BACKGROUND: Ruptured intracranial internal carotid artery dissecting aneurysm (IICDA) occurring in association with fibromuscular dysplasia (FMD) is exceptionally rare, and the optimal treatment strategy remains undefined.
OBSERVATIONS: A 43-year-old woman presented with sudden severe headache and was transferred to the authors' hospital 2 days later after developing impaired consciousness. Initial CT revealed a diffuse subarachnoid hemorrhage with right-sided predominance. Digital subtraction angiography demonstrated a "string-of-beads" appearance in the bilateral cervical internal carotid a

BACKGROUND: Aplastic or twig-like middle cerebral artery (Ap/T-MCA) is a rare cerebrovascular anomaly characterized by unilateral stenosis of the proximal middle cerebral artery (MCA) with a plexiform arterial network along the horizontal (M1) segment, requiring distinction from moyamoya disease. Genetically confirmed cases remain rare.
OBSERVATIONS: We report the cases of 3 patients with Ap/T-MCA carrying the RNF213 (ring finger protein 213) variant. The first patient, a man in his 60s, presented with subarachnoid hemorrhage. The second patient, a woman in her 20s, developed involuntary move

BACKGROUND: Middle cerebral artery (MCA) aneurysms rarely cause ophthalmic complications due to their anatomical distance from ocular structures. We report an exceedingly rare case of acute unilateral blindness and complete ophthalmoplegia following MCA aneurysm clipping via a pterional/trans-sylvian approach, a complication not previously documented in the literature with concurrent optical coherence tomography (OCT) findings.
CASE PRESENTATION: A 49-year-old man with a history of hypertension presented with a severe headache. Computed tomography (CT) and angiography confirmed a subarachnoid

Background: Temporary arterial occlusion (TAO) is a key adjunct in microsurgical clipping of middle cerebral artery (MCA) aneurysms, but its safe duration and impact on perioperative ischemia-particularly in subarachnoid hemorrhage (SAH)-remain uncertain. Methods: A retrospective cohort of 245 patients undergoing microsurgical clipping of MCA aneurysms (154 incidental, 91 SAH) at a tertiary neurovascular center (2010-2020) was analyzed. TAO use, cumulative duration (>5, >8, >10, >15 min), number of applications, and occlusion site were extracted alongside clinical, radiographic, and outcome da

Stroke remains a leading cause of death and disability worldwide. Despite hundreds of neuroprotective agents showing promise in preclinical studies, nearly all have failed in clinical trials, largely due to inconsistencies between animal models and the human condition. This review provides a comprehensive overview of experimental stroke models, encompassing ischemic stroke (middle cerebral artery occlusion, photothrombosis, thromboembolic models, endothelin-1-induced vasoconstriction, global ischemia), hemorrhagic stroke (intracerebral hemorrhage, subarachnoid hemorrhage, epidural hematoma), c

INTRODUCTION: Vertebral artery dissection (VAD) is a leading cause of cerebrovascular disease in young and middle-aged adults. Although medical claims databases facilitate large-scale observational studies, validated case-identification algorithms for unruptured VAD and its complications remain limited. We evaluated the diagnostic accuracy of International Classification of Diseases, Tenth Revision (ICD-10) codes for identifying unruptured VAD and its complications in Japanese medical claims data.
METHODS: We conducted a single-center cross-sectional validation study using electronic medical

The superficial temporal artery (STA) is one of the final branches of the external carotid artery. STA pseudoaneurysms are a rare vascular complication following blunt trauma, and even rarer following craniotomy. A 44-year-old man presented to hospital with altered level of consciousness and left-sided paresis. Neuroimaging showed a right middle cerebral artery bifurcation aneurysm, subarachnoid hemorrhage, and a large right-sided intracerebral hematoma (ICH). An emergent fronto-temporo-parietal craniotomy was performed for hematoma evacuation and clipping of the ruptured aneurysm. On postoper

BACKGROUND: Retrieval of an atherosclerotic plaque-vessel wall complex as a single specimen from the middle cerebral artery during mechanical thrombectomy (MT) is exceedingly rare.
OBSERVATIONS: A 74-year-old man with atrial fibrillation underwent MT for right M1 occlusion. A red thrombus and a yellowish-white specimen were retrieved, and successful reperfusion was achieved. Histopathological examination of the retrieved specimen revealed an atherosclerotic plaque attached to the arterial wall, consistent with retrieval of the plaque-vessel wall complex as a single specimen. Postoperatively,

BACKGROUND: Vasospasms associated with dural arteriovenous fistula (dAVF)-related subarachnoid hemorrhage (SAH) are rare, and their management has not been well established. Clazosentan, a selective endothelin A receptor antagonist, is typically administered soon after an SAH to prevent cerebral vasospasms. However, the optimal timing remains controversial because of the risk of fluid retention and related complications. Here, we report the case of dAVF-related SAH in which clazosentan was introduced as rescue therapy to treat cerebral vasospasm.

BACKGROUND: Anterior cerebral artery (ACA) mirror intracranial aneurysms (IAs) are rare, underreported vascular lesions. This systematic review addresses current evidence on ACA mirror aneurysms by analysing patient demographics, clinical presentation, imaging findings, aneurysm characteristics, treatment strategies and outcomes.
METHODS: A PRISMA guided systematic search was conducted in MEDLINE, Scopus and Web of Science. Studies reporting ACA mirror IAs, defined as aneurysms arising symmetrically from the same segment of bilateral intracranial arteries, with demographic, clinical and proce

Traumatic intracranial aneurysms (TICAs) are rare lesions accounting for <1% of intracranial aneurysms and are associated with high morbidity and mortality. They frequently arise after blunt trauma and may develop days to months following the initial injury, often evading early vascular imaging. Pseudoaneurysms predominate and carry a high risk of rupture. We report the case of a patient who sustained a severe head injury with a comminuted open depressed temporoparietal skull fracture, epidural hematoma, subarachnoid hemorrhage, and mandibular fracture after a road traffic accident. Initial va

PURPOSE: This study aimed to evaluate the validity of the test bolus tracking (TBT) method for head computed tomography (CT) angiography (CTA) using four-dimensional (4D) CT data in patients with subarachnoid hemorrhage (SAH).
METHODS: We analyzed 39 patients who developed SAH and underwent 4D CT. Based on the imaging protocol, patients were classified into a test-injection (TI) group and a TBT group. Time-enhancement curves (TECs) of the M1 segment of the middle cerebral artery were measured. Validation was performed by assessing the concordance between the TEC peak time and the peak scan ti

UNLABELLED: Concurrent pituitary apoplexy and ruptured intracranial aneurysm are rare, but potentially fatal, if unrecognised. A 53-year-old hypertensive woman presented with acute confusion and transient hemiparesis. Brain CT demonstrated a haemorrhagic sellar-suprasellar mass with atypical subarachnoid and intraventricular haemorrhage. CT angiography (CTA) confirmed a ruptured right middle cerebral artery (MCA) aneurysm and two additional unruptured aneurysms. Subsequent endovascular coil embolisation was successful. The pituitary macroadenoma was managed conservatively.

Anterior communicating artery (AComm) aneurysms account for approximately 25% of all cerebral aneurysms.1-3 Their management remains challenging due to significant anatomic and morphologic variability, and a high recurrence rate after primary endovascular coiling.4 Recurrence is often due to coil compaction and thrombus incorporation into the aneurysm wall, necessitating more complex treatments such as stenting, flow diversion, or microsurgical clip reconstruction. The picket fence clipping technique is a strategy for treating large or recurrent aneurysms when conventional clipping across the

OBJECTIVE: Procedure-related subarachnoid hemorrhage (SAH) is a recognized complication of mechanical thrombectomy for M2 middle cerebral artery occlusion, but whether the first-line device strategy influences its occurrence remains unclear. This study was performed to determine whether a stent retriever (SR)-first strategy, compared with a contact aspiration (CA)-first strategy, is independently associated with procedure-related SAH.
METHODS: In this single-center retrospective study, 180 consecutive patients who underwent mechanical thrombectomy for isolated M2 occlusion were classified acc

Anterior communicating artery (AComA) aneurysms remain challenging because of their complex anatomy, rupture risk, and proximity to vital structures. This study compared the long-term durability, retreatment rates, complications, and outcomes of microsurgical clipping and endovascular treatment. This retrospective study included 313 patients (≥ 18 years) with ruptured or unruptured AComA aneurysms treated at a single center (2006-2024). Patients were categorized by treatment modality into two groups: (1) Microsurgical clipping and (2) Endovascular treatment. Primary outcomes included aneurysm
