal relationship between the 2 remains unclear. This study aims to assess the causal link between psoriasis and CAD, which encompasses coronary heart disease, myocardial infarction, and angina pectoris. After obtaining genome-wide association studies data on psoriasis and CAD, we selected appropriate single nucleotide polymorphisms for Mendelian randomization (MR) analysis. The inverse-variance weighted method was used as the main analytical method. The inverse-variance w

Major adverse cardiovascular events after liver transplantation: a population cohort analysis of English transplant centres.

BACKGROUND: Major adverse cardiovascular events (MACE) are common early after liver transplantation (LT) as reported in the United States, but incidence is not well described in a European cohort. With lower MACE rates reported after kidney transplantation in the United Kingdom versus the United States, it is important to determine if a similar incongruity exists after LT. METHODS: In this large retrospective, population cohort study, we studied all LT procedures performed in England between 1st April 2002 and 31st March 2023 (n = 10,213). MACE data was obtained from Hospital Episode Statisti

Cardiovascular outcomes and adverse drug reactions associated with the use of ezetimibe in combination with a statin in diabetes mellitus patients with higher risk for myocardial infarction: a meta-analysis.

BACKGROUND: It is well known that diabetes mellitus (DM) is an important risk factor for the development of cardiovascular diseases (CVD). Ezetimibe, an inhibitor of cholesterol absorption, is now a second-line lipid-lowering therapy with apparently more benefits. Ezetimibe can be used in addition to a statin in order to further reduce low density lipoprotein (LDL) levels. In this analysis, we aimed to systematically compare the cardiovascular outcomes and the adverse drug reactions associated with the use of ezetimibe in combination with a statin versus statin monotherapy in type 2 DM patient

NMR-based metabolic measures of chronic stable angina and myocardial infarction in patients with diabetes mellitus.

BACKGROUND AND AIMS: Diabetes mellitus (DM) accelerates the onset and progression of coronary artery disease (CAD), yet metabolomic profiling in individuals with both conditions remains limited. NMR-based metabolomics offers a comprehensive assessment of metabolic alterations and may improve cardiovascular risk stratification. Hence, the objective of this study was to characterize serum metabolic signatures associated with varying CAD severity in diabetic patients and evaluate their relationship with conventional clinical measures. METHODS: Eighty-eight diabetic patients undergoing coronary a
