The durability of different anchoring points and anchoring types in the left ventricle in an ex vivo porcine model.

OBJECTIVE: Surgical mitral valve repair, including anchoring in different parts of the mitral apparatus, is the standard of care for severe mitral regurgitation. Various percutaneous approaches have been developed and are currently under development, but are struggling with durable results. This study investigates the durability of different anatomic anchoring points and anchoring types in the left ventricle. MATERIAL AND METHODS: Anchoring was tested with PTFE sutures or a 5 mm helix at three locations: the head of the papillary muscle, the lateral wall of the ventricle, and the apex of the

Interobserver Agreement in Transthoracic Echocardiography between Cardiology Residents and Attending Cardiologists: A Cross-Sectional Study.

BACKGROUND: Transthoracic echocardiography (TTE) is essential for making clinical decisions regarding cardiovascular disorders. Cardiology trainees frequently conduct and interpret TTE examinations; however, discrepancies between their interpretations and those of attending cardiologists can impact patient management. The objective of this study was to assess the concordance between cardiology trainees and attending cardiologists in the interpretation of TTE findings. METHODS: This study included 374 inpatients and outpatients referred to for TTE examinations. TTE studies were independently p

Radiographic Comparison of Pulmonary Lobar Vasculature Dimensions in Medically Managed and Drug-Naive Dogs With Left Heart Failure Secondary to Myxomatous Mitral Valve Disease.

Myxomatous mitral valve disease (MMVD) is the most common acquired cause of left heart failure (LHF) in dogs, and radiographic identification of pulmonary venous enlargement is traditionally used to support the diagnosis of LHF. However, the absence of venous enlargement has been anecdotally observed after initiation of therapy. Therefore, we aimed to compare radiographic pulmonary vessel size in LHF patients with ongoing medical management ± receiving acute rescue therapy. Dogs with echocardiographically confirmed MMVD and radiographic cardiogenic pulmonary edema were retrospectively selected

Automatic transesophageal echocardiographic evaluation of patients with mitral valve prolapse: comparison with patients without structural cardiac chamber remodeling (an exploratory study).

BACKGROUND: There is a paucity of information concerning automatic mitral valve apparatus (MVA) analysis of patients with mitral valve prolapse (MVP). We aimed to study with an automatic three-dimensional (3D) transesophageal echocardiography (TEE)-dedicated software, patients with moderate and severe mitral regurgitation when compared to patients with no structural cardiac chamber alterations [patients with patent foramen ovale (PFO) who underwent TEE]. METHODS: We employed TEE software dedicated to automatic analysis of 34 parameters of the MVA, comparing MVP patients with moderate and seve

Identifying the High-Risk Arrhythmic Mitral Valve Prolapse Phenotype by Imaging.

Within arrhythmogenic mitral valve prolapse (AMVP), a distinct subset of mitral valve prolapse (MVP), there is a small group that carries a higher risk of malignant ventricular arrhythmias and sudden cardiac death called malignant MVP. The malignant MVP phenotype is over-represented in young women with unexplained sudden cardiac death on autopsy. AMVP is defined by myxomatous, often bileaflet MVP with mitral annular disjunction, abnormal valvular-myocardial mechanics, complex ventricular ectopy, and nonsustained ventricular tachycardia. Excessive mechanical force from systolic annular expansio

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Based on the modified World Health Organization (mWHO) classification, she was categorized as Class I

Definition and Prognosis of Mitral Regurgitation in Transthyretin Amyloid Cardiomyopathy: A Multicenter International Cohort Study.

BACKGROUND: In transthyretin amyloid cardiomyopathy, mitral regurgitation (MR) severity may be underestimated by conventional (semi-)quantitative echocardiographic criteria due to restrictive low-flow hemodynamics. We sought to define and validate disease-specific MR risk thresholds and compare their prognostic performance with guideline definitions. METHODS: In a multicenter cohort study (8 centers, 4 countries), 1101 patients with newly diagnosed transthyretin amyloid cardiomyopathy (derivation, n=722; validation, n=379) underwent standardized echocardiography with blinded core laboratory a

How does rewarming temperature affect bleeding after mitral valve repair?

ObjectiveSo far, evidence is scarce regarding the impact of rewarming temperature on bleeding after cardiac surgery under cardiopulmonary bypass (CPB). We seek to evaluate how rewarming temperature affects postoperative bleeding in patients undergoing mitral valve repair.MethodsClinical, anesthesia and CPB data were analyzed for 379 adults undergoing isolated mitral regurgitation repair. Platelet levels (1000/μL) were divided into three groups: low, 53-164 (n = 128); medium, 165-208 (n = 126); and high, 209-906 (n = 125). The primary endpoint was chest drainage volume (mL) at 24 postoperative

Clinical value of RT-3DE in evaluating mitral valve configuration and the occurrence of VFMR in patients with ischemic cardiomyopathy: a clinical investigation.

PURPOSE: This study used real-time three-dimensional echocardiography (RT-3DE) to obtain three-dimensional (3D) geometric parameters of the mitral annulus and leaflets, including annular structural indices, leaflet angles, and tenting volume (VTent). We further compared the remodeling patterns between atrial functional mitral regurgitation (AFMR) and ventricular functional mitral regurgitation (VFMR). In addition, RT-3DE combined with speckle tracking imaging (STI) was applied to evaluate structural and functional changes of the left ventricular (LV) and mitral valve (MV) in patients with isch

Risk of Atrial Fibrillation Recurrence After Modified Cox-Maze IV Surgery: Comparing Atrial Functional Mitral Regurgitation and Rheumatic Mitral Regurgitation.

BACKGROUND: The comparative efficacy of the modified Cox-Maze IV procedure (CMP IV) in treating atrial fibrillation (AF) in patients with atrial functional mitral regurgitation (AFMR) and those with rheumatic mitral regurgitation (RMR) remains inadequately characterized. Therefore, this study aimed to evaluate and compare postoperative AF recurrence rates and clinical outcomes between these two etiologically distinct groups. METHODS: A retrospective analysis was conducted on 221 patients (AFMR group: n = 53; RMR group: n = 168) with concomitant long-standing persistent AF who underwent mitral

Associations of Mild or Trace Valvular Abnormalities With Cardiac Remodeling in Older Adults: The ARIC Study.

BACKGROUND: The impact of mild/trace valvular abnormalities on cardiac remodeling is not well understood, despite their association with poor cardiovascular outcomes. METHODS: Using data from 4684 participants in the ARIC (Atherosclerosis Risk in Communities) Study (2011-2013), aged 66 to 90 years, we examined cross-sectional associations of mild/trace valvular abnormalities (aortic stenosis, aortic regurgitation, and mitral regurgitation) with cardiac parameters using multivariable linear regression models. Cardiac parameters included cardiac structure (eg, left ventricular mass index), syst

[Mitral valve disease: diagnosis and management revisited].

Mitral valve disease ranks among the most frequent valvular conditions and carries significant morbidity and mortality if left untreated. This review addresses anatomy, the Carpentier classification, and echocardiographic severity grading, as well as current management of primary and secondary mitral regurgitation (MR) and mitral stenosis. Transcatheter therapies, including edge-to-edge repair and transcatheter mitral valve replacement, receive particular attention and are discussed in the context of four clinical cases from a specialized valve center.

Regional strain patterns in patients with mitral valve prolapse.

PURPOSE: Echocardiographic studies have shown that patients with mitral valve prolapse (MVP) exhibit altered regional contraction patterns. With cardiovascular magnetic resonance (CMR) becoming increasingly important in the evaluation of global and regional cardiac function, we aimed to quantify the regional contraction patterns of patients with MVP using CMR feature tracking (FT) strain analysis in comparison to a control group. METHODS: Of all patients who underwent CMR scans from 2013 to 2022 at our institution, 132 patients with MVP were identified and compared to 50 controls without any
