BACKGROUND: Early changes in left ventricular ejection fraction (LVEF) after transcatheter aortic valve replacement (TAVR) in patients with pure aortic regurgitation (AR) may signal functional recovery potential and prognostic relevance, yet systematic evidence is lacking.
METHODS: We retrospectively analyzed 128 consecutive patients with pure AR who underwent TAVR between 2018 and 2023 and experienced an acute postprocedural LVEF reduction (≥5%). LVEF was assessed using the Simpson's biplane method at discharge and at 1 month. Patients were categorized according to a 1-month LVEF change into

Neonatal truncal valve insufficiency carries high postoperative mortality. Mechanical replacement is challenging, and leaflet-sparing repair alone may be insufficient due to fragile tissue and annular dilatation. We report a successful case using geometry-based root reconstruction guided by body-weight-adjusted reference values. This native-tissue approach restored physiologic root geometry, preserved the valve, and enabled growth potential, demonstrating a feasible strategy for durable neonatal repair.

OBJECTIVES: This study aims to compare the immediate and mid-term outcomes of Aortic Valve Neocuspidization (AVNeo) with surgical aortic valve replacement using a bioprosthesis (BioSAVR) to determine if neocuspidization can overcome limitations of current techniques.
METHODS: From December 2016 to December 2023, 155 patients received AVNeo at the Heart Institute, while 301 underwent BioSAVR. Baseline characteristics were balanced using 1:1 propensity matching.
RESULTS: 132 identical patient pairs were included in the analysis. Neocuspidization had longer ischemic times (98.67 ± 28.47 min vs.

BACKGROUND: The non-coronary sinus pivot implantation (NCPI) technique has notably improved procedural success rate in transcatheter aortic valve replacement (TAVR) for cases of severe pure native aortic regurgitation (PNAR) when utilizing self-expanding transcatheter heart valves (THVs). Nonetheless, the intrinsic anchoring mechanism remains obscure.
AIMS: The aim of this study was to assess the feasibility of implementation of the NCPI method using self-expanding THVs for TAVR in severe PNAR and dilated ascending aortas (AAOs), and to elucidate the anchoring mechanism of the NCPI.

AIMS: Cardiac magnetic resonance (CMR) compliments transthoracic echocardiography (TTE) for heart valve evaluation, however TTE remains more widely available. We sought to optimize TTE quantification of significant aortic regurgitation (AR) by developing TTE-based algorithms to identify CMR-defined severe AR.
METHODS AND RESULTS: Patients with moderate-to-severe AR undergoing both TTE and CMR within 3-months were studied. A historical cohort 2006-18 (n = 193) was used to derive TTE-based decision tree regression algorithms to best identify severe AR based on holodiastolic flow reversal (HDR)

BACKGROUND: Aortic valve repair is preferred for congenital aortic valve disease in children, but its durability is limited. Although acceptable immediate repair results improve outcomes, late failures still occur. This study aimed to identify risk factors for suboptimal outcomes after an initially acceptable repair.
METHODS: We retrospectively reviewed 287 patients undergoing primary aortic valve repair (2014-2023). Acceptable repair was defined as less than moderate aortic stenosis (AS) and mild or less aortic insufficiency (AI) after bypass. Suboptimal outcomes were defined as more than mo

IMPORTANCE: Left ventricular (LV) dilatation is an established prognosticator in aortic regurgitation (AR). Current guidelines recommend aortic valve surgery (AVS) using LV end-systolic diameter index (LVESDi) with a uniform threshold, irrespective of sex. While LV end-systolic volume index (LVESVi) may better characterize LV remodeling, it was only recently included in European guideline recommendations, with a threshold of 45 mL/m2 for both men and women.
OBJECTIVE: To assess sex differences in LV remodeling using linear and volumetric dimensions and their association with outcomes in AR.

AIM: Whether aortic valve sparing (valve sparing root replacement [VSRR]) and Bentall surgery differently affect myocardial dysfunction is not clear. This study aimed to clarify whether the type of aortic root surgery, in case of at least grade 2 aortic regurgitation associated with myocardial dysfunction, affects the outcomes.
METHOD: Extraction from the Heart Valve Society aortic valve database (Aortic Valve Insufficiency and ascending aorta Aneurysm InternATiOnal Registry [AVIATOR]) was performed and two groups of patients operated between July 2007 and December 2022 were identified: Group

BACKGROUND: Mixed aortic valve disease (MAVD) is defined by the presence of concurrent aortic stenosis (AS) and aortic regurgitation (AR). Transcatheter aortic valve replacement (TAVR) is effective in MAVD patients with mortality rates comparable to pure aortic stenosis (PAS). While most TAVR complications also occur at similar rates between the MAVD and PAS populations, recent studies have shown conflicting results.
METHODS: A systematic literature review was conducted on PubMed and Embase for studies on the outcomes of TAVR in MAVD from inception until May 2024. Primary outcomes were short-

Conservative management of aortic insufficiency relies on three physiological strategies preserving native valve function and root dynamics: aortic valve repair; the Ross procedure; and personalized external aortic root support (PEARS). Aortic valve repair is the cornerstone of conservative surgery for dystrophic bicuspid and tricuspid aortic insufficiency, with or without aneurysm, as it restores normal life expectancy while reducing lifelong prosthetic valve-related complications. Standardized techniques combining calibrated annuloplasty with systematic cusp effective height assessment have

Cutibacterium acnes is a low-virulence pathogen that can cause prosthetic valve endocarditis (PVE), presenting significant diagnostic challenges due to its slow growth and indolent clinical course. We report a case of a 57-year-old male with a history of tissue aortic valve replacement, who presented with 6 months of intermittent fever and a nonproductive cough. Initial tests showed elevated inflammatory markers and moderate aortic regurgitation, but no vegetations were identified on transthoracic or transoesophageal echocardiography. After 13 months of symptoms, a PET scan showed significant

OBJECTIVES: To develop and internally validate a novel scoring system for bicuspid aortic valve (BAV) reparability based on routinely collected transoesophageal echocardiography (TEE) parameters.
METHODS: We conducted a retrospective analysis of 203 consecutive BAV patients who underwent surgery for aortic regurgitation between 2018 and 2024. Ten preoperative TEE parameters associated with valve repair were systematically analysed; significant variables were included in a multivariate analysis and then integrated into a scoring system.
RESULTS: Four baseline TEE measurements emerged as indep

AIMS: Mixed aortic valve disease poses unique haemodynamic challenges. This study compared the clinical outcomes of concomitant moderate aortic stenosis (AS) and moderate aortic regurgitation to isolated AS.
METHODS AND RESULTS: We analysed a multicentre cohort of valvular heart disease between 2008 and 2022 at three tertiary centres. The entire cohort was divided into three groups: moderate AS accompanied by moderate aortic regurgitation (moderate ASR), isolated severe AS, and isolated moderate AS. The primary outcome was a composite of cardiac death and hospitalization for heart failure. Th

BACKGROUND: Paravalvular leak (PVL) remains a significant complication after transcatheter aortic valve replacement (TAVR). This study investigated the relationship between perioperative serum albumin levels and post-TAVR paravalvular leak. METHODS: This retrospective observational study analyzed 1463 consecutive patients who underwent TAVR between 2013 and 2023. Serum albumin levels were measured preoperatively, immediately after procedure, and on postoperative days 1–5 and before discharge. Additional biomarkers including NT-proBNP and high-sensitivity troponin T were collected. Postprocedur

AIMS: Multivalvular heart disease (MVD), defined as coexisting lesions in two or more valves, poses significant diagnostic and prognostic challenges. While isolated valvular heart disease (VHD) is well characterized, the impact of moderate aortic regurgitation (AR) with another concomitant moderate VHD remains unclear. Current guidelines focus mainly on isolated severe lesions, potentially underestimating moderate MVD. This study assessed the prevalence and prognostic impact of concomitant moderate VHD in patients with moderate AR.
METHODS AND RESULTS: We retrospectively analyzed 794 patients

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos
