Frailty and intraoperative hypotension in older adults undergoing hip fracture surgery: a retrospective cohort study.

BACKGROUND: Frailty is common among older adults and is associated with adverse postoperative outcomes. Hip fractures are increasingly prevalent in aging populations and frequently require urgent surgical intervention. Intraoperative hypotension (IOH) is a common perioperative complication associated with serious postoperative morbidity. However, the relationship between frailty and IOH remains unclear. This study aimed to investigate the association between frailty, assessed using the 11-item modified frailty index (mFI-11), and IOH in older adults undergoing hip fracture surgery. METHODS: T

Effects of LBBAP Pacing-Site Position under a Modelled Proximal LBBB: A 24-Model Electrophysiological Simulation Study.

Left bundle branch area pacing (LBBAP) may recruit the distal left-sided conduction
system, but the electrical effect of pacing-site position along the left bundle branch (LBB)
remains incompletely defined. We evaluated this effect in 24 four-chamber virtual heart
anatomies using a Reaction-Eikonal finite-element model with rule-based myocardial
fibres and algorithmically generated His-Purkinje networks. A modelled proximal His
to-LBB disconnection preserved the distal LBB-Purkinje network while interrupting its
proximal connection. Six conditions were simulated in

Cardiac Resynchronization Therapy Using a Conduction System Pacing Defibrillation Lead in Patients Meeting CRT-D Indications: Six-Month Clinical Outcomes Results From the ASCEND CSP Study.

BACKGROUND: Conduction system pacing (CSP) via left bundle branch area pacing (LBBAP) is a physiologic alternative to conventional biventricular (BiV) pacing for cardiac resynchronization therapy (CRT), but prospective data using purpose-built implantable cardioverter-defibrillator (ICD) leads are limited. OBJECTIVES: The purpose of this study was to evaluate procedural success, defibrillation efficacy, safety, and 6-month clinical outcomes of left bundle branch-optimized cardiac resynchronization therapy (LOT-CRT) delivered with a purpose-built CSP ICD lead and compare outcomes to convention

Abdominal Pain as the First Presentation of Severe Heart Failure With Reduced Ejection Fraction and Left Ventricular Thrombus in a Young Asymptomatic Patient: A Diagnostic Interception.

A young, physically active patient with no cardiac symptoms presented with abdominal pain and was found, through incidental findings of elevated BNP, cardiomegaly, and new ECG changes, to have severe HFrEF with a left ventricular apical thrombus. Early cardiac evaluation changed management and led to complete thrombus resolution.

Antenatally detected right ventricular outflow tract aneurysm with neonatal ventricular arrhythmia.

A large right ventricular outflow tract aneurysm in a fetus was incidentally detected during a routine third-trimester ultrasound of a 30-year-old pregnant woman. The pregnancy itself was otherwise uneventful, and the infant was delivered at term. Postnatal echocardiography confirmed the presence of the right ventricular outflow tract aneurysm. Initially, the neonate was stable; however, he developed frequent ventricular premature complexes, with electrocardiographic features indicating a right ventricular outflow tract origin. Early initiation of amiodarone therapy led to a prompt resolution

Cross-Domain Chest Radiograph Report Generation Under Dataset Shift: Bidirectional Audit Study.

BACKGROUND: Cross-domain chest radiograph report generation may produce formally valid reports that are nevertheless repetitive, weakly case-specific, or missing clinically relevant findings. These failures can be missed when evaluation relies on a single language-overlap or clinical-label score. OBJECTIVE: This study aimed to evaluate cross-domain report generation with a bidirectional layered audit, characterize direction-specific and end point-specific failure profiles under a composite training-side control and 3 decoding policies, and explore whether selected audit dimensions remained in

Immune Checkpoint Inhibitor Myocarditis on FDG PET/CT and Cardiac MR.

Immune checkpoint inhibitor-related myocarditis is one of the rare immune-related adverse effects, but with disproportionately high mortality. A 54-year-old man with stage 4 squamous cell carcinoma on pembrolizumab presented with symptoms of heart failure. FDG PET/CT revealed new cardiomegaly with intense myocardial uptake. Contrast-enhanced cardiac MRI performed concurrently demonstrated myocardial edema and gadolinium enhancement, consistent with myocarditis. The specific etiology of this case of myocarditis was further substantiated by the resolution of cardiomegaly and abnormal myocardial

Discharge NT-proBNP predicts 2-year outcomes after TAVR but not after SAVR in preserved ejection fraction: a propensity-matched cohort study.

BACKGROUND: Discharge N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a recognised prognostic marker after transcatheter aortic valve replacement (TAVR), but its value after surgical aortic valve replacement (SAVR) remains uncertain, possibly reflecting differences in perioperative physiology. AIMS: To compare the prognostic utility of discharge NT-proBNP after TAVR versus SAVR in patients with severe aortic stenosis and preserved ejection fraction, and to examine the association between NT-proBNP reduction and Left Ventricular Mass Index (LVMI) regression. METHODS: This propensity-

DNA methylation analysis of aortic wall tissue in ascending aortic dilatation/aneurysm and dissection.

Ascending aortic dilatation/aneurysm and dissection are life-threatening cardiovascular conditions. However, the epigenetic regulatory mechanisms within aortic cells underlying these pathologies remain poorly understood. This study aimed to identify differentially methylated genes (DMGs) in aortic tissues from patients with ascending aortic dilatation/aneurysm and dissection by performing a re-analysis of publicly available DNA methylation datasets. A re-analysis was conducted on three DNA methylation datasets (GSE50192, GSE202047, GSE84274) from the Illumina Infinium HumanMethylation450 BeadC

Recurrent Large-Vessel Occlusive Stroke Requiring Repeated Mechanical Thrombectomy in Severe Dilated Cardiomyopathy With Extremely Low Left Ventricular Ejection Fraction and HIV Screening Positivity: A Case Report.

Severe dilated cardiomyopathy with markedly reduced left ventricular ejection fraction can rarely present as recurrent large-vessel occlusive stroke. This case demonstrates that timely mechanical thrombectomy and sustained anticoagulation are crucial for neurological recovery and secondary prevention, while HIV screening positivity adds complexity but should not independently preclude reperfusion therapy.

Perioperative Management of a Toddler With Tuberous Sclerosis and Dilated Cardiomyopathy Undergoing Open Abdominal Aortic Aneurysm Repair: A Case Report.

Toddler abdominal aortic aneurysms (AAA) are rare and lack defined perioperative guidelines. Aneurysms may be associated with tuberous sclerosis complex (TSC) but rarely occur with concurrent dilated cardiomyopathy. This case report describes the anesthetic and surgical considerations for a two-year-old male with TSC and dilated cardiomyopathy with low ejection fraction undergoing open infrarenal AAA repair. Our most important perioperative considerations included advanced hemodynamic monitoring, management of intraoperative blood loss, and the physiological changes associated with aortic clam

Case Report: When cardiomyopathy is the clue: late-presenting propionic acidemia manifesting as acute heart failure.

BACKGROUND: Propionic acidemia (PA) is a rare inborn error of metabolism that typically presents in infancy; however, late-onset variants may manifest with atypical features, including cardiac involvement. Dilated cardiomyopathy (DCM) is an uncommon but serious complication. CASE SUMMARY: A 15-year-old with no prior medical history but a significant maternal family history of cardiomyopathy of unknown etiology from an Amish background presented with altered mental status, increased work of breathing, and progressive edema in the context of preceding viral illness. Evaluation revealed severe b

Prognostic Value of Late Gadolinium Enhancement Findings in Isolated Left Ventricular Dilatation and Dilated Cardiomyopathy.

BACKGROUND: Non-ischemic dilated cardiomyopathy (DCM) and isolated left ventricular dilatation (ILVD) are defined as left ventricular (LV) dilatation, with reduced or preserved left ventricular ejection fraction (LVEF), respectively. We aimed to assess the prognostic value of late gadolinium enhancement (LGE) findings, including LGE extent, location, and number of areas to predict all-cause death in ILVD and DCM, separately. METHODS: Between 2008 and 2021, we conducted a bicentric retrospective study including all consecutive ILVD and DCM patients referred for cardiovascular magnetic resonanc

Risk stratification in early non-ischaemic cardiomyopathy: the interplay of genetics and cardiac imaging.

Preventing sudden cardiac death (SCD) in early non-ischaemic cardiomyopathy (NICM) remains a major clinical challenge. Traditional risk stratification centred on left ventricular ejection fraction (LVEF) poorly captures arrhythmic risk, particularly in early dilated cardiomyopathy (DCM) and non-dilated left ventricular cardiomyopathy (NDLVC). Increasing recognition of fibrosis-based and genotype-specific arrhythmogenic substrates has prompted a shift towards multimodal risk assessment strategies integrating imaging, electrocardiographic, and genetic data. This review examines contemporary appr

Decoding Atrial Fibrillation Heterogeneity: A Nationwide Phenome-Wide Association Study in England.

BACKGROUND: Age and sex influence atrial fibrillation (AF) prevalence and outcomes, yet comprehensive data on sex- and age-specific health care utilization in patients newly diagnosed with AF remain limited. OBJECTIVES: We aimed to address this gap using a phenome-wide association study (Phe-WAS) approach. METHODS: We conducted a retrospective cohort study using nationwide Clinical Practice Research Datalink data linked to Hospital Episodes Statistics. Adults diagnosed with AF between 1998 and 2016 (n = 199,433) were matched 1:1 by age and sex to non-AF control subjects. Primary and secondar

Silent Ears, Failing Heart: Dilated Cardiomyopathy in an Infant With LARS2-Related Perrault Syndrome Type 4 (PRLTS4).

We report a 19-month-old girl with molecularly confirmed Perrault syndrome type 4 (PRLTS4), due to a homozygous LARS2 variant (c.457A>C, p.Asn153His), who developed dilated cardiomyopathy (ejection fraction 21%) at 2 months of age despite a normal neonatal echocardiogram, alongside bilateral profound sensorineural hearing loss and global developmental delay. No second causative cardiac variant was identified. To our knowledge, this is the first reported case of dilated cardiomyopathy in molecularly confirmed LARS2-related PRLTS4, expanding the recognised phenotypic spectrum of this disorder an

Heart-reactive antibody levels differentially associate with ventricular function in hereditary cardiomyopathies.

BACKGROUND: gCMP are a major cause of cardiovascular morbidity and mortality. While agonistic anti-β1-adrenergic receptor antibodies (anti-β1AR-ECII) have a recognized pathogenic role in idiopathic dilated cardiomyopathy (DCM), the relevance of HRA in gCMP remains unclear. METHODS: One hundred gCMP patients with confirmed genetic variants in cardiomyopathy genes were compared with 100 age- and sex-matched healthy individuals. Antibody reactivities were quantified using bead-based flow cytometric assays against cardiac antigens: The second extracellular loop of the β1 adrenergic receptor (β1AR
