Gerrymandering in the heart: How tissue heterogeneities promote premature ventricular contractions.

Premature ventricular contractions (PVCs) are abnormal heartbeats that can trigger life-threatening arrhythmias. One mechanism responsible for PVCs is triggered activity. In this process, spontaneous calcium (Ca2+) release from the sarcoplasmic reticulum and subsequent Ca2+ waves at the subcellular level promote transient inward currents that lead to delayed afterdepolarizations at the cellular level. In uniform, healthy tissue, such events are often benign because the surrounding healthy cells act as an electrical sink, suppressing the abnormal signals from a few pathological source cells. Th

Adverse outcomes due to atrioventricular synchrony loss induced by temporary VVI pacing in patients with pre-existing DDD stimulation: insights from a device recall scenario.

AIMS: Temporary reprogramming of dual-chamber (DDD) pacemakers to non-synchronous ventricular modes (VVI/VVIR) may be required in selected clinical scenarios, but short-term consequences of abrupt atrioventricular (AV) synchrony loss remain incompletely characterized.Leveraging a unique natural experiment, this study aimed to quantify the clinical and echocardiographic consequences of temporary VVI/VVIR pacing in patients in sinus rhythm chronically adapted to DDD stimulation. METHODS AND RESULTS: In this prospective observational study, 93 patients (79.9 ± 16.0 years; 63.4% AV block) with re

Conduction System Pacing-Induced Ventricular Arrhythmia: Case Report.

Left bundle branch-optimized cardiac resynchronization therapy (LOT-CRT), combining left bundle branch area pacing (LBBAP) with a left ventricular lead, offers physiological activation and improved synchrony compared with conventional biventricular pacing. We report a 78-year-old woman with pacing-induced cardiomyopathy who underwent LOT-CRT, resulting in QRS narrowing but notable QT prolongation and T-wave inversions. On the first post-implant day, she developed ventricular fibrillation triggered by premature ventricular complexes, without reversible metabolic or ischemic cause. This case hig

Left bundle branch pacing in anatomically repaired congenitally corrected transposition of the great arteries: a case report.

BACKGROUND: There are currently different approaches to conduction system pacing in unrepaired congenitally corrected transposition of the graet arteries (CCTGA) patients. However, these therapies are not feasible when patients have undergone anatomic repair. CASE SUMMARY: We describe our approach and considerations for left bundle branch (LBB) area pacing in a 12-year-old patient with CCTGA after double switch operation using Rastelli repair. Three-dimensional reconstructions derived from cardiac CT were both printed as a dry model and uploaded in a virtual reality environment to enhance ana

Assessment of Lactate Levels as Prognostic and Predictive Markers in Patients with Complete Atrioventricular Block.

OBJECTIVE: Complete atrioventricular (AV) block complicates management decisions, particularly regarding temporary pacemaker (TP) insertion. Lactate, an anaerobic byproduct, may reflect inadequate cardiac output and help guide this decision. We analyzed the prognostic value of lactate in patients with complete AV block. METHOD: We retrospectively evaluated the medical records of adult patients presenting with complete AV block to our coronary care unit. Initial and subsequent venous blood lactate levels were analyzed for associations with adverse in-hospital outcomes, defined as renal injury,

P-wave abnormalities in PRKAG2 cardiomyopathy: Expanding the electrocardiographic spectrum beyond preexcitation and hypertrophy.

PRKAG2 cardiomyopathy exhibits distinctive P-wave abnormalities-including notching, axis deviation, inter-atrial block, and enlarged P-wave terminal force in lead V1 (PTFV1)-that reflect underlying atrial involvement. These atrial electrocardiographic features, when integrated with comprehensive genetic testing, may enable earlier diagnosis, facilitating accurate genotype-phenotype correlation and refined risk stratification beyond traditional preexcitation or hypertrophy patterns, ultimately guiding timely intervention and family screening in this rare but potentially malignant inherited card

Glycosylation of the murine cardiac channel TRPM4 is altered by the pathogenic p.I376T variant.

TRPM4 is a calcium-activated, voltage-modulated, non-selective cation channel expressed in various tissues, including the heart. In 2016, we reported on a large French family with progressive heart block type I carrying the variant TRPM4 p.I376T. In the present study, the aim was to investigate the consequence of the channel variant TRPM4 p.I376T in cardiac physiology in a newly generated Trpm4 knock-in mouse line. Male and female Trpm4 knock-in (Trpm4I376T/I376T) and wild-type mice of different young ages (12, 18, 24 and 36 weeks old) were phenotyped using surface ECGs. Western blots were per

Distinct Purkinje electrogram phenotypes at ventricular fibrillation trigger sites in idiopathic and structural heart disease.

BACKGROUND: Purkinje fibers are established triggers of ventricular fibrillation (VF). In structural heart disease, septal fibrosis and border-zone remodeling may alter Purkinje electrograms at VF-triggering sites; however, classical electrogram criteria derived from structurally normal hearts have not been validated in scarred substrate. OBJECTIVE: The purpose of this study was to compare Purkinje electrogram morphology at VF-triggering sites in patients with idiopathic and structural heart disease undergoing catheter ablation. METHODS: 37 consecutive patients underwent Purkinje-targeted VF

Clinical outcomes of conduction system pacing versus right ventricular pacing in patients with atrioventricular block: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Conduction system pacing (CSP) has emerged as an alternative to right ventricular pacing (RVP) for atrioventricular block (AVB) aiming to avoid ventricular dyssynchrony and adverse heart failure outcomes yet clinical adoption remains limited due to insufficient randomised evidence. OBJECTIVES: This study aimed to assess the clinical outcomes of CSP versus RVP in patients with AVB based exclusively on randomised controlled trials (RCTs). METHODS: MEDLINE, Embase and Cochrane were searched from inception to October 2025. Clinical outcomes comparing CSP and RVP were analysed using H

Baseline QRS axis distinguishes para-Hisian PVC from outflow tract PVCs.

BACKGROUND: Para-Hisian premature ventricular contraction (PVC) is challenging and needs to be distinguished preprocedurally from other outflow tract PVCs (OT-PVCs) based on electrocardiographic morphology. OBJECTIVE: This study aimed to investigate whether para-Hisian PVCs can be distinguished from the right (RVOT) and left ventricular outflow tract (LVOT) by comparing the QRS axis of PVC and sinus rhythm (SR). METHODS: We retrospectively analyzed 160 idiopathic PVC cases without conduction abnormalities. The site of origin was defined by the successful ablation site. Baseline SR and PVC ax

Artificial Intelligence to Predict Major Arrhythmic Events Based on Left Ventricular Electroanatomic Mapping Data.

Background/Objectives: Electroanatomic mapping (EAM) provides high-resolution spatial and electrogram information, but the prognostic utility of quantitative EAM features has not been systematically evaluated with contemporary artificial intelligence (AI) methods. We investigated whether an AI analysis of quantitative EAM exports from the CARTO system enhances the prediction of major arrhythmic events (MAEs). Methods: In this retrospective, multicenter cohort study, 248 consecutive patients undergoing left ventricular EAM at four tertiary electrophysiology centers were analyzed. Numerical EAM

Sinus Node Dysfunction in Takotsubo Syndrome: An Uncommon Presentation.

BACKGROUND: Stress cardiomyopathy is an acute, usually reversible left ventricular (LV) systolic dysfunction often triggered by emotional or physical stress. Conduction system complications are uncommon. CASE SUMMARY: A 68-year-old patient presented with acute pulmonary edema, reduced LV ejection fraction, and troponin elevation. Coronary angiography excluded obstructive coronary artery disease, and cardiac magnetic resonance confirmed stress cardiomyopathy. During hospitalization, the patient developed sinus node dysfunction (SND). Despite complete recovery of LV systolic function, the condu

Advancements in pacing and ICD technologies: Potential implications for complex congenital heart disease in adults.

Adults with congenital heart disease (ACHD) represent a rapidly growing population with a high burden of bradyarrhythmias and ventricular arrhythmias requiring cardiac implantable electronic devices. However, conventional transvenous pacing and defibrillator systems often present important challenges in this population due to complex anatomy, limited venous access, intracardiac shunts, and the cumulative risk of lead-related complications over a patient's lifetime. Recent technological advances-including conduction system pacing, leadless pacemakers, subcutaneous and extravascular implantable

Left atrial myopathy development and progression in atrial fibrillation patients with recurrent left atrial ablation procedures: The LAMP-AF study.

BACKGROUND: Pulmonary vein isolation remains central to atrial fibrillation (AF) ablation; however, recurrence can occur despite durable isolation, and is associated with low voltage areas (LVAs), a surrogate for atrial fibrosis. OBJECTIVE: This study characterized longitudinal LVA changes in repeat ablation patients and identified predictors of substrate progression. METHODS: Consecutive adults undergoing de novo left atrial (LA) ablation for AF were enrolled, with repeat ablation cases analyzed. LVAs were quantified, with progression defined as >10% LVA increase. Conduction velocity (CV) a

ERBB4 activation as a new therapy for atrial remodeling and fibrillation.

BACKGROUND: The neuregulin-1/erythroblastic leukemia viral oncogene homolog (ERBB) signaling system is essential for cardiac embryonic development and function. Its activation during heart failure exerts compensatory effects by acting on cardiomyocytes and mitigating ventricular fibrosis. OBJECTIVE: We investigated whether selective ERBB4 activation suppresses atrial fibrillation (AF) in minipigs and mice in the absence of heart failure and assessed its effects on atrial cardiomyocytes and atrial fibrosis. ERBB4 stimulation was achieved using JK07, a recombinant fusion protein consisting of a

Delta (Δ) 12-lead electrocardiography and vectorcardiography to identify the origin of focally induced atrial and ventricular premature depolarizations in horses.

BACKGROUND: Cardiac arrhythmias are common in horses, but their clinical and prognostic relevance remains poorly defined. Despite the importance of localization for diagnosis and treatment, noninvasive methods to identify arrhythmogenic foci are limited. HYPOTHESIS/OBJECTIVES: Evaluate the ability of delta (Δ) 3- and 12-lead ECGs and 2-dimensional (2D) and 3-dimensional (3D) vectorcardiography (VCG) to identify the site of origin of focally induced atrial and ventricular premature depolarizations in horses. Type 1 Brugada Pattern Triggered by Low-Grade Fever: Implications for Diagnosis and Risk Stratification.

Brugada syndrome (BrS) is a rare but potentially life-threatening condition that may lead to sudden cardiac death. Among the causes, dysfunctions of ion channels involved in the cardiac action potential (specifically in SCN5A and SCN10A genes) are particularly significant. Among diagnosed Brugada patients, fever-induced episodes occur in 20-30% of cases. Fever worsens sodium channel dysfunction, as elevated temperature further reduces their conductance. First clinical manifestation of BrS occurs usually during a febrile episode, especially in young people. We performed a multiparametric examin

Successful Use of Ranolazine in SCN5A p.R222Q-Related Multifocal Ectopic Purkinje-Related Premature Contractions.

INTRODUCTION: Gain-of-function variants in SCN5A may cause multifocal ectopic Purkinje-related premature contractions (MEPPC), with a high premature ventricular contraction burden that may lead to PVC-induced cardiomyopathy. Pharmacological therapy is usually required, as catheter ablation is ineffective because of the multifocal origin of the ectopy. METHODS AND RESULTS: We report two siblings with the pathogenic SCN5A p.Arg222Gln variant. In both, ranolazine markedly suppressed ventricular ectopy, improved or preserved left ventricular function, and enabled amiodarone withdrawal. CONCLUSIO
