Cardiogenic brain embolism. Cerebral Embolism Task Force.

One in six ischemic strokes is due to cardiogenic embolism. The current knowledge about the natural history, diagnosis, prevention, and treatment of cardioembolic stroke is reviewed. Nonrheumatic atrial fibrillation is the most frequent substrate for brain embolism; patients with this dysrhythmia have a fivefold increase in the risk of stroke. Ischemic heart disease (25%), rheumatic mitral stenosis (10%), and prosthetic cardiac valves (10%) are other major sources of cardioembolic stroke. Mitral annulus calcification and calcific aortic stenosis infrequently serve as cardioembolic sources. As

[Antibodies to polysaccharides of group A Streptococcus in patients with rheumatism after mitral commissurotomy].

Gel precipitation was employed to detect antibodies to the A-polysaccharide specific antigenic determinant in blood serum of 131 patients with different versions of rheumatic fever. 21 patients with non-rheumatic myocarditis, 25 convalescents following quinsy, and 58 donors. It was established that high titers of antibodies to A-polysaccharide in patients with protracted and latent rheumatic fever correlate with the clinical and morphological signs of the process activity. Elevation of the antibody titers after mitral commissurotomy made to 107 patients was recorded only in those with exacerba

Morphologic features of the normal and abnormal mitral valve.

Anatomic and functional features of the normal and abnormal mitral valve are reviewed. Of 1,010 personally studied necropsy patients with severe (functional class III or IV, New York Heart Association) cardiac dysfunction from primary valvular heart disease, 434 (43%) had mitral stenosis (MS) with or without mitral regurgitation (MR): unassociated with aortic valve stenosis or regurgitation or with tricuspid valve stenosis in 189 (44%) patients, and associated with aortic stenosis in 152 (35%), with pure (no element of stenosis) aortic regurgitation in 65 (15%) patients, and with tricuspid val

Rapidly progressing, massive mitral annular calcification. Occurrence in a patient with chronic renal failure.

Calcification of the mitral annulus developed in a patient while undergoing dialysis. The rapid onset of events corresponded to the onset of end-stage renal failure and uncontrolled secondary hyperparathyroidism. Sequential echocardiograms verified the progression of calcification of the annulus as well as the valve. A new systolic and diastolic murmur and reduced valve orifice on two-dimensional echocardiography suggested acquired nonrheumatic mitral stenosis and insufficiency. We propose that metastatic calcium deposition rather than long-term hypertensive and degenerative effects was the pr

Management of cerebral embolism of cardiac origin.

The cardiac conditions most commonly associated with cerebral embolism are rheumatic heart disease (RHD), atherosclerotic heart disease (myocardial infarction and atrial arrhythmias) and other kinds of nonvalvular atrial fibillation (AF). The natural history of cerebral embolism from these cardiac sources is reviewed. Virtually all rheumatic hearts producing emboli have mitral stenosis, but not all of them are in AF. Of patients with RHD, 10--20% will experience a systemic embolus, and approximately half will have a recurrence, usually early. Of patients with a myocardial infarction, 5--12% wi

[Echocardiographic diagnosis of mitral valve insufficiency].

Seventy-two persons with various forms of mitral insufficiency of rheumatic and non-rheumatic etiology and 17 healthy individuals (control group) were examined. The results of echocardiography in 35 patients were compared with operative (mitral commissurotomy, valve plastics) findings. Several typical variants of a mitral echogram in patients with "pure", predominant and concomitant mitral insufficiency were distinguished. The predominantly "valvular" mechanism of mitral insufficiency in patients with rheumatic disease and the predominant role of "subvalvular" pathology in nonrheumatic insuffi

Ventriculographic and hemodynamic features of mitral regurgitation of cardiomyopathic, rheumatic and nonrheumatic etiology.

Quantitative angiographic findings were reviewed in 40 patients with significant mitral regurgitation classified into three etiologic groups: group I, primary mitral regurgitation (prolapse, ruptured chordae); group II, mixed stenosis and regurgitation of rheumatic origin; and group III, cardiomyopathic mitral regurgitation. For patients in both groups I and II, left ventricular end-diastolic volume was directly related to regurgitant fraction, and ejection fraction was generally well maintained. In contrast, patients in group III had a depressed ejection fraction (less than 0.40) and end-dias

Mitral annular calcification. Clinical and echocardiographic correlations.

Clinical, echocardiographic, and radiographic findings in 41 patients with mital annular calcification are presented. Clinical diagnosis included rheumatic heart disease (32%), noninflammatory calcific disease (34%), and chronic renal failure (32%). Mitral annular calcification is recognized echocardiographically as a dense echo band beneath the posterior mitral leaflet with motion paralleling that of the free left ventricular wall. Only two patients of 41 (5%) with mitral annular calcification had calcification on routine x-ray films, and six of 16 (38%) on cinefluoroscopy. The left atrial si

Assessment of ventricular elements of mitral valve by left ventriculography.

In the left ventriculogram in the right anterior oblique projection the plane of the mitral valve is seen in profile and the papillary muscle shadows are outlined. The distance from the middle of the papillary muscles to the plane of the mitral valve during systole was used to assess the average length of the chordae tendineae, and the area of the papillary muscle shadows was measured as an index of hypertrophy in a series of hearts with mitral valve disease. Valvar mitral stenosis is characterised by slight reduction in the length of chordae tendineae and more hypertrophy of the papillary mus

Granulomatous mitral valve obstruction.

Acquired non-rheumatic valvular mitral valve obstruction is extremely rare. A case of mitral valve obstruction is described which resulted from a granulomatous mass replacing the valve cusps. The problems in diagnosis and probable aetiology are discussed.

Non-rheumatic acute mitral insufficiency caused by ruptured chordae tendineae.

Five patients with proved mitral insufficiency caused by ruptured chordae tendineae were examined. Two patients came to autopsy without cardiac cateterization. Three patients, who had cardiac cateterization and surgical replacement of their mitral valves, improved dramatically. Two additional patients have been clinically diagnosed and have had corroborative cardiac catheterization and are being observed in anticipation of possible future valve replacement. Similar findings in these seven patients include adult onset of a characteristic systolic heart murmur which mimics aortic valvular stenos

The significance of the left atrial appendage in rheumatic heart disease.

The possible diagnostic value of an enlarged left atrial appendage (LAA) on the posterior-anterior or right anterior oblique chest film as a means of implicating a rheumatic etiology for mitral valve disease in children was investigated. Chest films were examined without prior knowledge of clinical or laboratory data, and the results were later correlated with this information in 113 children and adolescents. The clinical and laboratory data included application of the modified Jones criteria for the diagnosis of acute rheumatic fever, streptococcal antibody titers and clinical and cardiac cat

Cronic non-rheumatic valvular heart disease. An autopsy study.

The frequency of chronic non-rheumatic valvular heart disease in Iceland was investigated via autoposies performed from November 1965 through December 1974. During this period, about 12.400 Icelanders died at the age of 16 years and older and 28.8 per cent of these were included in the study. At autopsy, males outnumbered females by 2:1. The frequency of calcific aortic stenosis was found to be 3.63 per cent and the prevalence was calculated to be 3.17 per cent among males and 4.50 per cent among females. Calcific aortic stenosis in tricuspid valves was more frequent in females and calcific ao
