The Ross procedure is a common surgical option for congenital aortic valve disease, but little is understood about the long-term ventricular mechanics of patients who undergo the Ross procedure. This study evaluated the late-term diastolic function of Ross patients compared to patients who underwent other types of index surgical repairs for congenital heart lesions. Patients who underwent catheterization for transcatheter pulmonary valve implantation (TPVi) at a single center between 2010 and 2022 were retrospectively reviewed. Patients who had undergone a Ross procedure were compared to those

BACKGROUND: Home monitoring programs (HMPs) have been implemented for high-risk postoperative patients with congenital heart diseases.
OBJECTIVE: We recently extended the indications for HMPs to preoperative patients with tetralogy of Fallot (ToF). Thus, our purpose was to evaluate the current practice of home monitoring for preoperative patients with ToF, emphasizing the early detection of hypoxic spell events and the prevention of preoperative attrition. Furthermore, we sought to identify the risk factors for unplanned urgent operations during the home monitoring period and to evaluate the

BACKGROUND: Recurrent interventions are required following surgical treatment of congenital heart diseases involving pathologies of the right ventricular outflow tract (RVOT). Percutaneous pulmonary valve implantation (PPVI) is one of the most commonly employed catheter-based interventions for this purpose.
AIMS: The aim of this study is to share the early-term feasibility, effectiveness, and hemodynamic outcomes of PPVI using Meril's Myval pulmonary valve in the largest patient series from a single center.
METHODS: This study included patients with congenital heart disease who had RVOT dysf

Use of the Harmony valve within a failing surgical bioprosthesis has not been described to date, especially with respect to compression of the valve and valve housing when Harmony is used in smaller, non-compliant landing zones. Described is a case of a 45-year-old male with a history of congenital pulmonary valve stenosis who underwent surgical pulmonary valve placement with a 25-mm Biocor Epic bioprosthesis and developed pulmonary regurgitation. The patient underwent cardiac catheterization with high-pressure balloon valvuloplasty with intentional fracturing and resultant compression of the

Background: Mitral valve replacement in infants is highly challenging due to size limitations and the lack of suitable prosthetic valves. The Melody™ valve, originally developed for transcatheter pulmonary valve replacement, offers an off-label alternative for surgical implantation in the mitral position, with the possibility of balloon expansion and valve-in-valve procedures during somatic growth. Aim: To evaluate the long-term outcomes and technical feasibility of the Melody™ valve in the mitral position in infants and very young children following failed surgical repair. Methods: This singl

OBJECTIVES: To evaluate the right ventricle function by conventional echocardiographic methods and strain analysis in the long term after balloon pulmonary valvuloplasty. In addition, we investigated the relationship between pulmonary regurgitation, demographic data at the time of the procedure, and right ventricle dysfunction in late follow-up.
METHODS: The records of patients submitted to balloon pulmonary valvuloplasty from 2001 to 2015 in a single centre were reviewed. From that sample, a revised cohort was formed, and the patients were submitted to clinical and echocardiographic evaluati

BACKGROUND: In patients with right ventricular (RV) outflow tract stenosis and pulmonary regurgitation (PR), percutaneous pulmonary valve implantation (PPVI) aims to preserve RV and left ventricular (LV) integrity and function. Our study aimed to assess acute changes in biventricular intrinsic myocardial function occurring with PPVI.
METHODS: Twenty patients with RV outflow tract dysfunction (mean±1 SD; age, 23.0±10.9 years; mean peak echocardiographic RV outflow tract gradient, 64±25 mm Hg) underwent PPVI with biventricular assessment of pressure-volume loops using the conductance catheter t

BACKGROUND: Excellent mid-term outcomes are well-established for percutaneous pulmonary valve implantation (PPVI) in treating right ventricular outflow tract (RVOT) dysfunction. Despite this success, long-term data assessing PPVI performance are limited, especially within middle- to low-income country contexts.
OBJECTIVE: To report on the patient characteristics and long-term outcomes after PPVI.
METHODS: This retrospective study describes outcomes for 31 PPVI patients treated from 2012 to 2023 at the paediatric cardiology department, Universitas Academic Hospital, Bloemfontein, South Africa

OBJECTIVE: The Rastelli operation is accepted as 1 of the standard techniques for complete transposition of the great arteries (TGA) and TGA-type double-outlet right ventricle with left ventricular outflow tract (LVOT) obstruction. The half-turned truncal switch (HTTS) operation has been reported as an alternative to the Rastelli approach. The aim of this study was to compare these 2 operations for children with TGA and LVOT obstruction (TGA/LVOTO).
METHODS: Between 2012 and 2024, 11 patients underwent TGA/LVOTO repair. The median age at repair was 12 months and median weight was 8.4 kg. Preo

A 15-year-old post-REV patient underwent transcatheter pulmonary valve replacement with a 23 mm SAPIEN 3 for right ventricular outflow tract (RVOT) stenosis and severe regurgitation. At 9 months, recurrent stenosis revealed multiple frame fractures. Balloon redilation up to 24 mm was limited by aortic root compression and recoil, prompting surgery. This first reported case of early SAPIEN 3 fracture in the pulmonary position highlights structural risks in complex RVOT anatomies.

Although the long-term outcomes of the surgical grafts are well defined and reported, the data regarding the mid-and long-term results of the balloon-expandable percutaneous valves in the native right ventricular outflow tract (RVOT) is limited. We retrospectively evaluated 42 patients who underwent PPVI (Sapien® XT valve) to native RVOT due to severe pulmonary regurgitation (PR) and/or moderate to severe pulmonary stenosis (PS) between August 2015 and November 2020. The median patient age at the time of PPVI was 13.4 years (6.1-36.5 years). The median body weight of the patients was 42 kg (15

Information on mid-term outcomes of percutaneous pulmonary valve replacement (PPVR) with the Edwards Sapien (ES) valve in the native right ventricular outflow tract (RVOT) are limited. This study assesses mid-term outcomes in 76 patients who underwent PPVR between 2016 and 2022, comparing native (40.8%) and non-native (59.2%) RVOTs. The primary endpoint was a composite of endocarditis, reinterventions, and cardiovascular death and secondary outcomes included prosthetic valve dysfunction (PVD), tricuspid regurgitation (TR), right ventricular ejection fraction (RVEF), and indexed ventricular vol

Congenitally corrected transposition of the great arteries (ccTGA) is a complex cardiac abnormality that represents less than 1% of all congenital heart defects. It is characterized by a unique pathophysiology involving both atrioventricular and ventriculoarterial discordance and may occur with or without cardiac abnormalities such as ventricular septal defects, pulmonary stenosis, or tricuspid valve anomalies. A man in his 20s presented with a 3-week history of mild dyspnea during strenuous activities. The patient was diagnosed with isolated ccTGA based on electrocardiogram and echocardiogram

BACKGROUND: To determine the impact of tricuspid regurgitation (TR) on outcomes after transcatheter aortic valve implantation (TAVI), and to determine the evolution of TR severity after TAVI.
METHODS: This was an observational study of TAVIs for aortic stenosis from November 2012 to December 2021. Patients were dichotomized according to the severity of pre-TAVI TR: less than moderate versus moderate or greater. Logistic regression was used to determine the risk factors for progression or persistence of moderate or greater TR at 1 year after TAVI.
RESULTS: A total of 2250 patients were includ

Background: Severe neonatal Ebstein's anomaly (EA) is associated with a high risk of mortality. A new therapeutic approach aims to combine the advantages of Starnes' procedure in stabilizing critically ill neonates with the long-term superiority of biventricular physiology after cone reconstruction. Case report: The echocardiography of a male preterm (36 weeks' gestation; birth weight 2400 g) demonstrated EA Carpentier type C, membranous pulmonary atresia, and hypoplastic pulmonary arteries (PAs). After undergoing the Starnes procedure postnatally, multiple dilatations of the AP shunt and the

BACKGROUND: The long-term natural history of moderate tricuspid regurgitation (TR) patients with preserved left-ventricular ejection fraction (LVEF) and without severe left-sided valvular heart disease (VHD) remains ambiguous. We aimed to assess the outcomes of patients with moderate TR, preserved LVEF and without concomitant severe left-sided VHD.
METHODS: We evaluated patients diagnosed with moderate TR in our centers between 2012 and 2020. The primary outcome was all-cause death, the secondary outcome was the composite of all-cause death+heart failure (HF) hospitalization, also accounting
