Association of lipoprotein(a) with ischemia-reperfusion injury in patients with ST-elevation myocardial infarction.

BACKGROUND: Hypercholesterolemia adversely affects ischemia-reperfusion injury in acute ST-elevation myocardial infarction (STEMI). Lipoprotein(a) (Lp(a)) is an independent cardiovascular risk factor beyond traditional lipid markers; however, its impact on ischemic myocardial injury remains unclear. We therefore investigated the association of Lp(a) with myocardial and microvascular damage assessed by multiparametric cardiovascular magnetic resonance (CMR), as well as clinical outcomes in STEMI patients. METHODS AND RESULTS: In this prospective MARINA-STEMI (NCT04113356) cohort study, 518 STE

Neutrophil to high-density lipoprotein cholesterol ratio is positively associated with congestive heart failure among hypertensive adults: a population-based study.

BACKGROUND: Congestive heart failure (CHF) continues to pose a substantial global health issue. The neutrophil-to-high-density lipoprotein cholesterol ratio (NHR), a novel biomarker reflecting systemic inflammatory balance, has been linked to cardiovascular outcomes, yet its relationship with CHF in hypertensive individuals is unclear. METHODS: A cross-sectional investigation was adopted using data obtained from the National Health and Nutrition Examination Survey (NHANES) spanning the years 1999 to 2018. Weighted multivariate logistic regression was used to evaluate the independent correlati

A machine learning-driven and explainable prediction model for in-hospital mortality in patients with diabetes mellitus and congestive heart failure: integrating XGBoost and SHAP approaches.

BACKGROUND: Patients with congestive heart failure (CHF) and type 2 diabetes mellitus (T2DM) are at elevated risk of in-hospital mortality, particularly in intensive care units (ICUs). Conventional scoring systems may not fully capture the clinical complexity of such comorbidities. This study aimed to construct an interpretable machine learning model to predict in-hospital mortality in this high-risk group. METHODS: A retrospective study included 573 ICU patients with CHF and T2DM admitted to three hospitals in Henan, China (Jan 2025-Jun 2026). Seven predictors-body mass index (BMI), myocardi

Multimodal imaging of a coronary artery fistula in a Welsh Springer spaniel.

A four-year-old intact male Welsh Springer spaniel was presented for a prebreeding evaluation. Thoracic auscultation revealed a grade 2/6 left basilar systolic murmur. Echocardiography demonstrated left atrial and left ventricular dilation with systolic dysfunction, including a left atrium-to-aortic root ratio of 2.24, normalized left ventricular internal dimension in diastole of 2.44, and fractional shortening of 19%. Color and spectral Doppler interrogation identified continuous flow with a peak velocity of approximately 1.5 m/s in the left pulmonary artery. Transesophageal echocardiography,

Merkel Cell Carcinoma Masquerading as Dermatitis.

Merkel cell carcinoma (MCC) is a rare, highly aggressive neuroendocrine skin carcinoma that typically presents as a rapidly enlarging, firm, painless nodule on sun-exposed areas in elderly patients. This malignancy is characterized by high rates of local recurrence, regional spread, and distant metastasis. We report a case of a woman in her 80s who presented with an erythematous patch on the left anterior thigh, a non-sun-exposed site, initially diagnosed as inflammatory dermatitis and treated with topical corticosteroids. After two months of failed therapy and interval growth, a punch biopsy

Doxorubicin-induced Cardiotoxicity in Dogs: A Narrative Review.

Doxorubicin, an anthracycline widely used in both human and veterinary oncology, is highly effective against a variety of malignancies in dogs, including lymphomas, sarcomas and carcinomas. Despite its efficacy, doxorubicin exposure can be associated with cardiotoxicity, which may present acutely or chronically, potentially leading to severe cardiac arrhythmia and congestive heart failure. Mechanisms underlying cardiotoxicity are multifactorial, including oxidative stress, mitochondrial dysfunction, DNA damage via topoisomerase IIβ, inflammation and dysregulated cell death pathways such as apo

Optimizing entropy-based heart rate variability metrics: fixed tolerance improves congestive heart failure classification.

Entropy-based heart rate variability (HRV) metrics are widely used to characterize heartbeat dynamics and distinguish physiological from pathological states, but their classification performance is highly sensitive to parameter selection. This study investigated a fixed-tolerance approach derived from a healthy reference population as an alternative to conventional subject-specific tolerance normalization and assessed how entropy-parameter selection influences classification performance.

Approach:
Approximate entropy (ApEn), sample entropy (SampEn), and multiscale entropy (

Drug-Drug Interactions vs. Therapeutic Combinations in Patients with Congestive Heart Failure and Diabetes: An Ambispective Real-World Study.

Concomitant congestive heart failure (CHF) and diabetes mellitus (DM) patients frequently are treated with polypharmacy, thus potentially increasing the risk of drug-drug interactions (pDDIs). Conventional DDI screening methods often fail to distinguish clinically significant interactions from recommended therapeutic combinations. This study assessed the prevalence, severity, and clinical consequences of pDDIs in hospitalized patients with CHF and DM. This ambispective, cross-sectional study included 144 hospitalized patients with CHF and DM. We used the Micromedex and Lexicomp databases to ch

Anticoagulation strategies during peripheral ultrafiltration in diuretic-resistant congestive heart failure.

Introduction and objectives Peripheral ultrafiltration (UF) is an effective strategy for the management of diuretic-resistant congestion in patients with decompensated heart failure. However, extracorporeal circuit clotting remains one of the main technical limitations of the procedure, and the optimal anticoagulation strategy during UF has not been clearly established. The primary aim of this study was to evaluate the efficacy and safety of different anticoagulation strategies during UF. Methods This prospective, nonrandomized, single-center study included 87 consecutive patients who underwen

Equity Meets Primary-Care-Sensitive Performance: A Longitudinal Benchmark of Cost-Related Unmet Care and Avoidable Hospitalization in Europe.

BACKGROUND/OBJECTIVES: This study compares European health systems by considering the cost-related unmet-care inequality between income groups and primary-care-sensitive avoidable hospitalizations. METHODS: This was a longitudinal comparative benchmarking study. The main sample consists of 275 country-year observations from 20 European countries with at least ten years of data during 2009-2023. The first dimension is measured by the absolute difference between the lowest and highest income quintiles in the prevalence of unmet medical examination or treatment because it was too expensive. Distinct Temporal Patterns of Limb Loss and Mortality After Transcatheter Arterialization of the Deep Veins for No-Option CLTI.

OBJECTIVE: No-option chronic limb-threatening ischemia (CLTI) represents the most advanced stage of peripheral arterial disease, characterized by exhausted revascularization options and substantial systemic comorbidity. Contemporary registry data have reported amputation-free survival (AFS) as low as approximately 37% at 1 year with standard care alone. Deep venous arterialization (DVA) has emerged as a limb-salvage strategy for these patients by arterializing the venous circulation to restore perfusion to the distal extremity. Although early trial data have been encouraging, the durability of

The heart-skin connection: Carvajal syndrome in a child with a novel DSP gene variant-a case report and literature review.

Carvajal syndrome is a rare familial cardio-cutaneous disorder characterised by the triad of woolly hair, palmoplantar keratoderma, and dilated cardiomyopathy. It results from mutations in the DSP gene, (p 24) a desmosomal protein crucial for cell adhesion. Defective desmoplakin disrupts myocardial and epidermal integrity, leading to electrical and contractile dysfunction of the heart. The resulting cardiomyopathy progresses to life-threatening congestive heart failure. We describe a 7-year-old boy from Delhi, India, who presented with features of decompensated heart failure along with woolly
