Cardiac rupture (CR) remains among the most lethal mechanical complications of acute myocardial infarction (AMI), with in-hospital mortality exceeding 50%. Although primary percutaneous coronary intervention has reduced CR incidence to 1%-2%, mortality has not declined proportionally because the abrupt onset of rupture limits the opportunity for intervention. The early post-infarction period-particularly the first 72 h for free wall rupture and extending through day 7 for ventricular septal and papillary muscle rupture-represents the critical window for detection. This mini review synthesizes

INTRODUCTION: Traumatic tricuspid regurgitation resulting from blunt chest trauma is a rare complication, and the surgical options remain unclear.
CASE PRESENTATION: We describe the case of a 19-year-old male who sustained polytrauma in a scooter accident. Concomitant medical findings included massive right hemothorax, splenic burst with active bleeding, hemoperitoneum, L1-L3 spinous apophysis fracture, and 2 rib arches. Despite 6 hours of medical treatment, including high doses of noradrenaline and dobutamine, complete stabilization was not achieved. A transesophageal echocardiogram was perf

Over 800,000 acute myocardial infarction (AMI) events occur annually in the United States. Increased emphasis on primary prevention strategies has decreased the incidence of AMI.1,2 Treatment of AMI includes reperfusion of the culprit coronary arteries, and expeditious intervention has led to a decrease in the rate of post-AMI complications.3 However, these complications still occur in approximately 0.3% of patients presenting with AMI; this is estimated to be about 2,400 patients annually.4,5 Myocardial tissue necrosis secondary to AMI can lead to several different mechanical complications, i

Traumatic papillary muscle rupture is an uncommon cause of acute mitral regurgitation (MR). We report the perioperative management of a case of severe MR due to the posteromedial papillary muscle rupture in a 33-year-old man who had blunt chest trauma caused by fall of an electric lamp post over him. He also had an additional finding of a Ventricular Septal Defect (VSD). He successfully underwent a mitral valve replacement (MVR) with a VSD repair surgery and was discharged on the sixth day of the surgical correction. Sometimes respiratory symptoms because of hemothorax, rib fractures can mask

BACKGROUND: Transvenous lead extraction carries a small risk of vascular tears and tricuspid valve (TV) damage.
CASE SUMMARY: A 68-year-old woman with dilated cardiomyopathy and a primary prevention cardiac resynchronization therapy with a defibrillator presented with device infection and was referred for system extraction. During the procedure, she developed acute hemodynamic instability due to an anterior papillary muscle rupture, resulting in a flail anterior TV leaflet and severe tricuspid regurgitation. Emergent TV transcatheter edge-to-edge repair was performed successfully within 48 ho

BACKGROUND: Iatrogenic papillary muscle rupture (iPMR) is a rare but fatal complication of catheter interventions. No previous report to our knowledge has described iPMR with pathologic findings following transcatheter embolization for guidewire perforation of coronary artery.
CASE SUMMARY: An 83-year-old man with unstable angina underwent a percutaneous coronary intervention. On the same night as the percutaneous coronary intervention, a guidewire perforation of the left circumflex artery side branch was identified, resulting in cardiac tamponade. Transcatheter embolization of the culprit ve

Myocardial infarction (MI) is a leading cause of morbidity and mortality worldwide. Although rapid diagnosis and reperfusion in the acute setting rely primarily on clinical assessment, electrocardiography, echocardiography, and invasive coronary angiography, advanced cardiac imaging plays an essential role beyond the hyperacute phase. Cardiac computed tomography (CCT) and cardiac magnetic resonance (CMR) imaging provide complementary information that extends from coronary anatomy to myocardial tissue characterization and functional assessment. CMR imaging enables the comprehensive, multiparame

Sudden hemodynamic deterioration after myocardial infarction should prompt immediate evaluation for papillary muscle rupture, even in the absence of a murmur or large-vessel occlusion. Acute severe mitral regurgitation from papillary muscle rupture may mimic recurrent ischemia, and early echocardiography with prompt surgical intervention can be lifesaving.

Mitral valve papillary muscle rupture represents a rare yet critical complication of acute myocardial infarction, often leading to severe mitral regurgitation and subsequent pulmonary edema with high mortality. Despite optimal ventilatory support and pharmacological management, patients often exhibit refractory hypoxemia, rendering surgical intervention both technically demanding and high-risk. A 40-year-old Asian male presented to the emergency department of Liaocheng People's Hospital following percutaneous coronary intervention (PCI) for acute myocardial infarction at an outside facility. BACKGROUND: Adults with repaired tetralogy of Fallot (rTOF) frequently develop right heart failure, most commonly related to pulmonary regurgitation. Tricuspid regurgitation (TR) is an underrecognized contributor. Transcatheter edge-to-edge repair (TEER) has emerged as an alternative to surgery, but evidence in congenital heart disease remains limited.
CASE SUMMARY: We report the first case of isolated tricuspid valve repair using TriClip in an adult with rTOF and severe symptomatic TR. Preprocedural transesophageal echocardiography demonstrated type IIIb anatomy with a 6-mm coaptation gap. T

Papillary muscle rupture (PMR) is often underdiagnosed or diagnosed late due to its low incidence and nonspecific symptoms in acutely presenting, critically ill patients. This report presents a case of a 74-year-old male who developed acute mitral regurgitation secondary to PMR in the context of an initially conservatively managed inferior wall infarction following recent spine surgery. The discussion highlights the most appropriate diagnostic approach for critically ill patients presenting with acute hypoxic respiratory failure in the cardiac ICU. It also examines optimal management strategie

Mechanical complications after acute myocardial infarction (MI)-ventricular septal rupture (VSR), free-wall rupture (FWR), and papillary muscle rupture (PMR)-have become uncommon in the primary percutaneous coronary intervention (PCI) era, yet remain among the most lethal cardiovascular emergencies, with contemporary mortality largely driven by cardiogenic shock and delays to definitive treatment. Although major society documents agree on urgent imaging, early mechanical circulatory support when shock is present, and multidisciplinary decision-making, important transatlantic differences persis

Background and Clinical Significance: Mechanical complications and intracavitary thrombus are both recognized causes of clinical deterioration following acute myocardial infarction, yet they require fundamentally different therapeutic approaches. Distinguishing between these entities is critical, as misdiagnosis may lead to unnecessary surgical intervention or delayed anticoagulation with serious consequences. Left ventricular (LV) thrombus typically appears as a well-defined mass; however, atypical and highly mobile morphologies may closely mimic catastrophic post-infarction mechanical compli

Leadless pacemaker (LP) is reported to have serious complications including cardiac tamponade and ventricular perforation during and after implantation. We report a case of severe tricuspid regurgitation (TR) caused by papillary muscle rupture and hypoxia during LP re-implantation, which required surgical intervention. The patient was an 88-year-old man who had undergone LP implantation for complete atrioventricular block 2 years before. Due to the battery depletion, the additional LP was implanted. After the procedure, he developed persistent hypoxemia despite 9 l oxygen administration. He wa

BACKGROUND: Acute myocardial infarction complicated by cardiogenic shock (AMI-CS) carries the highest in-hospital mortality among acute coronary syndromes, with medical management and various adjunctive therapies employed to care for these critically-ill patients.
OBJECTIVE: Intra-aortic balloon pumps (IABP) are a widely-used therapy, thus we aim to evaluate differences in mortality, clinical outcomes, and hospital courses of patients with AMI-CS treated with IABP versus non-IABP medical management.
METHODS: We conducted a retrospective cohort study of the National Inpatient Sample database

The prevalence of mechanical complications following acute myocardial infarction has steadily declined in recent years owing to advances in prompt coronary revascularization, and they now occur in <1% of acute myocardial infarction cases. Nevertheless, significant haemodynamic impairment may already be present at hospital admission, requiring immediate diagnostic evaluation and urgent intervention. Until recently, surgical repair was the only treatment option, with non-negligible in-hospital mortality rates, particularly among patients with acute cardio-circulatory failure. Advances in transca

BACKGROUND: Catheter kinking during coronary angiography is a common complication, particularly in patients with tortuous vascular anatomy. Traditional retrieval methods may be unsuccessful in such scenarios.

UNLABELLED: Cardiogenic shock secondary to papillary muscle rupture carries high mortality and often precludes surgical repair. We present a case of acute mitral regurgitation following myocardial infarction managed with emergent transcatheter edge-to-edge repair (TEER) under combined veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and transvalvular assist device (Impella, Abiomed, Danvers, MA, USA) support. Real-time echocardiographic guidance required transient VA-ECMO flow reduction to optimize right atrial visualization and Impella adjustment to prevent device interference. Seq
