Atrial electrograms and activation sequences in the transition between atrial fibrillation and atrial flutter.

INTRODUCTION: The electrophysiologic mechanism of atrial fibrillation (AF) has a wide spectrum, and it seems that some atrial regions are essential for the occurrence of a particular type of AF. We focused on one type of AF: AF associated with typical atrial flutter (AFL), which was right atrial (RA) arrhythmia, and sought to investigate intra-atrial electrograms and activation sequences in the transition between AF and AFL. METHODS AND RESULTS: Intra-atrial electrograms and activation sequences in the RA free wall and the septum were evaluated in the transition between AF and AFL in seven pa

[Pathophysiologic mechanisms of atrial rhythm disorders. II. Atrial flutter].

Atrial flutter is a common cardiac dysrhythmia which responds for half of the supraventricular tachycardias with exception of atrial fibrillation. Activation and entrainment mapping studies in man and in animals confirmed the reentry mechanism of atrial flutter and demonstrated that the reentry circuit is located in the right atrium. The most important anatomical structures which enable the occurrence of atrial flutter are crista terminalis, Eustachian valve/ridge and tricuspid annulus. In typical atrial flutter the impulse rotates "counterclockwise". According to the present knowledge the ree

Electrical-anatomic correlations between typical atrial flutter and intra-atrial re-entry following atrial surgery.

It is well known that in typical (or type I) atrial flutter, conduction proceeds counterclockwise, up the interatrial septum and down the right atrial wall anterior to the crista terminalis (CT). Recent careful mapping studies using entrainment pacing have clearly shown the importance of the CT and the eustachian valve ridge (EVR), which act as fixed barriers to intra-atrial conduction and interact with other barriers, including the tricuspid valve, inferior vena cava (IVC), and coronary sinus os, to create a long macroreentrant circuit. Ablative lesions are directed at the isthmus between the

Hybrid pharmacologic and ablative therapy: a novel and effective approach for the management of atrial fibrillation.

INTRODUCTION: Maintenance of sinus rhythm in patients with recurrent atrial fibrillation is often difficult to achieve with pharmacologic therapy. Complex catheter ablative procedures are being developed, but efficacy and safety issues remain to be clarified. We hypothesized that combined pharmacologic and simple ablative therapies in a targeted subset of patients will improve success in the treatment of atrial fibrillation. METHODS AND RESULTS: We identified 13 patients (mean age 61.5 +/- 16.2 years) with atrial fibrillation who converted to electrocardiographic atrial flutter during antiarr

Unipolar waveforms and monophasic action potentials in the characterization of slow conduction in human atrial flutter.

Anisotropic propagation may be involved in the development of areas of slow conduction in atrial flutter. We evaluated monophasic action potentials (MAPs) and simultaneous unipolar (0.2-400 Hz) and bipolar electrograms from multiple atrial sites of patients undergoing RF ablation of idiopathic atrial flutter. Nine patients (mean age 46 +/- 20 years) with typical atrial flutter (one with both types) were studied. Unipolar electrograms with triphasic complexes of small amplitude and with a slow, negative deviation of the baseline preceding the rS deflection indicated transversal conduction in re

P-wave morphology during right atrial pacing before and after atrial flutter ablation--a new marker for success.

Fourteen patients with typical atrial flutter underwent pacing from the low lateral right atrium and the proximal coronary sinus in normal sinus rhythm before and after catheter ablation. During low lateral right atrial pacing, a positive change in P-wave morphology in the inferior leads was noted in every patient (n = 12) in whom bidirectional block was achieved; no recurrence was noted in any of these patients.

[Transcatheter subendocardial infusion: a novel technique for ablation of atrial arrhythmias].

UNLABELLED: Ablation of typical atrial flutter using radiofrequency energy is limitted by difficulties in creating a continuous line of block across the istmus between the tricuspid anulus and the inferior vena cava. Recent studies from our laboratory have shown that a novel infusion ablation technique can create large endocardial lesions in a safe and predictable fashion. The purpose of our study was to assess the feasibility of transcatheter subendocardial infusion of a mixture of 95% enthanol, iodinated contrast and glycerin to create atrial lesions resulting in complete ablation of the cri

Conduction properties of the inferior vena cava-tricuspid annular isthmus in patients with typical atrial flutter.

INTRODUCTION: A functional region of slow conduction located in the inferior right atrium has been postulated to be critical to the induction and maintenance of typical human atrial flutter. We reexamined the potential role of functional conduction delay in the annular isthmus between the tricuspid valve and the inferior vena cava; it is within this region that such delays have been postulated to occur, and where interruption of conduction by radiofrequency energy application has been shown to eliminate typical flutter. METHODS AND RESULTS: Thirty patients with type I atrial flutter (30 count

Mechanism and location of atrial flutter in transplanted hearts: observations during transient entrainment from distant sites.

OBJECTIVES: This study was designed to elucidate the location and mechanism of typical atrial flutter in the transplanted heart. BACKGROUND: Although the F wave morphology in atrial flutter is similar in nontransplanted and transplanted hearts, the surgical incision needed for the atrial anastomosis may create a distinct electrophysiologic substrate of atrial flutter. METHODS: Entrainment from the lateral wall of the right atrium and interatrial septum was used to determine the location of atrial flutter in five patients with a transplanted heart and six patients with a nontransplanted heart

Simplified electrophysiologically directed catheter ablation of recurrent common atrial flutter.

BACKGROUND: Despite verification of bidirectional conduction block after radiofrequency (RF) catheter ablation in the inferior vena cava (IVC)-tricuspid annulus (TA) isthmus, recurrence of common atrial flutter is relatively common. Although complete linear reablation is usually performed, we evaluated a simplified electrophysiological strategy selectively targeting recovered conducting isthmus tissue. METHODS AND RESULTS: Twenty-one patients (18 men and 3 women, age, 54+/-10 years) with a recurrence of typical atrial flutter 6+/-7 months after an apparently successful catheter ablation in th

Characterization of low right atrial isthmus as the slow conduction zone and pharmacological target in typical atrial flutter.

BACKGROUND: Previous electrophysiological studies in patients with typical atrial flutter suggested that the slow conduction zone might be located in the low right atrial isthmus, which is a path formed by orifice of inferior vena cava, eustachian valve/ridge, coronary sinus ostium, and tricuspid annulus. The conduction characteristics during atrial pacing and responses to antiarrhythmic drugs of this anatomic isthmus were unknown. METHODS AND RESULTS: Forty-four patients, 20 patients with paroxysmal supraventricular tachycardia (group 1) and 24 patients with clinically documented paroxysmal

Relationship between atrial fibrillation and typical atrial flutter in humans: activation sequence changes during spontaneous conversion.

BACKGROUND: A transitional rhythm precedes the spontaneous onset of atrial flutter in an animal model, but few data are available in man. METHODS AND RESULTS: In 10 patients, 16 episodes of atrial fibrillation (166+/-236 seconds) converting into atrial flutter during electrophysiological evaluation were analyzed. A 20-pole catheter was used for mapping the right atrial free wall. Preceding the conversion was a characteristic sequence of events: (1) a gradual increase in atrial fibrillation cycle length (150+/-25 ms after onset, 166+/-28 ms before conversion, P<.01); (2) an electrically silent

Characterization of the excitable gap in human type I atrial flutter.

OBJECTIVES: We sought to characterize the excitable gap of the reentrant circuit in atrial flutter. BACKGROUND: The electrophysiologic substrate of typical atrial flutter has not been well characterized. Specifically, it is not known whether the properties of the tricuspid valve isthmus differ from those of the remainder of the circuit. METHODS: Resetting was performed from two sites within the circuit: proximal (site A) and distal (site B) to the isthmus in 14 patients with type I atrial flutter. Resetting response patterns and the location where interval-dependent conduction slowing occurr

Relation between left atrial size and secondary atrial arrhythmias after successful catheter ablation of common atrial flutter.

Catheter ablation provides an effective cure for patients with typical atrial flutter. However, these patients may have the potential to develop atrial tachyarrhythmias other than common atrial flutter. This study examines clinical and echocardiographic predictors for the occurrence of uncommon atrial flutter or atrial fibrillation after abolition of common atrial flutter. The study population comprised 17 patients (12 men, 5 women, age 32-74 years) who underwent successful radiofrequency catheter ablation of common atrial flutter. Common atrial flutter did not recur in any patient during a me

Electrophysiologic characteristics and radiofrequency catheter ablation in patients with clockwise atrial flutter.

INTRODUCTION: Although the mechanism and radiofrequency catheter ablation of counterclockwise (typical) atrial flutter have been studied extensively, information about the electrocardiographic and electrophysiologic characteristics and effects of radiofrequency ablation in patients with clockwise atrial flutter is limited. METHODS AND RESULTS: Thirty consecutive patients with clinically documented paroxysmal clockwise atrial flutter were studied. Endocardial recordings and entrainment study using a "halo" catheter with 10 electrode pairs in the right atrium were performed. Radiofrequency ener

[Mechanism and location of typical atrial flutter. Information drawn from experimental models and clinical electrophysiology].

Several experimental models have been proposed to explain the electrocardiographic and electrophysiological characteristics of atrial flutter. In animal models based on anatomical obstacles, intercaval crush or Y like shaped lesion located in the right atrium, it has been possible to induce sustained atrial arrhythmias in which the entrainment criteria could be demonstrated. Additionally these tachycardias presented an atrialwave morphology similar to the F waves of type 1 or typical atrial flutter. Flutter type 2 could better be explained by models based on functional reentry like the leading

Results of radiofrequency catheter ablation for atrial flutter.

RF catheter ablation for symptomatic typical atrial flutter is associated with a high procedural success rate, but a second RF procedure may be required in up to one third of subjects, particularly those with right atrial enlargement. In those subjects with both established AF and flutter, RF ablation for atrial flutter may decrease the recurrence rate of AF. However, patients remain at risk for the development of newly documented AF, most likely secondary to the high incidence of underlying structural heart disease.
