Hemodynamic Valve Deterioration After Transcatheter Aortic Valve Replacement: Incidence, Predictors, and Clinical Outcomes.

BACKGROUND: Reports on the durability of transcatheter aortic valve replacement (TAVR) prostheses are scarce and confounded by varying definitions and competing risks of death. OBJECTIVES: The authors sought to determine the incidence, predictors, and clinical outcomes of hemodynamic valve deterioration (HVD) according to the Valve Academic Research Consortium 3 definition after TAVR. METHODS: We analyzed consecutive patients undergoing TAVR in the prospective Bern TAVI (Transcatheter Aortic Valve Implantation) registry between August 2007 and June 2022 for the incidence and predictors of HV

Valve-Sparing vs Bio-Bentall Aortic Root Replacement in Patients Aged 60 to 75 Years: Survival, Reintervention, and Aortic Regurgitation.

BACKGROUND: Valve-sparing root replacement (VSRR) with the David technique is an established therapy for aortic root pathology in young patients. This study evaluated short- and long-term outcomes between VSRR and aortic root replacement (ARR) with a biological-valved conduit in sexagenarians. METHODS: A multicenter retrospective review from 2002 to 2022 identified 299 sexagenarians undergoing aortic root surgery, among whom 82 (27.4%) underwent VSRR and 217 (72.6%) underwent bio-Bentall. Inverse probability of treatment weighting (IPTW) was applied to balance clinical variables. Median follo

Sub-coronary ROSS operation in a young man with quadricuspid aortic valve and anomalous aortic origin of the left coronary artery: A case report.

RATIONALE: Quadricuspid aortic valve (QAV) is a rare condition with a very low incidence. Anomalous aortic origin of a coronary artery (AAOCA) is the most prevalent form of coronary anomaly. One variant of AAOCA is the anomalous aortic origin of the left coronary artery from the right coronary sinus (L-AAOCA). It is even rarer for QAV to occur in combination with AAOCA. PATIENT CONCERNS: In this case report, we present a patient who was diagnosed with QAV and L-AAOCA using preoperative transesophageal echocardiography and 3-dimensional computed tomography (3D-CT). DIAGNOSES: Cardiac-enhanced

Left ventricular rigor mortis interferes with postmortem aortic root geometry.

Aortic regurgitation is a common valve disease and can be caused by delineated findings such as fenestrations or hardly discernible alterations of the aortic root geometry. Therefore, aortic regurgitation can be a challenging diagnosis during an autopsy. Cardiac surgeons, however, are confronted with comparable problems during surgery and have developed a refined knowledge of the anatomy of the aortic root including its geometry. Transferring this knowledge from the operating room to the dissection would further complement the panel of postmortem diagnostic tools. To foster translation of the

Aortic regurgitation in ankylosing spondylitis-an echocardiography follow-up study.

OBJECTIVES: To investigate the long-term course of aortic regurgitation (AR) and the width of the proximal ascending aorta (PAA) in patients with ankylosing spondylitis (AS). METHOD: This is a follow-up cohort study of patients with AS examined with echocardiography at inclusion (2009 to 2011). Out of the initial 187, a subgroup of 52 patients (54% men, mean age 62 years) was selected for follow-up based on presence/absence of AR at baseline; 26 with AR (18 mild, 7 moderate, 1 severe) and 26 age/sex-matched without AR. These patients were re-examined with echocardiography in 2014 by an indepe

Suspected Transient Ischemic Attack Related to Dysfunctional Quadricuspid Aortic Valve.

We describe a 30-year-old Caribbean-Black woman with a clinical presentation suggestive of a transient ischemic attack (TIA) with no conventional cerebrovascular risk factors, albeit with a newly diagnosed quadricuspid aortic valve (QAV) with moderate aortic regurgitation (AR). Although QAV is a recognized congenital cardiac defect, its association with TIA remains elusive. This case highlights the importance of considering potential atypical etiologies, such as QAV, in the evaluation and management of young patients presenting with cerebrovascular events.

Autograft dilation after Ross procedure in children and young adults is mitigated by autograft reinforcement: A retrospective MRI study.

Limited magnetic resonance imaging (MRI) data on autograft dilatation following the Ross procedure in congenital cohorts presents challenges in understanding its evolution and impact on clinical outcomes. This study, spanning from February 2003 to December 2022, included patients under 40 years at the time of the Ross procedure, with MRI follow-ups assessing dimensions at key aortic sites. Among 307 patients, 132 MRIs were analyzed from 76 individuals, revealing that autograft z-scores increase primarily with time post-procedure (Coef. 0.13; 95% CI:0.051-0.216; P = 0.002). Additionally, older

Image-Based Analysis of Correlation Between Valve Stent Deformation and Valve Function in Transcatheter Aortic Valve Replacement.

BACKGROUND AND OBJECTIVES: Currently, there remains a paucity of research on the deformation and valve function of transcatheter heart valves (THV) in patients with aortic regurgitation (AR) following transcatheter aortic valve replacement (TAVR). This study aimed to thoroughly explore the correlation between THV deformation and postoperative hemodynamics in these patients. METHODS: In this study, we assessed 39 AR patients treated with the J-Valve THV system during TAVR. We utilized postoperative cardiac-enhanced computed tomography angiography (CTA) to examine the extent of stent deformatio

Aortic valve repair in adults: long-term clinical outcomes and echocardiographic evolution in different valve repair techniques.

OBJECTIVES: Aortic valve repair/sparing techniques have been established as effective treatments for aortic regurgitation and/or aortic aneurysms. However, concerns remain regarding long-term durability, reproducibility and patient selection. This study aims to asses long-term clinical and echocardiographic outcomes, with a focus on aortic regurgitation grade and left ventricular ejection fraction evolution, in adults undergoing these procedures. METHODS: Adult patients in the Heart Valve Society Aortic Valve Database, undergoing any aortic valve repair/sparing technique were included. Time-t

Propensity score matching analysis of valve-sparing versus aortic root replacement in type A aortic dissection patients.

This study compared long-term survival and reintervention rates between Valve-Sparing Root Replacement (VSRR, n = 244) and Aortic Root Replacement (ARR, n = 499) in 743 patients undergoing Type A acute aortic dissection (AAD), given the lack of prospective comparative data. Multivariable analysis is identifying advanced age, high Body Mass Index (BMI), Marfan syndrome, severe aortic regurgitation, bicuspid aortic valve, increased aortic root diameter, and reduced aortic cross-clamp time (ACC) as significant factors associated with ARR. After Propensity Score Matching (PSM), VSRR is showing sig

Midterm Outcomes of Minimally Invasive Aortic Valve Replacement via Right Lateral Minithoracotomy.

OBJECTIVE: Minimally invasive aortic valve replacement (MIAVR) procedures have been found to have good short-term results. However, no known reports regarding outcomes of MIAVR via a right lateral minithoracotomy (LT) approach over longer terms have been presented. The aim of the present study was to analyze the midterm outcomes of the use of MIAVR with the right LT approach over an 8-year period. METHODS: Between September 2014 and February 2023, MIAVR was performed for 348 patients with severe aortic valve stenosis and regurgitation at our hospital. Operative mortality, all-cause mortality,

The hemi-Yacoub procedure and valve repair in a patient with Turner syndrome and bicuspid aortic valve.

Bicuspid aortic valve is a common congenital anomaly often associated with aortic valve degeneration and ascending aorta dilation, leading to stenosis or insufficiency. We present the case of a 46-year-old female with Turner syndrome, a Sievers type 1 bicuspid aortic valve, moderate aortic insufficiency and a dilated aortic root. Given her age and anatomical features, a valve-sparing aortic root replacement using the hemi-Yacoub technique was performed. This technique preserved the patient's native aortic valve, thereby avoiding the need for anticoagulation therapy. Postoperatively, no residua

Aortic annuloplasty with internal geometric ring; single-center midterm outcomes.

OBJECTIVE: Surgical aortic valve replacement has been the mainstay of treatment against severe aortic insufficiency despite the high incidence of prosthesis-related complications and better long-term outcomes following aortic valve repair. Annuloplasty and leaflet reconstruction are the integral parts of the procedure. Safety and efficacy of HAART internal annuloplasty ring are hereby investigated through mid-term outcomes of a single referral center. METHODS: Forty-three consecutive patients with either trileaflet or bicuspid aortic insufficiency along with ascending aorta and/or aortic root

Neoaortic Dilation and Polyvalvar Dysfunction: The Coexistence of Marfan Syndrome with Hypoplastic Left Heart Syndrome.

We present a unique case of progressive neoaortic root and neoascending aortic dilatation, with poly-valvar regurgitation in a patient with Marfan syndrome and hypoplastic left heart syndrome palliated to Fontan procedure. This progressive dilation and valvar dysfunction necessitated a modified Bentall procedure and tricuspid valve repair. Our case highlights the impact of both a primary and secondary aortopathy complicating the Fontan procedure and the need for mechanical aortic valve replacement.

A murine model of aortic regurgitation generated by trans-apical wire destruction of the aortic valve.

BACKGROUND: The mechanisms underlying cardiac remodeling in aortic valvular (AoV) disease remain poorly understood, partially due to the insufficiency of appropriate preclinical animal models. Here, we present a novel murine model of aortic regurgitation (AR) generated by trans-apical wire destruction of the AoV. METHODS: Directed by echocardiography, apical puncture of the left ventricle (LV) was performed in adult male C57BL/6 mice, and a metal guidewire was used to induce AoV destruction. Echocardiography, invasive LV hemodynamic and histological examination were conducted to assess the de

Current status of aortic valve repair surgery.

Severe aortic valve regurgitation, if not timely treated, can significantly impact patients' survival both for tricuspid aortic valve and bicuspid aortic valve patients, with the latter being significantly younger. Increased understanding of the root anatomy and its physiology has opened the way to techniques of aortic valve repair surgery. The techniques mainly relate to re-establishing a correct root and annular geometry and eliminating leaflet prolapse. These techniques are applied both in the presence of a dilated and normal root and are equally valid for tricuspid or bicuspid valve. Techn
