Machine learning for enhancing the ablation outcomes using recurrence quantification analysis in human persistent atrial fibrillation.

BACKGROUND: Several ablation strategies, including dominant frequency (DF), complex fractionated atrial electrograms (CFAEs), and rotors, have been used to target the drivers of atrial fibrillation (AF). The success rate of these strategies remains suboptimal. OBJECTIVE: This work aims to develop a model using recurrence quantification analysis (RQA) variables and machine learning (ML) algorithms to predict the responses of ablating intracardiac electrograms (EGMs) and their impact on terminating AF and cycle length changes. METHODS: 3206 non-contact EGMs were collected from 10 persistent AF

Learning Curve of Pentaspline Pulsed-Field Ablation With Posterior Wall Isolation for Persistent Atrial Fibrillation.

BACKGROUND: Pentaspline pulsed-field ablation (PFA) has emerged as a novel modality for pulmonary vein isolation (PVI) and posterior wall isolation (PWI) in persistent atrial fibrillation (AF). However, the learning curve associated with combined PVI + PWI and the contribution of anatomical factors to acute lesion completeness remain incompletely defined. METHODS: We analyzed 182 consecutive patients with persistent AF undergoing fluoroscopy- and intracardiac echocardiography-guided PFA. Acute incomplete lesions were defined as residual conduction requiring additional PFA applications after s

Pre-ablation rate control therapy and direct-to-catheter ablation in patients with persistent atrial fibrillation: A DECAAF II subanalysis.

BACKGROUND: For patients with persistent atrial fibrillation (PersAF), initial treatment strategies often involve rate or rhythm control before offering ablation. This study compared two approaches: (1) catheter ablation without prior medication (referred to as direct-to-catheter ablation, DTCA) and (2) catheter ablation after initial rate control with either diltiazem or metoprolol. OBJECTIVE: This study included two independentanalyses of patients with persistent atrial fibrillation (PersAF) undergoing catheter ablation. Aimed at evaluating the potential impact of pre-ablation rate control

Risk factors of failed electrical cardioversion in patients with persistent or long-standing persistent atrial fibrillation.

BACKGROUND/AIMS: Identifying clinical risk factors of failed electrical cardioversion (ECV) for persistent or long-standing persistent atrial fibrillation (AF) can guide selection of rhythm control. METHODS: A total of 1058 patients who underwent ECV for persistent or long-standing persistent AF at multiple centres were retrospectively reviewed. Patients were divided into three groups: group 1 maintained sinus rhythm (SR) for >1 year, group 2 maintained SR ≤1 year after ECV, and group 3 had failed ECV. SR maintenance was assessed via regular electrocardiography follow-ups or Holter. RESULTS:

From bench to bedside: The clinical relevance of atrial electrical remodeling in atrial fibrillation therapy.

Atrial electrical remodeling spans molecular, electrical, and structural alterations that shorten refractoriness, facilitate reentry, and ultimately create the atrial substrate underlying atrial fibrillation (AF). These changes include ion channel dysfunction, calcium-handling abnormalities, oxidative injury, and deranged proteostasis that are tightly intertwined with atrial fibrosis and cardiomyopathy. High-density mapping and advanced imaging now allow in vivo staging of this substrate: left atrial low-voltage areas and functional conduction phenomena identify regions of fibrosis and conduct

Rivaroxaban Treatment Prevents Atrial Myocyte Hypertrophy in Goats with Persistent Atrial Fibrillation by Inhibition of Protease-Activated Receptor-1.

Atrial fibrillation (AF) is associated with an increased risk of stroke and hypercoagulability. Coagulation factors mediate remodeling processes via protease-activated receptors (PARs) in various organs. We evaluated whether inhibition of factor Xa (FXa) via rivaroxaban protects against atrial structural remodeling in goats with persistent AF and explored FXa and thrombin hypertrophic effect on human iPSC-derived cardiomyocytes (hiPSC-CMs).Three groups of goats were tested: CTRL AF (control AF, n = 10), RIVA AF (rivaroxaban treatment during AF, n = 11), and SHAM (no AF, n = 10). Pacing-induced

A Retrospective Long-Term Follow-Up of the Randomized Study: Total Endoscopic Ablation of Patients With Long-Standing Persistent Atrial Fibrillation.

OBJECTIVES: Minimally invasive surgical ablation for atrial fibrillation is an alternative to catheter ablation. Achieving a lasting sinus rhythm in long-standing persistent atrial fibrillation is challenging, and long-term data after surgery are limited. In 2016, we published a randomized trial comparing totally endoscopic box lesion ablation of the left atrium (case) to medical therapy (control) during 1 year in patients with long-standing persistent atrial fibrillation. This study presents data from a follow-up of our previous cohort to investigate the rhythm outcome long-term. METHODS: Th

A Simple Method Is Used to Identify PVI-Responsive Patients With Persistent Atrial Fibrillation: Electrical Cardioversion and Noninduction Before Procedure.

BACKGROUND: Pulmonary vein isolation (PVI) demonstrates less efficacy in persistent atrial fibrillation (PersAF) compared to paroxysmal AF. Although recent multiple-center RCTs have shown that additional ablation, except for the ethanol intervention of the Marshall vein, has not significantly benefited in persistent AF. Therefore, the most important issue is how to identify PVI-responsive PersAF. OBJECTIVES: This exploratory cohort study assessed a novel direct current cardioversion-electrophysiological study (DC-EPS) protocol before ablation, and evaluated whether inducibility can be guided

A rare case of congenital absence of left atrial appendage diagnosed by multimodal imaging.

Congenital absence of the left atrial appendage (LAA) is an exceptionally rare cardiac anomaly. We reported a patient in his 60s with symptomatic persistent atrial fibrillation (AF) and no history of cardiac surgery or other diseases. During preprocedural evaluation for AF cardioversion, transesophageal echocardiography and cardiac computed tomography revealed a complete absence of the LAA. The patient was managed with regular anticoagulation therapy and maintained in sinus rhythm, without recurrence or related complications during a 1-year follow-up.

Short-Term Procedural Outcomes of Posterior Wall Isolation Using the Affera Pulse Field Ablation System.

BACKGROUND: Posterior wall isolation (PWI) is often added to pulmonary vein isolation (PVI) for persistent atrial fibrillation (AF) and at redo ablation. Still, procedural performance with the Affera pulsed field ablation (PFA) platform is not well described. METHODS: We retrospectively analyzed consecutive patients who underwent PWI with the Affera Sphere-9 lattice-tip catheter from April to August 2025. Baseline characteristics and procedural data, including PWI time, number of lesions, and acute success, were analyzed. The primary procedural endpoint was acute PWI success. Secondary measur

Pulsed-Field Ablation With a Persistent Left Superior Vena Cava: Potential for Transmural Ablation.

BACKGROUND: A persistent left superior vena cava (PLSVC) is an important source of nonpulmonary vein triggers in patients with atrial fibrillation. However, experience with pulsed-field ablation (PFA) in the presence of a PLSVC is limited. CASE SUMMARY: A 63-year-old man with persistent atrial fibrillation presented for PFA. A previously unrecognized PLSVC was identified. A decapolar catheter was positioned in the PLSVC. During PFA, attenuation of the PLSVC electrograms was observed, consistent with a potentially transmural effect. Given this observation and the lack of preprocedural discussi

Real word data with no evidence for additional left atrial substrate modification at first PVI procedure in patients with persistent atrial fibrillation - Results from the German Ablation registry.

BACKGROUND: Pulmonary vein isolation (PVI) is the cornerstone of interventional treatment for atrial fibrillation. Patients with persistent atrial fibrillation have relatively high recurrence rates after catheter ablation irrespective of treatment strategy. Randomized data suggest that adding substrate modification to PVI at the first ablation procedure does not improve arrhythmia-free survival. To date, real-world data from experienced centers is limited. METHODS AND RESULTS: The German Ablation registry maintained by the Institut für Herzinfarktforschung Ludwigshafen, Germany included 909 p

Ensuring long-term arrhythmia freedom: what are the options for long-standing persistent atrial fibrillation?

INTRODUCTION: Long-standing persistent atrial fibrillation (LSPAF) is among the most challenging arrhythmia phenotypes to manage, due to extensive atrial remodeling, fibrosis, and the presence of complex non-pulmonary vein substrates and triggers. Conventional antiarrhythmic drug (AAD) therapy provides limited efficacy and is constrained by short- and long-term toxicity. Catheter ablation (CA) remains the cornerstone of rhythm control but offers modest single-procedure success. AREAS COVERED: This review examines current treatment strategies for LSPAF, including pharmacologic rhythm control,

Longitudinal Esophageal Wall Rupture and Mediastinitis Due to Esophageal Warming Balloon Dysfunction During Left Atrial Ultralow Cryoablation for Persistent Atrial Fibrillation: A Case Report.

INTRODUCTION: Atrial fibrillation (AF) is a common supraventricular tachyarrhythmia associated with an increased risk of stroke and mortality. Catheter ablation using pulmonary vein isolation is a standard treatment for patients refractory to antiarrhythmic drugs, where radiofrequency ablation and cryoablation are the two most commonly used procedures. Although generally safe, esophageal baro- and thermal injuries remain a rare but life-threatening complication due to the close anatomical relationship between the esophagus and the left atrium. Various protective strategies, such as esophageal

Rhythm control in persistent atrial fibrillation improves endothelial function without uniform anti-inflammatory effects: A 9-month prospective cohort study.

BACKGROUND: Atrial fibrillation (AF) is associated with systemic inflammation, endothelial dysfunction, and adverse cardiovascular outcomes. While there is robust evidence, that inflammation contributes to AF pathogenesis, the existence of a reverse relation - whether AF contributes to inflammation - remains elusive. This study therefore evaluates the impact of rhythm control on systemic inflammation and endothelial function. METHODS: In this prospective observational study, 124 patients with persistent AF undergoing successful rhythm control therapy (electrical cardioversion or catheter abla

Yucatan Miniswine Model of Atrial Fibrillation: Clinical Relevance.

INTRODUCTION: Persistent atrial fibrillation (AFib) is the most common chronic arrhythmia in adults in the United States and is associated with significant morbidity, including thromboembolic events, stroke, and heart failure. Despite available therapies such as catheter ablation and antiarrhythmic drugs, AFib remains incurable for many patients. Our study aims to develop a large-animal model of AFib in Yucatan miniswine to support investigation of new therapeutic approaches for this disease. METHODS: Yucatan miniswine were selected for their physiological similarity to humans and suitable si

Clinical outcomes of pulsed field versus radiofrequency ablation, incorporating posterior wall isolation, in persistent atrial fibrillation.

BACKGROUND: Persistent atrial fibrillation (AF) remains challenging to treat with catheter ablation. The left atrial posterior wall (PW) may represent an important non-pulmonary vein (PV) substrate; however, randomised trials have not demonstrated improved outcomes with adjunctive PW isolation (PWI), potentially reflecting technical limitations of thermal ablation rather than a lack of mechanistic relevance. Pulsed-field ablation (PFA) is a non-thermal ablation modality that selectively targets myocardial tissue and may enable safer and more consistent PWI. We compared real-world outcomes of P
