Transcatheter mitral valve replacement.

Transcatheter mitral valve interventions have seen a tremendous and constant increase in use over the past decade. Treatment is mainly aimed at mitral regurgitation (MR), the most common valve disease in industrialized countries. While transcatheter edge-to-edge repair has been established as the main therapy for this pathology, it is not a one-for-all option, and some types of MR would be better addressed using mitral valve replacement. Furthermore, non-rheumatic mitral valve stenosis and mitral annular calcification can only be treated using mitral valve replacement. Yet, although numerous d

Association of systemic inflammatory indicators with incident valvular heart disease: a prospective analysis of the UK Biobank.

BACKGROUND: Systemic inflammation has been implicated in valvular degeneration, but prospective evidence across major non-rheumatic valvular phenotypes remains limited. We investigated the associations of circulating inflammatory indicators with incident valvular heart disease in the UK Biobank. METHODS: This prospective cohort study included 250,532 participants free of cardiovascular disease, valvular heart disease, and autoimmune disease at baseline. Baseline systemic inflammation response index (SIRI), neutrophil-to-lymphocyte ratio (NLR), neutrophil-to-platelet ratio (NPR), lymphocyte-to

Prevalence and predictors of left atrial thrombus in patients with rheumatic atrial fibrillation undergoing cardiac surgery: a cross-sectional study.

BACKGROUND: Unlike non-rheumatic atrial fibrillation (AF), where left atrial thrombus (LAT) is predominantly confined to the left atrial appendage (LAA), a significant proportion of LAT in rheumatic AF occurs within the left atrial cavity (LAC). However, LAC thrombosis in rheumatic AF has not been extensively studied. This study aimed to evaluate the prevalence of LAT and its subtypes and identify potential predictors of LAT. METHODS: This retrospective study included adult patients with rheumatic AF who underwent open-heart surgery for mitral valvular abnormalities between January 2019 and D

Changing trends in burden of calcific aortic valve diseases and degenerative mitral valve diseases among people aged 60-89 years from 1990 to 2030.

BACKGROUND: The growing elderly population has increased the prevalence of nonrheumatic valvular heart diseases, such as calcific aortic valve disease (CAVD) and degenerative mitral valve disease (DMVD), raising major health concerns. METHODS: The Global Burden of Disease Study 2019 was used to assess the global burden of CAVD and DMVD. Estimated annual percentage change (EAPC) and joinpoint regression analyzed 30-year trends, while future trends for the next decade were projected using the Nordpred model. RESULTS: In 2019, there were 6.9 (95% UI [5.9-8.2]) million CAVD and 15.4 (95% UI [14.

Demographics and mortality trends of valvular heart disease in older adults in the United States: Insights from CDC-wonder database 1999-2019.

BACKGROUND: Valvular heart disease (VHD) represents a spectrum of cardiac conditions, including valvular stenosis, valvular regurgitation, or mixed lesions affecting single or multiple valves. The severity of VHD has emerged as a major cause of cardiovascular (CV) morbidity and mortality among the older population in the United States (U.S). OBJECTIVE: To evaluate temporal trends in mortality associated with VHD in the elderly U.S population between 1999 and 2019. METHODS: We utilized the CDC WONDER database for VHD mortality in adults ≥75 from 1999 to 2019, using ICD-10 codes. Age-adjusted

Profibrotic VEGFR3-Dependent Lymphatic Vessel Growth in Autoimmune Valvular Carditis.

BACKGROUND: Rheumatic heart disease is the major cause of valvular heart disease in developing nations. Endothelial cells (ECs) are considered crucial contributors to rheumatic heart disease, but greater insight into their roles in disease progression is needed. METHODS: We used a Cdh5-driven EC lineage-tracing approach to identify and track ECs in the K/B.g7 model of autoimmune valvular carditis. Single-cell RNA sequencing was used to characterize the EC populations in control and inflamed mitral valves. Immunostaining and conventional histology were used to evaluate lineage tracing and vali

Prevalence and predictors of non-rheumatic valvular heart disease in patients with sickle cell disease: insights from the National In-Patient Database in 2016 and 2017.

Sickle cell disease (SCD) life expectancy has increased in high-income countries, approaching the fifth decade in the United States. Aging in SCD has raised concerns about chronic organ damage due to adaptative and maladaptive cardiac remodeling. This study aims to assess the prevalence and predictors of non-rheumatic valvular heart disease (NRVHD) in SCD patients using the United States National Inpatient Sample database from 2016 and 2017. We conducted a weighted analysis on SCD patients during their index hospitalization. We obtained the prevalence of NRVHD and calculated adjusted odds rati

In-hospital and Readmission Outcomes With Percutaneous Balloon Mitral Valvuloplasty.

Percutaneous balloon mitral valvuloplasty (PBMV) is primarily performed for rheumatic mitral stenosis (MS). Therefore, limited data exist on PBMV in countries with a low incidence of rheumatic disease. Using the Nationwide Readmission Database, we examined trends in in-hospital mortality and 30-day readmission among patients who received PBMV for rheumatic and non-rheumatic MS. We also examined the change in 90-day hospitalization rate before vs after PBMV. Between 2016 and 2019, there were 1109 hospitalizations in which patients received PBMV for rheumatic (n = 955, 86.1%) vs non-rheumatic MS

Mitral stenosis in a teenager after rheumatic mitral valve regurgitation valve repair: A case report.

INTRODUCTION: Mitral stenosis (MS) is a widely known complication of mitral valve repair for non-rheumatic mitral regurgitation (MR). Few reports are available on the occurrence of MS after mitral valve repair for rheumatic MR in young populations. CASE SUMMARY: A 14-year-old girl presented with orthopnea, abdominal distension, and bilateral lower-limb edema. She was cachectic, with a high-pitched holosystolic murmur best heard at the cardiac apex, bilateral basal crackles, tender hepatomegaly, pitting pedal edema, and jugular venous distension. Antistreptolysin O (ASO) titer was elevated. Tr

A Rare Presentation of Systolic Anterior Motion Occurring Eight Years After Mitral Valve Replacement.

Systolic anterior motion (SAM) is the dynamic displacement of mitral valve leaflets anteriorly toward the left ventricular outflow tract (LVOT) during systole. SAM-like physiology has been reported to occur shortly after mitral valve replacement (MVR) surgery; occurrence beyond two years after surgery is very rare. A 55-year-old woman who had bioprosthetic MVR eight years earlier for non-rheumatic mitral stenosis presented to the emergency room with progressive dyspnea and sudden-onset chest pressure. Physical examination noted a grade 3/6 systolic murmur at the cardiac apex, a soft diastolic

How to Manage Mitral Stenosis Due to Mitral Annular Calcification.

PURPOSE OF REVIEW: Mitral annular calcification (MAC) is associated with cardiovascular comorbidities and events and in the presence of mitral stenosis (MS) represents a high-risk cohort with limited treatment options. Emerging hybrid, minimally invasive, and transcatheter therapies that use circumferential MAC as an anchor for mitral valve replacement are emerging, but none are consistently associated with ideal outcomes. RECENT FINDINGS: In patients with MAC and nonrheumatic calcific mitral stenosis who are severely symptomatic, mitral intervention may be indicated. Surgical decalcification

A case report: cardiac dysphagia-a ghost of the past?

BACKGROUND: Rheumatic heart disease has become rare in developed countries and physicians have grown unfamiliar with the disease and its clinical course. The mitral valve is most commonly affected leading to mitral regurgitation and/or stenosis. The chronic volume and/or pressure overload leads to atrial remodelling and enlargement, driving the development of atrial fibrillation and thrombo-embolic events. CASE SUMMARY: A 87-year-old patient with a history of rheumatic mitral stenosis and mitral valve replacement was admitted to the neurology department for vertigo. A stroke was suspected, an

Etiology and distribution of isolated aortic stenosis in Indian patients - A study from a large tertiary care hospital in north India.

BACKGROUND: Isolated aortic valve disease (IAVD) has traditionally been a disease of elderly, etiology being either senile degeneration of a tricuspid aortic valve or calcification of a bicuspid aortic valve. However, there is scarcity of Indian data regarding demographic distribution and etiological patterns of IAVD in context of emerging therapies like transcatheter aortic valve implantation (TAVR). METHODS & RESULTS: A retrospective observational analysis of 60,560 echocardiograms over three years revealed 3728 newly diagnosed cases of valvular heart disease (VHD). Isolated mitral valve di

Non-Rheumatic Mitral Annular Calcification as a Cause of Late-Onset Mitral Stenosis.

While 'senile' calcific mitral stenosis is a rare cause of mitral stenosis as compared with rheumatic heart disease worldwide, it is thought to be more common in developed nations. Due to differences in the mechanism of dysfunction compared to rheumatic mitral stenosis, treatment options for senile calcific mitral stenosis are more limited and technically challenging. We describe a case of symptomatic severe mitral stenosis that was managed by surgical bioprosthetic valve replacement.
