BACKGROUND: National data on the durability of surgical mitral repair and outcomes of reintervention are lacking. This study aimed to quantify the national incidence and outcomes of mitral reintervention after surgical repair.
METHODS: The US Centers for Medicare & Medicaid Services claims identified patients aged ≥65 years who underwent surgical mitral repair followed by reintervention (replacement, repeat repair, or transcatheter edge-to-edge repair [TEER]) between 2012 and 2023. The 30-day and 3-year mortality, stroke, and heart failure readmission after reintervention were assessed using

OBJECTIVES: Right minithoracotomy mitral valve surgery has become a minimally invasive alternative to median sternotomy, although long-term outcomes remain incompletely defined. This study compared early and late outcomes of the 2 approaches in patients with degenerative mitral regurgitation.
METHODS: A single-center retrospective analysis was performed of 976 patients (mean age 60 ± 13 years) who underwent mitral valve repair, with or without concomitant procedures, between 2001 and 2022. Patients were categorized by surgical approach: sternotomy (n = 437) or right minithoracotomy (n = 539).

BACKGROUND: Left atrial (LA) strain analysis is an emerging echocardiographic technique for assessing LA function. This study aimed to compare LA strain parameters in patients with mitral stenosis (MS) and mitral regurgitation (MR) to identify discriminators between these conditions.
METHODS: This cross-sectional study included 44 patients with moderate-to-severe MR and 44 patients with severe-to-very severe MS, all with normal left ventricular ejection fraction. Patients with left ventricular systolic dysfunction, ischemic mitral regurgitation, or atrial fibrillation were excluded. Clinical

Monitoring cardiac output (CO) is helpful in the perioperative management of the patient with severe mitral regurgitation (MR). We assessed the accuracy and precision of the Cheetah CO monitor in patients with moderate or severe MR undergoing right and left heart catheterization as part of their pre-operative evaluation for mitral valve surgery. Cheetah CO was obtained concurrently with thermodilution CO (TD CO). Bias data was non-normally distributed; therefore, a non-parametric equivalent to Bland and Altman limits of agreement was used. Additionally, the proportions of differences between t

OBJECTIVE: Minimally invasive cardiac surgery for mitral valve (MV) disease is a rising strategy. Axillary access is linked to reduced pain and faster recovery, but its efficacy and safety compared with median sternotomy for MV surgery (MVS) remain unclear. We conducted a meta-analysis comparing the clinical outcomes of MVS via axillary access and median sternotomy.
METHODS: Four databases were analyzed. The primary endpoint was perioperative mortality. Secondary endpoints included cardiopulmonary bypass (CPB) and cross-clamp times, rethoracotomy, wound complications, mechanical ventilation d

BACKGROUND: Long-term outcomes of transcatheter mitral valve replacement (TMVR) with balloon-expandable aortic prostheses remain uncertain.
OBJECTIVES: The aim of this study was to evaluate long-term clinical and hemodynamic outcomes after TMVR.
METHODS: All patients undergoing TMVR at the authors' center were included. Balloon-expandable transcatheter heart valves were implanted in all cases, usually using a trans-septal approach. The primary outcome was a composite of death or mitral reintervention (surgical or transcatheter replacement or transplantation). Secondary outcomes included mort

BACKGROUND: Large V waves on pulmonary capillary wedge pressure tracings are common in patients with severe mitral regurgitation (MR) and typically decrease after transcatheter mitral valve therapy (TMVT). A persistently elevated V wave despite MR resolution may reflect impaired left atrial (LA) compliance.
OBJECTIVES: The aim of this study was to identify noninvasive predictors of invasive LA compliance and V-wave response in patients undergoing TMVT.
METHODS: Patients who underwent transcatheter edge-to-edge repair or valve-in-valve transcatheter mitral valve replacement between 2019 and 2

AIMS: Mitral valve edge-to-edge repair (M-TEER) is increasingly being used for the treatment of severe mitral regurgitation; however, outcomes in inoperable patients with very complex or even unfavourable mitral valve (MV) disease as 'ultima ratio' are unknown. The study aimed to evaluate patient outcomes according to mitral valve anatomical complexity, as suggested in a recent review article.
METHODS AND RESULTS: In this single-centre, retrospective analysis, consecutive patients who underwent M-TEER were categorized as 'Non-Complex' (NC), 'Complex' (C), 'Very Complex' (VC), and 'Ultimately

BACKGROUND: Transcatheter edge-to-edge mitral valve repair is a key therapeutic option for patients with severe symptomatic mitral regurgitation at high surgical risk. This prospective study aimed to develop a novel end-to-end deep learning model for preoperative artificial intelligence assessment in transcatheter edge-to-edge mitral valve repair (TEERAI-pre) candidates using multiview, multimodal echocardiography.
METHODS: TEERAI-pre, a video vision transformer-based classification model, predicts morphological suitability for transcatheter edge-to-edge mitral valve repair from multiview, mu

BACKGROUND: Current guidelines for degenerative mitral regurgitation (MR) emphasize left ventricular (LV) dysfunction. However, subclinical alterations in left atrial (LA) or LV strain may manifest earlier in severe MR.
OBJECTIVES: This study sought to determine whether combined LA and LV strain measurements improve long-term mortality prediction in severe degenerative MR, especially in asymptomatic individuals.
METHODS: This retrospective derivation cohort included 1,314 patients (mean age: 55 ± 13 years; 35% women) who underwent valve repair or replacement for severe MR. Preoperative peak

OBJECTIVES: Mitral valve repair (MVR) is the preferred surgical approach for paediatric mitral valve disease, but predictors of long-term durability remain poorly defined. We aimed to describe long-term outcomes after MVR in children and to identify echocardiographic predictors of mitral valve reintervention.
METHODS: We retrospectively reviewed all consecutive patients younger than 18 years who underwent MVR between 2000 and 2024 at a single tertiary centre. Patients with atrioventricular septal defects or single-ventricle physiology were excluded. Preoperative and predischarge echocardiogra

BACKGROUND: Transcatheter edge-to-edge repair (TEER) has emerged as an important therapy for severe mitral regurgitation. Current guidelines lack evidence-based recommendations for optimal postprocedural antithrombotic strategies, leading to heterogeneous clinical practices. And there are no randomized controlled trials (RCTs) available to compare different antithrombotic strategies following TEER.
METHODS: STAR-TEER trial is an investigator-initiated, multicenter, randomized, parallel controlled, open-label trial, aiming to assess the safety and efficacy of de-escalated antithrombotic strate

Cardiac amyloidosis is characterized by the deposition of amyloid fibrils, which cause complications such as mitral regurgitation (MR). The efficacy of transcatheter mitral edge-to-edge mitral repair (TEER) in patients with amyloidosis remains uncertain. We herein present four cases of TEER for transthyretin cardiac amyloidosis. TEER reduced the MR in all cases. However, two of the four individuals died within one year, and in one case, a recurrent MR deterioration occurred. Although TEER can reduce MR in individuals with cardiac amyloidosis, the long-term outcomes appear to be poor owing to d

BACKGROUND: Functional mitral regurgitation (FMR) contributes significantly to morbidity and mortality and may result from left ventricular (VFMR) or atrial (AFMR) remodelling. Left atrial reservoir strain (LASR) is a sensitive marker of atrial dysfunction and may offer incremental prognostic value. This study evaluated whether LASR predicts all-cause mortality and heart failure (HF) hospitalizations in FMR, its performance in VFMR versus AFMR, and its utility over standard echocardiographic parameters.
METHODS: We retrospectively analyzed 102 patients (mean age 68 ± 14 years, 41.2% female) w

Acute mitral regurgitation (MR) is a critical condition arising from diverse etiologies, including infective endocarditis, ischemic heart disease, stress-induced cardiomyopathy, and iatrogenic injury. While surgical repair or replacement has traditionally served as the standard of care, particularly in emergent cases, the evolution of transcatheter mitral valve therapies offers viable alternatives for patients deemed high-risk for conventional surgery. This review examines the pathophysiological mechanisms underlying acute MR in specific clinical contexts, and explores the expanding role of tr

BACKGROUND: Transcatheter mitral edge-to-edge repair (M-TEER) has transformed the management of patients with severe mitral regurgitation (MR) at high or prohibitive surgical risk. However, data on long-term survival and causes of death after M-TEER remain limited.
METHODS AND RESULTS: This single-center registry included consecutive patients undergoing M-TEER with the MitraClip device for severe MR between February 2016 and June 2020. The primary objective was long-term mortality trends and causes of death. Over a median follow-up of 3.3 years (IQR 1.3-5.1; maximum 8.5 years), 130 of 218 pat

BACKGROUND: Patients undergoing transcatheter edge-to-edge repair (TEER) of the mitral or tricuspid valve are predominantly elderly and exhibit high rates of frailty. The procedure regularly involves general anaesthesia, necessitating the monitoring of patients on an intensive or intermediate care unit (ICU/ImCU). This study aimed to investigate the outcome of patients being admitted to a general ward as opposed to an ICU/ImCU for postprocedural care after TEER. METHODS: This is a retrospective study analysing the course of 209 patients that underwent TEER at a university hospital centre in Ge

Ischemic mitral regurgitation (IMR) commonly complicates pPCI in STEMI patients. GDF11, a protective cardiovascular factor, may influence IMR progression and prognosis. Among 310 STEMI patients, 126 with recent normal echocardiography underwent pPCI with pre-procedural GDF11 testing. Cox regression, ROC analysis, and Spearman's correlation assessed GDF11's predictive value for IMR exacerbation and cardiac function. Lower GDF11 predicted IMR worsening (HR = 0.982, P = 0.004; AUC = 0.744, P < 0.001). High GDF11 patients had reduced IMR progression (P = 0.0013) and heart failure risk (P = 0.0003)

Due to variable involvement of mitral valve (MV) anomalies in hypertrophic obstructive cardiomyopathy (HOCM), MV repair may be added to septal myectomy. This study reports on our experience with HOCM surgery, including the effect of MV surgery. The HOCM population was divided per treatment in group 1 = septal myectomy/ablation only, and group 2 = septal myectomy + MV surgery. Clinical endpoints as survival and reoperation rate, and echocardiographic results were compared. The study comprised 47 patients (group 1 = 18 - group 2 = 29). Group 2 showed more MV anomalies (group 1:29% - group 2:79%,

BACKGROUND: Left ventricular rupture after mitral valve surgery is a rare but catastrophic complication with an extremely high mortality rate. Cases developing massive postoperative bleeding often require urgent surgical re-exploration. However, other causes of periannular bleeding, including iatrogenic coronary artery injury, should also be considered. Herein, we describe the case of a patient with periannular bleeding after mitral valve repair initially suspected to be left ventricular rupture but ultimately diagnosed as a distal left circumflex artery injury and successfully treated with co
