Case Report: A rare case of ANCA-positive Q fever endocarditis-associated glomerulonephritis.

BACKGROUND: Q fever is a zoonotic disease caused by Coxiella burnetii and is endemic worldwide. Q fever endocarditis is commonly found in adults and is rarely seen in children. Infectious endocarditis can also lead to renal damage. Here, we present a case of Q fever endocarditis-associated glomerulonephritis in a Chinese boy with positive Antiproteinase 3 antibody (PR3)-antineutrophil cytoplasmic antibody (ANCA). CASE PRESENTATION: A 12-year-old Chinese boy presented with intermittent fever and hematuria for 2 months. He was diagnosed with Tetralogy of Fallot at birth and underwent multiple c

Querying Q fever: spatiotemporal patterns and environmental drivers in French Guiana.

BACKGROUND: Q fever is a zoonotic infection caused by the bacterium Coxiella burnetii. French Guiana, largely covered by the Amazon rainforest, is considered a hyper-endemic region. While ruminants are the primary reservoirs worldwide, the reservoir in French Guiana remains debated, likely relying on wild fauna. This study aimed to identify spatiotemporal clusters of human Q fever in the Cayenne area and investigate their relationships with environmental factors using remote sensing data. METHODS: A retrospective study was conducted on acute Q fever human cases from January 2007 to December 2

Coxiella burnetii seroconversion in neonatal calves in an infected dairy herd attributable to ingestion of imported colostrum replacer rather than vertical transmission.

In April-June 2017, a cluster of 4 third-trimester abortions caused by Coxiella burnetii was diagnosed on a dairy farm with 356 Holstein cows in Uruguay. We investigated 1) the seroprevalence of C. burnetii in cows delivering normal calves in this herd, 2) whether non-aborting cows gave birth to seropositive and/or PCR-positive newborn calves in the year of the abortion cluster, and 3) whether seronegative newborn calves seroconverted after ingesting colostrum replacer. In March-September 2017, serum samples were collected from 133 calving cows and their calves immediately after birth and befo

Inhaled Ciprofloxacin as an Alternative Treatment for Infection with Coxiella burnetii.

BACKGROUND/OBJECTIVES: Q fever, caused by Coxiella burnetii, is typically treated with doxycycline, but its efficacy is limited in chronic cases and may be poorly tolerated. Systemic ciprofloxacin shows limited activity for acute Q fever. However, inhaled liposomal formulations may provide therapeutic benefit. METHODS: Two inhaled ciprofloxacin formulations (Lipoquin® and Apulmiq®) were evaluated in an A/J mouse model of Q fever and compared with intraperitoneal ciprofloxacin and oral doxycycline. Initially, pharmacokinetic studies were performed to determine an appropriate dosing regimen for

Q fever endocarditis complicating biventricular failure: diagnostic and therapeutic insights from a case report and literature review.

BACKGROUND: Q fever endocarditis (QFE) is a rare, life-threatening infection caused by Coxiella burnetii, accounts for 5-6% of culture-negative endocarditis cases and predominantly affects prosthetic or structurally abnormal valves. Diagnosis is frequently delayed by negative blood cultures and subtle or atypical echocardiographic findings. CASE PRESENTATION: We describe a 45-year-old man with dual mechanical valves and ischemic cardiomyopathy who presented in cardiogenic shock with acute decompensated heart failure and cardiogenic shock. Transesophageal echocardiography revealed severe prost

Nationwide serological, molecular, and spatial assessment of Q fever in dromedary camels (Camelus dromedarius) in Jordan: A One Health perspective.

BACKGROUND AND AIM: Q fever, caused by Coxiella burnetii, is a globally distributed zoonosis with major public health and livestock production implications. Dromedary camels (Camelus dromedarius) are increasingly recognized as important reservoirs, particularly in arid and semi-arid regions. In Jordan, however, comprehensive national-level data integrating serology, molecular detection, and spatial epidemiology are lacking. This study aimed to estimate the serological and molecular prevalence of Q fever in camels, identify associated risk factors, and describe the spatial distribution of infec

Coxiella burnetii shedding in dairy cattle and its correlation with humoral and cell-mediated immune responses.

Coxiella burnetii is the intracellular bacterium responsible for the zoonotic disease Q fever, with cattle among the reservoirs capable of shedding the bacterium for prolonged periods. This longitudinal study examined temporal variation in C. burnetii shedding and immune responses among 192 Australian cows in a naturally infected Australian dairy herd. Blood, placenta, faeces, vaginal mucus, and milk were collected, with the same cows sampled at up to five time points from before calving to mid-lactation (∼200 days in milk). Coxiella burnetii DNA was detected by multiplex qPCR, serum C. burnet

Q fever in the Irish dairy herd.

INTRODUCTION: Coxiella burnetii, the causative agent of Q fever, is a notifiable zoonotic pathogen in Ireland. While typically subclinical in ruminants, infection is associated with reproductive losses. In humans, disease can range from asymptomatic to more serious complications. Ruminants have been identified as the main reservoir for human infection. Ireland’s dairy cattle industry has expanded substantially in recent years, yet current data on the national prevalence of Coxiella burnetii in dairy herds are limited. Understanding the herd-level prevalence and associated risk factors is essen

Coxiella burnetii, the causative agent of Q fever, is a globally distributed zoonotic pathogen affecting humans, domestic animals, and arthropod vectors. In Algeria, fragmented data suggest widespread circulation, yet no comprehensive quantitative assessment exists. This study aimed to systematically review and meta-analyse the prevalence of C. burnetii in humans, animals, and vectors in Algeria, using a One Health perspective. A systematic search of PubMed, Scopus, Web of Science, and Google Scholar was conducted up to January 2026. Studies reporting serological or molecular detection of C. b

Genome characteristics and type IV effector protein repertoire of Coxiella burnetii depend rather on Genomic Groups than on host species.

BACKGROUND: Q fever is a zoonotic disease with virtually worldwide dissemination. Its bacterial agent, Coxiella burnetii, is primarily found in cattle and small ruminants. Disease manifestation is highly variable, i.e. asymptomatic, acute or chronic in humans, and subclinical or present as reproductive disorders in ruminants. Different genomic lineages of C. burnetii have been recognized and are considered to show host preferences and influence the disease outcome. The virulence of C. burnetii is essentially determined by effector proteins that modulate host cell processes, allowing the bacter

[Long-term follow-up of a Coxiellosis outbreak in a goat flock].

ABSTRACT: In January 2020 Coxiella (C.) burnetii was detected in a small goat flock after the observation of a weak born kid, an abortion as well as a malformed kid. ABSTRACT: All animals older than 3 months were subsequently primarily vaccinated with Coxevac (2x three weeks apart) until May 2020 and were revaccinated once in October/November 2020 and in May 2021. Young animals were primarily vaccinated annually except for 2023. Surveillance was performed by qPCR-testing of milk samples, vaginal and preputial (genital swabs, GT), nasal (NT) and environmental swabs (UT). Vaccination was contro

Quantitative three-dimensional imaging of Coxiella burnetii infection by focused ion beam-scanning electron microscopy.

Coxiella burnetii is a highly virulent intracellular pathogen and the causative agent of acute and chronic Q fever in humans. Central to its pathogenicity is the Dot/Icm type IV secretion system, which translocates more than 100 effector proteins into host cells to drive the biogenesis of Coxiella-containing vacuoles (CCVs), which are specialized, lysosome-derived compartments that support bacterial replication. During intracellular growth, C. burnetii undergoes a distinctive biphasic developmental cycle, transitioning between the infectious, dormant small cell variant and the replicative larg

Epidemiological investigation and genetic characterization of Coxiella burnetii carried by parasitic ticks in the southeastern coastal region of China.

BACKGROUND: The pathogen of Q fever, Coxiella burnetii, can persist in the environment for yearsand it can form infectious aerosols that facilitate long-distance airborne transmission of Q fever under certain conditions. Ticks are obligate hematophagous ectoparasites with a broad host range. Accumulating evidence suggests that, within natural ecosystems, ticks may contribute to the maintenance and circulation of Coxiella burnetii among wildlife and livestock, thereby representing a potential component of its transmission cycle. In recent years, cases of Q fever have been reported in Fujian Pro

Diagnosis of Q-fever pneumonia by tNGS of bronchoalveolar lavage fluid in an elderly patient.

This study explores the value of targeted next-generation sequencing (tNGS) in the etiological diagnosis of unexplained pneumonia in elderly patients. A 76-year-old male patient presenting with fever and cough was initially diagnosed with lobar pneumonia. Bronchoalveolar lavage fluid (BALF) was analyzed using tNGS, which detected Coxiella burnetii (4 sequences), confirming the diagnosis. Treatment with moxifloxacin and doxycycline led to significant symptom improvement and complete resolution of pulmonary lesions on imaging. The results demonstrate that tNGS can provide rapid and accurate diag
