Right Atrium-Dominant Single-Loop Biatrial Tachycardia Mimicking Reverse Typical Atrial Flutter After Mitral Valve Repair via a Superior Transseptal Approach.

Following a superior transseptal approach, extensive postoperative septal conduction impairment facilitated a right atrium-dominant single-loop biatrial tachycardia mimicking reverse typical atrial flutter. Detailed biatrial mapping and entrainment revealed limited left atrial participation, highlighting the importance of recognizing biatrial circuits in patients with prior septal surgery.

Adjunctive Low-Voltage Area Ablation Reduces Atrial Fibrillation Recurrence in Persistent Atrial Fibrillation: A Systematic Review, Meta-Analysis, and Trial Sequential Analysis.

BACKGROUND: Adjunctive low-voltage area ablation (LVAA) has been proposed as a substrate modification strategy to improve outcomes of catheter ablation in persistent atrial fibrillation (AF); however, randomized controlled trials (RCTs) evaluating LVAA have yielded inconsistent findings. OBJECTIVES: This meta-analysis sought to evaluate the efficacy of LVAA in addition to pulmonary vein isolation (PVI) compared with PVI alone in patients with persistent AF. METHODS: Electronic databases were searched from inception to September 2026 for eligible RCTs. The primary endpoint was recurrent AF/at

Increased Heart Failure Risk After Low-Voltage-Area Ablation in Patients With Paroxysmal Atrial Fibrillation.

BACKGROUND: Extensive atrial ablation may impair atrial function; however, its impact on long-term heart failure (HF) risk remains unclear. OBJECTIVES: This study sought to evaluate the association between low-voltage area (LVA) ablation and HF outcomes after catheter ablation for paroxysmal atrial fibrillation (AF). METHODS: This was a long-term follow-up analysis of the Volatile Coronary Lesion and Obstructive Nodal Atrial Fibrillation trial. A total of 398 patients were followed for up to 7 years and classified into 3 groups: no LVAs (group A), LVAs with LVA ablation (group B), and LVAs w

Unraveling the Enigma of Postablation Rhabdomyolysis: Diagnostic Dilemmas and Therapeutic Strategies.

Rhabdomyolysis is characterized by the breakdown of skeletal muscle tissue and the release of intracellular contents into the circulation, and it poses diagnostic challenges due to its heterogeneous presentation and potential for serious complications. We present a case of a 44-year-old male with a history of hypertension, anxiety, and persistent atrial fibrillation (AF) who developed rhabdomyolysis the morning after a scheduled catheter ablation. He presented with myalgias, muscle cramps, weakness, and brown-colored urine; laboratory studies revealed a creatine phosphokinase (CPK) of 23,757 U

Transient changes in P-wave terminal force after electrical cardioversion of persistent atrial fibrillation.

BACKGROUND: P-wave terminal force in lead V1 (PWTFV1) reflects left atrial remodeling and is associated with atrial fibrillation (AF) outcomes, but its time course after electrical cardioversion of persistent AF is unclear. METHODS: We retrospectively analyzed 108 patients with persistent AF after successful electrical cardioversion, with 12‑lead electrocardiograms at 30 min, 1 month, and 3 months. Terminal P-wave duration (PNTV1) and amplitude (PNAV1) in lead V1 were measured and PWTFV1 calculated as their product; changes are expressed in absolute magnitude. Correlations with left atrial di

One-year outcomes in patients with persistent atrial fibrillation using CARTOFINDER mapping with OCTARAY: a Shinshu-AB registry study.

INTRODUCTION: Persistent atrial fibrillation (PerAF) has no established method of catheter ablation. Although catheter ablation using CARTOFINDER has shown some therapeutic effects, the associated prolonged procedure time remains a challenge. The identification of focal or rotational activation sites by CARTOFINDER using OCTARAY™ Mapping Catheter (OCTARAY) suggests that it can more efficiently detect the activation sites owing to its greater coverage of the left atrium (LA). This study aimed to compare the outcomes of catheter ablation guided by CARTOFINDER versus BOX isolation (BOXI) in patie

Enhancing Persistent Atrial Fibrillation Ablation: The Added Value of Vein of Marshall Ethanol Infusion in a Tailored Strategy.

BACKGROUND: Outcomes following catheter ablation for persistent atrial fibrillation (AF) remain suboptimal. Low-voltage areas (LVA), as a surrogate for atrial fibrosis, have been targeted due to their association with arrhythmia recurrence. Ethanol infusion in the vein of Marshall (VoM) has emerged as an adjunctive strategy that may enhance ablation efficacy. We evaluated the effectiveness of a tailored ablation strategy guided by the presence of LVA and incorporating VoM ethanol infusion in patients with persistent AF. METHODS: This prospective, single-center study included patients referred

Predictors of Recurrence and Quality of Life After Cryoballoon Ablation in Atrial Fibrillation: Data From a Multicenter Taiwanese Registry With 24-Month Follow-Up.

BACKGROUND: Contemporary Asian data on 24-month outcomes after cryoballoon ablation (CBA) in predominantly paroxysmal atrial fibrillation (AF) are limited. METHODS: We conducted a multicenter registry across four Taiwanese tertiary centers (2018-2023). Consecutive patients undergoing CBA were analyzed for documented atrial-arrhythmia recurrence (≥ 30-s episodes after a 90-day blanking period) and 12-month quality of life (QoL; EQ-5D-3L index and visual analogue scale [VAS]). Rhythm follow-up relied mainly on intermittent ECG, Holter, event-recorder, or clinically indicated rhythm assessment.

Pulmonary Vein Isolation Alone Versus Adjunctive Ablation for Paroxysmal Atrial Fibrillation.

BACKGROUND: Pulmonary vein isolation (PVI) is the standard ablation strategy for paroxysmal atrial fibrillation (PAF). Despite guideline recommendations favoring PVI alone in PAF, some operators routinely perform extra pulmonary vein ablation (PVI+) during initial procedures. OBJECTIVE: To determine frequency of PVI+ strategy for treatment of PAF in the real world and examine its impact on arrhythmic recurrence and progression compared to PVI alone. METHODS: Using the TriNetX research network, we identified adults with PAF who underwent catheter ablation between January 2020 and January 2024

Lower airway microbiome transcriptional and metabolic activities influence responsiveness to immunotherapy in NSCLC.

Atrial fibrillation (AF) is an abnormal heart rhythm related to an increased risk of heart failure, dementia, and stroke. The distinction between valvular and non-valvular AF remains a debate. In this study, proteomics and metabolomics were integrated to describe the dysregulated metabolites and proteins of AF patients relative to sinus rhythm (SR) patients. Totally 47 up-regulated and 41 down-regulated proteins in valvular AF, and 59 up-regulated and 149 down-regulated proteins in non-valvular AF were recognized in comparison to SR patients. Moreover, 58 up-regulated and 49 significantly down

Building a surgical atrial fibrillation program: Programmatic evolution, safety, and three-year rhythm outcomes of hybrid ablation.

OBJECTIVE: Surgical ablation is a guideline-supported treatment for symptomatic persistent and long standing persistent atrial fibrillation (AF) refractory to catheter ablation, but how a program is built, refined, and sustained has not been described. We sought to describe the development of a de novo surgical ablation program as well as its procedural evolution, safety profile, and long-term rhythm outcomes. METHODS: We reviewed the programmatic evolution of our program and growth of procedural volume and complexity. We also analyzed the long-term electrophysiologic and safety outcomes of a

Rationale and design of a multicenter randomized trial of adjunctive non-pulmonary vein trigger ablation in persistent atrial fibrillation: The EXPORT trial.

BACKGROUND: Pulmonary vein (PV) isolation in paroxysmal atrial fibrillation (AF) is strongly endorsed with a class IA recommendation for those with symptomatic AF who are refractory to antiarrhythmic drug (AAD) therapy to reduce symptoms, recurrence, and disease progression. However, in persistent AF, the efficacy of PV isolation alone is less pronounced than in paroxysmal AF, underscoring the necessity of adjunctive ablation strategies. Whether ablation of non-PV triggers after PV isolation improves the long-term procedure outcome is unknown. The EXPORT trial aims to evaluate the effects of s

Ibrutinib as the Unifying Culprit: A Case Report of Multisystem Complications in an Elderly Patient With Low-Grade B-Cell Lymphoproliferative Disorder.

Ibrutinib, an irreversible inhibitor of Bruton's tyrosine kinase (BTK), is a cornerstone of therapy for low-grade B-cell lymphoproliferative disorders (B-cell LPD), including Waldenstrom's macroglobulinaemia (WM). While highly effective, its adverse effect profile is broad and potentially life-threatening, particularly in elderly patients with multiple comorbidities. We report an 82-year-old woman with a seven-year history of CD5-negative low-grade B-cell LPD and IgM paraproteinaemia, managed sequentially with rituximab and ibrutinib. During ibrutinib therapy, she developed a cascade of compli

Pericardio-peritoneal communication following emergency subxiphoid pericardiocentesis in acute complex percutaneous coronary intervention: a case report.

BACKGROUND: Emergency pericardiocentesis represents a life-saving intervention in patients with acute cardiac tamponade. During complex percutaneous coronary intervention (PCI), abrupt haemodynamic deterioration secondary to pericardial tamponade may necessitate immediate subxiphoid drainage. Although the procedure is generally safe when performed by experienced operators, catheter malposition can result in ineffective pericardial decompression and may be associated with unexpected and potentially serious complications. CASE SUMMARY: An 83-year-old patient with multiple cardiovascular risk fa
