ALCAPA syndrome: a rare cause of sudden cardiac arrest in adulthood?-a case report.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare coronary artery anomaly occurring in approximately 1 in 300,000 live births. Typically diagnosed in infancy, ALCAPA can lead to significant cardiovascular complications, including myocardial ischemia, heart failure, and sudden cardiac death if untreated. The adult form of ALCAPA is less common and often identified incidentally through modern cardiac imaging techniques. CASE PRESENTATION: We describe the case of a 30-year-old white German woman who suffered an out-of-hospital cardiac arrest. I

Diagnostic challenges in infant death associated with maternal cocaine abuse during pregnancy and lactation: a forensic case report and review of the literature.

INTRODUCTION: Maternal cocaine abuse during pregnancy and lactation represents a critical public health crisis. Recent 2024-2025 data indicate approximately 4.3 million users in the United States and 2.7 million young adults in Europe. While epidemiological evidence suggests an independent association between prenatal cocaine exposure and Sudden Infant Death Syndrome (SIDS), forensic interpretation remains complex due to the lack of validated toxicological thresholds for the pediatric population. METHOD: We report the case of a 36-day-old infant who died suddenly and unexpectedly. Post-mortem

Spanish founder variant in MYBPC3: implications of diagnostic criteria, clinical phenotype, and prognosis.

INTRODUCTION AND OBJECTIVES: MYBPC3 is the most frequently mutated gene in hypertrophic cardiomyopathy (HCM). This study presents the clinical phenotype of a founder variant in MYBPC3, p.Arg891Alafs*160, present in 47 families from southern Spain, and compares it with other described founder truncating variants in MYBPC3. METHODS: We studied 259 relatives of 47 index cases with HCM carrying the p.Arg891Alafs*160 variant. Clinical evaluation, including recently proposed diagnostic criteria, pedigree analysis and genotyping in relation to outcomes, were performed. RESULTS: A total of 159 carri

[Diagnostic awareness and red flags in hypertrophic cardiomyopathy in daily clinical practice].

Hypertrophic cardiomyopathy is the most common structural myocardial disease. Based on the literature, its estimated prevalence in the adult population is 1 : 500. It is distinguished by an exceptionally heterogeneous clinical presentation and a broad spectrum of disease trajectories. The disorder is recognised in only about 10% of the patients. In 90% of cases, patients remain undiagnosed, often due to the absence of symptoms. Hypertrophic cardiomyopathy is frequently the underlying cause of sudden cardiac death. In recent years, mortality of hypertrophic cardiomyopathy has declined due to im

Non-Dilated Left Ventricular Cardiomyopathy: Exploring the Spectrum from Phenotype to Disease Entity.

Non-dilated left ventricular cardiomyopathy has recently emerged as a distinct morpho-functional phenotype defined by non-ischemic left ventricular scarring or fatty replacement or by isolated global left ventricular hypokinesia without scarring in the absence of ventricular dilatation. Whether it represents a distinct biological entity or a shared phenotypic expression across cardiomyopathies remains unresolved. This narrative review examines evidence from genetics, multimodality imaging, natural history, and outcomes. Overlap with dilated and arrhythmogenic cardiomyopathies is extensive: no

Tpeak-Tend interval predicts VT inducibility in arrhythmogenic mitral valve prolapse syndrome.

BACKGROUND: Mitral valve prolapse (MVP) is commonly benign, but an arrhythmic phenotype (AMVP) with increased risk for sudden cardiac death has been described. A prolonged Tpeak-Tend interval has been association with increased arrhythmic risk in different clinical settings. The aim of this study was to investigate a possible correlation between prolonged Tpeak-Tend interval and ventricular tachycardia (VT) inducibility in MVP. METHODS AND RESULTS: Sixty-five patients fulfilling the EHRA criteria of AMVP syndrome underwent programmed ventricular stimulation between 2016 and 2024. Sustained po

Wernicke Encephalopathy Complicating a Distinctive POLG Phenotype With MNGIE-Like Features.

BACKGROUND: Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is an extremely rare autosomal recessive disease caused by variants in the thymidine phosphorylase gene (TYMP), primarily characterized by severe gastrointestinal and neurological symptoms. The complete phenotype of MNGIE has not been linked to any gene other than TYMP. METHODS: We describe two identical twins who exhibited delayed psychomotor development, infantile bilateral cataract, congenital demyelinating polyneuropathy, and severe progressive gastrointestinal dysmotility with recurrent pseudo-obstruction episodes,

Ventricular arrhythmia inducibility predicts clinical events in mitral valve prolapse syndrome.

BACKGROUND: Mitral valve prolapse (MVP) is commonly benign, but an arrhythmic phenotype (AMVP) with increased risk for sudden cardiac death has been described. Current risk stratification is mainly based on clinical data and imaging markers, while the role of programmed ventricular stimulation (PVS) remains uncertain. METHODS: In this prospective single-center cohort study, 42 patients fulfilling the EHRA consensus criteria for AMVP underwent an electrophysiological study with programmed ventricular stimulation between 2016 and 2025. Stimulation was performed from the right ventricular apex a

Association of atrial tachyarrhythmia, heart block, and right ventricular dysfunction: a case report and review of literature.

BACKGROUND: The association of right ventricular dysfunction, atrial tachyarrhythmias, and cardiac conduction system disease is rarely reported in literature. Previous case reports include right ventricular dysfunction associated with sudden cardiac death and ventricular arrhythmia. Other case reports with similar clinical presentation triad differ by either left ventricular involvement or having an abnormal presentation of systemic diseases. CASE PRESENTATION: We report a case series of right ventricular dysfunction, atrial tachyarrhythmias, and cardiac conduction system disease in a single

Active learning for the accelerated discovery of complex concentrated NiCoCr alloys in additive manufacturing.

BACKGROUND: According to the World Health Organization, air pollution is closely associated with climate change and, in particular, with global warming. In addition to melting of ice and snow, rising sea level, and flooding of coastal areas, global warming is leading to a tropicalization of temperate marine ecosystems. Moreover, the effects of air pollution on airway and lung diseases are well documented as reported by the World Allergy Organization. METHODS: Scientific literature was searched for studies investigating the effect of the interaction between air pollution and climate change on

Acute Decompensation Requiring Cardiopulmonary Resuscitation in a 15-Year-Old With Undiagnosed Lyme Carditis.

We describe the case of a previously healthy teenager who developed third-degree atrioventricular block due to Lyme disease. While Lyme carditis is common in children, severe conduction anomalies remain rare but can be life-threatening. This case highlights the importance of early recognition, prompt testing, and timely intervention to prevent unnecessary procedures, such as permanent pacemaker implantation, and to ensure favorable outcomes. This case also supports Lyme disease testing in pediatric sudden cardiac death in cases with autopsy findings suggestive of myocarditis or pericarditis.

Clinical Presentation, Etiology, and Outcomes of HIV-Associated Cardiomyopathy: A Systematic Review of Published Case Reports.

HIV-associated cardiomyopathy is a significant cause of morbidity and mortality among people living with HIV, contributing to heart failure, arrhythmia, and sudden cardiac death. Despite its clinical importance, its individual-patient clinical spectrum has not been systematically synthesized. We conducted a systematic review of published English-language case reports and small case series describing cardiomyopathy in HIV-infected individuals. Etiologies were classified using a framework distinguishing cardiomyopathy arising from uncontrolled HIV from that occurring despite virologic control. S

Late Gadolinium Enhancement Entropy as a Novel Imaging Biomarker of Myocardial Tissue Heterogeneity-A Comprehensive Review.

Late gadolinium enhancement (LGE) cardiac magnetic resonance is the reference standard for non-invasive myocardial tissue characterization. Conventional LGE analysis focuses on the presence and extent of fibrosis, yet these measures incompletely describe the spatial complexity of myocardial scar that underpins arrhythmogenesis. Entropy, derived from radiomic analysis of LGE signal-intensity distributions, has emerged as a surrogate marker of myocardial tissue heterogeneity. Mechanistically, heterogeneous fibrosis promotes electrical conduction alterations, and entropy serves as a global descri

Accessory pathways in arrhythmogenic right ventricular cardiomyopathy: A two-case series and literature review.

BACKGROUND: Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a genetic cardiomyopathy characterized by fibrofatty replacement of ventricular myocardium, associated with ventricular arrhythmias and sudden cardiac death (SCD). While supraventricular arrhythmias occur in approximately 25% of ARVC patients, the coexistence of accessory pathways is exceptionally rare, particularly concealed pathways without ventricular pre-excitation. CASE PRESENTATION: We describe two young patients with ARVC and concealed accessory pathways. Case 1: A 27-year-old male with family history of SCD presente

Fatal Acute Thyroiditis in a Giraffe (Giraffa camelopardalis) Associated with Clostridium perfringens Type A: A "Local Proliferation-Systemic Intoxication" Pathogenic Model.

This report presents the first case of fatal acute thyroiditis associated with Clostridium perfringens type A in a captive giraffe (Giraffa camelopardalis) and describes findings consistent with the proposed "local proliferation-systemic intoxication" pathogenic model. A 5-year-old female giraffe in Hebei Province, China, presented with diarrhea, emaciation, and sudden death. Necropsy revealed marked thyroid enlargement, intestinal mucosal hemorrhage and shedding, and pericardial effusion. C. perfringens type A was isolated from thyroid tissue, and molecular analysis detected the presence of t

Multi‑target mechanisms of paeoniflorin in inflammatory regulation: A network pharmacology (Review).

BACKGROUND: Children with clinical levels of conduct problems are at high risk of developing mental health problems such as persistent antisocial behavior or emotional problems in adolescence. Serious conduct problems in childhood also predict poor functioning across other areas of life in early adulthood such as overweight, heavy drinking, social isolation and not in employment or education. It is important to capture those children who are most at risk, early in their development. The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) is commonly used in clinical settings, to iden

Exposed yet unmapped: evidence of differential flood exposure in deprived urban areas using citizen science.

BACKGROUND: Due to their immature immune system, neonates are at high risk of infection. This vulnerability when combined with limited resources and health education in developing countries can lead to sepsis, resulting in high global neonatal mortality rates. Many of these deaths are preventable. The BabyGel pilot trial tested the feasibility of conducting the main randomised trial, with the provision of alcohol handgel to postpartum mothers for prevention of neonatal infective morbidity in the rural community. This secondary analysis sought to evaluate the methods of detecting infections in
