When Infective Endocarditis Masquerades as Gastrointestinal Disease: A Case Report.

AIMS/BACKGROUND: Infective endocarditis (IE) is a serious and potentially life-threatening condition characterised by inflammation and infection of the endocardium. While cardiac manifestations are well-recognised, extracardiac manifestations of IE, particularly in the gastrointestinal tract, are less commonly reported. We report the first known case of IE presenting with colitis-like symptoms in the absence of identifiable colonic pathology. CASE PRESENTATION: A 34-year-old male patient presented to primary care with gastrointestinal symptoms, including fever, weight loss, and bloody diarrho

Preprocedural Three-Dimensional Vena Contracta Area in Ventricular Secondary Mitral Regurgitation: Association with Outcome after Mitral Valve Intervention.

OBJECTIVE: Increasing effective regurgitant orifice area (EROA) has been associated with poor outcomes in patients with ventricular secondary mitral regurgitation (VSMR). However, measuring EROA with two-dimensional (2D) echocardiography has important limitations. Overcoming most of these limitations, three-dimensional (3D) color Doppler transesophageal echocardiography allows direct assessment of the vena contracta area (VCA), whose prognostic value, however, has not been specifically investigated in patients with VSMR undergoing mitral valve intervention. METHODS: A total of 218 patients (6

Predictive Utility of EROA/LVEDV Ratio in Mitraclip Outcomes: Retrospective Multicenter Cohort Study.

Background: The effective regurgitant orifice area to left ventricular end-diastolic volume (EROA/LVEDV) ratio has been proposed to distinguish proportionate from disproportionate functional mitral regurgitation and to guide patient selection for transcatheter edge-to-edge repair (TEER). Methods: We conducted a multicenter, retrospective cohort study of 221 patients undergoing TEER with the Mitraclip system. Preprocedural echocardiographic parameters, including EROA, LVEDV, diastolic indices, and chamber volumes, were systematically collected. The primary outcome indicative of symptom worsenin

Comparative Analysis of Clinical, Echocardiographic, and Hemodynamic Features Across Functional Mitral Regurgitation Subtypes.

Functional mitral regurgitation (FMR) includes atrial (aFMR), ventricular (vFMR), or combined atrial and ventricular (avFMR) phenotypes. This study evaluated differences in clinical, echocardiographic, and invasive hemodynamic features among aFMR, vFMR, and avFMR in patients undergoing transcatheter edge-to-edge repair (TEER). We retrospectively analyzed patients who underwent TEER for significant FMR at the Cleveland Clinic from 2019 to 2023. FMR was diagnosed in patients with structurally normal mitral leaflets and regurgitation secondary to atrial or ventricular remodeling. aFMR was defined

Severe mitral regurgitation after aortic annular enlargement with Y-incision: a case report.

BACKGROUND: The Y-incision technique enables implantation of larger prosthetic valves by facilitating effective aortic annular enlargement. Although this approach improves hemodynamic outcomes, rare and clinically significant complications may occur and remain insufficiently described. CASE PRESENTATION: A patient undergoing surgical aortic valve replacement with concomitant Y-incision annular enlargement developed early postoperative severe central mitral regurgitation. Standard transthoracic and transesophageal echocardiography demonstrated abnormal aorto-mitral geometry and annular distort

Myectomy with secondary chordal resection in hypertrophic obstructive cardiomyopathy: the mid-term outcomes.

BACKGROUND: It is not clear whether septal myectomy with secondary chordal resection (SM-SCR) can achieve similar postoperative outcomes among hypertrophic obstructive cardiomyopathy (HOCM) patients exhibiting different severity of septal hypertrophy. METHODS: This was a single-center retrospective cohort study with a nested case-control study. HOCM patients received SM-SCR between 2014 and 2023 were retrospectively included and were grouped based on their septal thickness. The main outcome was the relief of the left ventricular outflow tract (LVOT) obstruction. Besides, predictors for recurr

Minimally Invasive, Echocardiography-Guided Mouse Model of Degenerative Mitral Regurgitation Recapitulates Cardiac Remodeling and Arrhythmogenesis.

BACKGROUND: Degenerative mitral regurgitation (MR) is a progressive valvular disorder that causes atrial enlargement, cardiac remodeling, heart failure, and arrhythmias. Current rodent models are limited by thoracotomy or intubation, which increase procedural risk and reduce reproducibility. METHODS AND RESULTS: We established a minimally invasive, transthoracic echocardiography-guided mouse model of MR by puncturing the anterior mitral leaflet with a 27G needle introduced through the left ventricular apex. Ten-week-old C57BL/6J mice underwent baseline imaging before MR induction, and sham co

Thrombotic mitral valve obstruction during venoarterial extracorporeal membrane oxygenation with left ventricular venting following complex valve surgery.

BACKGROUND: Left ventricular (LV) unloading during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is essential for managing refractory low cardiac output after complex cardiac surgery. However, direct LV venting is associated with an increased risk of intracardiac thrombosis, particularly when anticoagulation is delayed. CASE PRESENTATION: A 69-year-old woman with hypertrophic cardiomyopathy and severe left ventricular outflow tract obstruction presented with exertional dyspnea. Echocardiography showed severe mitral regurgitation with marked annular calcification, moderate aortic

Pushing the Limits of Balance in Atrioventricular Septal Defect and Complex Techniques for Valve Re-repair in Atrioventricular Septal Defect.

Repair of atrioventricular septal defect (AVSD) remains one of the most technically demanding procedures in congenital cardiac surgery. In patients with balanced anatomy, biventricular repair provides excellent long-term survival; however, borderline and unbalanced AVSD continue to pose major challenges, particularly in children with Down syndrome. Contemporary practice emphasizes that balance is not fixed but can be influenced by surgical planning, ventricular recruitment, and meticulous management of the left atrioventricular valve (LAVV). LAVV regurgitation remains the principal cause of ea

The 2026 American Association for Thoracic Surgery Expert Consensus Document: Management of atrial functional mitral regurgitation.

OBJECTIVE: Atrial functional mitral regurgitation (AFMR) represents a distinct pathophysiologic entity characterized by MR secondary to left atrial dilation and annular remodeling, in the absence of significant left ventricular disease. With the increasing prevalence of atrial fibrillation and heart failure with preserved ejection fraction, AFMR has emerged as an important and potentially underrecognized etiology of MR. This expert consensus statement provides an evidence- and experience-based framework for the evaluation and management of AFMR, encompassing diagnosis, medical and rhythm thera

Transcatheter edge-to-edge repair and left ventricular assist devices for secondary mitral regurgitation in advanced heart failure: a scoping review.

BACKGROUND: Secondary mitral regurgitation (SMR) worsens outcomes in advanced heart failure. Transcatheter edge-to-edge repair (TEER) and left ventricular assist device (LVAD) implantation represent advanced treatment options for patients with persistent symptoms despite guideline-directed medical therapy (GDMT). METHODS: A scoping review using a PICO framework identified contemporary studies published up to 2025 involving adults with advanced heart failure (NYHA III-IV, INTERMACS 1-7) and ≥moderate SMR. Outcomes included mortality, heart failure hospitalizations, functional status, mitral re

Transcatheter Mitral and Aortic Paravalvular Leak Closure: Innovations, Challenges, and Future Directions.

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos

Beyond the usual suspects: unmasking ALCAPA in pediatric cardiomyopathy - case report.

INTRODUCTION: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare but potentially lethal congenital heart disease. Delayed recognition may result in irreversible myocardial injury. Although the classic presentation occurs in infancy, atypical forms may appear in older children, making this entity a diagnostic challenge. CASE PRESENTATION: We report the case of a 9-year-old boy with a history of dilated cardiomyopathy (DCM) and poorly controlled asthma who was admitted for an asthma exacerbation. Transthoracic echocardiography revealed Left ventricular eje

Angiotensin Receptor Neprilysin Inhibitor in Heart Failure With Preserved Ejection Fraction and Secondary Mitral Regurgitation: The PRAISE-MR Randomized Trial.

BACKGROUND: Atrial functional mitral regurgitation (AFMR) characterizes a high-risk phenotype in heart failure with preserved ejection fraction (HFpEF). Although sacubitril/valsartan reduces functional mitral regurgitation (MR) in HF with reduced EF (HFrEF), its impact on exercise hemodynamics and the dynamic burden of AFMR in HFpEF remains to be elucidated. METHODS: This multicenter, randomized, open-label trial with blinded primary endpoint assessment assigned 84 patients with symptomatic HFpEF and at least moderate AFMR within the previous year to sacubitril/valsartan (n=41) or standard-of

Management Approach for Residual and Recurrent Mitral Regurgitation After Transcatheter Edge-to-Edge Repair.

Mitral valve transcatheter edge-to-edge repair (M-TEER) is an established treatment option for patients with severe mitral regurgitation (MR) at high risk for surgery. However, the rate of recurrent MR ≥ 3+ after M-TEER may be up to 10% at 1 year. As the volume of M-TEER procedures increases, the number of patients experiencing recurrent MR post-TEER is also rising. M-TEER reshapes valve anatomy and complicates further treatment strategies for MR. In this review, the authors provide practical management considerations for patients with residual or recurrent MR after M-TEER.

Phenotypic Diversity and Outcomes of Transcatheter Mitral Repair for Secondary Mitral Regurgitation: Insights From the COAPT and MITRA-FR Trials.

BACKGROUND: Mitral valve transcatheter edge-to-edge repair (M-TEER) is effective for treating patients with secondary mitral regurgitation (SMR), but therapeutic response varies because of anatomy and patient heterogeneity. OBJECTIVES: The aim of this study was to identify distinct SMR phenotypes and analyze their response to M-TEER in the COAPT (Cardiovascular Outcomes Assessment of the MitraClip Percutaneous Therapy for Heart Failure Patients With Functional Mitral Regurgitation) and MITRA-FR (Percutaneous Repair or Medical Treatment for Secondary Mitral Regurgitation) trials. Indirect mitral annuloplasty using Carillon device after conduction system pacing resynchronization in patient with severe mitral regurgitation: a case report.

BACKGROUND: The prevalence of secondary mitral regurgitation (SMR) is increasing due to the aging population. SMR management is a complex disease because at least 2 pathologies are involved. SMR and severely reduced EF are independent risk factors for adverse outcomes. We present the case of a 57-year-old man who presented with dyspnea NYHA III. On echocardiography, a combination of SMR and DCM with EF of 20% were diagnosed. Indirect mitral annuloplasty after CRT-CSP was performed based on the patient's clinical profile and high surgical repair risk. The 3-month follow-up was uneventful. DISC

Streamlining the workflow: evaluation of ECG-gated computed tomography as a comprehensive imaging modality prior to intervention for mitral valve regurgitation.

BACKGROUND: ECG-gated computed tomography (CT) is not commonly utilized as the primary imaging modality for the diagnosis and management of degenerative mitral valve (MV) disease and preparation for MV intervention typically involves the use of several imaging studies. We hypothesized that CT can accurately identify MV pathology and exclude obstructive coronary artery disease (CAD) in this population potentially obviating the need for routine use of additional imaging modalities. METHODS: CT was compared to invasive coronary angiography (ICA), transthoracic (TTE) and transesophageal (TEE) ech
