Short- and mid-term outcomes of surgical repair of acute type A aortic dissection and concomitant coronary artery bypass grafting or extracorporeal membrane oxygenation support.

OBJECTIVES: In this study, we aimed to retrospectively analyse the risk factors for failing to wean from CPB and short- and mid-term outcomes of surgical repair of acute type A aortic dissection (TAAD) and concomitant coronary artery bypass grafting (CABG) versus extracorporeal membrane oxygenation (ECMO) support in patients. METHODS: Two hundred and three patients were enrolled and categorized into the simple TAAD group (n = 106) or the complex TAAD group (n = 97). Following propensity score matching, 68 patients in the complex TAAD group were distributed between the CABG subgroup (n = 34) a

Unplanned coronary artery bypass graft in aortic root replacement†.

OBJECTIVES: Trouble with coronary artery reimplantation at aortic root replacement (ARR) may require unexpected coronary artery bypass graft (CABG). This study aims to elucidate the outcomes of such unplanned CABG during ARR. METHODS: This is retrospective study from 2 aortic centres that underwent ARR from 2004 to 2021. Planned CABG for atherosclerotic coronary artery disease were excluded, while other concomitant CABG were defined as 'unplanned'. Propensity score matching (PSM) was performed to compare patients who underwent ARR or ARR + Unplanned CABG alongside landmark analysis at 90 days

Multistage Hybrid Treatment of Aortic Aneurysm and Management of Postoperative Complications: A Case Report.

The Frozen Elephant Trunk (FET) is a surgical procedure developed for simultaneous repair of the aortic arch and the proximal descending thoracic aorta. Experience has shown its technical feasibility and good clinical results, although complications remain possible. Case Presentation: A 66-year-old male presented for evaluation due to findings from computed tomography angiography of aneurysms in the ascending aorta, aortic arch, and abdominal aorta. The patient had a history of a quadruple coronary artery bypass graft (CABG) and placement of two stents in the descending thoracic aorta, which w

Aortic root replacement with a mechanical valve conduit: A single-institutional experience.

BACKGROUND: There is a lack of consensus regarding the best strategy for aortic root replacement with current guidelines supporting the use of mechanical valves in younger patients when a valve repair is not possible. We sought to describe our institutional experience with mechanical aortic root replacement. METHODS: This was a descriptive study using an institutional database of aortic root replacements with a mechanical valve composite graft performed at our center from 2010 to 2022. All patients undergoing aortic root replacements with a mechanical valve were included. Patients undergoing

Surgical management of a patient with anomalous origin of the left circumflex coronary artery undergoing aortic and mitral valve surgery.

BACKGROUND: The anomalous origin of the left circumflex coronary artery is rare and, when isolated, typically has minimal pathological significance. However, it can cause damage or compression of the coronary artery during aortic and mitral valve surgery. CASE PRESENTATION: The patient was a 34-year-old male diagnosed with severe aortic regurgitation due to a bicuspid aortic valve following infective endocarditis at the mitral valve. He was referred to our hospital owing to worsening heart failure. Preoperative evaluation revealed a mitral valve aneurysm and an anomalous left circumflex coron

Aortic Root Reconstruction With Pledgeted Sutures in Patients With Acute Type A Aortic Dissection.

BACKGROUND: The aortic root is invariably dissected in patients with acute type A aortic dissection (ATAD). We have used an aortic root reconstruction technique using pledgeted sutures to reattach the dissected wall without surgical glue. We reviewed our 2-decade experience to evaluate the results of aortic root reconstruction with pledgeted sutures in ATAD. METHODS: All open ATAD repairs from our institution from January 1999 to December 2024 were reviewed, and early and late outcomes of supracoronary aortic repair with aortic root reconstruction were evaluated. Outcomes of adverse aortic ro

Acute Type A Aortic Dissection With an Anomalous Aortic Origin of the Right Coronary Artery: A Rare Entry Formation.

Anomalous aortic origin of a coronary artery (AAOCA) is a rare malformation that can lead to sudden death from obstruction of the coronary artery running between the aorta and the pulmonary trunk. In cases of acute aortic dissection with AAOCA, exceptional coronary reconstruction may be required during aortic root reconstruction. This report presents a surgical case of acute aortic dissection with AAOCA in a 75-year-old woman. Aortic root reconstruction was necessary as a result of a ruptured aortic root dissection. A tear in the intramural segment of the right coronary artery required an aort

The Endo-Bentall Procedure in the Treatment of Ascending Aorta Pathologies: A Scoping Review.

OBJECTIVE: The ascending aorta remains a challenging area for endovascular repair, with limited anatomical suitability of current stent grafts due to proximal landing zone constraints. This scoping review summarised, existing experiences with the Endo-Bentall procedure, evaluated its use across pathologies, identified obstacles, and proposed future advances. DATA SOURCES: PubMed, Cochrane, and Embase were systematically searched (English language, up to 30 November 2024) for studies evaluating the Endo-Bentall procedure in treating ascending aortic pathologies. REVIEW METHODS: The Preferred

Are high-dose CT examinations on the rise?

OBJECTIVE: To analyse high-dose CT examinations (≥50 mSv) over a 10-year period to study temporal changes and identify causes. METHODS: CT dose provided by the dose management system (Radimetrics) was assessed for the years 2013-2022. CT protocols that led to ≥50 mSv exams were identified. Using body mass index (BMI), patients were classified into underweight, normal weight, overweight, and obese, and correlation with dose was studied. Stratified analysis was performed for diagnostic and interventional examinations according to procedure type, patient age, and protocol. RESULTS: Over 10 year

Giant right coronary artery aneurysm with associated dissection: diagnosis and surgical management.

Coronary artery aneurysms are defined as a focal dilatation exceeding 1.5 times the diameter of the adjacent normal segment. They are found in up to 5% of patients undergoing coronary angiography. Giant coronary artery aneurysms (GCAAs), typically defined as >20 mm in diameter, with an incidence of ⁓0.02%. We present the case of an 82-year-old female who underwent coronary artery bypass grafting due to a giant right coronary artery aneurysm measuring 85 mm, identified on preoperative contrast-enhanced computed tomography (CT). The patient exhibited symptoms consistent with pericardial irritati

Inflammation, abdominal aortic aneurysm enlargement and rupture. Lessons learned from the Covid19 pandemic.

Patients with moderate-severe COVID19 infection suffer from several cardiovascular diseases: heart failure (3 %-33 %), myocardial ischemia (0.9 %-11 %), ventricular dysfunction (10 %-47 %), arrhythmias (9 %-17 %), venous thrombo-embolism (25 %) and arterial thrombosis (1 %-3 %). Although intracranial and coronary arterial aneurysms have been described in adults and children with COVID19, few reports have correlated COVID19 infection and sudden degeneration of aortic aneurysms and dissections. We analyzed the risk factor for enlargement and rupture of aortic aneurysms in patrients with moderate

Personalized external aortic root support: The Dutch experience.

INTRODUCTION: Patients with connective tissue disease and aortic root aneurysm are at risk of dissection and progression of the dilatation. Standard treatment of aortic root aneurysm is either a total root replacement or valve-sparing root replacement. Both procedures have drawbacks, such as the need for anticoagulation and long-term exposure to the chance of reoperation or endocarditis. The ExoVasc personalized external aortic root support (PEARS) is a perivascular stent that has been introduced to reduce these risks while aiming for a similar beneficial outcome on late aortic complications.

Impact of the frozen elephant trunk technique in operation for acute type A aortic dissection: from the Japan cardiovascular surgery database.

OBJECTIVES: We compared early outcomes of total arch repair (TAR) and ascending aorta replacement (AAR) for acute aortic dissection type A (AADA) using the nationwide Japan Cardiovascular Surgery Database (JCVSD) and examined the role of the frozen elephant trunk (FET) technique in TAR. METHODS: Of 17,075 JCVSD patients undergoing urgent or emergent conventional surgery for AADA in 2013-2016, redo cases and those undergoing valve/root or coronary surgery were excluded (included n = 12,288). Four propensity score-matched analyses for treatment with AAR or TAR (n = 4272 each) were established.

Sandwich Technique for Iliac Aneurysm Rupture After Treatment of Iatrogenic Type B Aortic Dissection: A Case Report.

AIM: Type B iatrogenic acute aortic dissection (IAAD) is a rare complication of diagnostic or interventional cardiac procedures. The STent Assisted Balloon Induced intimaL dISruption and rElamination in aortic dissection repair (STABILISE) technique is being increasingly used for the treatment of complicated aortic dissections. However, hemodynamic changes and the pre-existence of aneurysmal arteries could lead to "unexpected" complications. CASE PRESENTATION: This case report shows how rescue techniques can be employed in response to such challenging handling situations, especially in emerge

Aortic Arch Pseudoaneurysm Causing Acute Coronary Artery Occlusion and Cardiogenic Shock in a Postoperative Patient Presenting to the Emergency Department.

BACKGROUND: Pseudoaneurysm is one complication of type A aortic dissection (TAAD) arch repair. It occurs in 10-24% of patients after TAAD repair and typically warrants elective operative intervention in hemodynamically stable patients. CASE REPORT: We present a rare case of a patient with a pseudoaneurysm compressing the left coronary artery after repaired TAAD who presented to the emergency department (ED) in cardiogenic shock. Given hemodynamic instability, resuscitation began in the ED and the patient was managed nonoperatively with salvage maneuver revascularization in the cardiac cathete

Successful Tip Detection-Antegrade Dissection and Re-Entry for Chronic Iatrogenic Coronary Artery Dissection Using a 6Fr Guiding Catheter: A Technical Challenge Overcome.

A 57-year-old man presented with recurrent exertional angina 1 month after an unsuccessful PCI attempt elsewhere. Angiography revealed severe stenosis and a chronic dissection of the posterior lateral artery (PLA). PCI was performed via a 6 Fr right transradial guiding catheter. Initial intravascular ultrasound (IVUS) using OptiCross HD (Boston Scientific; 3.1 Fr outer diameter [OD], 20 mm transducer-to-tip distance [TTD]) confirmed guidewire placement in the subintimal space within the extensive chronic dissection. TD-ADR was successfully performed using the OptiCross HD IVUS and a low-profil
