Granulicatella adiacens infective endocarditis in pregnancy: diagnostic contribution of metagenomic sequencing-a case report.

BACKGROUND: Granulicatella adiacens is a fastidious Gram-positive coccus and is a rare but recognized cause of infective endocarditis. Infective endocarditis during pregnancy is uncommon but carries substantial maternal and fetal risk. Plasma metagenomic analysis of microbial cell-free DNA has emerged as a complementary diagnostic tool in culture-negative infections. CASE SUMMARY: We describe a 35-year-old pregnant woman with known mitral valve prolapse who presented at 21 weeks of gestation with an acute ischemic stroke. Initial etiological work-up, including transesophageal echocardiography

Mitral Valve Prolapse Associated with Mitral Annular Disjunction: Diagnosis, Prognosis, and Arrhythmic Risk.

Mitral valve prolapse (MVP) is the most common valvular heart disease, with a reported prevalence of 2-3% in the general population, and represents the leading structural cause of mitral regurgitation in Western countries. MVP has long been considered predominantly a benign condition; however, it may be associated with adverse clinical events, including severe mitral regurgitation, atrial fibrillation, congestive heart failure, endocarditis, stroke, and ventricular arrhythmias, potentially leading to sudden cardiac death.

Cardiac Magnetic Resonance for the Prediction of Arrhythmic Events in Mitral Valve Prolapse: A Systematic Review and Meta-analysis.

Cardiac magnetic resonance (CMR) has gained ground in the assessment of mitral valve prolapse (MVP) patients. In this meta-analysis, we assessed the prognostic value of key CMR-derived imaging biomarkers -late gadolinium enhancement (LGE), mitral annular disjunction (MAD), and systolic curling- in patients with MVP. We also aimed to define the clinical and imaging features of patients with LGE on CMR. A total of 15 studies comprising 1,994 patients with MVP were included in this systematic review. All 3 MVP-related imaging biomarkers were significantly associated with increased risk of arrhyth

Malignant Mitral Valve Prolapse With Life-Threatening Ventricular Arrhythmias: A Multidisciplinary Team-Guided Stepwise Management.

CLINICAL CONDITION: A 45-year-old woman with long-standing bileaflet myxomatous mitral valve prolapse (MVP), mitral annular disjunction, and mild mitral regurgitation (MR) developed near-syncope and was found to have frequent polymorphic ventricular tachycardia on ambulatory monitoring. Despite medical therapy, she experienced recurrent ventricular fibrillation and multiple implantable cardioverter-defibrillator (ICD) shocks. KEY QUESTIONS: Can malignant arrhythmogenic MVP occur in the absence of severe MR or detectable myocardial fibrosis on cardiac magnetic resonance imaging? Which combinat

Impact of Transcatheter or Surgical Mitral Repair on Arrhythmic Burden in Patients With Mitral Valve Prolapse.

OBJECTIVES: Arrhythmic mitral valve prolapse (AMVP) is associated with malignant ventricular arrhythmias, yet the impact of surgical repair versus transcatheter edge-to-edge repair (TEER) on arrhythmic outcomes remains unclear. We evaluated ventricular arrhythmia occurrence and premature ventricular contraction (PVC) burden following mitral repair in patients with and without AMVP. METHODS: We conducted a retrospective cohort study of 1911 patients who underwent surgical mitral valve repair or TEER between 2004 and 2024 at a single institution. AMVP was defined per EHRA/ESC criteria, requirin

Incidence and type of recurrent ventricular arrhythmia in patients with arrhythmic mitral valve prolapse and implantable cardioverter defibrillator for secondary prevention.

AIMS: Mitral valve prolapse (MVP) is a common valvular disorder associated with ventricular arrhythmias (VAs) in a subgroup of patients, known as arrhythmic MVP (AMVP). Arrhythmic MVP patients with malignant VAs typically receive an implantable cardioverter-defibrillator (ICD) for secondary prevention. However, data regarding the incidence of appropriate ICD therapy and the type of arrhythmia recurrence in this population remain scarce. This study aimed to evaluate the incidence of appropriate ICD therapies and type of arrhythmia recurrence in AMVP patients with an ICD implanted for secondary

Serum peptidomic profiling and peptide mass fingerprinting reveal signatures associated with peroxisomal and mitochondrial pathways in MMVD-associated cardiorenal syndrome in dogs.

Cardiorenal syndrome in dogs, particularly those with myxomatous mitral valve degeneration (MMVD), involves complex neurohormonal interactions, yet molecular mechanisms linking heart and kidney dysfunction remain poorly understood. This study aimed to identify serum peptide and protein signatures that differentiate disease stages and clarify pathophysiology. We hypothesized that alterations in the serum peptide mass fingerprint and protein profile would serve as sensitive biomarkers for kidney disease progression in MMVD-affected dogs. Dogs were classified into five groups following ACVIM cons

Automated echocardiographic detection of mitral valve prolapse and mitral regurgitation with video-based artificial intelligence algorithms.

AIMS: We aimed to develop and evaluate fully automated artificial intelligence (AI) system for detection of mitral valve prolapse (MVP) and mitral regurgitation (MR) from echocardiographic studies. METHODS AND RESULTS: We used a dataset of 24 869 echocardiographic studies from the University of California San Francisco (UCSF) to train a multi-view deep neural network (DNN) to detect MVP using apical four-chamber, two-chamber, and parasternal long-axis views. A separate dataset of 27 906 studies from UCSF was used to train a second multi-view DNN model to detect moderate-to-severe or severe MR

Doming Volume in Mitral Valve Prolapse: Pathophysiology, Imaging Implications and Clinical Relevance.

Mitral valve prolapse represents the most common cause of primary mitral regurgitation in Western countries and has traditionally been viewed as a disorder driven by valvular incompetence and chronic volume overload. Within this paradigm, left ventricular enlargement was expected to correlate with regurgitant severity. However, patients with myxomatous bileaflet prolapse often exhibit left ventricular dilatation disproportionate to the degree of regurgitation, leading to the hypothesis of an intrinsic myocardial disease process. Cardiovascular magnetic resonance imaging has challenged this con

A Reverse Saddle Mitral Annulus Shape Explains Apparent Systolic Mitral Annular Disjunction in Mitral Valve Prolapse: A Three-Dimensional Transesophageal Echocardiography Analysis.

AIMS: Mitral annular disjunction (MAD) is considered an abnormal anatomical/functional feature in patients with mitral valve prolapse (MVP). Recent evidence suggests that in most cases, systolic disjunction is only apparent (Pseudo-MAD) because of altered annular dynamics, with the leaflet hinge point remaining anchored to the annulus. The aim of this study was to analyze mitral annular shape and dynamic function in patients with MVP by three-dimensional (3D) transesophageal echocardiography (TEE). METHODS AND RESULTS: Patients with MVP and grade 3 or greater mitral regurgitation (MR) who und

Cardiac arrest secondary to mitral annular disjunction: a case report.

BACKGROUND: Mitral annular disjunction with bileaflet mitral valve prolapse is increasingly recognized as a substrate for malignant ventricular arrhythmia and sudden cardiac arrest. Prior myocardial fibrosis adjacent to papillary muscles and frequent ventricular ectopy increase arrhythmic risk. CASE SUMMARY: We describe the case of a woman in her 40s who suffered an out-of-hospital ventricular fibrillation cardiac arrest with immediate bystander CPR and ambulance arrival within 5 min. Return of spontaneous circulation occurred approximately 30 minutes after multiple shocks. She had a backgrou

Recent advances in omics-based research of mitral valve disease.

Mitral valve disease (MVD), particularly mitral valve prolapse (MVP), is a common heart valve condition affecting millions of people worldwide. Its pathological mechanism is complex, involving multiple factors such as extracellular matrix remodeling, inflammatory responses, and genetic susceptibility. In recent years, with the development of high-throughput sequencing and analysis technologies, omics approaches including genomics, epigenomics, transcriptomics, proteomics, metabolomics and multi-omics integration have been widely used in MVD research. These technologies have greatly deepened ou

Association of Mitral Annular Disjunction on Magnetic Resonance Imaging With Cardiovascular Outcomes.

BACKGROUND: Mitral annular disjunction (MAD) is increasingly recognised on cardiac magnetic resonance (CMR) imaging, yet its clinical significance remains under discussion. We aimed to determine the prevalence, prognostic impact, and association with native T1 relaxation time of MAD assessed in both end-systole and end-diastole in an all-comer CMR cohort. METHODS: We analysed data from 1969 patients enrolled in a prospective CMR registry at the Vienna General Hospital. MAD was defined as ≥ 1 mm separation between the left atrial wall at the mitral valve hinge point and the adjacent left ventr

Beating-heart mitral valve replacement assisted by rapid ventricular pacing after Bentall procedure: a case report.

BACKGROUND: Beating-heart open mitral valve replacement (MVR) precludes aorta clamping, cardioplegic arrest, and reperfusion injury in cardiac surgery. However, beating-heart MVR still poses the risk of air embolism. The main cause of air embolisms is effective ventricular contraction and stroke. Rapid ventricular pacing (RVP) is commonly used in transcatheter valve implantation to temporarily halt cardiac output. However, its application in open beating-heart surgery has not explored in depth. Recently, we successfully employed RVP to perform MVR on an on-pump beating heart, without cross-cla

Infective Endocarditis in a Previously Healthy Adolescent Caused by Streptococcus sanguinis: Diagnostic Challenges and Clinical Implications.

Infective endocarditis (IE) is uncommon in children, particularly in those without congenital heart disease or indwelling catheters. We describe an adolescent patient without prior history of cardiac disease or previously diagnosed predisposing conditions who developed Streptococcus sanguinis endocarditis of the native mitral valve. Despite appropriate antibiotic therapy, the clinical course was complicated by acute mitral regurgitation and heart failure requiring valve replacement. This report highlights the diagnostic and therapeutic challenges of pediatric IE in the absence of traditional r

Cardiac mechanopathies: regional stress maldistribution as a primary determinant of myocardial disease.

The heart relies on finely tuned spatial heterogeneity in wall structure, fibre orientation, blood distribution and excitation-contraction coupling to maintain mechanical homeostasis. Even minor deviations may interact with anisotropy to promote maladaptation and disease. While cardiomyopathies have classically been attributed to primary cellular defects, emerging evidence suggests that abnormal regional stress may act as a primary driver of pathology even in the absence of intrinsic sarcomeric dysfunction. This review proposes the concept of 'cardiac mechanopathies', conditions in which misdi
