An Innovative Echocardiographic Scoring System for Bicuspid Aortic Valve Repair.

OBJECTIVES: To develop and internally validate a novel scoring system for bicuspid aortic valve (BAV) reparability based on routinely collected transoesophageal echocardiography (TEE) parameters. METHODS: We conducted a retrospective analysis of 203 consecutive BAV patients who underwent surgery for aortic regurgitation between 2018 and 2024. Ten preoperative TEE parameters associated with valve repair were systematically analysed; significant variables were included in a multivariate analysis and then integrated into a scoring system. RESULTS: Four baseline TEE measurements emerged as indep

Prognostic implications of moderate aortic stenosis with concomitant aortic regurgitation in degenerative aortic valve disease: insights from a multicentre cohort.

AIMS: Mixed aortic valve disease poses unique haemodynamic challenges. This study compared the clinical outcomes of concomitant moderate aortic stenosis (AS) and moderate aortic regurgitation to isolated AS. METHODS AND RESULTS: We analysed a multicentre cohort of valvular heart disease between 2008 and 2022 at three tertiary centres. The entire cohort was divided into three groups: moderate AS accompanied by moderate aortic regurgitation (moderate ASR), isolated severe AS, and isolated moderate AS. The primary outcome was a composite of cardiac death and hospitalization for heart failure. Th

Relationship between perioperative serum albumin levels and paravalvular leak after transcatheter aortic valve replacement.

BACKGROUND: Paravalvular leak (PVL) remains a significant complication after transcatheter aortic valve replacement (TAVR). This study investigated the relationship between perioperative serum albumin levels and post-TAVR paravalvular leak. METHODS: This retrospective observational study analyzed 1463 consecutive patients who underwent TAVR between 2013 and 2023. Serum albumin levels were measured preoperatively, immediately after procedure, and on postoperative days 1–5 and before discharge. Additional biomarkers including NT-proBNP and high-sensitivity troponin T were collected. Postprocedur

Prognostic significance of concomitant valvular disease in moderate aortic regurgitation.

AIMS: Multivalvular heart disease (MVD), defined as coexisting lesions in two or more valves, poses significant diagnostic and prognostic challenges. While isolated valvular heart disease (VHD) is well characterized, the impact of moderate aortic regurgitation (AR) with another concomitant moderate VHD remains unclear. Current guidelines focus mainly on isolated severe lesions, potentially underestimating moderate MVD. This study assessed the prevalence and prognostic impact of concomitant moderate VHD in patients with moderate AR. METHODS AND RESULTS: We retrospectively analyzed 794 patients

Transcatheter Mitral and Aortic Paravalvular Leak Closure: Innovations, Challenges, and Future Directions.

Paravalvular leak (PVL) is a well-known complication of valve replacement, occurring in up to 10% of aortic and 17% of mitral prosthetic valves. Although many leaks are small and clinically insignificant, ≈1% to 5% of cases are associated with clinically relevant complications, such as heart failure symptoms and hemolytic anemia. Surgical reoperation is currently considered the first-line option; however, it encompasses high mortality and morbidity risks. In the past years, transcatheter PVL closure has proven to be highly effective when performed in experienced centers. Transcatheter PVL clos

Successful management of left ventricular high lateral wall rupture during bentall procedure: a case report.

Left ventricular rupture is a highly fatal complication, with reported in-hospital mortality rates as high as 80%, largely due to the challenge of achieving timely surgical intervention. We present the case of a 59-year-old female who underwent a Bentall procedure for moderate-to-severe aortic regurgitation with concomitant aortic sinus and ascending aortic dilation. Shortly after weaning from cardiopulmonary bypass, a sudden rupture of the high lateral left ventricular wall occurred. Successful management involved a combined intracardiac and extracardiac repair: the external tear was first ex

Design and rationale of the COMPARE-TAVI 2 trial: An all-comers head-to-head comparison of Evolut FX+ and Sapien 3 Ultra Resilia transcatheter heart valves.

INTRODUCTION: The COMPARE-TAVI trial framework was launched for direct comparison of transcatheter aortic valve implantation (TAVI) valves. The COMPARE-TAVI 1 trial, comparing Myval/Myval Octacor versus Sapien 3/Sapien 3 Ultra transcatheter heart valves (THVs), was recently published. Here, we present the design and rationale for the COMPARE-TAVI 2 trial comparing the Evolut FX+ self-expandable THV with the Sapien 3 Ultra Resilia balloon-expandable THV. METHODS AND ANALYSIS: In the COMPARE-TAVI 2 trial (ClinicalTrials.gov NCT06470022), patients will be randomized 1:1 between the THVs. The tri

Transcatheter Aortic Valve Implantation in Complex Aortic Pathology: Managing Aortic Regurgitation With Chronic Dissection and Previous Endovascular Repair.

Transcatheter aortic valve implantation (TAVI) is generally contraindicated in patients with severe aortic regurgitation associated with anatomically complex scenarios such as prior aortic dissection and/or aortic graft replacement. However, it may be considered in selected patients following heart team evaluation and optimal procedure planning. We report the case of an 81-year-old man with severe aortic regurgitation (AR) who had previously undergone ascending aorta replacement and aorto-bisiliac endovascular aneurysm repair (EVAR) in two consecutive procedures, due to an acute aortic dissect

Hemodynamic analysis of the aortic root based on differences in outflow graft angle in patients with implantable left ventricular assist devices.

Aortic insufficiency (AI) that develops during long-term support with continuous-flow left ventricular assist devices (LVADs) remains a significant clinical problem. Abnormal flow patterns and altered wall shear stress (WSS) on the aortic valve are believed to contribute to leaflet remodeling, yet the hemodynamic influence of outflow graft orientation in patient-specific anatomies is not fully understood. This study investigated how different outflow graft angles affect aortic valve WSS and coronary perfusion using computational fluid dynamics (CFD). Three LVAD recipients who underwent preoper

Valve-sparing root replacement versus Bentall procedure with bioprosthetic valves in patients with aortic insufficiency: a meta-analysis.

OBJECTIVE: The use of bioprosthetic valves in the Bentall procedure (bio-Bentall) has increased for its anticoagulant-free lifestyle benefits. Valve-sparing root replacement (VSRR) offers an alternative to the bio-Bentall with the preservation of native valves. However, comparative evidence between VSRR and bio-Bentall is scarce. This meta-analysis aims to address this knowledge gap. METHODS: A systematic review was conducted on databases from inception through December 2025. Studies reporting adjusted outcomes were included. The primary outcome was mid-term all-cause mortality and aortic valv

Staphylococcus lugdunensis endocarditis presenting with P wave asystole, valvular destruction and spontaneous closure of an acquired Gerbode defect.

Staphylococcus lugdunensis is a highly virulent coagulase-negative staphylococcus, capable of causing rapidly destructive infective endocarditis (IE). We report an elderly male who developed P wave asystole as the initial manifestation of S. lugdunensis IE, occurring in the absence of fever or classical IE symptoms. Transoesophageal echocardiography (TOE) revealed severe aortic regurgitation from cusp destruction, a perivalvular abscess and an acquired Gerbode defect-a rare left ventricular-to-right atrial communication. An urgent aortic valve replacement was performed, and intraoperatively, t

Infectious seeds of valve calcification: Exploring the bacterial hypothesis in the pathogenesis of calcific aortic valve disease.

BACKGROUND: Calcific aortic valve disease (CAVD) is a multifactorial condition characterized by progressive leaflet calcification with a potential role for bacterial colonization in its pathogenesis. This study investigates clinical, microbiological and molecular features of calcified versus non-calcified aortic regurgitation (AR) valves. METHODS: This is a prospective, observational study, whose primary objective was to compare the occurrence of bacterial detection between CAVD and AR. The secondary objectives included the evaluation of bone-related calcification markers in valves from CAVD

Explant of a 21-mm homograft with redo aortic root replacement with a 25-mm bioprosthesis.

Re-operative management of a failing aortic homograft presents substantial technical challenges due to progressive calcification of the valve leaflets, annulus, coronary button attachments and surrounding aortic root structures. In this video tutorial, we demonstrate the explantation of a heavily calcified 21-mm aortic homograft and performance of a redo aortic root replacement using a handmade composite Bentall graft with a 25-mm bioprosthesis and 28-mm straight Dacron graft. A 73-year-old man, 23 years post-homograft root replacement, presented with torrential aortic regurgitation and a smal

Quantifying myocardial function characteristic with myocardial work measurement in patients with different aortic regurgitation severity.

BACKGROUND: Assessing subclinical myocardial dysfunction in aortic regurgitation (AR) with preserved left ventricular ejection fraction (LVEF) is challenging due to load-dependent conventional parameters. Non-invasive myocardial work (MW) integrates left ventricular pressure and strain to provide afterload-adjusted performance indices. This study aimed to characterize MW across AR severity in patients with preserved LVEF. METHODS: We prospectively enroled 130 patients categorized as control (n = 30), mild (n = 30), moderate (n = 34), and severe (n = 36) according to guidelines. MW parameters

Acute myocardial injury with normal coronary arteries in a young male: a case report of aortic dissection with rapidly progressive aortic regurgitation.

Type A aortic dissection (TAAD) is a life-threatening condition that often presents diagnostic challenges, particularly when mimicking acute myocardial infarction (AMI). This case report describes a 31-year-old male with a history of fever and no cardiovascular history who presented with acute chest pain and rapidly developed hemodynamic instability. Initial assessments revealed mild aortic regurgitation (AR) and ascending aortic dilation but no aortic dissection was reported. ECG showed ST-segment changes, yet coronary angiography ruled out obstructive coronary disease. Serial daily echocardi

Neo-Aortic Valve Insufficiency and Root Dilatation During and After the Staged Palliation for Hypoplastic Left Heart Syndrome and Variants.

In hypoplastic left heart syndrome (HLHS) and variants, the pulmonary valve functions as the neo-aortic valve after the Norwood procedure. Neo-aortic valve insufficiency (Neo-AI) represents an emerging complication with implications for long-term survival. This study investigated the clinical impact and patterns of Neo-AI development following staged palliation. Neonates undergoing the Norwood procedure (2001–2023) were retrospectively reviewed. Neo-AI was evaluated using serial echocardiograms, with patients classified into two groups: early onset (during the Norwood hospitalization) and late

Mechanical versus biological aortic valve replacement in patients aged 50-70 years: a systematic review and meta-analysis.

OBJECTIVES: The optimal prosthetic aortic valve replacement (AVR) for long-term outcomes in patients aged 50-70 years remains uncertain. International guidelines differ; contemporary randomised trials are lacking, and use of bioprosthetic valves has increased. This review synthesised evidence comparing efficacy and safety outcomes between valve types in this age group. METHODS: A systematic review and meta-analysis of randomised and observational studies comparing mechanical and bioprosthetic AVR in patients aged 50-70 years was conducted. Medline, Embase and the Cochrane Library were searche
