Transcatheter Aortic Valve Explants: Calcification Patterns and Leaflet Thickening to Inform Repeat Interventions.

BACKGROUND: Degeneration is an inevitable outcome for bioprosthetic heart valves. In the setting of transcatheter aortic valve (TAV) replacement (TAVR), calcifications and leaflet thickening have implications for feasibility and outcomes of repeat interventions, including leaflet modification techniques. In this study, we aimed to investigate the frequency and pattern of leaflet calcification and thickening in explanted TAVs to inform repeat interventions. METHODS: TAV explants were obtained from the Explanted THV registry and clinical institutions. Micro-computed tomography imaging and histo

Advancing aortic stenosis assessment: Validation of fluid-structure interaction models against 4D flow MRI data.

Systematic in vivo validations of computational models of the aortic valve (AV) remain scarce, despite successful validation against in vitro data. Utilizing a combination of computed tomography and 4D flow magnetic resonance imaging data, we developed patient-specific fluid-structure interaction models of the AV immersed in the aorta for five patients in the pre-transcatheter AV replacement configuration. Our computational models are subjected to rigorous validation against 4D flow measurements. Our results demonstrate the models' capacity to accurately replicate flow dynamics. In addition, w

Incidence of Worsening Aortic Regurgitation After Mapping and Ablation in the Aortic Cusps.

BACKGROUND: Ventricular arrhythmias from the aortic cusps (ACs) represent a subset of left ventricular outflow tract arrhythmias. Although ablation in this region can be highly effective, concerns persist regarding aortic valve injury and iatrogenic aortic regurgitation (AR). Contemporary data on AR incidence after AC mapping/ablation remain limited. OBJECTIVES: This study sought to determine the incidence, severity, and clinical significance of AR in a multicenter cohort after mapping and/or radiofrequency ablation in the ACs or commissures. METHODS: We retrospectively analyzed patients at

Reoperation after previous aortic valve repair in children: Redo repair versus Ross procedure.

OBJECTIVE: The study objective was to compare the long-term outcomes of redo aortic valve repair versus the Ross procedure in children requiring reintervention after prior aortic valve repair. METHODS: We performed a retrospective, single-center cohort study of patients aged less than 18 years who underwent primary aortic valve repair between 1980 and 2024. Patients were included if their first aortic valve reoperation was redo repair or a Ross procedure. Outcomes of interest were survival free from heart transplantation, freedom from semilunar valve reoperation, and freedom from recurrent 2+

Differential Effect of Early Intervention for Moderate Aortic Stenosis by Extravalvular Damage.

INTRODUCTION: It is unknown whether early intervention may be beneficial for patients with moderate aortic stenosis (AS) and if so, for whom. We compared the effect of early intervention with conservative treatment in moderate AS patients according to extravalvular cardiac damage. METHODS: This multicenter retrospective cohort study included consecutive patients with moderate AS (aortic valve area, 1.0-1.5 cm2, peak velocity 3-4 m/s, or mean pressure gradient 20-40 mm Hg, with any parameter beyond these limits defined as severe AS) from 2008 to 2020. Patients were categorized by extravalvular

Sex differences in reverse remodeling after transcatheter aortic valve replacement in low-flow aortic stenosis.

OBJECTIVES: Sex-specific differences in left ventricular (LV) remodeling after transcatheter aortic valve replacement (TAVR) in low-flow aortic stenosis (LFAS) remain incompletely defined, and the influence of LFAS subtype on remodeling by sex is uncertain. The authors sought to characterize post-TAVR remodeling in LFAS by sex and subtype and identify predictors of remodeling. METHODS: This retrospective cohort study included 488 patients with LFAS (303 men, 185 women) who underwent TAVR. Smoothed conditional mean curves assessed longitudinal changes in LV mass index (LVMi), relative wall thi

miR-181d-5p promotes valvular calcification and directly targets Wnt inhibitory factor 1 in calcific aortic valve disease.

This study aimed to investigate the role of miR-181d-5p in the pathogenesis of calcific aortic valve disease (CAVD), identify its downstream targets, and provide novel insights into therapeutic strategies for this condition. The differential expression of miRNAs in valve samples from CAVD patients was assessed via qRT-PCR to identify key regulatory miRNAs. Target gene prediction software and four Gene Expression Omnibus (GEO) datasets (GSE12644, GSE235995, GSE51472, and GSE77287) were employed to identify differentially expressed genes (DEGs) in CAVD and predict the downstream targets of miR-1

REsting FLOW rate to determine true Aortic valve Severity (REFLOW-AS).

BACKGROUND: Patients with low gradient severe aortic stenosis (AS) are clinically challenging cohort requiring additional imaging modalities to secure diagnosis of severe AS. We aimed to assess whether resting aortic valve area (AVA) can determine hemodynamic severity of AS in patients with high trans-valvular flow rate (TVFR). METHODS AND RESULTS: This was a retrospective single centre analysis of consecutive patients undergoing dobutamine stress echo (DSE) for AS work-up. A standard protocol for low-dose DSE was performed and data on resting as well as stress echocardiographic parameters we

Single-Stage Computed Tomography Angiography, Percutaneous Coronary Intervention, and Transcatheter Aortic Valve Replacement in an Octogenarian With Femoral Neck Fracture, Non-ST-Elevation Myocardial Infarction, and Severe Aortic Stenosis: A Case Report and Literature Review of Multidisciplinary Approaches.

Geriatric femoral neck fractures are associated with high mortality. Early surgery within 48 hours improves survival, whereas delayed arthroplasty is associated with a 41% increase in mortality. Multimorbidity in this population often prolongs preoperative optimization, thereby exacerbating perioperative risk. Current evidence indicates that 5%-10% of patients with hip fractures have concomitant severe aortic stenosis (AS), which independently increases the risk of anesthesia-related complications. Perioperative acute myocardial infarction further increases mortality risk, making cardiac manag

Bentall Procedure in a Patient with Loeys-Dietz Syndrome: A Case Report.

Loeys-Dietz syndrome (LDS) is a rare autosomal dominant connective tissue disorder characterized by aggressive aortopathy, craniofacial anomalies and systemic involvement. Rapid aortic root dilatation and valvular dysfunction mandate an early surgical intervention. We report a case of 15-year-old boy with LDS, rapidly progressive aortic root dilatation and severe aortic regurgitation (AR) who underwent successful Bentall procedure. The case highlights the need of early detection, multidisciplinary involvement and meticulous anesthetic care.

Emergency Transcatheter Aortic Valve Implantation for Decompensated Aortic Stenosis accompanied by Cardiogenic Shock.

Patients with severe aortic stenosis (AS) often present acutely with decompensated heart failure, cardiogenic shock or cardiac arrest requiring immediate intervention. Treatment options for these patients include palliative care, balloon aortic valvuloplasty (BAV), emergency transcatheter aortic valve implantation (TAVI), or surgical aortic valve replacement. TAVI procedure is performed electively in majority of the patients. However, it can be considered a rescue therapy in patients with cardiogenic shock secondary to acutely decompensated valvular AS. Here, we report the successful use of em

Conduction System Versus Right Ventricular Pacing in Patients Undergoing TAVR: A Multicenter Propensity-Matched Study.

BACKGROUND: Pacemaker implantation after transcatheter aortic valve replacement (TAVR) is common and has been associated with adverse clinical outcomes. Cardiac physiological pacing (CPP) may preserve ventricular synchrony and reduce heart failure (HF) events compared with right ventricular pacing (RVP). OBJECTIVES: This study sought to compare long-term clinical outcomes of CPP vs RVP in patients requiring permanent pacing after TAVR. METHODS: Registro de Estimulación Fisiológica-TAVR is a retrospective, multicenter nationwide registry including patients undergoing TAVR and pacemaker implan

Expression of Desmocollin-1 in carotid atherosclerotic plaques and calcific aortic valves.

INTRODUCTION: Atherosclerosis and calcific aortic valve stenosis are active, regulated processes sharing features such as lipid infiltration, extracellular matrix deposition, cellular proliferation, and calcification. Their exact mechanisms remain unclear. High-density lipoprotein (HDL) is central to reverse cholesterol transport, and Desmocollin-1 (DSC1) has recently been implicated in HDL biogenesis. This study assessed DSC1 expression and localization in atherosclerotic plaques and calcific aortic valves. METHODS: Human carotid artery specimens (19 subjects: plaques and normal tissue) and

Lipoprotein(a) Inhibition: Current Evidence, Therapeutic Landscape, and Future Directions.

Lipoprotein(a) [Lp(a)] is a genetically determined, apolipoprotein B-containing lipoprotein that contributes to cardiovascular risk through atherogenic, proinflammatory, and potentially antifibrinolytic mechanisms. Genetic, epidemiological, and mechanistic evidence supports a causal role for elevated Lp(a) in atherosclerotic cardiovascular disease and calcific aortic valve disease. In contrast, associations with other cardiovascular phenotypes are less consistent. Since circulating concentrations are largely inherited and only modestly affected by lifestyle measures and conventional lipid-lowe
