Successful staged anatomic repair for congenitally corrected transposition with both atrioventricular valves' regurgitation: a case report.

A 4-year-old boy with dextrocardia, congenitally corrected transposition of the great arteries, pulmonary valve stenosis, and ventricular septal defect demonstrated progressive hypoxemia and mild-to-moderate mitral and tricuspid regurgitation. Cardiac magnetic resonance imaging revealed insufficient right ventricular volume for Senning-Rastelli type anatomical repair. Given the elevated surgical risk associated with mitral regurgitation and inadequate ventricular volume, an initial palliative approach was undertaken at 2 years of age, consisting of systemic-to-pulmonary shunt placement and mit

Percutaneous balloon valvuloplasty in a small cat with pulmonary valve stenosis and concomitant tricuspid valve dysplasia.

A 14-month-old, male castrated, 3,9-kg Singapura cat was referred for treatment of severe pulmonary valve stenosis associated with tricuspid valve dysplasia. The cat was pretreated with atenolol and scheduled for pulmonary balloon valvuloplasty. Because the right femoral vein could not accommodate a 5 Fr introducer, vascular access was obtained via the left external jugular vein, followed by right ventricular catheterisation, pressure measurement and angiography. A 10 mm low pressure balloon catheter was advanced over a guidewire to the level of the pulmonary valve and inflated once, resulting

Development and validation of a prognostic model of postnatal circulation in fetuses with pulmonary atresia or critical pulmonary stenosis with intact ventricular septum: protocol for an observational cohort study with prospective validation.

BACKGROUND: Pulmonary atresia or critical pulmonary stenosis with intact ventricular septum (PA/CS-IVS) is a rare and heterogeneous congenital heart disease with highly variable postnatal outcomes. Existing prenatal prognostic models are limited by small sample sizes, heterogeneity in echocardiographic variables, and suboptimal validation. Robust prenatal prediction of postnatal circulation is essential for parental counselling, perinatal planning, and selection of candidates for potential fetal intervention. METHODS: This international observational cohort study will include non-intervened f

Severe pulmonary valve stenosis associated with advanced atrioventricular conduction disease: A rare case report.

Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Pulmonary hypertension in patients with Noonan syndrome.

BACKGROUND: Noonan syndrome is a RASopathy inherited in an autosomal dominant manner, mainly caused by gain-of-function variants activating the RAS/mitogen-activated protein kinase signalling pathway. Pulmonary hypertension (PH) may occur in Noonan syndrome, but its mechanisms, clinical characteristics and outcomes remain poorly defined. METHODS: We analysed data from the French PH Network to characterise the phenotype of Noonan syndrome patients who develop PH, and conducted a systematic analysis of the literature. RESULTS: Seven patients were identified from the French PH Network (male/fem

The association between paternal race and ethnicity and a spectrum of birth defects in a national case-control study.

PURPOSE: To estimate associations between paternal race and ethnicity and a spectrum of birth defects. METHODS: We analyzed data from the National Birth Defects Prevention Study for infants with birth defects and controls delivered between 1997-2011. Using unconditional logistic regression, we assessed associations between paternal race and ethnicity and 32 birth defects, before and after adjusting for maternal race and ethnicity and 14 other factors. RESULTS: Data from 33,455 fathers were analyzed (889 Asian/Pacific Islander [A/PI], 8394 Hispanic, 4139 non-Hispanic Black [NHB], and 20,033 n

Giant pericardial cyst with coronary artery involvement: a rare case of progressive right heart compression in an infant.

Pericardial cysts are rare, benign congenital cardiovascular malformations that account for approximately 7% of mediastinal masses. Epicardial cysts attached to the cardiac surface with intimate coronary artery involvement are even rarer and pose significant diagnostic and surgical challenges. This case highlights a giant pericardial cyst with intimate right coronary artery involvement in a 10-month-old infant, where subtotal resection was necessary to preserve coronary integrity. A 10-month-old male infant with a pericardial cyst initially detected at 27 weeks of gestation presented with prog

Balloon pulmonary valvuloplasty as a palliative strategy for late presenting adolescents and adult patients with cyanotic congenital heart disease.

BACKGROUND: Management of late presenters with cyanotic congenital heart disease (CCHD) and reduced pulmonary blood flow is challenged by multi-system derangements from longstanding hypoxia in addition to anatomic factors. Balloon pulmonary valvuloplasty (BPV) is a potentially viable palliative option for selected patients with severely stenotic pulmonary valve and are unsuitable for definitive surgical repair METHODS: A retrospective chart review was conducted (2002-2024, age >12 years). BPV was considered for those with a significant component of valvular pulmonary stenosis (PS) on echocardi

General Anesthesia With Remimazolam in Fontan Procedure: A Case Report.

The Fontan procedure requires anesthetics, allowing rapid awakening for early extubation and less circulatory depression owing to passive pulmonary blood flow hemodynamics. Remimazolam, a short-acting benzodiazepine, may be a suitable anesthetic choice; however, its use in this procedure is unreported. We present a 2-year-old boy with a single-right ventricle and pulmonary valve stenosis who underwent the Fontan procedure under anesthesia with remimazolam (3-8 mg kg-1 h-1) and fentanyl (21.8 μg/kg). Hemodynamics remained stable under electroencephalographic monitoring, and the patient was extu

Dysregulated TGFβ-ERK Signaling Drives Aberrant Extracellular Matrix Production in Noonan Syndrome-Associated Pulmonary Valve Stenosis.

Pulmonary valve stenosis (PVS) is the most common congenital heart defect in Noonan syndrome (NS) and related RASopathies, yet the molecular mechanisms linking pathogenic variants to the valve pathology remain poorly defined. Here, we utilized a human iPSC-based valve differentiation platform to generate the cardiac valve cell lineages-including fibrosa and spongiosa valve interstitial cell (VIC) subtypes. CRISPR-edited iPSCs harboring NS gain-of-function RAS/MAPK and Noonan syndrome with multiple lentigines (NSML) dominant-negative RAS/MAPK variants exhibited early defects in mesodermal and e

Transvalvular pulmonary stent angioplasty for palliation of severe pulmonic stenosis in two cats.

Transvalvular pulmonic stent angioplasty has been described in dogs with severe pulmonic stenosis, when balloon valvuloplasty is not indicated or possible. We report two cats that underwent stent placement across the stenotic pulmonary valve for similar indications. Both cases had good short- to medium-term outcomes. One cat experienced immediate postoperative complications in the form of pulmonary edema, while the other cat had no observed complications. Pulmonic stent angioplasty has not been previously reported in cats. This report demonstrates a possible alternative intervention in cats wi

Decellularized Pulmonary Homograft Repair of Idiopathic Main Pulmonary Artery Aneurysm in SC Hemoglobinopathy.

BACKGROUND: Pulmonary artery aneurysm is a rare and potentially fatal condition, often diagnosed incidentally. The lack of specific guidelines makes it difficult to define clear intervention criteria and the ideal reconstructive technique. Decellularized pulmonary homografts (DPHs) have emerged as a promising alternative, although they are still scarcely reported. CASE SUMMARY: A 48-year-old previously healthy man presented with mediastinal widening on a routine chest x-ray. Work-up revealed an idiopathic main pulmonary aneurysm measuring 52 mm, associated with moderate pulmonary valve stenos

Severe pulmonary trunk stenosis associated with R2A-type coronary artery anomaly in a dog: echocardiographic findings and post-mortem contrast-enhanced CT confirmation.

A 1.5-year-old female French Bulldog was diagnosed with severe valvular pulmonic stenosis, coronary artery anomaly, and clinical signs of right-sided heart failure. Despite medical management, the patient’s condition deteriorated, and euthanasia was elected after 6 months of treatment. Post-mortem contrast-enhanced computed tomography additionally revealed a single right coronary ostium and confirmed a prepulmonary course of the left coronary artery, consistent with an R2A-type coronary anomaly. This anomaly could have significantly complicated potential surgical or catheter-based intervention

Optimizing Diagnostic Accuracy of Clinical Red Flags in RASopathies.

RASopathies are a group of genetic disorders caused by pathogenic variants in the RAS-mitogen-activated protein kinase (RAS-MAPK) signaling pathway, often presenting with congenital heart defects, craniofacial dysmorphisms, and developmental delays. To assess the diagnostic yield of genetic testing in patients with suspected RASopathies and to identify predictive clinical "red flags" using machine learning (ML). This retrospective study included patients evaluated at our institution between 2020 and 2023. All patients underwent cardiovascular evaluation, and a subset of individuals with suspec

Diagnostic pitfalls in constrictive pericarditis coexisting with right ventricular outflow tract obstruction and coronary artery anomaly.

Constrictive pericarditis associated with an anomalous left anterior descending artery is a very rare and complex condition. The aim of this report is to highlight the diagnostic challenges and surgical management in this disease. A 40-year-old man with right heart failure, dyspnea, and ascites, underwent comprehensive cardiac evaluation and surgery. The pulmonary valve was dysplastic, causing severe right ventricular outflow tract (RVOT) obstruction. During resection, the left anterior descending artery was inadvertently transected and bypassed with a saphenous vein graft. RVOT reconstruction
