Outcomes of acute kidney injury in critically ill adults with tropical acute febrile illness: Prospective observational study.

BACKGROUND: Tropical acute febrile illness (TAFI) is a major illness with a large number of intensive care unit (ICU) admissions in tropical regions. It is frequently complicated by acute kidney injury (AKI) and associated poor outcomes. The data on the predictors of outcomes, in terms of in-hospital mortality and renal outcomes, amongst critically-ill TAFI patients from our region remain limited. AIM: To evaluate the clinical profile and outcomes of AKI amongst critically-ill TAFI patients and to identify the factors associated with in-hospital mortality. METHODS: This prospective observati

Ruxolitinib combined with azithromycin for scrub typhus-associated hemophagocytic lymphohistiocytosis in a child: a case report and narrative literature review.

BACKGROUND: Scrub typhus-associated hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening complication in children, with reported mortality of 11.9%-30%. Conventional immunomodulation with corticosteroids and intravenous immunoglobulin often provides insufficient control of the hyperinflammatory state, while etoposide-based chemotherapy carries significant toxicity. JAK1/2 inhibition targeting the interferon-gamma pathway represents a promising therapeutic strategy, but its application in scrub typhus-associated HLH has not been previously reported. CASE PRESENTATION: A 5-ye

Small-molecule inhibition of the Orientia tsutsugamushi deubiquitylating enzyme OtDUB impairs bacterial reproduction.

Scrub typhus is a potentially fatal infectious disease caused by the obligate intracellular bacterium Orientia tsutsugamushi . While antibiotic treatment is generally effective, it requires extended treatment, and drug resistance and treatment failures have emerged. O. tsutsugamushi encodes a deubiquitylating enzyme, OtDUB, which interferes with host ubiquitin-dependent pathways. OtDUB cleaves ubiquitin from various substrates, but whether this activity can be selectively targeted by small molecules is unknown. Here we have screened a chemically diverse small-molecule library using a fluoresce

Transformation and allelic exchange in Orientia tsutsugamushi.

Orientia tsutsugamushi is a mite-transmitted obligate intracellular bacterium that causes the potentially deadly zoonosis, scrub typhus. The absence of genetic tools for Orientia have limited studies of the microbe-host interactions that underlie scrub typhus. To address this gap, we developed a protocol for transforming and achieving allelic exchange in O. tsutsugamushi str. Ikeda. From evaluating multiple cell lines and antibiotics, we found that contact-inhibited EA.hy926 human endothelial-like cells best supported Orientia replication and that chloramphenicol was an effective selection mar

Eschar, enigma, and encephalopathy: a case series of atypical and fatal presentations of scrub typhus in young adults from Northern India.

Scrub typhus is a re-emerging but often underdiagnosed tropical zoonosis in India. Its clinical heterogeneity, serological overlap with other endemic infections, and frequent absence of pathognomonic signs contribute to delayed diagnosis and high complication rates. Central nervous system, pulmonary, and renal involvement signify severe disease and are associated with considerable morbidity and mortality. We describe three cases of previously healthy young adult males (ages 24-32) who presented with severe, organ-threatening complications of scrub typhus: acute encephalitis syndrome (AES), acu

Mapping Scrub Typhus Across Multiple Hosts in India.

Scrub typhus is a significant public health concern in India. Despite numerous studies on its prevalence across hosts and vectors, country-specific data remain unavailable. This systematic review and meta-analysis provide comprehensive insights into the prevalence of scrub typhus across humans, animal hosts, and vectors to aid disease control programs. The PECOS framework was used to identify eligible studies. A systematic search across six databases (PubMed, Scopus, Web of Science, Google Scholar, Krishikosh, and Shodhganga) was conducted in March 2024. Data were classified by diagnostics and

Advancing the diagnosis of scrub typhus with targeted next-generation sequencing.

OBJECTIVE: Scrub typhus has nonspecific symptoms, making it easy to miss or underdiagnose. This retrospective study evaluated targeted next-generation sequencing (tNGS) for the early diagnosis of acute Orientia tsutsugamushi infection in terms of feasibility and application value. METHODS: This retrospective study included 49 patients with suspected scrub typhus as the case group. Among them, 28 patients with eschar or ulcers were assigned to the clinical diagnosis group. The remaining 21 patients without eschar or ulcers formed the clinically suspected group. Additionally, 48 patients with n

Droplet digital PCR-based nucleic acid amplification test for improved molecular and clinical diagnosis of severe fever with thrombocytopenia syndrome patients.

Severe fever with thrombocytopenia syndrome (SFTS) is caused by SFTS virus (SFTSV) and is characterized by high mortality and non-specific clinical manifestations, which make early and accurate molecular diagnosis essential. This study aims to investigate whether reverse transcription droplet digital PCR (RT-ddPCR) provides improved diagnosis of SFTS compared to existing molecular diagnostic platforms. Detection limits of RT-ddPCR, reverse transcription quantitative PCR (RT-qPCR), and reverse transcription nested PCR (RT-nested PCR) were determined using RNA extracted from metrologically valid

Genomic insights into emerging South Korean uncommon Orientia tsutsugamushi strains and their value as new surveillance resources.

OBJECTIVES: Orientia tsutsugamushi, the causative agent of scrub typhus, shows strain diversity and a highly repetitive genome. In South Korea, strain Boryong predominates, whereas non-Boryong strains remain under-characterized at the whole-genome level. This study performed whole-genome sequencing and comparative genomic analysis of two patient-derived less-common South Korean strains, Chuncheon17-2 and strain Chuncheon17-13. METHODS: Archived isolates were reactivated and confirmed by 56-kDa TSA PCR and sequencing. Hybrid sequencing was performed using PacBio HiFi and Illumina platforms. Ge
