Some investigators have suggested that children receiving stimulant medications to manage attention-deficit hyperactivity disorder should undergo screening electrocardiography to identify asymptomatic cardiac disease. However, no study to date has examined the efficacy and costs of this strategy. In the present study we sought to determine the utility of electrocardiographic screening in children with attention-deficit hyperactivity disorder. We reviewed the clinical experience of electrocardiographic screening of subjects with attention-deficit hyperactivity disorder <21 years of age from Apr

Sudden cardiac death may be the first event in the history of Wolff-Parkinson White syndrome: this is a very rare event although as ablation of the accessory pathway avoids this risk a potentially malignant form of the disease needs to be detected. Electrophysiological studies are the most reliable method. These may be performed by a trans-oesophageal or endocavity approach from the age of six to seven years onwards. Whilst it is rare to detect a potentially malignant form the results of these studies more often enable the person to play sports or continue their job without offering radio-freq

OBJECTIVE: To investigate whether the PJ interval in the patients with pre-excitation syndrome can be shortened by pathway conduction, and to explore the clinical implications of the prolonged PJ interval.
METHODS: 143 patients with single pathway, who experienced successful radiofrequency (RF) ablation, were divided into two groups: Group A (n = 132) with normal atrioventricular and ventricular conduction (sub-divided into 10 subsets further according to the location of the pathway) and Group B (n = 11) with first degree atrioventricular block or with bundle branch block. The ECG images with

AIMS: To determine the feasibility and the results of exercise testing (ET) and electrophysiological study (EPS) in outpatient asymptomatic children with a Wolff-Parkinson-White (WPW) syndrome.
METHODS AND RESULTS: Exercise testing and transesophageal EPS were performed in 55 outpatient asymptomatic children aged 6 to 19 years old (14 +/- 3) with WPW. Wolff-Parkinson-White persisted during maximal exercise. Isoproterenol was not required in five children younger than 10 years old, because they developed a catecholaminergic sinus tachycardia. Maximal rate conducted through accessory pathway (A

Radiofrequency ablation is the interventional therapy of choice for the definitive cure of patients with tachycardias facilitated by accessory pathways. Ablation of an accessory pathway at sites close to the His bundle is inevitably associated with an increased risk of causing complete atrioventricular block. Here we describe the case of a patient with pre-excitation syndrome and episodes of supraventricular tachycardia, in whom an electrophysiological study identified an accessory pathway so close to the His bundle that discrete sites between the pathway and the His bundle were extremely diff

A variety of clinical conditions not associated with ischemic heart disease can have ECG features mimicking those of ischemic etiology. Some of these entities are: ventricular hypertrophy, intraventricular conduction defects, pre-excitation syndrome, pericarditis, electrolyte disturbances, etc. This can lead to erroneous diagnosis and uncalled for hazardous treatment. A careful comprehensive evaluation with in depth analysis of the ECG can overcome such errors. We have reviewed some of these conditions and elaborated upon the differentiating features.

This brief review discusses the interesting early history of the pre-excitation syndrome. In 1913 Cohn and Fraser published the first patient with a short P-R interval, wide QRS complexes, and paroxysmal tachycardia. This was followed by other cases of pre-excitation syndrome, all of which were considered to be due to bundle branch blocks. In 1930 Wolff, Parkinson, and White reported 11 patients with the syndrome, which came to bear their name. Two years later, Holzmann and Scherf suggested bypass tracts as the most likely mechanism of pre-excitation syndrome. In 1942, Wood et al. documented t

We report three patients with pre-excitation syndrome that resembled an acute coronary syndrome. A 65 years old woman, consulting in the emergency room for palpitations and retrosternal pain. EKG showed regular tachycardia and ST depression that reverted spontaneously after an episode of vomiting. A subsequent EKG demonstrated a pre-excitation syndrome and the accessory pathway was fulgurated. A 18 years old male presenting with tachycardia and chest pain elicited during exercise. An EKG showed a pre-excitation syndrome and ST segment elevation in V2 and V3. A coronary angiogram was normal. Th

Atrioventricular node blocking agents including beta-adrenergic blockers, non-dihydropyridine calcium channel blockers and digoxin are usually effective in controlling ventricular rate in atrial fibrillation and flutter. Intravenous beta-blockers and non-dihydropyridine calcium channel blockers are equally effective in rapidly controlling the ventricular rate. The addition of digoxin to the regimen causes a favorable outcome but digoxin as a single agent is generally less effective in slowing the ventricular rate in acute setting. Clonidine, magnesium, and amiodarone have also been used for ac

A 52 year old man was admitted to an emergency department with a fast ventricular rate atrial fibrillation after an electrical shock. Electrical cardioversion was attempted after echocardiographic examination. This failed, but the heart rate slowed. Successful pharmacological cardioversion was achieved after 16 hours of amiodarone infusion. Pre-excitation syndrome was detected on baseline echocardiograph. Serum cardiac specific markers were all within normal limits. No abnormal findings were detected by chest radiography, echocardiographic, or coronary angiographic investigations. Acute onset

OBJECTIVE: Pre-excitation syndrome is considered to be autosomal dominant hereditary disease. The objective of this study was to search the genetic foundation of the pre-excitation syndrome.
METHODS: Genomic DNA was isolated from peripheral lymphocytes obtained from 44 cases of patients with pre-excitation syndrome and 53 normal persons. Polymorphic short tandem repeats(STR) were amplified using polymerase chain reaction (PCR) and analyzed by polyacrylamide gel electrophoresis. The genotype of each individual was determined by polymorphic STR including D7S505,D7S688,D7S483. Association analys

A 40-year-old man with Fabry disease, confirmed by decreased leukocyte alpha-galactosidase A activity in 2001, complained of sudden bilateral deafness, as evidenced by clinical history and audiometry. Magnetic resonance of the brain revealed features typical of Fabry disease. Other clinical manifestations of the disease included: angiokeratoma, mild proteinuria with normal renal function, lymphoedema of the lower limbs, pre-excitation syndrome, myocardial hypertrophy.

Dilated cardiomyopathy induces circulatory insufficiency with poor prognosis. Persistent tachyarrhythmias may be the cause of this disease. At particular high risk for heart damage and insufficiency are young people and children. The group of most dangerous arrhythmias consist of: incessant tachycardia in patients with preexcitation syndrome and supraventricular tachyarrhythmias (atrial flutter and fibrillation, ectopic atrial tachycardia) with high rate of ventricles. The result of arrhythmias is dilatation of the heart and thinness of ventricular walls with hemodynamic disorders. The effecti

BACKGROUND: Previous studies have shown that the risk of major cardiovascular events at 1 year is less than 1% in patients with normal myocardial stress perfusion study results. However, the racial distribution of patients enrolled in these studies is not known. Hence, the prognostic value of normal stress perfusion study results in black patients is not well established. Our objective was to determine the incidence of major cardiovascular events in black patients with normal stress perfusion study results over a 12-month period.
METHODS AND RESULTS: We searched the nuclear cardiology databas

This paper discusses the importance of critical care and emergency nurses having an understanding of why pre-existing cardiac disorders can influence antiarrhythmic treatment. The patient with a pre-excitation syndrome is usually managed in a coronary care unit. However, these patients may be admitted to an intensive care unit (ICU) with complications of Wolff Parkinson White (WPW) syndrome; for example post cardiopulmonary arrest or WPW as a co-morbidity. It is common practice in critical care areas for registered nurses to administer antiarrhythmics without a doctor's prescription in life-th

Rhabdomyomas are not uncommon in infants with tuberous sclerosis. We describe a neonate who presented with hydrops fetalis arising from a tachyarrhythmia during fetal life related to rhabdomyomas. After reversion of the arrhythmia, pre-excitation was noted on an interval electrocardiogram. Following regression of the tumours, the delta wave disappeared with no further arrhythmias noted.

Tissue Doppler imaging or myocardial velocity imaging is a variation of conventional Doppler. This modality allows the quantification of the Doppler shift within the range of myocardial tissue motion. The velocity of motion at a variety of myocardial sites can be determined and distinguished very rapidly using Doppler techniques. The velocity of moving tissue can be studied with pulsed wave tissue Doppler sampling, which displays the velocity of a selected myocardial region against time, with high temporal resolution. In addition, the velocities can be calculated with time velocity maps and di

Supraventricular tachycardia is the most frequent form of symptomatic tachydysrhythmia in children. Neonates and infants with paroxysmal supraventricular tachycardias generally present with signs of acute congestive heart failure. In school-aged children and adolescents, palpitations are the leading symptom. Chronic-permanent tachycardia results in a secondary form of dilated cardiomyopathy. Therapy for episodes of tachycardia depends on the individual situation. In severe haemodynamic compromise, or if ventricular tachycardia is suspected, tachycardia should immediately be terminated by exter
