Causal relationships between psoriasis and coronary artery disease: A two-sample Mendelian randomization study.

Previous studies have revealed a potential association between psoriasis and coronary artery disease (CAD). However, the causal relationship between the 2 remains unclear. This study aims to assess the causal link between psoriasis and CAD, which encompasses coronary heart disease, myocardial infarction, and angina pectoris. After obtaining genome-wide association studies data on psoriasis and CAD, we selected appropriate single nucleotide polymorphisms for Mendelian randomization (MR) analysis. The inverse-variance weighted method was used as the main analytical method. The inverse-variance w

NMR-based metabolic measures of chronic stable angina and myocardial infarction in patients with diabetes mellitus.

BACKGROUND AND AIMS: Diabetes mellitus (DM) accelerates the onset and progression of coronary artery disease (CAD), yet metabolomic profiling in individuals with both conditions remains limited. NMR-based metabolomics offers a comprehensive assessment of metabolic alterations and may improve cardiovascular risk stratification. Hence, the objective of this study was to characterize serum metabolic signatures associated with varying CAD severity in diabetic patients and evaluate their relationship with conventional clinical measures. METHODS: Eighty-eight diabetic patients undergoing coronary a

Reduction in circulating endothelial progenitor cells caused by obstructive sleep apnea-related hypoxemia and its association with the severity of coronary artery disease.

BACKGROUND: Obstructive sleep apnea (OSA) is a risk factor for coronary artery disease (CAD), while endothelial progenitor cells (EPCs) are critical for vascular repair. This study investigated the associations among OSA-related hypoxemia, circulating EPC, and CAD severity. METHODS: This prospective study enrolled patients with unstable angina undergoing coronary angiography. All participants underwent overnight polysomnography to determine the apnea-hypopnea index (AHI) and the sleep apnea-specific hypoxic burden (SASHB). Circulating EPCs were quantified using flow cytometry. CAD severity wa

Prognostic Role of the PRECISE-DAPT Score in Acute Coronary Syndrome and Different Antithrombotic Treatment Strategies.

INTRODUCTION: The PRECISE-DAPT score is a useful tool for predicting the risk of bleeding after percutaneous coronary intervention (PCI) requiring dual antiplatelet therapy. We aimed to validate the PRECISE-DAPT score as a mid-term mortality predictor in acute coronary syndrome (ACS). METHODS: All patients with ACS hospitalized between October 2018 and October 2023 were analyzed. Mortality data were acquired in cooperation with the Institute of Health Information and Statistics of the Czech Republic. We used a standard PRECISE-DAPT threshold ≥ 25. A receiver operating characteristic (ROC) cur

The effect of inflammatory factors on unstable angina risk, from the gene level.

Inflammation and its genetic regulation play crucial roles in the development of unstable angina (UA), a key manifestation of coronary artery disease (CAD). However, the genetic determinants that influence cytokine production and interindividual inflammatory responses remain incompletely understood, particularly in the Chinese population. This study aimed to clarify the relationship between inflammatory cytokine gene polymorphisms, serum cytokine expression, and UA risk. A hospital-based case-control study was conducted in 160 patients with UA and 280 age- and sex-matched healthy controls. Par

Coronary slow flow and myocardial contractile reserve: a speckle‑tracking echocardiography study.

BACKGROUND: The Coronary Slow Flow Phenomenon (CSF) is increasingly recognized as a distinct phenotype of Ischemia with Non-Obstructive Coronary Arteries (INOCA). However, debate persists regarding whether CSF represents fixed structural microvascular remodeling or functional dysregulation. Conventional Left Ventricular Ejection Fraction (LVEF) is often insensitive to the subtle subendocardial mechanics associated with this condition. OBJECTIVES: This study aimed to interrogate the functional reserve of the left ventricle in patients with angiographically defined CSF using Two-Dimensional Spe

Giant Aortic Lambl's Excrescence: An Unusual Cause for Angina Pectoris.

BACKGROUND: Lambl's excrescences (LEs) are thin, filamentous valvular strands that are usually small and asymptomatic, however when they are large and mobile, they can cause embolic or obstructive events. CASE SUMMARY: A 59-year-old man presented with recurrent angina-like chest pain. Coronary angiography showed an intermediate lesion in the left anterior descending artery without hemodynamic relevance. Echocardiography revealed a highly mobile, 20- to 30-mm filamentous structure attached to the left coronary cusp, intermittently prolapsing toward the left coronary ostium and causing moderate

Correlation of Lp(a), ApoB and oxLDL with Endothelial Damage Reading in Patients with Different Degrees of Coronary Atherosclerosis.

This pilot hypothesis-generating study evaluated whether lipid-related biomarkers (Lp(a), ApoB, and oxLDL), endothelial injury markers (endocan, vimentin), and extracellular matrix glycoproteins (TSP-1, TSP-2) reflect the severity of coronary artery disease (CAD) in patients with stable angina pectoris. 93 patients underwent invasive coronary angiography/coronary CT angiography. CAD severity was evaluated using Gensini, SIS, SSS, and CAD-RADS scores. CAD was confirmed in 76.3% (n = 71). OxLDL correlated with Gensini (r = 0.455; p = 0.006), atherosclerotic segments (r = 0.469; p = 0.005), arter

Exercise treadmill testing for efficacy evaluation in randomized, controlled trials.

Exercise treadmill testing measures functional capacity and inducible myocardial ischemia and has historically served as an endpoint in phase 2 trials. The Precision Medicine with Zibotentan in Microvascular Angina trial evaluated the selective endothelin-A receptor antagonist zibotentan as a potential disease-modifying therapy for microvascular angina. The trial had a randomized, double-blind, cross-over design and the primary outcome was exercise duration. Compared with placebo, zibotentan at a dose of 10-mg daily for 12-weeks did not improve exercise duration or angina symptoms. In this pre

Systematic Druggable Genome-Wide Mendelian Randomization Identifies Genetically Supported Therapeutic Targets for Coronary Artery Disease.

OBJECTIVE: Coronary artery disease (CAD) remains a leading cause of mortality worldwide, with substantial unmet therapeutic needs. This study aimed to identify and prioritize genetically supported therapeutic targets for CAD using Mendelian randomization (MR). METHODS: We implemented a two-sample MR framework to infer the causal effects of blood druggable cis-expression quantitative trait loci (cis-eQTLs) on CAD. To consolidate MR findings, we applied Steiger filtering, Bayesian colocalization, and multiple sensitivity analyses. Mediation and phenomewide MR analyses were employed to investiga

An integrative strategy combining network pharmacology, metabolomics, and gut microbiota analyses to elucidate the bioactive ingredients and underlying mechanisms of Xin-Shu-Ning tablets against myocardial ischemia.

Xin-Shu-Ning tablets (XSNT) are clinically used for the treatment of angina pectoris and coronary insufficiency; however, their bioactive constituents and cardioprotective mechanisms remain insufficiently clarified. In this study, an integrated strategy combining network pharmacology, metabolomics, and 16S rDNA amplicon sequencing was employed to systematically elucidate the mechanisms of XSNT and identify its potential active components. The blood-absorbed constituents of XSNT were first characterized using an HPLC-MS/MS method. Network pharmacology analysis was then conducted to predict pote

Discovery of vasodilatory effects of Na+/H+ exchanger 1 inhibitors to treat vasospastic angina.

The best way of treating vasospastic angina pharmacologically is to induce vasorelaxation under hypoxia only. We previously established a new method to quantify the contraction and relaxation of vascular smooth muscle cells (VSMCs) in real time. In the present study, we used this method to screen the effects of chemicals on VSMC dilation under normoxia, and we chose the chemicals with negative results as candidates. Next, we tested the effects of the candidates on VSMC dilation under hypoxia, and we found that cariporide and empagliflozin induced VSMC relaxation under hypoxia but not normoxia.

Impact of invasive coronary function testing on management of ANOCA patients and secondary care resource utilisation: insights from a multicentre retrospective study in the UK.

BACKGROUND: Invasive coronary function testing (CFT) is indicated in patients with refractory angina with non-obstructed coronary arteries (ANOCA). Despite this, questions remain regarding patient selection for testing, safety and integration of CFT within clinical pathways and the impact that endotyping has on long term management of these patients. This study aims to investigate the safety of CFT in patients presenting with anginal chest pain, the prevalence of ANOCA endotypes in the tested population, and the impact of CFT on prescribed medical therapy and secondary care resource utilisatio

Exploring the effects of occupational noise on cardiovascular and cerebrovascular diseases: A Mendelian randomization study.

Existing observational studies have demonstrated a potential association between exposure to noisy workplaces and the occurrence of cardiovascular and cerebrovascular diseases. However, current research is still inconclusive and inconsistent. Therefore, we attempted to delineate the causal correlation between noisy workplace exposure and both cardiovascular and cerebrovascular diseases in the European population by using Mendelian randomization analysis. The necessary data on cardiovascular and cerebrovascular diseases, as well as noisy workplaces, were all accessible from the genome-wide asso
