BACKGROUND: Circular RNAs are implicated in various physiopathologic activities and play a crucial role in calcific aortic valve disease (CAVD) progression. However, the role of coding circular RNAs in CAVD remains unclear. In this study, we aimed to characterize coding circular RNAs and explore their functions in CAVD.
METHODS: Using a systematic approach from transcriptome sequencing to experimental validation, we identified circZBTB44, confirmed its translation into ZBTB44-342aa, and investigated the function and mechanism of this peptide in CAVD using both cellular and animal models.

PURPOSE: Transthyretin cardiomyopathy (ATTR-CM) and aortic stenosis (AS) are frequently associated. The diagnosis of ATTR-CM can be made by performing a cardiac amyloid radionuclide imaging (CARI) with a computed tomography (CT) scan to localize the signal, which allows visualization aortic valve calcifications (AVC). The aim of this study was to determine whether AVC on the CT performed at the time of the CARI for the diagnosis of ATTR-CM were predictive of AS and had an impact on prognosis. METHODS: CT images of patients diagnosed with ATTR-CM by CARI were reviewed for the presence of AVC an

Introduced as an alternative to open-heart surgery for elderly patients, Transcatheter Aortic Valve Implantation (TAVI) has recently been extended to younger patients due to comparable performance with the gold-standard. However, the long-term durability of the bio-prosthetic TAVI valves is limited by Structural Valve Deterioration (SVD), an inevitable degenerative process whose pathogenesis is still unclear. In this study, we aim to computationally investigate a possible relationship between aortic hemodynamics and SVD development. To this aim, we collect data from twelve patients with and wi

The use of transcatheter aortic valve implantation (TAVI) has become the most popular technique of aortic valve intervention. Classically, TAVI is performed via femoral arterial access. However, some patients who have severe peripheral arterial disease do not have safely accessible femoral arteries. In such patients, the use of carotid access has been well described via the 'clamp-and-sew' technique. We describe a minimally invasive carotid access technique for TAVI deployment. The right common carotid is accessed by a 3 cm suprasternal incision. The carotid sheath is entered, and the surgeon

OBJECTIVES: Aortic valve replacement with Trifecta valve provides a greater early haemodynamic performance with its externally mounted structure. However, failure rates in short-term post-implantation started to be reported, which brought great attention towards understanding the mechanisms of failure and optimal approaches for the replacement of the valve.
METHODS: Fifty-seven studies published in PubMed/Medline, Cochrane Central Register of Controlled Trials (CENTRAL), Embase, and ClinicalTrials.gov were explored until January 2025. Original research articles or clinical trial reports that

OBJECTIVES: Patients with aortic stenosis (AS) undergoing haemodialysis (HD) often experience more rapid disease progression and poorer prognosis than non-dialysis patients; however, the underlying mechanisms remain unclear. This study aimed to elucidate clinical and molecular differences between HD and non-HD patients with AS, focusing on transcriptomic profiling of resected aortic valves.
METHODS: We retrospectively analysed 183 patients with severe AS who underwent surgical aortic valve replacement at the University of Yamanashi Hospital from February 2015 to May 2024. Among them, 34 patie

Aortic valve stenosis (AS) is the most common valvular disease, with a growing impact in the aging population. AS can culminate in heart failure if left untreated. Early treatment with minimally-invasive transcatheter aortic valve replacement (TAVR) is being evaluated in clinical trials. We addressed an unmet clinical need for early detection, referral to echocardiography for AS evaluation, and possible treatments (e.g., lifestyle changes, drugs, or TAVR). As aortic valve calcification (AVC) is typically present in AS, we created a method to detect AVC in low-cost/no-cost non-contrast CT calci

BACKGROUND: The coexistence of aortic stenosis (AS) and transthyretin cardiac amyloidosis (CA) is common. If treated with transcatheter aortic valve replacement (TAVR), patients with the combined phenotype (AS-CA) have a similar survival at 1 year compared to those with lone AS. This study aims to evaluate the long-term outcomes of AS-CA compared to lone AS.
METHODS: Using a prospective, multicenter, observational, case-control design, we studied patients with severe AS referred for TAVR. All underwent bone scintigraphy to differentiate between AS-CA and lone AS. Outcomes were compared betwee

OBJECTIVES: Aortic stenosis (AS) induces left ventricular diastolic dysfunction, which is associated with left atrial (LA) dysfunction. However, the prognostic impact of LA dysfunction in the surgical intervention for AS remains unclear.
METHODS: Among the 812 patients who underwent isolated surgical aortic valve replacement (SAVR) for AS at 8 institutions between 2016 and 2021, 366 with available preoperative left atrial volume index (LAVI) were included. Based on the optimal value of 40 mL/m2-derived from receiver operating characteristic curve analyses-patients were stratified into a LA en

BACKGROUND: Infective endocarditis after transcatheter aortic valve implantation (TAVI-IE) is an uncommon but severe complication associated with substantial morbidity and mortality. Therapeutic strategies vary widely, since invasive management is often precluded by prohibitive risk and conservative medical therapy may be linked to poorer outcomes. Therefore, we aimed to compare outcomes between conservative and invasive management in patients with TAVI-IE.
METHODS: A systematic search of PubMed, Scopus, and Web of Science identified comparative studies evaluating conservative versus invasive

Available studies have found associations between several cytokines and calcific aortic valvular stenosis (CAVS). To better comprehend the causal link between circulating cytokines and CAVS, we performed a bidirectional Mendelian randomization (MR) study. Genetic variants associated with 41 cytokines were obtained from public genome-wide association study (GWAS), and summary statistics for GWAS of CAVS were obtained from the FinnGen consortium. Forward MR analysis was conducted to determine the impacts of 41 cytokines on CAVS risk and reverse MR analysis was used to determine whether genetic s

BACKGROUND: Aortic stenosis (AS) is a progressive disease requiring timely monitoring and intervention. While transthoracic echocardiography remains the diagnostic standard, deep learning-based approaches offer the potential for improved disease tracking. This study examined the longitudinal changes in a previously developed deep learning-derived index for AS continuum (DLi-ASc) and assessed its prognostic association with progression to severe AS.
METHODS: We retrospectively analyzed 2373 patients (7371 transthoracic echocardiographies) from 2 tertiary hospitals. DLi-ASc (scaled 0-100), deri

BACKGROUND: Mitigating prosthesis-patient mismatch (PPM) in surgical or transcatheter aortic valve replacement (SAVR/TAVR) requires early identification of small annulus size (SAS). While computed tomography (CT) is the gold-standard for annular sizing in TAVR, its routine use in SAVR planning is limited by resource constraints.
OBJECTIVES: To develop and validate the Patient Referral Evaluation to Determine Indication for CT (PREDICT) model - a predictive tool utilizing anthropometric and transthoracic echocardiography (TTE)-derived data to identify SAS at the point of referral.

OBJECTIVES: Severe aortic stenosis (AS) with a left ventricular ejection fraction (LVEF) of 50% to 54% is associated with worse outcomes than an LVEF greater than or equal to 55%. European guidelines consider aortic valve replacement (AVR) a Class IIa indication for asymptomatic patients with an LVEF of less than 55%, whereas American guidelines recommend AVR when the LVEF is less than 50%. The authors assessed outcomes of AVR vs conservative management in this range where guidelines differ.
METHODS: A registry was created for individuals with severe high-gradient AS (AVA ≤ 1 cm²), an LVEF of

OBJECTIVE: Transcatheter aortic valve implantation (TAVI) has revolutionized the treatment of severe aortic stenosis; however, early mortality risk stratification remains challenging. The Naples Prognostic Score (NPS), which integrates inflammatory and nutritional markers, has shown promise in cardiovascular disease prognosis. This study investigated the relationship between preprocedural NPS and 30-day mortality in patients undergoing TAVI.
METHOD: This retrospective, single-center study analyzed 308 patients aged ≥ 65 years who underwent elective transfemoral TAVI between August 2012 and De

BACKGROUND AND AIMS: The prognostic significance of global calcium burden-including coronary artery (CAC), mitral annular (MAC), aortic valve (AVC), and thoracic aortic calcification (TAC)-as assessed by cardiac computed tomography (CCT), remains incompletely understood in patients with severe aortic stenosis (AS) undergoing aortic valve replacement (AVR). This study aimed to evaluate the prognostic impact of overall calcification burden in patients with severe AS undergoing AVR.
METHODS: A retrospective analysis of 313 patients with severe AS undergoing CCT before AVR between 2016 and 2019 w

BACKGROUND: While female sex has historically been associated with worse outcomes following surgical aortic valve replacement, the evidence regarding sex-related differences after transcatheter aortic valve implantation (TAVI) remains inconclusive. Given that women are more frequently referred for TAVI, clarifying the role of sex in procedural outcomes is clinically relevant.
OBJECTIVES: To systematically evaluate and summarise randomised clinical trial (RCT) evidence comparing TAVI outcomes between women and men.
METHODS: A systematic review was conducted following Preferred Reporting Items

BACKGROUND: Diffuse fibrosis is central to the pathophysiology of aortic stenosis (AS), can be assessed using cardiovascular magnetic resonance (CMR) with extracellular volume fraction (ECV%), and is associated with mortality. The relevance of this signal to long-term prognosis remains unclear. We aim to assess predictors of long-term mortality with focus on diffuse fibrosis.
METHODS: Single-center prospective observational cohort study of patients with severe, symptomatic AS undergoing aortic valve replacement (AVR). Patients were assessed using echocardiography, high-sensitivity cardiac tro
