BACKGROUND: In some patients, transcatheter aortic valve implantation (TAVI) is associated with conduction disturbances, necessitating permanent pacemaker implantation (PMI). The clinical course of conduction disturbances after PMI, based on the type of TAVI valve, remains unclear. Therefore, the present study aimed to evaluate the effects of different types of TAVI valves on the clinical course of conduction disturbances.
METHODS: In total, 480 patients, including 230 with self-expandable (SE) valves and 250 with balloon-expandable (BE) valves, who underwent TAVI between January 2016 and Dec

This study aimed to determine the financial implications of isolated transcatheter aortic valve replacement (TAVR) versus surgical aortic valve replacement (SAVR) in the Brazilian Public Health System (SUS) using time-driven activity-based costing (TDABC). We performed a retrospective observational cohort study at a single center from 2019 to 2022, including 180 patients (TAVR: n = 65; SAVR: n = 115). TAVR patients were older and had higher surgical risk. The total cost of TAVR was significantly higher than SAVR (INT$ 40,776.9 vs INT$ 12,393.7, p <0.001), primarily driven by the prosthetic val

BACKGROUND: Sutureless aortic valve replacement (SU-AVR) and transcatheter aortic valve implantation (TAVI) have expanded treatment options for severe aortic stenosis, but comparative real-world data remain limited.
AIMS: We aimed to compare early and up to 6-month clinical and hemodynamic outcomes of SU-AVR versus transfemoral TAVI.
METHODS: All consecutive patients undergoing SU-AVR (n = 53) or TAVI (n = 245) between 2014 and 2023 were retrospectively analyzed. Propensity score matching (1:1 nearest neighbor, no replacement) generated 53 balanced pairs.
RESULTS: Paravalvular leak was more

BACKGROUND: Redo transcatheter aortic valve implantation (TAVI) is increasingly used to treat bioprosthetic valve dysfunction in patients who have undergone TAVI. As TAVI indications continue to expand to include younger patients, it is essential to systematically document redo TAVI procedures to better understand their long-term efficacy and safety. This study aimed to compare outcomes between redo TAVI and first TAVI procedures using a propensity score-matched analysis.
AIMS: To compare the long-term clinical outcomes, including death, stroke and procedural adverse events, between redo TAVI

BACKGROUND AND AIMS: Unlike systolic heart function, the assessment of diastolic heart function is often demanding using cardiovascular magnetic resonance (CMR) imaging. This study aims to assess the value of semi-automatic CMR-derived biplane functional parameters in patients with cardiac light chain (AL) and transthyretin (ATTR) amyloidosis or hypertrophic cardiomyopathy (HCM).
METHODS: This retrospective, multi-center, multi-vendor study included patients with a diagnosis of HCM or cardiac AL/ATTR amyloidosis. Proven healthy volunteers served as controls. CMRs were assessed semi-automatica

BACKGROUND: Moderate aortic stenosis (AS) poses a substantial risk for adverse outcomes, yet its prognostic determinants and cardiac remodeling markers remain underexplored. This study aimed to evaluate prognostic factors, including left atrial strain, and their predictive value for adverse cardiac events in patients with moderate AS.
METHODS: A retrospective cohort study included 1125 patients diagnosed with moderate AS between 2008 and 2019. Cardiac remodeling indices, including left atrial reservoir strain (LARS) and left ventricular global longitudinal strain, were assessed using speckle-

BACKGROUND: The optimal management of coronary artery disease in patients undergoing transcatheter aortic valve replacement (TAVR) remains unclear, and evidence supporting routine percutaneous coronary intervention (PCI) beforehand is limited. This study aimed to evaluate whether PCI before TAVR provides clinical benefit compared with conservative management in patients with significant coronary artery disease, using nationwide Swedish registry data.
METHODS: This observational study included 2578 Swedish patients with significant coronary artery disease (≥50% angiographic stenosis or physiol

BACKGROUND: Previously proposed staging classification based on cardiac damage highlighted that right ventricular (RV) damage reflects the most advanced stage in patients with severe aortic stenosis. This study aimed to investigate the association between RV damage detected by RV free wall longitudinal strain (RVFWLS) and outcomes in patients with severe aortic stenosis who underwent transcatheter aortic valve implantation (TAVI).
METHODS: A total of 768 patients with severe aortic stenosis who underwent TAVI and echocardiographic images for RV functional assessment were included from a retro

BACKGROUND AND AIMS: Lipoprotein(a) [Lp(a)] and LDL cholesterol (LDL-C) are causally linked to aortic valve calcium (AVC) and aortic stenosis (AS). Lipoprotein(a) has anti-fibrinolytic properties; therefore, aspirin may reduce cardiovascular disease risk among individuals with high Lp(a). This analysis sought to determine the association of aspirin with incident AVC and AS across Lp(a) and LDL-C levels.
METHODS: This observational study included up to 6598 participants in the Multi-Ethnic Study of Atherosclerosis. Aortic valve calcium was measured on non-contrast cardiac computed tomography.

BACKGROUND AND AIMS: Predicting coronary artery occlusion due to sinus sequestration (SS) during a second transcatheter aortic valve (TAV) procedure using pre-procedural computed tomography (CT) after the first TAV replacement (TAVR) is underreported. This study aimed to investigate the accuracy of SS risk prediction using pre-procedural CT by comparing it to the actual SS risk observed on post-procedural CT during TAV-in-TAV.
METHODS AND RESULTS: We retrospectively evaluated 139 consecutive patients who underwent TAVR with a balloon-expandable valve. The predicted valve-to-sinotubular juncti

BACKGROUND: Coarctation of the aorta (CoA) is a well-known congenital heart disease, which is often associated with other cardiac and vascular anomalies, most frequently a bicuspid aortic valve. Aortic coarctation is an unusual cause of hypertension during pregnancy, and its management is not clarified.
CASE SUMMARY: We report a case of a 24-year-old pregnant woman with long-standing hypertension, who presented at the end of the first trimester with severe refractory hypertension. The diagnostic investigation culminated in the diagnosis of a CoA. Facing risks for pursuing pregnancy such as ao

IMPORTANCE: Selection of patients with a bicuspid aortic valve and aortopathy for prophylactic aortic surgery remains challenging. In thoracic aortopathy, aortic medial elastin fiber fragmentation initially leads to microcalcification but later declines with progressive loss of elastin content and reduced structural integrity.
OBJECTIVE: To determine whether aortic microcalcification detected using fluorine F 18-labeled [18F]-sodium fluoride positron emission tomography (PET) is associated with future aortic diameter expansion.
DESIGN, SETTING, AND PARTICIPANTS: This prospective longitudinal

A sinus of Valsalva aneurysm (SOVA) is an aneurysm of the aortic root between the aortic valve annulus and the sinotubular junction. Unruptured SOVAs are often asymptomatic and discovered incidentally during cardiac imaging. Rupture of a SOVA can lead to acute or insidious hemodynamic compromise. When SOVAs are diagnosed, they are accompanied by the decision to recommend surgical intervention versus conservative medical therapy. We are reporting a challenging case of a young female patient presenting with an incidental finding of an aneurysm of the sinus of Valsalva. A previously healthy 21-ye

Hypertrophic cardiomyopathy (HCM) is a genetic myocardial disease. In 20% to 30% of patients, a disease-causing variant can be identified and may also be present in relatives. Individuals carrying a pathogenic variant (G+) without left ventricular hypertrophy (LVH) are classified as genotype-positive/phenotype-negative (G+/P-). Their risk of developing LVH or HCM-related events remains uncertain. The aim of the article is to describe the clinical course of G+/P- individuals during long-term follow-up. G+/P- individuals were recruited from relatives of HCM patients at a tertiary center. All und
