Clinical impact of combined assessment of myocardial inflammation and fibrosis using myocardial biopsy in patients with dilated cardiomyopathy: a multicentre, retrospective cohort study.

BACKGROUND: Among patients with dilated cardiomyopathy (DCM), myocardial inflammation and fibrosis are risk factors for poor clinical outcomes. Here, we investigated the combined prognostic value of these two factors, as evaluated using myocardial biopsy samples. METHODS: This retrospective and multicentre study included patients with DCM-defined as LVEF of ≤45% and left diastolic diameter of >112% of predicted value, without evidence of secondary or ischaemic cardiomyopathy. In myocardial biopsy samples, inflammatory cells were counted using immunohistochemistry, and Masson's Trichrome stain

Outcomes of ventricular tachycardia ablation facilitated by pre-procedural cardiac imaging-derived scar characterization: a prospective multi-centre international registry.

AIMS: Pre-procedural imaging can facilitate scar-related ventricular tachycardia (VT) ablation, although only limited data have been reported. This prospective registry aimed to analyse procedural data and outcomes in a multi-centre setting of a pre-defined VT ablation strategy facilitated by the integration of pre-procedural imaging into the navigation system. METHODS AND RESULTS: Consecutive patients referred for scar-related left-sided VT ablation were prospectively enrolled at five European tertiary hospitals. Pre-procedural cardiac magnetic resonance (CMR)-derived scar maps and/or multi-

Non-ST elevation myocardial infarction from aortic root thrombus after LVAD placement.

Aortic root thrombosis (ART) is a rare but serious complication following the placement of continuous-flow left ventricular assist devices (CF-LVAD). We present the case of a patient with idiopathic non-ischaemic cardiomyopathy who developed a non-ST elevation myocardial infarction (NSTEMI) secondary to ART after CF-LVAD placement. The patient was treated conservatively with antiplatelet therapy and increased international normalized ratio (INR) goal with resolution of thrombus on repeat imaging. ART may be an under-recognised complication in patients with CF-LVAD. Stasis of blood at the aorti

Modulation of cell fate by shock wave therapy in ischaemic heart disease.

AIMS: Cardiac shockwave therapy (SWT) improves left ventricular (LV) function in patients with ischaemic cardiomyopathy. Shockwave therapy activates Toll-like receptor 3 (TLR3), a receptor-inducing chromatin remodelling and nuclear reprogramming of cardiac cells. We hypothesized that mechanical activation of TLR3 facilitates reprogramming of fibroblasts towards endothelial cells restoring myocardial perfusion and function. METHODS AND RESULTS: Human cardiac fibroblasts were treated by mechanical stimulation via SWT or TLR3 agonist Poly(I:C) in the presence of endothelial induction medium. A l

Prevalence of ventricular parasystole in patients with cardiac sarcoidosis: correlation between parasystole and inflammation in ventricular fibrillation.

BACKGROUND: Ventricular parasystole is strongly associated with ventricular fibrillation (VF) in patients with non-ischaemic cardiomyopathy. However, the relationship between ventricular parasystole and cardiac sarcoidosis (CS) remains unclear. The purpose of this study was to examine the prevalence of parasystole in patients with CS. METHODS: This was a retrospective observational study of 214 consecutive patients diagnosed with CS (mean age: 69±12 years, 104 males, median follow-up period: 6.8 years (IQR: 3.2-10.7) in our centre. We investigated parasystole in the patients who developed ven

A Mendelian randomization analysis of cardiac MRI measurements as surrogate outcomes for heart failure and atrial fibrillation.

BACKGROUND: Drug development and disease prevention of heart failure (HF) and atrial fibrillation (AF) are impeded by a lack of robust early-stage surrogates. We determined to what extent cardiac magnetic resonance (CMR) measurements act as surrogates for the development of HF or AF. METHODS: Genetic data were sourced on the association with 21 atrial and ventricular CMR measurements. Mendelian randomization was used to determine CMR associations with AF, HF, non-ischaemic cardiomyopathy (NICM), and dilated cardiomyopathy (DCM), noting that the definition of NICM includes DCM as a subset. Microaxial pump-supported coronary surgery without CPB to optimize outcome in severely impaired left ventricles.

AIMS: Cardiopulmonary bypass (CPB) is the standard approach for coronary artery bypass grafting (CABG) in advanced ischaemic cardiomyopathy. Microaxial pump support has been envisioned to allow for beating-heart CABG without CPB (MPCAB), thereby avoiding CPB-inherent complications. This study aims to compare the in-hospital and follow-up outcome of MPCAB versus CPB-CABG in patients with severely impaired left ventricular function. METHODS AND RESULTS: Eleven patients suffering from three-vessel coronary artery disease with median ejection fraction of 27% and deemed appropriate for CABG accord

Proteome Alterations in Cardiac Fibroblasts: Insights from Experimental Myocardial Infarction and Clinical Ischaemic Cardiomyopathy.

Ischaemic heart disease (IHD) is a chronic condition that can cause pathological cardiac remodelling and heart failure (HF). In this study, we sought to determine how cardiac fibroblasts were altered post-experimental myocardial infarction (MI). Female C57BL6 mice underwent experimental MI by permanent left coronary artery ligation. Cardiac fibroblasts were isolated from extracted heart tissue of experimental MI mice and subsequently treated with the pro-fibrotic cytokine, TGF-β, for 24 h and analysed using high throughput LC-MS/MS analysis. Findings were validated using mass spectrometry data

Primary prevention ICD in non-ischaemic cardiomyopathy: an ongoing search for improvement of current indications : A retrospective study analysing the impact of the new Dutch guideline on the use of ICDs.

INTRODUCTION: Patients with non-ischaemic cardiomyopathy (NICMP) have a class IIa primary prevention indication for an implantable cardioverter-defibrillator (ICD). Recent studies have shown that the evidence for a survival benefit following ICD implantation in this patient group is not particularly robust. In 2023, the Dutch Society of Cardiology published an update of the ESC guideline to better select patients with NICMP for ICD implantation. The objective of this study was to analyse the impact of this guideline on the number of indications in a retrospective cohort of patients who had rec

Diagnostic value and clinical impact of cardiac magnetic resonance imaging in patients after sudden cardiac arrest: a retrospective study.

INTRODUCTION: Cardiac MRI (CMRI) is an important investigation in cases of unclear cause of sudden cardiac arrest (SCA). It demonstrates diagnostic utility in assessing reversibility and tissue scar burden and ultimately aids in further treatment planning. METHODS: A retrospective analysis of all adult patients referred for CMRI after SCA between 2007 and 2022 by local intensive care units in our institution was performed. The patient cohort is highly selective, excluding those who did not reach the hospital, had cerebral oedema or had confirmed acute myocardial infarction as the cause of SCA

Obesity Phenotypes Causally Affect Cardiac MRI Structure and Induced Non-ischaemic Cardiomyopathy.

BACKGROUND: The growing obesity epidemic highlights the need to understand how various obesity phenotypes affect myocardial structure and increase non-ischaemic cardiomyopathy (NICM) incidence. The aim of this study was to investigate the causal effect of eight obesity-related traits on NICM and 16 cardiac MRI parameters. Potential mediators between obesity and NICM were also investigated. METHODS: Two-sample Mendelian randomisation was used to explore the causal relationship between eight obesity-related traits and NICM and assess their impact on cardiac MRI indicators. The study also used v

Pulsed field ablation of refractory alternating atrial fibrillation and atrial flutter using Impella CP in a patient with cardiogenic shock: a case report.

BACKGROUND: Supraventricular tachycardia (SVT) can lead to cardiogenic shock, particularly in patients with severely impaired left ventricular function. Acute management typically includes electrical cardioversion combined with antiarrhythmic drugs to restore sinus rhythm. In cases of therapy-resistant SVT, atrioventricular nodal ablation may be considered, although this results in permanent pacemaker dependency. Definitive treatments, such as atrial fibrillation (AF) or atrial flutter (AFLUT) ablation, which could potentially avoid pacemaker implantation, are seldom pursued in such critical s

Treat-and-repair strategy for atrial septal defect with left ventricular systolic dysfunction: a case report of cardiac resynchronization therapy responder.

BACKGROUND: The treatment strategy for closing atrial septal defect (ASD) in patients with left ventricular (LV) dysfunction remains to be elucidated. Current guidelines recommend a balloon occlusion test to determine whether the ASD should be closed, fenestrated, or not. CASE SUMMARY: A 56-year-old man was referred to our hospital for secundum ASD with LV dysfunction. He was diagnosed with non-ischaemic cardiomyopathy with LV ejection fraction of 24%. A secundum ASD with a diameter of 18 mm also existed, with a pulmonary blood flow to systemic blood flow ratio over 2.0. Initially, ASD closur

Hydroxychloroquine-Induced Cardiomyopathy in a Young Female With Systemic Lupus Erythematosus: A Case Report.

Hydroxychloroquine (HCQ) is widely used in the treatment of systemic lupus erythematosus (SLE) and other autoimmune diseases due to its immunomodulatory and anti-inflammatory properties. Although generally well-tolerated, rare cases of HCQ-induced cardiomyopathy have been reported, often leading to irreversible cardiac dysfunction. We present a case of a 21-year-old female with SLE and biopsy-proven class II lupus nephritis who developed reversible cardiomyopathy associated with HCQ use. Prompt discontinuation of HCQ and initiation of guideline-directed medical therapy (GDMT) resulted in norma
