Evaluation and Management of Post-Cardiotomy Syndrome.

Post-cardiac injury syndrome (PCIS) encompasses inflammatory pericardial syndromes occurring after cardiac injury, including post-pericardiotomy syndrome (PPS) following cardiac surgery, post-myocardial infarction (Dressler syndrome), and post-traumatic pericarditis. With the expanding use of cardiac surgical and interventional procedures, PCIS has become increasingly prevalent despite a marked decline in Dressler syndrome in the contemporary reperfusion era. The syndrome is believed to result from an immune-mediated inflammatory response to myocardial and pericardial injury, leading to perica

Bilateral Bloody Pleural Effusions With Discordant Cellular Patterns: A Case of Concurrent Parapneumonic Effusion and Dressler Syndrome.

Bilateral pleural effusions typically suggest systemic aetiologies; however, concurrent distinct localised pathologies are rare. A 73-year-old man presented with fever and cough. Chest imaging revealed pneumonia and bilateral pleural effusions developed during antibiotic treatment. The right effusion was neutrophil-dominant, consistent with parapneumonic effusion, while the left was lymphocyte-dominant, suggesting Dressler syndrome triggered by pneumonia-induced systemic inflammation. Both effusions were bloody, an appearance possibly exacerbated by the concomitant use of a direct oral anticoa

[Rare side effect in the treatment of bacterial endocarditis].

A patient went to the hospital due to persistent febrile temperatures. Positive blood cultures for Enterococcus faecalis as well as a vegetation on the mitral valve in the echocardiography were detected. Therefore the diagnosis of E. faecalis endocarditis was made. An appropriate antimicrobial combination therapy with amoxicillin and gentamicin was initiated. During the course of treatment, gentamicin was switched to ceftriaxone because of an acute kidney injury KDIGO stadium AKI 1. Furthermore a mechanical mitral valve replacement was performed. After a continuous 5-week antimicrobial therapy

Evolution of a Pseudoaneurysm Due to Microvascular Obstruction in Myocardial Infarction Assessed by Cardiac Magnetic Resonance.

BACKGROUND: Ventricular pseudoaneurysm (PsA) is a rare but serious complication after acute myocardial infarction (MI) that can lead to heart failure, arrhythmias, thromboembolism, and death. CASE SUMMARY: A PsA was diagnosed in a patient with an unrevascularized inferolateral MI. Cardiovascular magnetic resonance showed late gadolinium enhancement and microvascular obstruction, followed several weeks later by wall rupture and PsA formation. DISCUSSION: A left ventricular PsA should be differentiated from a true aneurysm and other post-MI complications such as ventricular septal defect and D

Partial versus Complete Sternotomy for Aortic Valve Replacement-Multicenter Study.

BACKGROUND: The benefits of minimally invasive techniques in cardiac surgery remain poorly defined. We evaluated the short- and mid-term outcomes after surgical aortic valve replacement through partial upper versus complete median sternotomy (MS) in a large, German multicenter cohort. METHODS: A total of 2,929 patients underwent isolated surgical aortic valve replacement via partial upper sternotomy (PUS, n = 1,764) or MS (n = 1,165) at nine participating heart centers between 2016 and 2020. After propensity-score matching, 1,990 patients were eligible for analysis. The primary end point was

Dressler's Syndrome as a Late Complication of Myocardial Infarction: A Case Report.

Dressler's syndrome, a rare complication of myocardial infarction, is a form of secondary pericarditis that typically develops within one to six weeks following the infarction. In this report, we present a case of a 68-year-old woman who was diagnosed with Dressler's syndrome after nine weeks from acute coronary syndrome. The patient first presented with an ill-tolerated episode of atrial fibrillation. Based on clinical, laboratory, and imaging findings, accompanied by a history of respiratory tract infections, an infection of an unknown origin was initially suspected. Lack of response to the

A case report of left circumflex stent infection and mycotic aneurysm: a rare but life-threatening complication of percutaneous coronary intervention.

BACKGROUND: Coronary stent infections are an uncommon but deadly complication of percutaneous coronary intervention. Mortality remains as high as 40-60% even with adequate treatment. We report such an interesting case of left circumflex stent (LCX) infection and mycotic aneurysm that was successfully managed with antibiotics and surgery. CASE PRESENTATION: A middle-aged man who underwent percutaneous coronary intervention (PCI) to the left circumflex artery four weeks prior was referred as a case of pyrexia of unknown origin, not responding to antibiotics, and colchicine started for suspected

Post-cardiac Injury Syndrome Following Catheter Ablation of Atrial Flutter: A Case Report.

A woman in her 70s presented to accident and emergency (A&E) with shortness of breath and fever following atrial flutter ablation. Initial investigations revealed a new onset of pleural and pericardial effusion with raised inflammatory markers. After systematically ruling out chest infection and heart failure, a diagnosis of post-cardiac injury syndrome (PCIS) was made. After a short course of steroids and colchicine, she showed significant improvement in her symptoms, and subsequent follow-up showed resolution of her pleural and pericardial effusion.

Postcardiac Injury Syndrome After Cardiac Surgery: An Evidence-Based Review.

Postcardiac injury syndrome (PCIS) serves as a comprehensive term encompassing a spectrum of conditions, namely postpericardiotomy syndrome, postmyocardial infarction (MI) related pericarditis (Dressler syndrome), and post-traumatic pericarditis stemming from procedures like percutaneous coronary intervention or cardiac implantable electronic device placement. These conditions collectively give rise to PCIS, triggered by cardiac injury affecting pericardial or pleural mesothelial cells, leading to subsequent inflammation syndromes spanning from uncomplicated pericarditis to substantial pleural

Post-Cardiac Injury Syndrome Following Permanent Pacemaker Implantation Presenting Exclusively As Massive Pleural Effusion: A Rare Occurrence.

Post-Cardiac Injury Syndrome (PCIS) akin to Dressler's syndrome is late-onset pericarditis that is triggered by the body's immune system and presents commonly as pleuro- pericardial symptoms and raised inflammatory markers. Its occurrence following the insertion of a pacemaker has been reported infrequently and varies in different studies with an estimated prevalence of 1-2%. Our case reports a unique incidence of isolated pleural effusion following the implantation of a pacemaker in a 62-year-old female with complete heart block with no evidence of pericardial effusion on imaging. Post-cardiac Injury Syndrome Following Permanent Dual-Chamber Pacemaker Implantation.

Post-cardiac injury syndrome is a heterogeneous group of conditions that result from autoimmune-mediated inflammation of the pericardium, epicardium, and myocardium. Interventions such as pacemaker lead insertions, percutaneous coronary interventions, radiofrequency ablations, cardiac surgeries, and Swan-Ganz catheterizations can cause myocardial injury leading to post-traumatic pericarditis. This phenomenon can lead to chest pain, recurrent effusions, and fever along with possible complications of heart failure, arrhythmias, conduction abnormalities as well as cardiac tamponade. Herein, we pr

Dressler Syndrome: Not Just a Relic of the Past.

Dressler syndrome, also known as post-myocardial infarction syndrome, is a condition in which sterile pericarditis develops shortly after myocardial injury. It is characterized by pericardial inflammation typically including pericardial effusion, pleuritic chest pain, and elevated inflammatory markers. While its incidence has greatly decreased in the modern era of coronary revascularization, it remains a clinically important entity with the potential for serious morbidity. Here we present a case of presumed Dressler syndrome in a 54-year-old male who presented to the emergency room with a recu

Persistent ST Segment Elevation After Repeated Percutaneous Coronary Intervention: A Dressler Syndrome?

In the era of percutaneous coronary intervention (PCI), Dressler syndrome has become an extremely rare phenomenon. Originally known as post-myocardial infarction syndrome, it is characterized by fever, pleuritic chest pain, and pericardial or pleural effusion after myocardial infarction. It is one of the sub-entities of post-myocardial infarction pericarditis (PMIP). A 62-year-old man presented with persistent chest pain and diffuse ST segment elevation even after repeated PCIs. This condition was accompanied by fever and bilateral pleural effusion upon chest X-ray. The patient showed improvem

Cardiac tamponade secondary to Dressler's syndrome.

A 56-year-old woman presented to hospital with chest pain. Following review and investigations in the medical assessment unit, she was diagnosed with costochondritis and discharged home. She represented 10 days later and was mottled and hypotensive with a high lactate, raised inflammatory markers, an acute kidney injury and bilateral loin pain. A CT of the thorax, abdomen and pelvis showed pleural effusions and a large pericardial effusion with features of cardiac tamponade on subsequent echocardiography. A pericardiocentesis was performed and she was admitted to intensive care for haemofiltra

Shockwave cardiac injury in thoracoabdominal gunshot wound.

INTRODUCTION AND IMPORTANCE: Cardiac injuries are associated with high mortality rates and most affected individuals succumb to their injuries before arrival to the hospital. Even though they have a higher fatality rate, penetrating cardiac injuries are relatively easy to diagnose and they have straightforward management protocols. On the other hand unexpected non-penetrating cardiac injuries, especially in haemodynamically stable patients, are not looked out for and can be difficult to diagnose. They may have a delayed presentation leading to poor patient outcomes. Clinicians should have a hi
