Trend of zoonotic diseases outbreaks in India: a secondary data analysis of Integrated Disease Surveillance Programme data, 2010-2023.

BACKGROUND: Zoonotic diseases are occurring more frequently in different parts of the world. In India, most outbreaks are reported by the Integrated Diseases Surveillance Programme (IDSP). The objective of this study was to find the trend of zoonotic diseases reported in the IDSP portal. METHODS: Data were collected from the IDSP portal from January 2010 to December 2023. Data were collected regarding each outbreak's start date, the number of cases and the number of deaths. A total of 10 diseases were included in the study. RESULTS: From all reported outbreaks, a total of 1158 were reported

Azithromycin with or without cefixime for suspected or culture-confirmed uncomplicated typhoid fever in Nepal, Bangladesh, and Pakistan (ACT-South Asia): a double-blind, parallel-group, randomised, placebo-controlled, phase 4 trial.

BACKGROUND: WHO recommends azithromycin for uncomplicated typhoid fever treatment. Cefixime is also commonly used, but both drugs have reported failure rates of 10% or more. We hypothesised that combining azithromycin's intracellular activity with the extracellular activity of cefixime could overcome each drugs' pharmacokinetic gaps and enhance tissue sterilisation. The study objective was to determine if a combination of azithromycin and cefixime would reduce treatment failure rates compared with azithromycin alone. METHODS: We did a double-blind, parallel-group, randomised, placebo-controll

Case Report: Eschar-like subcutaneous lesion in disseminated melioidosis revealing anti-interferon-γ autoantibody-associated adult-onset immunodeficiency and a review of the literature.

BACKGROUND: Melioidosis, caused by the environmental saprophyte Burkholderia pseudomallei, is acquired from soil and surface water and rises sharply in incidence after heavy rainfall and flooding-something that extreme-weather trends under climate change are expected to make more frequent and more widespread. The disease presents with a broad clinical spectrum, but eschars or ecthyma gangrenosum-like necrotic skin lesions are very rarely reported and may be mistaken for scrub typhus in coendemic regions. CASE PRESENTATION: A 66-year-old woman from northeast Thailand with no known comorbidity

Comparison of carboplatin-based chemotherapy versus cisplatin-based chemotherapy in the treatment of malignant gonadal germ cell tumor: a systematic review and meta-analysis.

To evaluate the role of carboplatin-based chemotherapy in patients diagnosed with malignant gonadal germ cell tumors (GCTs), we conducted a systematic review and meta-analysis. We searched PubMed, MEDLINE, Embase, Cochrane library, and Web of Science. Randomized controlled trials or cohort studies on gonadal GCTs between January 1, 1970 and April 26, 2023 were enrolled. The treatment failure rate and mortality rate were the primary outcomes. Subgroup analysis based on the primary tumor site and dose of carboplatin was also conducted. In total, 8 studies with 1,409 patients were included. Compa

Cardiovascular Implications in Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD): A State-of-the-Art Review.

Preclinical animal models are essential for evaluating the biological performance, safety, and translational potential of bone regeneration strategies before human application. Because no single species can fully replicate human skeletal biology, careful selection of an appropriate model is necessary to generate reliable and clinically meaningful evidence. This narrative review summarizes the characteristics, advantages, and limitations of commonly used experimental species, with particular emphasis on their anatomical and biomechanical similarity to humans and their suitability for modeling l

C-C Motif Ligand 7 and C-C Motif Chemokine Receptor 3 Dysregulation in Patients With Scrub Typhus and Association With Mortality.

BACKGROUND: Scrub typhus, caused by Orientia tsutsugamushi, involves infiltration of a mixture of perivascular lymphocytes and macrophages into affected organs. We investigated if this is characterized by chemokine dysregulation. METHODS: mRNA expression of chemokines and receptors was screened in whole blood by cDNA microarray in a subgroup of patients and controls. Regulated transcripts were analyzed in plasma by enzyme immunoassays (chemokines) and in whole blood by quantitative polymerase chain reaction (receptors) from patients with scrub typhus (n = 129), patients with similar febrile i

Predictors of encephalitis in children with scrub typhus-associated acute febrile illness.

Scrub typhus (ST) is an emerging public health concern in India. Despite being treatable, 20-30 % of acute febrile illnesses (AFI) progress to encephalitis in endemic regions. This study aimed to identify early markers for encephalitis development in children hospitalized with AFI and positive Orientia tsutsugamushi (Ots) serology. This prospective cohort study was conducted during January-December 2018, involved AFI patients (age ≤ 16 years) tested positive for IgM antibodies against Ots. These patients were followed up prospectively to monitor the development of encephalitis. The clinical an

The diagnostic accuracy of point-of-care nucleic acid-based isothermal amplification assays for scrub typhus: a systematic review and meta-analysis.

INTRODUCTION: The diagnosis and detection of pathogens such as Rickettsia and Orientia is a cause of major concern among the public health community. Unavailability of rapid, cost-effective diagnostic assays contributes to delayed diagnosis and timely treatment. Using the methodology of systematic reviewing and meta-analysis, the study aimed to synthesize and compare the diagnostic performances of all the available isothermal assays for the detection of classical rickettsial diseases. METHODS: Studies were retrieved from PubMed and Scopus, and selection and screening were conducted using pre-

Scrub typhus in pregnancy: A 10-year multicenter study in resource-limited settings in China.

BACKGROUND: Despite its association with high mortality rates and negative fetal outcomes, large-scale epidemiological studies on scrub typhus (ST) during pregnancy remain scarce. METHODS: We conducted a retrospective, multicenter study by collecting 260 pregnant women with ST in China across a 10-year time period to evaluate how clinical characteristics changed over this time and identify risk factors for poor fetal outcome. RESULTS: In total, 78.5% (n = 204) presented with pathognomonic eschars or ulcers, and 75.4% (n = 196) had Weil-Felix test, in which 46.4% (n = 91) had a titer of ≥1:16

A CRISPR-Cas12a-based universal rapid scrub typhus diagnostic method targeting 16S rRNA of Orientia tsutsugamushi.

Scrub typhus is caused by Orientia tsutsugamushi infection and occurs frequently in an area called the Tsutsugamushi Triangle. Currently, there is no vaccine for O. tsutsugamushi, and its infection is treated with antibiotics such as doxycycline. Scrub typhus responds to effective treatment, and early treatment shortens the course of the disease, reduces mortality, and accelerates recovery. Therefore, it is important to rapidly diagnose O. tsutsugamushi infection to ensure successful outcomes. Here, we developed a CRISPR-Cas12a-based diagnostic method targeting the bacterial 16S rRNA to detect

A Novel Strain of Orientia tsutsugamushi Detected from Chiggers (Acari: Trombiculidae) on Wild Rodents.

Scrub typhus is caused by intracellular bacteria belonging to the genus Orientia. Until 2010, the endemic region was thought to be restricted to the Asia-Pacific region. Orientia species have recently been discovered in South America, Africa, Europe, and North America. In accordance with these circumstances, we tried to find new or novel bacterial strains in the Republic of Korea (ROK). We found that a new strain of O. tsutsugamushi formed a unique clade based on a 56-kDa type-specific antigen gene and showed 63.2-77.8% similarity to other strains of the same species. Additionally, we identifi

Scrub Typhus and Influenza A Co-Infection: A Case Report.

Scrub typhus, caused by Orientia tsutsugamushi, is a neglected and reemerging disease that causes considerable morbidity and mortality. It now extends beyond the Tsutsugamushi Triangle, the region wherein it has traditionally been endemic. Influenza has also resurged since the infection control measures against COVID-19 were relaxed. A few cases of scrub typhus and influenza co-infection have been reported. Herein, we report the case of a 74-year-old woman with fever and upper respiratory symptoms diagnosed with influenza A and treated with oseltamivir; however, her fever persisted, and she de

Scrub Typhus Infection Precipitating Hemolysis in a Patient With G6PD Deficiency: A Case Report.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a well-known red blood cell enzymopathy and a cause of intravascular hemolysis. This case report presents a child with underlying G6PD deficiency who experienced an acute episode of extensive intravascular hemolysis induced by a scrub typhus infection. The key takeaway from this report is that scrub typhus infection can trigger extensive hemolysis in patients with even "mild" G6PD deficiency, and normal G6PD levels found during the acute phase of hemolysis do not rule out the possibility of underlying G6PD deficiency.
