Cardiac diagnostic modalities in polycythemia vera: A narrative review.

Polycythemia vera is a chronic myeloproliferative neoplasm in which erythrocytosis, Janus kinase-signal transducer and activator of transcription activation, inflammation, endothelial dysfunction, and abnormal blood-cell activation converge to increase cardiovascular risk. This narrative review examines how electrocardiography, echocardiography, coronary angiography, cardiac magnetic resonance imaging, and right-heart catheterization contribute to recognition and characterization of cardiac involvement in polycythemia vera. Reported abnormalities include QT and P-wave changes, atrial arrhythmi

Fragmented Iliac Venous Stent Migration Requiring Tricuspid Valve Replacement.

BACKGROUND: A 36-year-old man with May-Thurner syndrome and a family history of cardiac disease underwent a screening echocardiogram, which revealed a mass in the right ventricle. Upon further imaging, the mass was determined to be an iliac stent migration. CASE SUMMARY: Our patient presented with a migrated iliac stent 2 years after placement, and a transthoracic echocardiogram visualized a circular and metallic mass. Computed tomography angiography demonstrated stent fragmentation with embolization to the right ventricle, pulmonary vasculature, and inferior vena cava, with entrapment of the

Systemic thrombolysis for an intracardiac thrombus in a premature child with a severe diaphragmatic hernia.

Intracardiac thrombus is a rare but difficult-to-treat complication in premature infants, with limited therapeutic options. If an urgent neonatal surgical procedure is needed, rescue systemic thrombolysis may be considered as an option to hasten thrombus resolution. In the present case, we illustrate the successful lysis of a right ventricle intracardiac thrombus in a premature child prior to a neonatal emergent surgery for a severe congenital diaphragmatic hernia. The 1-day-old premature child with right-sided diaphragmatic hernia contracted preoperatively a right ventricle thrombus in the co

Severe peripartum cardiomyopathy complicated by multiple left ventricular thrombi and systemic embolic events: a case report.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a potentially life-threatening cause of heart failure occurring towards the end of pregnancy or in the months following delivery. Severe PPCM may be complicated by intracardiac thrombi and systemic embolization, favoured by left ventricular (LV) systolic dysfunction, ventricular dilatation, and the hypercoagulable state of the puerperium. CASE SUMMARY: A 36-year-old woman presented 1-month postpartum with acute decompensated heart failure. Transthoracic echocardiography showed severe LV systolic dysfunction, with a LV ejection fraction (LVEF) of

Recurrent bilateral lower limb arterial embolism secondary to left ventricular thrombus in peripartum cardiomyopathy successfully treated with systemic streptokinase thrombolysis: a rare case report.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is an uncommon but potentially life-threatening form of heart failure occurring toward the end of pregnancy or during the months following delivery in women without pre-existing heart disease. Left ventricular systolic dysfunction predisposes patients to intracardiac thrombus formation and systemic thromboembolic complications, which are associated with significant morbidity and mortality. CASE PRESENTATION: A 35-year-old multiparous woman presented shortly after spontaneous vaginal delivery of an intrauterine fetal demise with acute right lower-li

Clinical features and advances in the genetics of periodic paralysis.

Cardiomyopathies cause most intracardiac thrombosis (ICT), and Behçet's syndrome (BS) is a rare inflammatory disease that can be responsible for a proportion of ICT. Other inflammatory disorders involved in the aetiology of ICT include antiphospholipid syndrome, Henoch-Schonlein purpura, COVID-19, and Loeffler endocarditis. ICT usually occur during the active phase of BS, and they have a close relationship with vascular involvement. Atrial myxomas are benign cardiac tumours arising from the interatrial septum. They can lead to a substantial acute phase response, making them difficult to distin

Massive hemoptysis after left atrial appendage occlusion in a patient with atrial fibrillation and left atrial appendage thrombus: a case report.

We report the case of an 86-year-old male with atrial fibrillation and a persistent left atrial appendage thrombus despite prolonged oral anticoagulation therapy. The patient successfully underwent intracardiac echocardiography-guided left atrial appendage occlusion with the thrombus in situ. However, he developed a rare but serious complication-massive hemoptysis. The patient then underwent bronchial artery angiography with successful embolization of the dilated bronchial arteries. Following the procedure, the hemoptysis was effectively controlled without any sequelae, and no complications we

Adrenocortical Carcinoma With Right Atrial Extension in a Three-Year-Old Child: A Case Report.

Pediatric adrenocortical carcinoma is a rare and aggressive malignancy frequently associated with germline tumor protein p53 (TP53) mutations and can mimic congenital adrenal hyperplasia (CAH). We present a three-year-old child with progressive virilization, initially presumed to be CAH, who presented in adrenal crisis with abdominal distention. Imaging revealed a large right adrenal mass, with the mass extending from the right hepatic vein through the inferior vena cava (IVC) and into the right atrium, causing an almost complete obstruction of the tricuspid valve. After evaluation and stabili

Single-stage laparotomy for intracardiac leiomyomatosis without open-heart surgery: a case report.

BACKGROUND: Intracardiac leiomyomatosis is a rare leiomyoma that originates from the uterus. It can be easily misdiagnosed because of its nonspecific manifestations. In this case, we aim to emphasize the complete resection of the ICL with a single-stage laparotomy. CASE PRESENTATION: Here, we report a case of a 36-year-old female who presented with dyspnea and a mobile tumor in the right atrium that was detected in transthoracic echocardiography prior to cosmetic surgery. Case Report: Embolus in transit vs. in situ PFO thrombus.

A crossing patent foramen ovale (PFO) thrombus is a thrombus that straddles both atria through a PFO, also called a transseptal thrombus or an impending paradoxical embolism. Although rare, this condition represents a highly critical clinical emergency. Clinically, such thrombi are usually classified as primary intracardiac (in situ) thrombosis or emboli in transit from the venous system. We report two contrasting cases of a thrombus straddling the foramen ovale documented with high-quality multimodality imaging and serial transesophageal echocardiography (TEE) during follow-up. Case 1 involve

[Research progress on risk factors and risk prediction of cardiovascular diseases related to antiphospholipid syndrome].

Antiphospholipid syndrome (APS) is a systemic autoimmune disease characterized by clinical manifestations such as thrombocytopenia, thrombosis, and miscarriage caused by persistent positive antiphospholipid antibodies. The cardiovascular system is one of the main target systems involved, including valvular heart disease, coronary artery disease, myocardial disease, pulmonary hypertension, and intracardiac thrombus. The 2023 American College of Rheumatology(ACR)/European League Against Rheumatology(EULAR) Antiphospholipid Syndrome Classification Criteria were released, incorporating various cli

Fatal Mesenteric Ischemia From a Presumed Paradoxical Embolism Through a Patent Foramen Ovale.

Acute mesenteric ischemia is a highly lethal vascular emergency most often caused by arterial embolism. Paradoxical embolism through a patent foramen ovale (PFO) is a well-recognized cause of cryptogenic stroke, but it is a rare and underrecognized cause of mesenteric arterial occlusion. A 68-year-old woman with a history of paradoxical embolism, chronic deep vein thrombosis, and a patent foramen ovale presented with sudden, severe abdominal pain and vomiting. She had previously undergone brachial artery embolectomy and had been maintained on long-term apixaban, which she later discontinued af

Undetected intracardiac thrombus and subsequent embolism following cardiopulmonary bypass: a case report.

BACKGROUND: Heparin is commonly administered to prevent thrombosis during cardiopulmonary bypass (CPB). However, the development of intracardiac thrombi under CPB has still been reported. A 79-year-old man with three-vessel coronary artery disease and a history of bladder cancer underwent on-pump coronary artery bypass grafting. His cardiac function was compromised by a low ejection fraction. After aortic declamping, the arterial waveform in the right radial arterial line disappeared abruptly, and emergent thrombectomy was performed for thrombotic occlusion of the right radial artery. Abdominal aortic aneurysm repair in an infant with tuberous sclerosis, cardiac rhabdomyoma, and hypercoagulability: The role of multidisciplinary management.

An infant diagnosed prenatally with tuberous sclerosis (TS) and cardiac rhabdomyoma presented at 6 months of age with an infrarenal abdominal aortic aneurysm. A multidisciplinary team comprising pediatric cardiologists, vascular surgeons, and pediatric cardiac anesthesiologists collectively managed the surgical repair. Given the possibility of diastolic dysfunction related to intracardiac rhabdomyomas, transesophageal echocardiography (TEE) was employed intraoperatively to guide fluid and inotrope management. Platelets were transfused based on clinical judgment due to ongoing blood loss; howev

Comparison between intracardiac and transesophageal echocardiography for the detection of thrombus in the left atrial appendage in relation to in-hospital and one-year outcomes among patients undergoing atrial fibrillation ablation.

BACKGROUND: Transesophageal echocardiography (TEE) is the standard method for excluding left atrial appendage (LAA) thrombus before atrial fibrillation (AF) ablation. Intracardiac echocardiography (ICE), used during ablation, allows LAA assessment at the procedure's beginning. Preliminary data suggest that ICE may provide thrombus visualization, but evidence remains limited. AIMS: To compare ICE and TEE for LAA thrombus detection and procedural outcomes in AF ablation. METHODS: This retrospective cohort study included 610 adults undergoing catheter ablation for AF or atrial flutter between 2

Left atrial appendage thrombus in a term neonate with ventricular dysfunction.

Left atrial appendage thrombosis is exceedingly rare in neonates and may present with nonspecific findings. We report a term neonate presenting with poor feeding and a gallop rhythm, who was found to have a large left atrial appendage thrombus with ventricular dysfunction. Surgical excision resulted in rapid recovery, highlighting the importance of early echocardiographic evaluation in subtle neonatal heart failure.
