Pulmonary hypertension in dogs with myxomatous mitral valve disease.

Pulmonary hypertension (PH) is a clinically significant and increasingly recognized complication of myxomatous mitral valve disease (MMVD) in dogs, contributing to worsening morbidity and mortality. The development of PH in the context of MMVD is associated with increased symptom burden, including exercise intolerance, syncope, and respiratory distress, and has been correlated with reduced survival times. Echocardiography is the primary modality for detection and monitoring, relying on Doppler estimation of tricuspid regurgitation velocity, but limitations in sensitivity and accuracy remain. T

Tricuspid valve endocarditis in people who inject drugs: strategies to reduce mortality and recurrent infection.

Tricuspid valve infective endocarditis (TV-IE) in people who inject drugs (PWID) has become an increasingly important clinical and public health concern, driven largely by the expanding opioid epidemic. Although PWID with TV-IE are typically young and often lack major comorbidities, they experience disproportionately high rates of mortality, reinfection, and recurrent hospitalizations. Repeated intravenous injection introduces contaminated particulate matter and pathogens into the venous circulation, promoting microbial adherence to the tricuspid valve, with Staphylococcus aureus including met

When an Atrial Septal Defect Is No Longer Silent: A Case Report of a Large Symptomatic Defect Presenting in Adulthood.

Atrial septal defects (ASDs) are among the most common forms of congenital heart disease diagnosed in adulthood. Ostium secundum defects represent the majority of cases and may remain clinically silent for decades. Delayed diagnosis can lead to progressive right-sided cardiac chamber dilation, pulmonary hypertension, arrhythmias, and heart failure. The choice between percutaneous and surgical closure depends on anatomical characteristics such as defect size and the adequacy of surrounding septal rims. We report the case of a 49-year-old woman with hypertension, type 2 diabetes mellitus, and hy

Carcinoid Heart Disease: Surgical Timing, Right Ventricular Risk Stratification and Operative Strategy.

Carcinoid heart disease is a progressive right-sided valvulopathy caused by serotonin and other vasoactive mediators released by metastatic neuroendocrine tumours. As oncological therapies have extended survival, cardiac disease has become a leading determinant of mortality. Operative mortality has decreased to 5-6% in contemporary high-volume centres, and long-term survival appears increasingly determined by tumour biology rather than cardiac disease when surgery is appropriately timed. The principal determinant of operative outcome is preoperative right ventricular function; symptom-based re

Effectiveness of Rhythm Control of Atrial Fibrillation in Moderate-to-Severe or Severe Secondary Tricuspid Regurgitation.

BACKGROUND: Secondary tricuspid regurgitation (STR) is heterogeneous and associated with increased morbidity and mortality, but there remains limited evidence on the optimal management strategy. Atrial fibrillation (AF) increases the risk of all types of STR. OBJECTIVES: The authors aimed to study the association of successful rhythm control of AF with decreased severity of moderate-severe/severe STR across clinically relevant subgroups and different etiologies of STR. METHODS: Patients with moderate-severe/severe STR, AF, and follow-up echocardiograms were identified retrospectively. STR ty

The Tricuspid Valve Maladapts in a Pulmonary Hypertension Rat Model.

Tricuspid valve regurgitation is a frequent valve lesion and, if severe, an independent predictor of mortality. In most patients, the valve itself has historically been considered intact. Yet, we have previously shown that the valve may not be an innocent bystander. In multiple sheep models, we have shown that the tricuspid valve thickens and stiffens. This remodeling may contribute to valve disease. Our goal is to extend our investigation of tricuspid valve remodeling to a rodent model, potentially opening scientific opportunity and enabling scaling our studies. To this end, we used pulmonary

State of the art: Transcatheter tricuspid valve intervention.

Tricuspid regurgitation (TR) has a community prevalence of 3%, which increases with age and affects up to 7% of elderly patients. TR is secondary (functional) in 90% of cases (usually resulting from left heart disease and/or pulmonary hypertension), although atrial dilatation is an increasingly recognised cause. Awareness of the poor prognosis associated with increasing TR severity has refocused the quest for effective treatments. Surgical intervention has been hindered by high in-hospital mortality (up to 10%), even in carefully selected cases, and transcatheter tricuspid valve interventions

Forgotten but Striking: Complete Tricuspid Valve Degeneration Unmasked by Lancisi's Sign.

Tricuspid regurgitation (TR) is increasingly recognized as an important cause of right-sided heart failure and adverse clinical outcomes. Although severe TR typically presents with a holosystolic murmur, advanced disease may produce atypical findings that obscure the diagnosis. Lancisi's sign, characterized by prominent systolic jugular venous pulsations, remains an important but underrecognized bedside clue to severe TR. We present the case of a 60-year-old man with a remote history of presumed infective endocarditis who presented with progressive dyspnea, anasarca, and peripheral edema. Phys

Clinical Risk Assessment of Acute Intermediate-Risk Pulmonary Embolism Using Repeat Transthoracic Echocardiography.

BACKGROUND: Current risk stratification algorithms for intermediate-risk pulmonary embolism (PE) inadequately identify patients who may benefit from escalated care interventions (ECIs) beyond anticoagulation and do not incorporate temporal assessments of right ventricular (RV) function. OBJECTIVES: To evaluate whether changes in tricuspid annular plane systolic excursion (TAPSE) on repeat transthoracic echocardiography (TTE) before ECIs are associated with clinical outcomes in acute intermediate-risk PE. METHODS: Acute intermediate-risk PE patients at a single quaternary care center were ret

Percutaneous Iatrogenic Atrial Septal Defect Closure Post Transcatheter Mitral Valve Repair: Revisiting MITHRAS Trial in a Larger Cohort.

Introduction Transcatheter mitral valve repair (TMVR) creates an iatrogenic atrial septal defect (iASD) that may persist and affect hemodynamics. Although the MITHRAS (Closure of Iatrogenic Atrial Septal Defects Following Transcatheter Mitral Valve Repair) trial showed no benefit of routine closure, its small size and selective population limit generalizability, and observational data remain conflicting. We hypothesized that percutaneous iASD closure (PC-iASD) would be associated with differences in in-hospital clinical and hemodynamic outcomes compared with no closure. Methods Using the Natio

Sex-Specific Remodeling Phenotypes of the Tricuspid Valve Leaflets in an Ovine Model of Functional Tricuspid Regurgitation.

BACKGROUND: Moderate to severe tricuspid regurgitation (TR) affects approximately 1.6 million Americans, yet more than 90% of patients with significant TR remain untreated. Women exhibit higher TR prevalence and more rapid disease progression than men, but the valve-intrinsic mechanisms underlying these sex disparities remain unclear. We hypothesized that sex and circulating testosterone influence tricuspid leaflet remodeling during right-sided pressure overload. METHODS: Female, castrated male (C-Male), and non-castrated male (NC-Male) adult Dorset sheep (n = 45) underwent pulmonary artery b

Intracardiac Migration of an Iliac Venous Stent to the Right Ventricle Causing Tricuspid Regurgitation Managed by Surgical Extraction: A Case Report.

Venous stenting is increasingly used in the management of chronic obstructive venous disease, particularly in severe post-thrombotic syndrome, iliocaval obstruction, and extrinsic iliac vein compression. Although this technique is generally effective and safe, rare but potentially serious complications may occur, including stent migration to the right-sided cardiac chambers. We report the case of a 72-year-old man with a history of deep venous thrombosis treated by left iliac venous stenting approximately 20 years earlier. He presented with a cough and hemoptysis evolving over three months. Ch

From Valve to Vessel: Disseminated Staphylococcus aureus Endocarditis Leading to Splenic Artery Pseudoaneurysm and Mycotic Aneurysm.

Infective endocarditis (IE) is an infection of the endocardial surface of the heart, with Staphylococcus aureus as the leading cause in the United States. Identified risk factors include advanced age, male sex, intravenous drug use (IVDU), prior IE, structural or valvular heart disease, HIV infection, and chronic hemodialysis. In IVDU, IE most commonly affects the right-sided valves, while bivalvular involvement is uncommon but associated with more severe disease and increased risk of rapid deterioration. Multiple complications arise from IE, including variable vascular phenomena and systemic

Acute rheumatic fever or rheumatoid arthritis? The role of hydroxychloroquine: a case report.

BACKGROUND: Rheumatic fever is endemic in our part of the world. Arthritis is the most common clinical presentation and is characteristically migratory in nature. Rheumatoid-like arthritis in acute rheumatic fever is not described in literature. Although arthritis is typically treated with nonsteroidal anti-inflammatory drugs, the role of hydroxychloroquine remains less well studied. CASE PRESENTATION: A 24-year-old Pakistani female individual presented with symmetrical polyarthritis involving the small joints of the hands along with morning stiffness. She was diagnosed with rheumatic heart d

From Carcinoid Syndrome to Carcinoid Heart Disease: The Cost of Delayed Cardiac Surveillance.

Carcinoid heart disease (CHD) is a serious cardiac manifestation of carcinoid syndrome (CS), characterized by progressive fibrotic degeneration of cardiac valves and right-sided heart failure. We report the case of a 72-year-old man with a history of metastatic small-intestinal neuroendocrine tumor and CS diagnosed four years earlier, who presented with worsening dyspnea and peripheral edema. Transthoracic echocardiography revealed characteristic findings of advanced CHD, including severely thickened, retracted, and immobile tricuspid and pulmonary valve leaflets, resulting in severe tricuspid

Rheumatic heart disease and mitral stenosis.

Rheumatic heart disease (RHD) remains a leading cause of mitral stenosis (MS), particularly in developing countries like India, China or countries within Africa. Characterized by a long latent phase after acute rheumatic fever, rheumatic MS often leads to significant morbidity and mortality. This paper explores its epidemiology, natural history, and clinical consequences, including associations with atrial fibrillation, pulmonary hypertension, heart failure, systemic embolism, and infective endocarditis. We review diagnostic tools, particularly echocardiography, and management strategies such

Surgical Versus Non-surgical Management of Acute and Subacute Infective Endocarditis in Patients With Rheumatic Heart Disease During the COVID-19 Pandemic: A Propensity-Matched Analysis.

BACKGROUND: Infective endocarditis remains a life-threatening condition requiring timely medical and surgical management. The COVID-19 pandemic posed unprecedented challenges to healthcare delivery, yet the outcomes of surgical versus conservative management among patients with infective endocarditis and underlying rheumatic heart disease during this period remain poorly defined. OBJECTIVE: This study aims to compare 30-day outcomes following surgical versus conservative management among adult patients with acute and subacute infective endocarditis in the setting of rheumatic heart disease du

Twenty-five year outcomes of patients undergoing valve surgery for infective endocarditis.

OBJECTIVES: Infective endocarditis is associated with significant morbidity and mortality. Long-term outcome data after surgery are limited. We sought to analyze 25-year outcomes after valve surgery for endocarditis. METHODS: We conducted a retrospective cohort study of patients undergoing valve surgery for infective endocarditis at Toronto General Hospital (1990-2014), linking clinical and administrative data with follow-up until March 2024 (median 14 years). We assessed survival and freedom from major adverse valve-related events (MAVRE), stratified by remote endocarditis and active endocar

Prophylactic Versus Reactive Ventricular Tachycardia Ablation in Repaired Tetralogy of Fallot: A Narrative Review.

Ventricular tachycardia and sudden cardiac death remain the principal late causes of mortality in repaired tetralogy of Fallot. Clinical practice is evolving from a "reactive" paradigm centered on defibrillator therapy and post-event ablation toward a "proactive" paradigm targeting slowly conducting anatomical isthmuses before clinical arrhythmias become manifest. Monomorphic ventricular tachycardia in this population typically occurs due to a discrete, anatomically defined set of slowly conducting isthmuses bounded by surgical patches or incisions and valve annuli. Substrate-targeted catheter
