Ensuring long-term arrhythmia freedom: what are the options for long-standing persistent atrial fibrillation?

INTRODUCTION: Long-standing persistent atrial fibrillation (LSPAF) is among the most challenging arrhythmia phenotypes to manage, due to extensive atrial remodeling, fibrosis, and the presence of complex non-pulmonary vein substrates and triggers. Conventional antiarrhythmic drug (AAD) therapy provides limited efficacy and is constrained by short- and long-term toxicity. Catheter ablation (CA) remains the cornerstone of rhythm control but offers modest single-procedure success. AREAS COVERED: This review examines current treatment strategies for LSPAF, including pharmacologic rhythm control,

Longitudinal Esophageal Wall Rupture and Mediastinitis Due to Esophageal Warming Balloon Dysfunction During Left Atrial Ultralow Cryoablation for Persistent Atrial Fibrillation: A Case Report.

INTRODUCTION: Atrial fibrillation (AF) is a common supraventricular tachyarrhythmia associated with an increased risk of stroke and mortality. Catheter ablation using pulmonary vein isolation is a standard treatment for patients refractory to antiarrhythmic drugs, where radiofrequency ablation and cryoablation are the two most commonly used procedures. Although generally safe, esophageal baro- and thermal injuries remain a rare but life-threatening complication due to the close anatomical relationship between the esophagus and the left atrium. Various protective strategies, such as esophageal

Rhythm control in persistent atrial fibrillation improves endothelial function without uniform anti-inflammatory effects: A 9-month prospective cohort study.

BACKGROUND: Atrial fibrillation (AF) is associated with systemic inflammation, endothelial dysfunction, and adverse cardiovascular outcomes. While there is robust evidence, that inflammation contributes to AF pathogenesis, the existence of a reverse relation - whether AF contributes to inflammation - remains elusive. This study therefore evaluates the impact of rhythm control on systemic inflammation and endothelial function. METHODS: In this prospective observational study, 124 patients with persistent AF undergoing successful rhythm control therapy (electrical cardioversion or catheter abla

Yucatan Miniswine Model of Atrial Fibrillation: Clinical Relevance.

INTRODUCTION: Persistent atrial fibrillation (AFib) is the most common chronic arrhythmia in adults in the United States and is associated with significant morbidity, including thromboembolic events, stroke, and heart failure. Despite available therapies such as catheter ablation and antiarrhythmic drugs, AFib remains incurable for many patients. Our study aims to develop a large-animal model of AFib in Yucatan miniswine to support investigation of new therapeutic approaches for this disease. METHODS: Yucatan miniswine were selected for their physiological similarity to humans and suitable si

Clinical outcomes of pulsed field versus radiofrequency ablation, incorporating posterior wall isolation, in persistent atrial fibrillation.

BACKGROUND: Persistent atrial fibrillation (AF) remains challenging to treat with catheter ablation. The left atrial posterior wall (PW) may represent an important non-pulmonary vein (PV) substrate; however, randomised trials have not demonstrated improved outcomes with adjunctive PW isolation (PWI), potentially reflecting technical limitations of thermal ablation rather than a lack of mechanistic relevance. Pulsed-field ablation (PFA) is a non-thermal ablation modality that selectively targets myocardial tissue and may enable safer and more consistent PWI. We compared real-world outcomes of P

ST-Elevation Myocardial Infarction Secondary to Intracoronary Air Embolism During Atrial Fibrillation Ablation: Case Report and Successful Management.

We present the case of a 47-year-old woman with persistent atrial fibrillation who underwent catheter ablation. During the procedure, real-time ST-segment elevation was observed on surface ECG. An emergency coronary angiography revealed an air embolism in the proximal segment of the left anterior descending (LAD) coronary artery. Intracoronary adenosine was administered, successfully restoring coronary flow (TIMI 3). The procedure was aborted, and the patient was subsequently managed in the coronary care unit due to biventricular cardiogenic shock secondary to myocardial stunning. This case hi

Impact of linear ablation in persistent atrial fibrillation using a dual-energy, wide-footprint catheter: Analysis from the SPHERE Per-AF randomized trial.

BACKGROUND: Among patients with persistent atrial fibrillation, linear ablation lesions beyond pulmonary vein isolation (PVI) are often performed with variable efficacy and inconsistent added value. New catheter designs and energy sources have the potential to improve outcomes. OBJECTIVE: This study aimed to examine (1) the performance of a novel dual-energy (pulsed field/radiofrequency) lattice-tip mapping and ablation system for linear ablation and (2) the impact of the ablation strategy on outcomes in the randomized SPHERE Per-AF trial. METHODS: Linear lesion characteristics, safety, and

Cardiac Macrophages and Fibroblasts Modulate Atrial Fibrillation Maintenance.

BACKGROUND: Nonmyocytes may contribute to regional adaptive changes during persistent atrial fibrillation (PsAF), favoring its perpetuation. We aimed to investigate the differential features of fibroblast and macrophage populations within individual-specific atrial regions associated with PsAF maintenance. METHODS: The study was conducted in 2 pig models of PsAF with and without infarct-related substrate (N=27 and N=27, respectively) and further validated in humans with PsAF (N=20). Sham-operated pigs (N=9), healthy animals (N=4), and patients in sinus rhythm (N=7) were used as comparative co

Catheter Ablation for Persistent Atrial Fibrillation: Rationale, Evidence, and Contemporary Strategies Beyond Pulmonary Veins.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and is associated with substantial morbidity and mortality due to increased risks of stroke, heart failure, and death. Catheter ablation is now firmly established as a first-line or early rhythm control strategy in selected patients with paroxysmal AF and as a Class I indication in symptomatic patients with persistent AF refractory to antiarrhythmic drug therapy. However, outcomes remain more variable in persistent and long-standing persistent AF, reflecting greater atrial substrate complexity, procedural challenges, and

Left Atrial Epicardial Adipose Tissue Associated With Persistent AF Termination During Pulmonary Vein Isolation.

BACKGROUND: Termination of persistent atrial fibrillation (PersAF) during radiofrequency catheter ablation (RFCA) is closely related to the outcome of the ablation. Epicardial adipose tissue (EAT) plays an important role in the maintenance of AF. The role of EAT on the termination of PersAF has not been reported. The aim of this study was to evaluate the association between left atrial EAT (LA-EAT) and termination of PersAF during pulmonary vein isolation (PVI). METHODS: This was a single-center retrospective observational study. Consecutive enrolment of PersAF who underwent first-time radiof

Efficacy of Vein of Marshall Ethanol Infusion in Persistent Atrial Fibrillation Ablation: A Meta-Analysis of RCTs.

BACKGROUND: Catheter ablation for persistent atrial fibrillation (AF) exhibits suboptimal outcomes after pulmonary vein isolation (PVI) alone. The vein of Marshall (VOM) has emerged as an adjunctive ablation target, through which ethanol infusion can achieve transmural lesions in regions resistant to radiofrequency ablation. OBJECTIVE: To evaluate the efficacy and safety of adjunctive VOM ethanol infusion in patients undergoing catheter ablation for persistent AF. METHODS: We conducted a meta-analysis of randomized controlled trials (RCTs) comparing PVI ± linear ablation with versus without

AI-based prediction of heart failure progression in persistent atrial fibrillation using wearable electrocardiography: a brief research report.

BACKGROUND: Persistent atrial fibrillation (AF) frequently coexists with heart failure (HF), yet HF monitoring remains limited by the need for repeated blood-based biomarkers such as N-terminal pro-brain natriuretic peptide (NT-proBNP). Advances in wearable electrocardiography (ECG) and artificial intelligence (AI) now allow continuous extraction of digital physiologic signatures that may reflect hemodynamic stress. OBJECTIVE: To evaluate the feasibility of predicting HF progression using wearable ECG-derived features in patients with persistent AF. METHODS: Fifty patients with persistent AF

Clinical implications of failed electrical cardioversion in long-standing persistent atrial fibrillation.

Ku et al. recently reported clinical predictors of failed electrical cardioversion (ECV) in patients with persistent or long-standing persistent atrial fibrillation. While the multicentre cohort and comprehensive echocardiographic assessment provide valuable data, several methodological and interpretative issues merit further discussion. These include the retrospective design and potential residual confounding, heterogeneity in ECV protocols across centres, reliance on intermittent rhythm monitoring for outcome assessment, and limited integration of contemporary atrial cardiomyopathy concepts
