Cardiac involvement in eosinophilic granulomatosis with polyangiitis: acute eosinophilic myocarditis and chronic inflammatory cardiomyopathy.

OBJECTIVES: Currently, cardiac involvement is used to describe all eosinophilic granulomatosis with polyangiitis (EGPA) cardiac problems. However, heterogeneity exists among them. We aimed to depict the disease spectrum of EGPA cardiac involvement and identify the high-risk population. METHODS: We included EGPA patients hospitalized in our centre from 2012 to 2023 and in public databases. Based on the cardiac enzymes, cardiac MRI and endomyocardial biopsy results, the patients were divided into three groups: eosinophilic myocarditis (EGPA-EM), chronic inflammatory cardiomyopathy (EGPA-ICM) an

Eosinophilic myocarditis: case series and review of the literature.

Eosinophilic myocarditis (EM) is characterized by acute myocardial inflammation due to eosinophilic tissue infiltration. It is a rare and underdiagnosed condition, which may be either idiopathic or secondary to vasculitides, hypereosinophilic syndromes, drugs, or infections. Diagnosis is based on laboratory findings, echocardiography, cardiac magnetic resonance imaging and may sometimes need endomyocardial biopsy. Treatment depends on the underlying cause and often consists of immunosuppressive agents and anticoagulation therapy. This case series includes nine patients with EM, specifically se

Evolution in the Diagnosis and Treatment of Myocarditis in Recent Years: State of the Art.

Acute myocarditis (AM) is an inflammatory cardiac condition resulting from infections, toxic exposures, or immune-mediated mechanisms, with clinical presentations ranging from mild symptoms to heart failure (HF) or cardiogenic shock. Although viral infections remain the predominant cause, both the absolute prevalence and the relative distribution of different etiologies may change over time and across regions depending on endemic diseases. Immune checkpoint inhibitor (ICI)-associated myocarditis has emerged as a newly recognized entity, with diagnostic rates increasing in parallel with growing

Direct Fast Scarlet and Congo Red Staining in Identification of Eosinophils and Their Cell-Free Granules in Cardiac Tissue.

Cell-free eosinophil granules, harmful to the heart, are stained red by haematoxylin-eosin (HE); however, they can be overlooked in cardiac tissues. Direct fast scarlet (DFS) and Congo red (CR), known for staining amyloids, may offer clearer detection of eosinophil granules; however, no firm evidence exists. This study aimed to confirm that DFS and CR stain eosinophil granules and evaluate their advantages over HE.Paraffin-embedded endomyocardial biopsy samples from 6 patients with eosinophil-infiltrating cardiac disorders and 6 with lymphocytic myocarditis were stained. The distributions of re

A Particular Case of Myocardial Injury.

We report a case of a patient admitted to our coronary intensive care unit with chest pain and elevated cardiac troponin. Coronary angiography showed no obstructive coronary artery disease. The patient had a history of chronic rhinosinusitis and nasal polyps, treated with dupilumab. Laboratory tests revealed marked hypereosinophilia, prompting a cardiac MRI, which showed findings consistent with eosinophilic myocarditis. This diagnosis was later confirmed by endomyocardial biopsy. This case highlights the importance of differential diagnosis in cases of myocardial injury (characterized by incr

Fulminant Eosinophilic Myocarditis in a Young Adult: Successful Recovery After Extracorporeal Membrane Oxygenation (ECMO) Support and Multisystem Complications.

Eosinophilic myocarditis (EM) is a rare inflammatory cardiomyopathy with a highly variable clinical presentation, ranging from mild symptoms to fulminant cardiogenic shock. Its diagnosis is challenging due to non-specific clinical findings and requires a high index of suspicion, with endomyocardial biopsy (EMB) serving as the gold standard. Early initiation of immunosuppressive therapy and, in severe cases, mechanical circulatory support (MCS) are essential to improve outcomes. We report the case of a 21-year-old male who presented with progressive dyspnea, chest pain, and hypoxemia. Echocardi

Endomyocardial biopsy in clinical practice: the diagnostic yield and insights from a 5-year single-center experience.

OBJECTIVES: Endomyocardial biopsy (EMB) is a diagnostic tool for evaluating various cardiac conditions, such as myocarditis and myocardial infiltrative diseases. It is also the gold standard screening technique for detecting allograft rejection after heart transplantation. Despite advances in noninvasive imaging modalities for myocardial tissue characterization, EMB is still necessary for making a definitive diagnosis and determining treatment for certain conditions. Herein, we report our recent experience using EMB and its diagnostic yield. METHODS AND RESULTS: We retrospectively reviewed EM

Long-term efficacy and safety of tailored immunosuppressive therapy in immune-mediated biopsy-proven myocarditis: A propensity-weighted study.

AIMS: Standardized immunosuppressive therapy (IS) had been previously investigated in biopsy-proven (BP) lymphocytic myocarditis with heart failure (HF). This study evaluated efficacy and safety of tailored IS in BP immune-mediated myocarditis, irrespective of histology and clinical presentation. METHODS AND RESULTS: Consecutive BP myocarditis patients treated with long-term tailored IS on top of optimal medical therapy (OMT), were compared with OMT non-IS controls using propensity-score weighting. The primary outcome was a composite of death or heart transplant, the secondary outcome was a c

Eosinophilic Myocarditis Resulting in Ventricular Tachycardia Storm.

Eosinophilic myocarditis (EM) is a rare but potentially fatal complication of sustained eosinophilia that is characterized by eosinophilic infiltration into myocardial tissue. There are various etiologies of EM that can be classified into general categories: reactive, clonal, and idiopathic. We present a case of EM caused by chronic eosinophilic leukemia, a rare myeloproliferative neoplasm that frequently presents with sustained peripheral eosinophilia. This case displays several serious complications of EM, including recurrent ventricular tachycardia storm, cardiogenic shock, and mural thromb

Cardiac Magnetic Resonance Imaging Used to Determine a Rare Etiology of a Layered Left Ventricular Apical Thrombus.

Eosinophilic myocarditis (EM) is a rare disease, often associated with hypereosinophilic syndrome (HES). Historically, the diagnostic gold standard was endomyocardial biopsy (EMB). We present a unique case of a 58-year-old female who presents after a syncopal episode and was found to have a layered left ventricular (LV) thrombus. Using laboratory studies and cardiac magnetic resonance imaging (MRI), we were able to delineate the etiology, avoiding any invasive testing.
